RESUMO

Objective: The objective of the present work was to establish a simple, precise, accurate and robust method for simultaneous estimation of gallic acid, curcumin and piperine from the marketed ayurvedic formulation by liquid chromatography. Methods: The separation was carried out on Hemochrom C18 Column (250 mm × 4.6 mm ID, 5 µm pore size) with a mobile phase methanol: acetonitrile: water (pH 3.2adjusted by using orthophosphate acid) in the ratio 70:20:10v/v by isocratic elution modeat 25 °C and the flow rate was setat0.8 ml/min. The analysis was carried out atisoabsorptive wavelength of 295 nm. Results: The retention time of gallic acid, curcumin and piperine was found to be 3.3(±0.2), 4.7 (±0.2) and 5.6 (±0.2) min, respectively. The linearity range for gallic acid, curcumin and piperine was found to be 10-70 μg/ml, 20-80 µg/ml and 2-14 µg/ml, respectively with the coefficient of linear regression greater than 0.99 for all markers. Mean percent recoveries for gallic acid, curcumin, and piperine were found within the limit of acceptance (99-100%). The percent relative standard deviation (%RSD) for precision and robustness was found less than 2%, which indicates the method is precise and robust. The developed method applied for quantification of these markers from the marketed ayurvedic formulation of Dekofcyn tablet. Conclusion: The developed method was found to be simple, rapid, precise and reproducible for standardization of Dekofcyn tablet and can be useful for other formulations containing these three markers.

Objective: This study aims to compare the effect of metformin monotherapy with the combination therapy of metformin and apple cider vinegar on the parameters of Body Mass Index and glycemic control of recently diagnosed type 2 diabetes mellitus patients. Methods: A total of 126 patients were enrolled in this open-label randomized controlled trial, divided into two parallel arms: Group I (Metformin 750 mg alone per day) and Group II (Metformin 750 mg+Apple Cider Vinegar 2 tablespoons per day) and assessed for weight, Body Mass Index, Fasting Blood Sugar and Hemoglobin A1c after which the post-treatment values of the aforementioned parameters were compared between the two groups. Informed consent from all patients and ethical approval from the institutional ethics board was obtained. A p-value of <0.05 was considered significant. Results: All the parameters underwent a significant reduction in the pre and post intra group comparison of each group (p-value<0.001). Moreover, a significant difference was found in the post-treatment comparisons of the Fasting Blood Sugar (p-value<0.001) and HbA1c (p-value=0.04) in between the two groups. However, no significant difference was obtained in the post-treatment comparisons of weight and Body Mass Index in between the two groups. Conclusion: Apple Cider Vinegar in combination with Metformin appears to be more effective in improving glycemic control versus Metformin monotherapy.

RESUMO

Lower limb reconstruction following trauma, chronic infection and cancer ablation remains challenging. However various options are now possible for lower limb reconstruction due to the more details available today about perforators. In recent years, the management of lower extremity has evolved with numerous new techniques and innovations and thus extremities are salvaged that would have been amputated in the past. Today we are able to cover various complex defects with the help of free flaps due to more of research and knowledge in this field which was previously unknown. In developed countries, all these procedures can be used because of the availability of good resources, good manpower, and the latest technologies. But in developing countries like India in spite of being aware of many of this methods, there is a limitation on using all these methods due to the limitation of resources, economic burden, limited manpower, and surgery time. The concept of the reconstructive ladder was proposed to achieve wounds with adequate closure using a stepladder approach from simple to complex procedures. The reconstructive elevator requires creative thoughts and consideration of multiple variables to achieve the best form and function rather than a sequential climb up the ladder. However, this paradigm of thought does not eliminate the concept of the reconstructive ladder but replaces it with a ladder of wound closure that makes its mark in the field where a variety of advanced reconstructive procedures and techniques is not readily available. Therefore the model of step ladder pattern of soft tissue defect management remains the good option for developing countries like ours. Also, various lower limb defects from simple to complex can be covered with simple methods rather than going for the most complex ones first. It helps in fast management and low economic burden to society with a lesser hospital stay.

RESUMO

We describe 10-month-old identical female twin infants, one with primary left-sided pulmonary agenesis and the other with primary left-sided pulmonary hypoplasia. They came to our outpatient clinic complaining of persistent dry cough. The clinical examination revealed decreased air entry over the left hemithorax. Chest x-rays showed complete left-sided radio-opacity in both the twins. The chest computed tomography scan with contrast confirmed the diagnoses of left-sided pulmonary agenesis [twin A] and left-sided hypoplasia [twin B]. No other associated congenital anomaly was noted in either of the twins. To our knowledge, such a condition in live monozygotic twins has not been previously reported in published studies

RESUMO

A 41 year male, diabetic on treatment presented with persistent erythema and. "taut" skin over neck and back since 1 year. The lesions showed symmetric and gradual progression. There was no history of prior sore throat. On examination effected skin was erythematous, woody hard and unpinchable. Scleredema diabeticorum (diutinum), although sharing clinical and historical features with Scleredema adultorum has no prodromal infection, is more extensive and affected individuals are characteristically obese, middle aged diabetics who often have accompanying microangiopathies and macroangiopathies. Recognition of scleredema by the physician has prognostic and therapeutic implications in the management of the coexisting diabetes.
