RESUMO

ObjectiveTo observe the clinical efficacy of Shengmaisan combined with polymyxin B in the treatment of carbapenem-resistant gram-negative bacillus infection with sepsis complicated with severe acute respiratory distress syndrome. MethodA total of 90 patients suffering from carbapenem-resistant gram-negative bacillus infection with sepsis complicated with severe acute respiratory distress syndrome were randomly divided into a control group and an observation group, with 45 cases in each group. The control group was treated with polymyxin B, and the observation group was treated with Shengmaisan combined with polymyxin B. The treatment course of both groups was seven days. The infection-related indicators [white blood cell (WBC) count, procalcitonin (PCT), neutrophil apolipoprotein (HNL)], inflammatory factors [interleukin-6 (IL-6), serum chemokine ligand 2 (CXCL2)], and T lymphocyte subpopulations (CD3+, CD4+, CD8+, and CD4+/ CD8+ value), acute physiological and chronic health Ⅱ (APACHE Ⅱ) score before and after treatment, as well as bacterial clearance rate and 28-day survival rate after treatment were observed. Result① The experiment was completed, and 81 cases were included, including 41 cases in the observation group and 40 cases in the control group. The general data of the two groups were comparable. ② The bacterial clearance rate of the observation group and the control group was 75.6% (31/41) and 52.5% (21/40), respectively, and the observation group was higher than the control group (χ2=4.7, P<0.05). ③ The WBC count, PCT, HNL, IL-6, CXCL2, and APACHE Ⅱ scores of the observation group and the control group all decreased after treatment (P<0.05). Except for the WBC count, the PCT, HNL, IL-6, CXCL2, and APACHE Ⅱ scores of the observation group were lower than those of the control group (P<0.05). ④ The values of CD3+, CD4+, and CD4+/CD8+ in the observation group were increased after treatment (P<0.05), and CD8+ was decreased (P<0.05). In the control group, only CD3+ value was increased (P<0.05). The values of CD3+, CD4+, and CD4+/CD8+ in the observation group were higher than those in the control group, and the value of CD8+ was lower than that in the control group (P<0.05). ⑤ The 28-day survival rate in the observation group was higher than that in the control group (χ2=4.3, P<0.05). ConclusionShengmaisan combined with polymyxin B in the treatment of carbapenem-resistant gram-negative bacillus infection with sepsis complicated with severe acute respiratory distress syndrome can better clear bacteria, control infection, reduce the level of inflammatory factors, regulate the immune state of the body, and improve the short-term prognosis.

RESUMO

Objective: To study the effect of diurnal temperature range on the number of elderly inpatients with ischemic stroke in Hunan Province. Method: Demographic and disease data, meteorological data, air quality data, population, economic and health resource data of elderly inpatients with ischemic stroke were collected in 122 districts/counties of Hunan Province from January to December 2019. The relationships between the diurnal temperature range and the number of elderly inpatients with ischemic stroke were analyzed by using the distributed lag non-linear model, including the cumulative lag effect of the diurnal temperature range in different seasons, extremely high diurnal temperature range and extremely low diurnal temperature range. Results: In 2019, 152 875 person-times were admitted to the hospital for ischemic stroke in the elderly in Hunan Province. There was a non-linear relationship between the diurnal temperature range and the number of elderly patients with ischemic stroke, with different lag periods. In spring and winter, with the decrease in diurnal temperature range, the risk of admission of elderly patients with ischemic stroke increased (Ptrend<0.001, Ptrend=0.002);in summer, with the increase in diurnal temperature range, the risk of admission of elderly patients with ischemic stroke increased (Ptrend=0.024);in autumn, the change in the diurnal temperature range would not cause a change in admission risk (Ptrend=0.089). Except that the lag effect of the extremely low diurnal temperature range in autumn was not obvious, the lag effect occurred in other seasons under extremely low and extremely high diurnal temperature ranges. Conclusion: The high diurnal temperature range in summer and the low diurnal temperature range in spring and winter will increase the risk of admission of elderly patients with ischemic stroke, and the risk of admission of elderly patients with ischemic stroke will lag under the extremely low and extremely high diurnal temperature ranges in the above three seasons.

RESUMO

Objective:To observe any effect of transcranial direct current stimulation (tDCS) on learning, memory ability and the morphology of neurons in the hippocampus and cortex of rats with cognitive impairment, and also to seek any correlation between the rats′ behavior and the thickness of the granular layer in the CA1 region of the hippocampus. Methods:Thirty Sprague-Dawley rats were randomly divided into an observation group, a model group and a control group, each of 10. Cognitive impairment was induced in the observation and model groups by intraperitoneal injection of scopolamine, while the control group was injected with saline solution over the same period of time. After successful modeling, the observation group was given tDCS, while the model and control groups were connected with electrodes but not given any electrical stimulation. After 16 consecutive days of treatment, behavioral changes of each group were quantified using a shuttle box and a Morris water maze. On the 30th day after the mode-ling, the brains were collected to observe any changes in the morphology of the hippocampal and cortical neurons. The thickness of the hippocampal granular layer was also measured. Results:In the observation group the average rate of electrical impulses after the intervention [(60.5±6.67)/min] was significantly less than in the model group [(145.8±19.31)/min], while the time to find a platform was significantly shorter. The rats of the observation group also crossed the D quadrant of the platform significantly more quickly than the model group, on average. Compared with the control group, the granular layer in the CA1 region of the hippocampus [(93.47±1.07)μm] was significantly thinner on average than in the model group but compared with the model group, the observation group had significantly thicker layers [95.17±1.49)μm] on average. The thickness was negatively correlated with the number of shocks and the time to find the platform, but positively correlated with the number of crossings of the platform in the D quadrant. Conclusions:The degree of impairment generated by intraperitoneal injection of scopolamine correlates with the thickness of the CA1 granular layer of the hippocampus, at least in rats. tDCS can improve the learning and memory of such rats. Its mechanism may be related to promoting structural recovery of hippocampal cortical neurons and increasing the thickness of the granular layer.

RESUMO

Objective: To investigate the effects of compound analgesia on ultra-pulsed fractional carbon dioxide laser (UFCL) treatment of post-burn hypertrophic s in children. Methods: A prospective randomized controlled study was conducted. From April 2018 to March 2020, 169 pediatric patients with post-burn hypertrophic s admitted to the First Affiliated Hospital of Air Force Medical University were randomly divided into general anesthesia alone group (39 cases, 19 males and 20 females, aged 35 (21, 48) months), general anesthesia+lidocaine group (41 cases, 23 males and 18 females, aged 42 (22, 68) months), general anesthesia+ibuprofen suppository group (41 cases, 25 males and 16 females, aged 38 (26, 52) months), and three-drug combination group with general anesthesia + lidocaine+ibuprofen suppository (48 cases, 25 males and 23 females, aged 42 (25, 60) months), and the pediatric patients in each group were treated with corresponding analgesic regimens when UFCL was used to treat s, and the pediatric patients were given comprehensive care throughout the treatment process. The pain degree of pediatric patients scar was evaluated by facial expression, legs, activity, cry, and consolability (FLACC) of children's pain behavior scale at 0 (immediately), 1, 2, and 4 h after awakening from the first anesthesia, respectively. At 4 h after awakening from the first anesthesia of postoperative pain assessment, the self-made analgesia satisfaction questionnaire was used to evaluate the satisfaction for the analgesic effect of the pediatric patients or their families, and the satisfaction rate was calculated. Within 2 h after the first operation, the occurrences of adverse reactions of the pediatric patients, such as nausea and vomiting, headache, dizziness, drowsiness, etc, were observed and recorded. Before the first treatment and 1 month after the last treatment, the Vancouver scar scale (VSS) was used to evaluate the pediatric patients scar, and the difference value between the two was calculated. Data were statistically analyzed with least significant difference test, Kruskal-Wallis H test, chi-square test and Fisher's exact probability test. Results: At 0 h after awakening from the first anesthesia, the FLACC scores of pediatric patients in general anesthesia+lidocaine group, general anesthesia+ibuprofen suppository group and three-drug combination group were significantly lower than those in general anesthesia alone group (P<0.01). The FLACC scores of the pediatric patients in anesthesia+ibuprofen suppository group and three-drug combination group were significantly lower than that in general anesthesia+lidocaine group (P<0.01), and the FLACC score of the pediatric patients in three-drug combination group was significantly lower than that in general anesthesia+ibuprofen suppository group (P<0.01). At 1 and 2 h after awakening from the first anesthesia, the FLACC scores of pediatric patients in general anesthesia+ibuprofen suppository group and three-drug combination group were both significantly lower than those in general anesthesia alone group and general anesthesia+lidocaine group (P<0.01), and the FLACC score of the pediatric patients in three-drug combination group was significantly lower than that in general anesthesia+ibuprofen suppository group (P<0.01). At 4 h after awakening from the first anesthesia, the FLACC scores of the pediatric patients in general anesthesia+ibuprofen suppository group and three-drug combination group were significantly lower than those in general anesthesia alone group and general anesthesia+lidocaine group (P<0.01). At 4 h after awakening from the first anesthesia, the satisfactions rate with the analgesic effect in the four groups of pediatric patients or their families were 79.49% (31/39), 85.37% (35/41), 87.80% (36/41), and 97.92% (47/48), respectively. The satisfaction rate of the pediatric patients in three-drug combination group was significantly higher than those in general anesthesia alone group, general anesthesia+lidocaine group, general anesthesia+ibuprofen suppository group. Within 2 h after the first operation, there was no significant difference in the overall comparison of adverse reactions such as nausea and vomiting, headache, dizziness, and drowsiness of pediatric patients among the 4 groups (P>0.05). The VSS scores of pediatric patients before the first treatment, 1 month after the last treatment, and and the difference value between the two in the 4 groups were not significantly different (P>0.05). Conclusions: Three-drug combination for analgesia has a good effect in the treatment of hypertrophic scars after burn in pediatric patients with UFCL. Pediatric patients or their families are highly satisfied with the effect, and the treatment effect and incidence of adverse reactions are similar to other analgesic regimens, so it is recommended to be promoted in clinical practice.

RESUMO

Anxiety disorders are a common mental illness that seriously endangered physical and mental health of human beings. The etiology of anxiety disorders is closely related to the abnormality of monoamines neurotransmitters, amino acids neurotransmitters and neuropeptides. The long-term use of anti-anxiety chemical drugs has some adverse effects, such as constipation, muscle relaxation, lethargy, tolerance and withdrawal symptoms. However, traditional Chinese medicines have advantages of multi-component, multi-target coordination, with less adverse reactions. Therefore, it is a promising prospect to develop novel anti-anxiety drugs from traditional Chinese medicines and formulas. This article reviewed some traditional Chinese medicines and formulas that can relieve anxiety symptoms. These include traditional Chinese medicines(Panax ginseng, Lycium ruthenium, Morus alba, Bupleurum plus dragon bone oyster soup, Chailong Jieyu Pills, and Naogongtai Formulas) with the effect on monoamine neurotransmitters, such as serotonin, dopamine, and norepinephrine; traditional Chinese medicines(Rehmannia glutinosa, Ziziphus jujuba Mill. var. spinosa, Jielv Anshen Decoction, Baixiangdan Capsules, Antianxietic Compound Prescription Capsules) with the effect on amino acid neurotransmitters, such as glutamic acid, γ-aminobutyrc acid; and traditional Chinese medicines(P. ginseng, Xiaoyao San, Shuyu Ningxin Decoction)with the effect on neuropeptide Y pathway, with the aim to provide theoretical basis for the further development of some novel and more effective anti-anxiety therapeutics from traditional Chinese medicine and formulas.

RESUMO

Objective: To estimate the prevalence of HIV/HCV co-infection and explore the influence factors and their interaction on HIV/HCV co-infection of patient's access to methadone maintenance treatment (MMT). Methods: A face to face interviews were conducted among 750 patients at two MMT clinics in Guangxi Zhuang Autonomous Region. The questionnaires information included demographic characteristics, HIV and HCV infection status, history of drug abuse, urine test for morphine, high risk sex behaviors, needle sharing, dropped out etc. Methods of χ(2) test one-way, multivariate logistic regression and interactions were used to analyze the related factors of HIV/HCV co-infection. Results: The study subjects included 750 participants, 18.31% (127/691) of patients were co-infected with HIV and HCV. The HIV/HCV co-infection rate in patients who shared needles with others or dropped out of treatment was 35.84% (81/226) and 19.88% (64/322) respectively, which were higher than those who have never shared needles or dropped out (9.89%, 46/465 and 17.07%, 63/369). Logistic regression analysis results showed that after adjusted for confounding factors, patients who shared needles (OR=4.50, 95%CI: 2.72-7.43) and dropped out of treatment (OR=1.71, 95%CI: 1.04-2.80) were more likely to be infected with HIV/HCV. Interaction analysis showed that sharing needles and dropping out of treatment exist additive effect on co-infection of HIV and HCV (RERI=4.21, AP=0.44, SI=1.95). Conclusions: Needle sharing and dropping out of treatment are associated with HIV/HCV co-infection. Health education, psychological counseling and other measures should be taken to reduce needle sharing and dropping out of MMT.

RESUMO

Objective: To explore the application of the cuff-leak test(CLT)guiding offline extubation in patients with tracheal intubation of mechanical ventilation. Methods: 64 patients with tracheal intubation who underwent mechanical ventilation were divided into CLT-negative group(47 cases)and CLT-positive group(17 cases)according to leakage situation of CLT.The CLT guiding offline extubation were adopted in the study and some basic situations, such as body mass index(BMI),APACHE-Ⅱ and so on,of the two groups were compared. And the relative situation of intubation, blood gas analysis index, vital signs, the occurrence rate of upper airway obstruction(UAO)post removing intubation and re-intubation rate between the two groups also were compared, and then the risk factors of influencing UAO were further analyzed. Results: The BMI of CLT-negative group was significantly smaller than that of CLT-positive group(t=2.44,P<0.05).The occurrence rate of UAO and re-intubation rate of CLT-negative group(6.38%and 2.13%)were significantly lower than that of CLT-positive group(35.29%and 17.65%)(x2=8.63,x2=5.13,P<0.05),respectively.The differences of BMI,APACHE-Ⅱ scores, intubation time, air sac pressure,PaO2and SpO2between patients with UAO and patients without UAO were significant(t=5.63,t=2.65,t=4.27,t=3.35,t=2.37,t=2.66,P<0.05).The results of Logistic regression analysis showed that the BMI,APACHEⅡ score, intubation time, air sac pressure were independent risk factors for occurring UAO post extubation. Conclusion: The CLT guiding offline extubation in patient with tracheal intubation of mechanical ventilation can effectively reduce the re-intubation rate. For these patients with obesity, high APACHE-Ⅱ score, long intubation time and big air sac pressure, the risk of occurring UAO is higher. Therefore, the number of intubation pre extubation should be reduced for them, and their physiological status should be comprehensively assessed so as to decrease the occurrence rate of UAO.

RESUMO

Purpose To investigate the clinicopathologic features of primary mediastinal germ cell tumors and to improve the diagnosis and treatment guidance. Methods The clinical features, histologic findings, molecular detection and biological behaviors of 56 PMGCT cases were analyzed retrospectively. Results The age of patients ranged from 9 to 48 years (median age 29.1 years), and mature teratoma(76.8%, 43/56) were the most common type.3 cases of mature teratoma were prepubertal patients.53 cases of postpubertal patients included 40 cases of mature teratoma, 2 cases of nonmature teratoma, 2 cases of yolk sac tumor, 5 cases of seminoma, 4 cases of mixed germ cell tumor. All malignant PMGCTs were male, and mature teratoma was found in the female. Histopathologic morphology and immune phenotype of primary mediastinal germ cell tumors were consistent with those of sexual gland. The isochromosome 12p was detected in various component of malignant GCTs, not in mature teratoma. All patients underwent surgical resection, with additional chemotherapy for malignant germ cell tumor cases. The prognosis of mature teratoma regardless of prepubertal or postpubertal patients was benign, but PMGCTs (except mature teratoma) of postpubertal type were malignant. Conclusion Primary mediastinal germ cell tumors are rare and mature teratoma is most common. The malignant PMGCTs mostly occur in young men. The abnormal 12p detected by FISH is helpful to differential diagnosis and guide the treatment.

RESUMO

Objective To investigate the gastric protective effect of Albizia chinensis extract on acute gastric ulcer model in rats and its mechanism related to H+,K+-ATPase in gastric parietal cells. Methods Rats were injected intraperitoneally with indo?metacin or kept under water immersion-restraint stress(WRS)to establish acute gastric ulcer model. Test samples were orally given 30 min before induction of gastric ulcer, and the gastric ulcer index and inhibitory rate were used to evaluate the effect of A. chinensis extract. Gastric mucosa pathological tissues in rats were observed by HE staining, the enzymatic activity of proton pump on isolated gastric parietal cells was measured, and the translocation of gastric proton pump was observed by immunofluorescence cell staining. Results The A. chinensis extract inhibited the gastric ulcer with the inhibition rate of 70.0%and 75.4%at 300 mg/kg in the WRS and indomethacin-induced rat model, respectively. The extract could also inhibit the enzymatic activity of gastric proton pump with the in?hibition rate of 21.5%,29.4%,44.1%,51.9%,57.7%and 62.9%(P<0.01)at 15,30,60,120 and 480 mg/L,respectively. The half-effective dosage(ID50)of the extract was determined to be 130 mg/L. The extract showed no significant effect on the translocation of gastric proton pump. Conclusion The A. chinensis extract(300 mg/kg,ig)could significantly prevent the formation of gastric ul?cer in rats via inhibiting the enzymatic activity of gastric proton pump.

RESUMO

Objective To investigate the anti-ulcer effects of Albizia chinensis(Osbeck)Merr. extract on rats with acetic acid-induced chronic gastric ulcer and explore the anti-inflammatory mechanism. Methods Wistar male rats(n=50)were randomized in-to five groups:control, model, famotidine,Albizia chinensis extract 100 and 200 mg/kg groups. In the experiment,30%acetic acid was used to induce chronic gastric ulcer in rats. The anti-ulcer effects of A. chinensis extract were evaluated by measuring the changes in general behavior of rats, gastric tissue morphology and the ulcer area after 12 d treatments. Wistar male rats(n=48)were random-ized into three groups:control, model and A. chinensis extract(200 mg/kg)groups. At 3 d after operation, the animals were sacrificed and stomachs were rapidly removed and processed for subsequent assays. The contents of TNF-αand IL-6 were evaluated by ELISA. The activity of myeloperoxidase(MPO)was tested by o-dianisidine method. Furthermore Western blot was used to examine the phos-phorylation of P38 and JNK. Results Compared with the model group, the gastric ulcer areas of rats were reduced by 38.4%(P<0.05)and the activity of MPO declined by 37.3%(P<0.01)in A. chinensis extract(200 mg/kg)group. Meanwhile the contents of TNF-αand IL-6 in A. chinensis extract(200 mg/kg)group respectively decreased by 43.8%(P<0.01)and 32.7%(P<0.01)com-pared with the model group. A. chinensis extract(200 mg/kg)obviously suppresed the phosphorylation of P38(P<0.01),but phos-phorylation of JNK and expression of P38 and JNK remained nearly unaltered. Conclusion A. chinensis extract(200 mg/kg)exhibits a protective effect on acetic acid-induced chronic gastric ulcer, which might be relevant to anti-inflammation by inhibiting the p38 sig-naling pathway but not JNK signaling pathway. A. chinensis extract(200 mg/kg)has no effect on JNK signaling pathway.
