RESUMO

Pancreaticopleuralfistula is a very rare complication of acute and chronic pancreatitis. High index of suspicion is needed to diagnose in patients with alcohol induced pancreatitis, presenting with recurrent or persistent pleural effusion. Patient typically presented with pulmonary symptoms, rather than abdominal complaints, leading to delay in the diagnosis. Here we present a case of a known chronic pancreatitis who presented to us with massive left sided pleural effusion. Blood coloured pleural fluid analysis showed lipase and amylase levels in hundred thousand and ten thousand ranges. Diagnosis was made by CECT abdomen and confirmed with MRCP. Patient was treated with first line medical management of thoracocentesis and ERCP with pancreatic duct stenting.

RESUMO

Burn injury is a serious public health problem in developing countries. The causes vary in different communities and so this study was done to know the epidemiology and outcome of burn injuries. A one year cross sectional study was done of all the burn injury patients admitted during April 1st, 2004 to March 31st, 2005. Data was collected using a pre designed and pre tested proforma from the patients themselves or their relatives and analyzed using chi square test and percentages. A total of 76 burn injury patients were admitted. Majority were females (52.63%) and sustained burn injuries at home (p=0.000). Maximum number of females were wearing synthetic clothes (p=0.000) and suffered from flame injuries (0.006).The case fatality rate was 31.58%.Burn injuries can be reduced by bringing about regulations to develop safer cooking appliances, promoting less inflammable fabrics to be worn at home and educating the community especially women.

RESUMO

Double-barrel common bile duct is rare. We report a 50-year-old woman with defective canalization of the common bile duct, presenting with extrahepatic biliary obstruction due to stones in one compartment. CT scan highlighted this anomaly. After failed attempts at stone extraction at ERCP, she was successfully operated on.

Duodenal and colonic duplication presenting as mass in a neonate is rare. We report a 52-cm-long tubular, total colonic and 5-cm-long cystic duodenal duplication in a two-day-old neonate. Both the duplications could be excised without resection of the normal bowel.
