RESUMO

Objective: To investigate the expression of MSI1 and HER2 in mammary Paget's disease, and the correlation between the expression levels of MSI1 and HER2 and the clinicopathologic characteristics and prognosis of the patients. Methods: Clinical data and paraffin-embedded specimens of 34 pairs of mammary Paget's disease and underlying breast cancer were collected at the Department of Pathology, Affiliated Lianyungang Oriental Hospital of Xuzhou Medical University from March 2011 to December 2019. Immunohistochemistry was used to detect the expression of MSI1 and HER2 in mammary Paget's disease and the accompanying breast cancer, and to analyze the correlation between the expression levels of MSI1 and HER2 and their clinicopathologic features, as well as their influence on prognosis. Results: In mammary Paget's disease, the positive rate of MSI1 was 91.2% (31/34) and the positive rate of HER2 was 88.2% (30/34); the expression of MSI1 and HER2 was positively correlated (P=0.001, r=0.530). The expression of MSI1 was positively correlated with menopausal status (r=0.372, P=0.030) and lymph node metastasis (r=0.450, P=0.008). HER2 expression was positively correlated with menopausal status (r=0.436, P=0.010), and negatively correlated with ER expression (r=-0.365, P=0.034). The co-expression of MSI1 and HER2 was positively correlated with age (r=0.347, P=0.044) and menopausal status (r=0.496, P=0.003), and negatively correlated with ER expression (r=-0.461, P=0.006). Conclusions: MSI1 and HER2 are highly expressed in mammary Paget's disease and their expression levels are positively correlated. The correlation analysis between clinicopathological features and prognosis suggests that both of them may be involved in the occurrence and development of mammary Paget's disease and are potential therapeutic targets for mammary Paget's disease.

RESUMO

Nonalcoholic fatty liver disease (NAFLD) is a common chronic liver disease in China. There is no specific drug approved for marketing at home and abroad. At present, some adjuvant drugs are mainly used to improve the progress of the disease. Being familiar with and mastering the pharmacokinetic characteristics is very important for the selection and use of drugs. According to the characteristics of drug absorption, distribution, metabolism and excretion, reasonable drugs are selected to improve the therapeutic effect and reduce adverse reactions. Therefore, this paper reviews the pharmacokinetics and clinical significance of drugs commonly used to improve nonalcoholic fatty liver disease in hope of providing basis for clinical medication.

RESUMO

Objective To discuss the application of deep hypothermic circulatory arrest in surgical treatment of complex thoracoabdominal aortic aneurysms and its near-midterm effect.Methods The clinical data of 34 cases of thoracoabdominal aortic aneurysm in the center from August 2009 to June 2018 were analyzed retrospectively.All the patients underwent surgery under deep hypothermic circulatory arrest.There were 23 males and 11 females; aged 23 -67 years, mean(42.26 ±10.96) years old; Crawford type Ⅰ in 12 cases and Crawford type Ⅱ in 22 cases; aneurysms with a maximum diameter of 50 -120 mm, mean(65.26 ±16.09) mm;Marfan syndrome 15 cases, atherosclerosis 14 cases, aortic coarctation in 5 cases;22 cases of hypertension;28 cases of first aortic surgery, 6 cases of re-aortic surgery.Surgical transthoracic and abdominal incision, ext-racapsular approach, femoral artery and inferior vena cava intubation, deep hypothermic circulatory arrest technique to complete proximal anastomosis, arterial tube reconstruction of intercostal artery, abdominal organ blood supply artery and four The bifur-cated vessels were anastomosed, and the bifurcated vessels were anastomosed with the "Y"type artificial blood vessel trunk. The bilateral radial arteries were end-to-end anastomosis in the 10 mm artificial blood vessels of the "Y"type artificial blood vessels.Results There were no complications of cranial nerve system in the whole group , deep hypothermic circulatory arrest (17.68 ±4.88) min, ventilator assist time(34.88 ±16.04) hours, postoperative renal failure in 5 cases, after CRRT treat-ment After recovery, 1 case of paraplegia after operation, muscle strength recovered after cerebrospinal fluid drainage and de-compression, and 1 case died in the whole group, and died of multiple organ failure.The patients were followed up for 3 months to 5 years, and the results were satisfactory.The survivors did not die.The survivors did not die.However, 5 patients underwent thoracic aortic replacement under deep hypothermic circulatory arrest for the first time , and 4 patients underwent reo-peration because of distal vasodilation.The reconstructed intercostal artery occlusion occurred in 4 patients, but no paraplegia occurred.Conclusion When cross clamping the aorta is not feasible,it is safe to perform proximal anastomosis with deep hy-pothermic circulatory arrest.

Objective:To observe the effect of storage period of three experimental dentin self-etching primers on the shear bond strength.Methods:Three experimental primers with 2-hydroxyethyl methacrylate (HEMA)or N-methacryloyl glycine (NMGly)as a hydrophilic monomer and 1 0-methacryloxydecyl dihydrogen phosphate(MDP)or N-methacryloyl-2-aminoethylphosphonic acid (NMEP) as an acid monomer,were designed.Immediately after preparation of the samples of MDP-HEMA,MDP-NMGly and NMEP-NMGly, the primers were stored at 40 ℃for 0,6 and 1 4 weeks.At the end of each storage period,the shear bond strength of resin to dentin(n=1 4)and the pH value of the primers were measured.Results:After storage at 40 ℃ for 1 4 weeks,the shear bond strength of MDP-HEMA decreased from (1 7.61 ±1 .56)MPa to (7.53 ±1 .76)MPa;however the bond strenth of MDP-NMGly and NMEP-NMGly did not decrease;most of the specimens of MDP-HEMA and MDP-NMGly exhibited an interfacial failure of the resin from the dentin sur-face;however NMEP-NMGly exhibited a cohesional failure of the dentin and/or resin.The SEMimages showed a separation of resin and dentin of MDP-HEMA and MDP-NMGly had storaged at 40 ℃for 1 4 weeks;while the interface of NMEP-NMGly did not change.Con-clusion:The NMEP-NMGly primer has noticeably higher hydrolytic stability than the MDP-HEMA and MDP-NMGly primers.

Objective To explore the relationship between physical activity ( PA) in college students and self?efficacy,and provide guarantee for improving health education in colleges. Methods International physical ac?tivity questionnaire( IPAQ) ,scales of physical behavior,scales of stage of change in physical exercise behavior,and scales of self?efficacy were used to test 504 students from some of colleges in Zhejiang province. Results Signifi?cantly more males (38.52%) than females (27.27%) were in the high activity tertile (P<0.01); the amount of PA per week in males and females were (4608±872.07)MET and(4361±674.03)MET(P<0.01); the scores of males and females were (3.32±0.44) and(3.04±0.36),(P<0.01). The number of males (35.5%) in maintenance period and period of action is greater than that of females (28.9%). Males had higher self?efficacy and more scores of behavior than females (P<0.05) . There were significant characteristics of PA in their self?efficacy,score of be?havior and stages of change( F=18.38, F=15.08, F=34.71, P<0.01),and a significant correlation between PA and self?efficacy,stages of change and physical exercise behavior ( r=0.336, r=0.36, r=0.46, P<0.01).Conclu?sion Self?efficacy may have important effect on the level of PA. Stages of change and scores of behavior may ex?plain difference of PA between males and females.

Objective To compare the differences in physical activity (PA), self-efficacy, stages of change and physical exercise behaviour in college students. Methods International physical activivty questionnaire( IPAQ), scales of self-efficacy, questionnaire of stage of change in physical exercise behaviour, and scales of physical behaviour were used to make a study on college students from Zhejiang province. Results The rate of males (40.43%) was signficant higher than that of females (26.09%) MET in the high activity tertile. There was significantly more energy expenditure in males (( 3462.79 ± 976.31 ) MET) than in females (( 3118.63 ±944.29 ) MET) (P < 0.05 ). The rate of males (44.68 % ) was more than females (25.00%) in maintainanee period and period of action. PA characteristics were shown significant defferences in different stages (F= 31. 380, F=6.012, P < 0.01 ). Characteristics in stages of change were shown high in behaviour score (F= 13.071, F= 7. 461,P <0. 01 ). Four factors as attitude,cognitive controls, behavior disposition and self-efficacy in males were entered into regression model of MET( t = 2.138 ~ 3. 789, P < 0.05 ), while in females only the factor of behaviour attitude was entered. Conclusion There is significant difference in PA, stages of change, behaviour characteristics, and self-efficacy between male and female college students.

Objective The college and university students' physical exercise behavior scale has been de-veloped. Methods Baaed on the theory of planning behavior,after investigation and analysis,a questionnaire was made to test 1180 students from some colleges and universities in Changjiang Delta area. Its reliability and validity was tested,and confirmatory factor analysis was carried out. Results The scale consists of 4 factors: behavioral intention,subjective criterion,cognitive behavior,and integrative attitude. Its retest reliability was 0.920, correla-tion coefficient of the factors between 0.732 and 0.854 ,and the total scale Cronbach α 0.931. The correlated co-efficients between subseales and total scales range 0.455~0.785 ;the IFI, CFI,and NFI fit indices of the four di-mensions were over 0.9 ;RMSEA 0.042. Conclusion The reliability and validity of the scale are suitable as an effective tool to test university students' sports exercise behavior.
