RESUMO

Objective:To investigate the causes of buccal masses after temporal autologous fat grafting, and to summarize the therapeutic methods to avoid medical disputes. Methods:Ten female patients (25-40 years; mean age, 33.1±7.5 years) with buccal mass complication after temporal autologous fat grafting were retrospectively analyzed, and statistics was done on their fat injection history, postoperative buccal mass symptom onset time, intraoperative mass conditions, postoperative pathological and fluid bacterial culture results, and postoperative recovery. Results:The unilateral temporal fat injection volume was 15-30 ml, and the buccal masses occurred 1-6 months after temporal autologous fat grafting. Lipoid masses were found in the buccal space during the operation, and some of them contained liquid. The postoperative pathological results indicated local necrosis in fibrotic adipose tissues. All the fluid bacteria cultures were negative, only one showed positive culture of acid-fast bacilli. 6-12 months postoperative follow-up visit was performed, and 9 patients recovered and 1 patient recovered after re-operation.Conclusions:The overfilled fat and liquefied necrotic fat after temporal fat grafting may move down to buccal space to form buccal masses, which should be treated by operation early to achieve a good prognosis.

RESUMO

Objective:To understand the psychology of patients undergoing breast augmentation, and to provide reference for preoperative communication between clinicians and patients. Methods:From September 2016 to September 2017, the Guangdong Second Provincial General Hospital formulated a 37-question questionnaire based on BREAST-Q and Chinese national conditions, and investigated patients who planned to undergo breast augmentation in 34 hospitals. Results:A total of 2066 valid questionnaires were collected. Fifty-four percent of women were dissatisfied with their nude appearance, which was more dissatisfied compared with their appearance when dressed. Surgical safety (20.1%), surgical effect (16.8%), surgical method and incision selection (16.5%), breast implant material (14.5%) and doctor′s skill (11.6%) were the five most important factors for considering breast augmentation. Only 26.6% of the respondents were supported by their family members or partners when they made the decision to have breast augmentation. Most women had limited knowledge of breast augmentation, with only 24.9 percent of respondents having a thorough understanding of the procedure. Conclusions:Women undergoing breast augmentation have an urgent need to improve their appearance, and their decision-making process is affected by many factors, among which surgical safety is the most worrying factor for patients. This group of people have limited knowledge about surgery, and the popularization of breast augmentation by plastic surgeons remains to be strengthened.

RESUMO

Objective@#To evaluate the accuracy of the simulation with a 3D stereophotogrammetric imaging system(Vectra) for the prosthetic breast augmentation.@*Methods@#From July 2014 to December 2015, 32 patients received Vectra scanning prior to breast augmentation. The simulations of post-operation images were created by the software. The distance of SN-N line, N-N line and the volume of the simulated breast was measured by the software. The patients came back for a follow-up 1-6 months post-operation. At that time, the same type of data was collected again, and the difference of simulated and real post-operation breast was calculated. 18 professional doctors scored the accuracy of the simulation using a 1-10 scale. We defined 6-10 as"of great accuracy". The scores were analyzed using the independent-sample t test.@*Results@#The simulated distance of SN-N line(L/R), N-N line(L/R) and the volume(L/R) was (19.3±1.4) cm/(19.3±1.2) cm、(19.0±1.5) cm and (369.0±64.5) ml/(370.0±82.7) ml. The real distance of SN-N line(L/R), N-N line(L/R) and the volume(L/R) was (19.0±1.3) cm /(19.0±1.2) cm、(18.4±1.3) cm and (365.6±81.0) ml/(360.2±84.2) ml. Those data of simulating and real post-operation breast was statistically insignificant (P>0.05). The score of the simulation accuracy by 18 professional doctors was 7.9±1.4, which means the doctors agreed the accuracy of Vectra for the breast augment simulation was great.@*Conclusions@#The Vectra can simulate the figure of breast augmentation with good accuracy.

RESUMO

Objective To explore the indication of implant prosthesis immediately after removing polyacrylamide hydrogel for augmentation mammaplasty. Methods MRI and pathologic examinations were made in 226 patients with the postoperative complications of augmentation mammaplasty by injecting polyacrylamide hydrogel and with the specific conditions in operation to analyze the possibility of implant prosthesis while removing polyacrylamide hydrogel. Based on MRI examination, physical examination and specific conditions in operation, we chosen 61 cases who requested for implant prosthesis while removing polyacrylamide hydrogel. Results The patients who did the operation for simply removing polyacrylamide hydrogel recovered well and symptoms such as pain, sclerosis, mass and others disappeared almostly; the patients who did implanting at the same time were satisfied with the breast shape and had no side-effect occured. Conclusion With MRI examination, physical examination and specific conditions in operation, it can achive good effects to do implanting immediately after removing polyacrylamide hydrogel for those suitable and willing to implant prosthesis.

RESUMO

Objective To introduce and evaluate an improved technique of vertical mammaplasty for correction of severe breast ptosis. Methods Vertical mammaplasty that was first reported by Ma-deleine Lejour in Brussels is a technique that uses adjustable markings, an upper pedicle for the areola, and a central breast reduction with lower skin undermining. To shorten vertical scar, it was important to locate new nipple position and move up new inframammary fold. The shape of the breast was crea-ted by suturing the gland and did not rely on the skin. No scar was produced in submammary fold. Re-sults 36 cases were treated with vertical mammaplasty in our department since August 1999. The shape of the new breast was satisfactory in all patients after 3-24 months following-up. Areora necro-sis was not found in all the cases. Conclusion The vertical mammaplasty is an optical technique for correction of severe breast ptosis, Stable results are produced because the gland is strongly sutured.

RESUMO

OBJECTIVETo investigate the correlation between the expression of key proto-oncogenes playing major roles in tumorigenic process and abnormal sarring.

METHODSImmunohistochemical technique was performed to detect the expressions of c-myc, c-fos and ras p21 proteins on hypertrophic scars, keloids and normal skin. Image analysis was used to compare their quantitative difference of expression.

RESULTSC-myc and c-fos expressions on the nucleus of fibroblasts of hypertrophic and keloid scars were significantly higher than normal skin controls, and there was no difference between the two lesions. Ras p21 expression was not detected on the fibroblasts of hypertrophic and keloid scars.

CONCLUSION1. c-myc and c-fos oncogenes are activated on hypertrophic and keloid scars, which may contribute to proliferation and differentiation of fibroblasts, synthesis and degradation of collagen and regulation of cytokines and induce abnormal scarring, the mechanisms of their effects remain to be further studied. 2. Ras gene may not mutate or its mutations may not play a major role in the process of abnormal scarring. 3. Only part of proto-oncogenes moderately expressed on abnormal scars. The expression of multiple oncogenes does not coexist in abnormal scars may be the cause of their less chances to induce malignant transformation.

RESUMO

Objective To explore the application and formulas of the tumescent technique in the ultrosonic liposuction for better and more predictable reseults. Methods The tumescent technique formulas were present and analysed through 318 cases of ultrosonic liposuction. Results The following regimen was recommended as a result of the above studies: Lactated Ringer's solution or nosmal sodium was the fluid of choice; Lidocaine (500 mg/l) was a reference anesthetic. The maximal safe dose recommended for liposuction was 35 mg/kg body weight and total dose of epinephrine was 2~3 mg/l. But its concentation should be lower than 1:1 000 000. Concentration of sodium bicarbonate was 500 mg/l, and higer doses were of no value but harmless. Conclusions Safer formulas are provided for the tumescent technique administration in the ultrosonic liposuction. The tumescent technique may reduce potential complications related to ultrosonic liposuction
