RESUMO

Objective: This study was aimed to compare and analyze the effects and safety of minimally invasive and craniotomy in the treatment of hypertensive intracerebral hemorrhage


Methods: A total of 130 patients with hypertensive intracerebral hemorrhage were recruited. The patients were randomly divided into two groups [research and control group]. Research group was treated with endoscopic minimally invasive surgery, while control group was treated with craniotomy and hematoma clearance. The basic situation, clinical effects, prognosis, nerve function and inflammatory factors of the two groups were compared while the condition of postoperative complications was also observed


Results: The operative time of patients in research group showed statistically significant [P<0.05] difference when compared with control group. Hematoma clearance rate and intraoperative blood loss of research group was significantly better than control group. There was no significant difference [P>0.05] between the two groups in preoperative hemorrhage and edema around the hematoma, however hemorrhage and edema around the hematoma after four weeks of surgery in the research group was significantly [P<0.05] lower than control group. After four weeks of treatment, the BI and SSS score, SP and IL-2 level of the research group were significantly higher than control group [P<0.05], while MRS score, IL-6, hs-CRP, TNF-? and SF was significantly lower than control group [P<0.05]


Conclusion: Compared with craniotomy, minimally invasive surgery is more effective in the treatment of hypertensive intracerebral hemorrhage, as well as it is more conducive to restore neurological function, improve prognosis and reduce serum inflammatory factor levels

RESUMO

Objective To discuss the value of pregnantal ultrasonography(US) on fetus' complicate congenital heart diseases(CHD). Methods The uhrasonographic fetures of fetus complicate CHD were analyzed and compared with the pathological diagnosis. Resells Twenty fetus' complicate CHD were diagnosed by pregnantal US. Four cases with single ventricle, two eases with transposition of great arteries,two eases with double outlet right ventricle,two cases with truncus arteriosus,three cases with hypoplastic left heart syndrome, three cases with endocardial cushion defect, two cases with hypoplastic right heart syndrome,one case with tetralogy of Fallot and one case with pulmonary atresia were included. The diagnosing according rate was 100%. Conclusions Pregnantal US can fully evaluate the fetus' complicate CHD.

RESUMO

[Objective]To investigate diagnosis of lumbar instability,segment of spinal canal decompression and principle of fixation and fusion.[Method]158 cases of degnenerative lumbar instability from Feb 2001 and Oct 2006 in our hospital were analyzed retrospectively,the patients were operated with posterior decompression,instrumentation with pedicle screw and interbody or inter-transverse fusion.Patients all were examined with lumbar radiographys of lateral flexion and extension and with lumbar CT befor operation,Patients began to walk after operation one week,6 months later,restore to normal activities.[Result]The preoperative spinal canal area was 56~114 mm2,averaged 74 mm2,the postoperative spinal canal area 204~296 mm2,averaged 221 mm2.The patients were followed up from 6 months to 5 years(averaged 2 years and 5 months).The score was evaluated according to the JOA score.The preoperative score was averaged 3.7,the postoperative score was averaged 13.2,the improving rate was averaged 85%.108 case excellent,46 good,3 fairand 1 bad,The excellent and good rate was 97.5% in this group.[Conclusion]The spinal stability can be effectively maintained and reconstructed by decompression of spinal canal and instrumentation with pedicle screw system and fusion,which is a good method for treating Degnenerative instability of lumbar spine.
