RESUMO

<p><b>OBJECTIVE</b>To evaluate the performance of thromboelastography (TEG) to monitor in vivo blood coagulation status and the efficacy of antiplatelet aggregation drugs in the patients with coronary heart disease (CHD) after oral anticoagulation.</p><p><b>METHODS</b>Seventy one CHD patients were enrolled in CHD group and 380 healthy persons with normal TEG were enrolled in the control group. After admission, all CHD patients were administrated with routine anti-platelet aggregation drugs at a clinically recommended dose. Then, TEG was applied to monitor the basic blood coagulation indexes, such as R value, K value, α angle, MA value, CI value and a series of related indexes on platelet inhibition.</p><p><b>RESULTS</b>Above 80% of the basic blood coagulation indexes in TEG were within normal reference range in the CHD group. the R value, MA value, α angle and CI value in the CHD group were not significanly different, from that in the control group, but the K value significantly increased (P<0.05). Compared with the control group, relatively higher ratio of male was included in the CHD patients at much older age (P<0.05), 83.1% of the CHD patients achieved significant anti-platelet aggregation effect (platelet inhibition rate>50%). Other antiplatelet aggregation indexes, MA, MAck and MA suggested a 9.86%, 4.23% and 12.68% risk of thrombogenesis, respectively. Among all the related antiplatelet aggregation indexes, MAck showed the strongest correlation with age (correlation coefficient, 0.111), and ADP% most highly correlated with body mass (correlation coefficient, 0.160).</p><p><b>CONCLUSION</b>TEG results can provide valuable coagulation information for clinicians, thus certainly guiding in the treatment for CHD patients receiving anti-platelet therapy. Moreover, the application of TEG can also provide accurate information for further individualized treatment of CHD patients, which would funther inprove the safety of anti-thrombotic therapy.</p>

<p><b>OBJECTIVE</b>To investigate the main risk factors for asthma in Chinese children, and to provide a reference for the prevention and treatment of asthma.</p><p><b>METHODS</b>The databases including CNKI, Wanfang Data, China Biology Medicine disc, VIP Database for Chinese Technical Periodicals, Web of Science, and PubMed were searched for studies on risk factors for asthma in Chinese children published up to September 2017. Stata 12.0 was used for the Meta analysis.</p><p><b>RESULTS</b>A total of 24 case-control studies were included, with 5 309 cases in the case group and 6 404 cases in the control group. The Meta analysis showed that a family history of asthma (OR=5.246, 95%CI: 3.435-8.011), a family history of allergy (OR=4.627, 95%CI: 2.450-8.738), atopic constitution (OR=4.659, 95%CI: 2.511-8.644), allergic rhinitis (OR=11.510, 95%CI: 6.769-19.574), a history of eczema/dermatitis (OR=4.919, 95%CI: 3.514-6.886), a history of allergies (OR=4.732, 95%CI: 2.802-7.989), a history of food allergies (OR=5.890, 95%CI: 3.412-10.166), a history of drug allergies (OR=4.664, 95%CI: 2.637-8.252), mold contamination at home (OR=2.483, 95%CI: 1.671-3.690), flowers at home (OR=1.748, 95%CI: 1.383-2.209), a history of house decoration (OR=2.823, 95%CI: 2.206-3.935), and cesarean section (OR=1.894, 95%CI: 1.166-3.077) were risk factors for asthma in children, while breastfeeding was a protective factor against asthma (OR=0.508, 95%CI: 0.396-0.653).</p><p><b>CONCLUSIONS</b>The development of asthma in Chinese children is associated with a variety of factors, among which a family history of asthma, a family history of allergy, atopic constitution, a history of allergies, allergic comorbidities, cesarean section, and bad family environment can increase the risk of asthma in children, while breastfeeding can reduce the risk.</p>

RESUMO

<p><b>BACKGROUND</b>Obstructive sleep apnea (OSA) syndrome has a negative impact on the health of millions of adolescents and youth. The aim of this study was to evaluate the associations of OSA syndrome with obesity and cardiometabolic risk factors among adolescents and youth at risk for metabolic syndrome (MS).</p><p><b>METHODS</b>A total of 558 subjects aged 14-28 years were recruited from the Beijing Child and Adolescent Metabolic Syndrome Study. Each underwent a 2-h oral glucose tolerance test (OGTT), echocardiography, and liver ultrasonography. Anthropometric measures, blood levels of glucose, lipids, and liver enzymes were assessed. Subjects with high or low risk for OSA were identified by Berlin Questionnaire (BQ).</p><p><b>RESULTS</b>Among the subjects in obesity, 33.7% of whom were likely to have OSA by BQ. Subjects with high risk for OSA had higher neck and waist circumference and fat mass percentage compared to those with low risk for OSA (P < 0.001). Moreover, significant differences in levels of lipids, glucose after OGTT, and liver enzymes, as well as echocardiographic parameters were found between the two groups with high or low risk for OSA (P < 0.05). The rates of nonalcoholic fatty liver disease (71.0% vs. 24.2%), MS (38.9% vs. 7.0%), and its components in high-risk group were significantly higher than in low-risk group.</p><p><b>CONCLUSIONS</b>The prevalence of OSA by BQ was high in obese adolescents and youth. A high risk for OSA indicates a high cardiometabolic risk. Mechanisms mediating the observed associations require further investigation.</p>

Objective To study the relationship between adipokines and metabolic syndrome (MS),and the predictive value of adipokines on diagnosis of MS.Methods According to the IDF consensus worldwide definition on MS in 2005,we divided the subjects into 4 groups:115 in MS0 (with none of MS component) ; 118 in MS1 (with one MS component) ; 77 in MS2 (with two MS components) and 104 in MS3 (with none of MS component).Serum levels of leptin,visfatin adiponectin and resistin were measured in these groups,using the enzymelinked immunosorbent assay (ELISA) method.Retinol-binding protein 4 (RBP-4) was assessed by radioimmtmoassay (RIA).Result ( 1 ) Serum adiponectin level decreased while the serum level of leptin and RBP-4 increased in women with the number of MS components gathered.However,the level of visfatin obviously decreased only in the MS3 phase.The level of resistin showed no changes with MS components gathered.(2) Detection rates of the MS components such as high blood pressure,hyperglycemia,dyslipidemia,insulin resistance and obesity were significantly higher in the Q4 group with high level of leptin when compared to the Q1 group with lower level (odd ratio:Q4/Q1 is 1.3,1.8,1.6,5.2,3.0respectively).(3) The higher level of serum RBP-4 was not only associated with greater risk for impaired glucose regulation (odd ratio:Q4/Q1=2.6),but also significantly with the risks for hyperglycemia ( odd ratio:Q4/Q 1 =1.6 ) dyslipidemia ( odd ratio:Q4/Q 1 =1.9 ),obesity ( odd ratio:Q4/Q 1 =1.5 ) and MS (odd ratio:Q4/Q 1 =2.7).In the Q4 group with higher levels of RBP-4,the positive rates of MS reached 50%.(4) The detection rates of MS components such as dyslipidemia,obesity,hyperglycemia,and insulin resistance were significantly higher in the Q1 group with the lowest level of adiponeptin when compared to the Q4 group with the highest level.Conclusion The levels of adipokines,serum adiponectin,leptin and RBP-4 showed significant associations with MS.Our findings suggested that these adipokines might serve as the key factors that participating in MS and could be used as markers for early prediction of MS as well as the new targets for therapy.

<p><b>OBJECTIVE</b>To probe the best therapy for the patient after operation of hypertensive cerebral hemorrhage.</p><p><b>METHODS</b>One hundred cases were randomly divided into a treatment group and a control group, 50 cases in each group. The control group were treated by routine western medicine (Mannitol, Nifedipine and Caftriaxone sodium and so On), and the treatment group by the medication of the control group plus acupuncture 9 days after operation. Acupuncture was given at Neiguan (PC 6), Shuigou (GV 26), Sanyinjiso (SP 6), Jiquan (HT 1) and Weizhong (BL 40) and so on, combined with scalp acupuncture, with "restoring consciousness and inducing consciousness" needling method, for 10 days. The nervous function defect was assessed by Chinese Stroke Scale and the criteria for assessment of therapeutic effects.</p><p><b>RESULTS</b>Acupuncture could improve significantly nervous function defect. The total effective rate was 86.000 in the treatment group and 14.0% in the control group with a significant difference between the two groups (P < 0.01).</p><p><b>CONCLUSION</b>The therapeutic effect of the treatment group is better than that of the control group. Acupuncture combined with western medicine has cooperation for treatment of hypertensive intracerebral hemorrhage with a better therapeutic effect.</p>

<p><b>OBJECTIVE</b>To evaluate the efficacy and adverse effects of transdermal fentanyl in management of patients with cancer pain.</p><p><b>METHODS</b>A total of 4492 patients (aged 3-90) with cancer pain were enrolled in this multicenter study. The mean age was 58.5 (3 approximately 90) years old. All patients received transdermal fentanyl. The patients were asked to record the attacks of pain, quality of life, and any side effects of the treatment.</p><p><b>RESULTS</b>Baseline mean pain intensity was 7.37. On days 1, 3, 6, 9, 15, and 30, the mean scores of pain were decreased to 4.04, 2.98, 2.52, 2.19, 1.85 and 1.61, respectively (P < 0.01). The effective rate was 96.8%. The mean doses of fentanyl were 32.37 microg/h (25-200 microg/h) on the initial day, 42.57 microg/h and 49.57 microg/h (25-225 microg/h) on days 15 and 30. The quality of life was significantly improved after treatment (P < 0.01). The common side effects were constipation (9.8%), nausea (13.6%), dizziness (6.5%), vomiting (3.9%), sedation (2.0%) and respiratory depression (0.2%). The incidence of constipation was related to age, and the incidence of vomiting and difficulty of urination was related to gender. The majority (84.5%) of patients preferred continuation of the treatment with transdermal fentanyl.</p><p><b>CONCLUSION</b>Transdermal fentanyl for the patients with cancer pain is effective, safe, convenient and can improve the quality of life. Transdermal fentanyl can be recommended as one of first-line drugs for the treatment of patients with moderate to severe cancer pain.</p>

<p><b>BACKGROUND</b>To study the correlativity between HBV-DNA and the markers of hepatitis B virus infection and different clinical types of hepatitis B.</p><p><b>METHODS</b>The fluorescence quantitation (FQ) of HBV-DNA of 105 patients with hepatitis B was performed by PCR, and the correlativity between the fluorescence quantitation of HBV-DNA and the markers of hepatitis B virus and different clinical types of hepatitis B was analyzed.</p><p><b>RESULTS</b>Ninety-seven percent of the patients were found HBsAg(+), HBeAg(+), HBcAb(+); 75% were HBsAg(+), HBeAb(+), HBcAb(+); 60% were HBsAg(+), HBcAb(+); 40% were HBsAg(+); in HBsAb(+), HBeAb(+), HBcAb(+) (or both HBsAb and HBcAb were positive) group the HBV DNA was undetectable. The analysis indicated that there was a significant difference among different groups (P less than 0.05).HBV-DNA was detected in 72.2% in acute hepatitis B group, in 75% of chronic hepatitis B group, and in 70% of cases of liver cirrhosis with hepatitis B group. The analysis indicated that there was no significant difference among the different clinical types of hepatitis (P greater than 0.05).</p><p><b>CONCLUSION</b>The levels of viral replication were not correlated with different clinical types of hepatitis B; the concentration of HBV-DNA in serum was related to hepatitis B antigen.</p>

<p><b>OBJECTIVE</b>To evaluate the efficacy and safety of domestic Gemcitabine in the treatment of patients with stage IIIB approximately IV non-small cell lung cancer (NSCLC).</p><p><b>METHODS</b>124 NSCLC patients were randomized into three groups: Group A: single drug group, 40 cases, gemcitabine 1 000 mg/m(2) + NS 100 ml or 200 ml was infused within 30 approximately 60 minutes on D1, 8 and 15, with 28 days taken as one cycle. Group B: combined treatment group, 36 cases, in addition to the above protocol, cisplatin 30 mg/m(2) was infused within 60 approximately 120 min, on D1, 2 and 3. Group C: combined control group: 39 cases, the protocol applied was the same as group B except domestic gemcitabine being replaced by imported gemzar. The efficacy and side effects of treatment were evaluated after 8 weeks of treatment.</p><p><b>RESULTS</b>115 patients could be evaluated for response rate. PR was observed in 9/40 (22.5%) of group A, 15/36 (41.6%) in group B and 15/39 (38.36%) in group C. There was no significant difference of PR rates between group B and group C (P = 0.552). 117 patients who received the second cycle of treatment were evaluated for toxicity. The incidence of grade III approximately IV nausea, vomiting and loss of appetite was much higher in group B. Hematological toxicity of groups B and C was higher than that of group A. There was no significant difference between groups B and C.</p><p><b>CONCLUSION</b>The efficacy and incidence of side effects between domestic gemcitabine and the imported gemzar are similar.</p>

RESUMO

Purpose:To study the efficiency and the safety of recombinant human erythropoietin (rHuEPO) in the treatment of chemotherapy-induced anemia.Methods:41 tumor patients were randomized into two groups:EPO-treatment group and control group,in order to observe the changes of the haemoglobin level,transfusion requirement and quality of life for the two groups respectively.Results:①the increase of haemoglobin level and the quality of life in the EPO-treat- ment group compared with that in the control group was statistically significant.②the reduction of transfusion requirement in the EPO-treatment group compared with that in the control group was not statistically significant.③treatment of EPO has few side-effects and a good tolerance.Conclusions:rHuEPO in the treatment of chemotherapy-induced anemia is effec- tive and safe.Not only can it increase the haemoglobin level but also improve the quality of life for the patients,so it should be used widely.
