RESUMO

Objective To explore the therapeutic effect and safety of endoscopic resection and laparoscopic surgical treatment for colorectal submucosal tumors under the guidance of ultrasound endoscopy.Methods The clinical data of 90 patients with colorectal submucosal tumors in Tengzhou Central People′s Hospital form January 2013 to January 2018 were retrospectively analyzed.Among them,60 patients underwent endoscopic resection ( hereinafter referred to as endoscopic resection group) and 30 patients underwent laparoscopic resection ( hereinafter referred to as laparoscopic resection group).The complete resection rate, average diameter of resected lesions, average operation time, average hospital stay, average intraoperative bleeding volume, incidence of complications and recurrence rate of colorectal submucosal tumors were compared between the two groups.Results The complete resection rate of endoscopic resection groups was 96.7%,The complete resection rate of laparoscopic surgical treatment groups was 96.6%,there was no statistical difference between the two groups( χ2=0.16,P=0.759).The average diameter of the lesion was larger in laparoscopic surgical treatment group (( 29.7 ± 5.3) mm) than that in endoscopic resection group((14.9±4.3) mm,P=0.013).The operation time was shorter in endoscopic resection group ((32.1±6.1) min) than that in laparoscopic surgical treatment group((75.3±9.8) min,P=0.007).There was statistically statistical difference in the intraoperative blood loss between endoscopic resection group ((13.2± 5.5) ml ) and laparoscopic surgical treatment group (( 28.3 ± 7.6) ml, P = 0.009).The postoperative hospital stays was less in endoscopic resection group((3.1±1.4) d) than that in laparoscopic surgical treatment group((6.9±2.2) d,P=0.007).The complicationgs rate in endoscopic resection group (3.33%) was significantly lower than that in laparoscopic surgical treatment group(13.3%),(χ2=4.89,P=0.021).The average hospitalization expenses of endoscopic resection group was significantly lower than that of the laparoscopic surgical treatment group(( 18 745± 6 281) vs(42 961± 10 423) yuan,P=0.006) .There were no relapse and distant metastasis in two groups,followed up for 6 to 12 months.Conclusion Endoscopic resection and laparoscopic resection are both safe and effective methods for the treatment of rectal submucosal tumors.Endoscopic resection has less trauma,quicker recovery,shorter hospitalization time and lower hospitalization costs.

Objective To investigated the change of endogenous heme oxygenase-1 (HO-1)/carbon monoxide (CO) system and resolvin E1( RvE1 ) in patients with crohn disease (CD), and its effects on the pathogenesis of ulcerative colitis. Methods The distribution and expression of HO-1 proteins in the colonic mucous in 40 cases with CD and 15 cases of normal control group were detected by SABC immunohistochemistry. The average optical density value of HO-1 was analyzed with an Image Analyzing systems. The concentration of CO in the colonic mucosa was detected with chemical methods. The levels of RvE1 in serum was detected by enzyme linked immunosorbent assay (ELISA). Results The expression of HO-1 proteins in patients with active CD were significantly higherin the colonic mucous membrane epithelia. The average optical density value of HO-1 in patients with active CD was higher than that in normal group and remission of CD( 0.358 ± 0.021 vs. 0.163 ± 0.018 and 0.177 ± 0.032)(P<0.01). The concentrations of CO in active CD was significantly higher than that in the normal control group and remission of CD [(23.79 ± 1.81) μmol/g vs. (18.68 ± 1.64) and (17.13 ± 1.23) μmol/g] (P <0.01). The levels of RvE1 in patients with active CD were significantly higher than those in normal group and remission of CD [(41.35 ± 2.76) ng/ml vs. (15.34 ± 2.19) and (14.51 ± 1.47) ng/ml] (P < 0.01). Conclusions The abnormal expression of HO-1/CO system and RvE in activity of the CD may play an important role in the pathogenesis of CD.

Objective@#To compare the therapeutic effect and safety of endoscopic multi-band mucosetomy (EMBM) and endoscopic mucosal resection (ESMR) in the treatment of colorectal submucosal tumors.@*Methods@#From January 2012 to June 2014, the clinical data of 40 patients with colorectal submucosal tumors in Tengzhou Central People′s Hospital were divided into EMBM group (22 cases) and ESMR group(18 cases) according to different treatment method.The complete resection rate, operation time and complications were compared between the two groups.@*Results@#The complete resection rate of the EMBM group was 95.45%, which of the ESMR group was 94.44%, the difference between the two groups was not statistically significant (χ2=0.273, P>0.05). The operation time was shorter in the EMBM group[(31.3.5±4.0)min]than that in the ESMR group[(47.2±4.5)min), t=-15.194, P<0.05]. The incidence rate of hemorrhage in the EMBM group was 9.0%, which was significantly lower than 33.3% in the ESMR group (χ2=3.952, P<0.05). There was no statistically significant difference between the two groups in the incidence of postoperative colorectal stricture (P>0.05). All the patients were followed up for 3~24 months after operation, and no local recurrence occurred.@*Conclusion@#EMBM is safe and effective for colorectal submucosal tumors and the complications are less.

Objective To explore the therapeutic effect and safety of endoscopic multi-band mucosetomy (EMBM) and endoscopic submucosal dissection(ESD) for colorectal submucosal tumors under the guidance of ultrasound endoscopy. Methods From January 2013 to June 2016,the clinical data of sixty patients with colorectal submucosal tumors in Center People's Hospital of Tengzhou who received either EMBM (32 cases) or ESD ( 28 cases) were included in this retrospective study. The complete resection rate, average diameter of resection lesion, operative time, complications and recurrence rate of two groups were compared. Results The complete resection rate of EMBM groups was 96. 9%(31/ 32),the complete resection rate of ESD groups was 96. 4%(27/ 28),there was no statistically significant difference between the two groups (χ2 = 0. 07,P= 0. 812). The average diameter of was ESD group (12. 3±2. 6) mm,significantly greater than that of EMBM group ((9. 1± 2. 2) mm) (P = 0. 038),and the mean operation time of EMBM group was (20. 5 ±5. 1) min,which was significantly less than that of ESD group ( (36. 8±4. 5) min) (P = 0. 008),and the percentage of perforation in EMBM group was 0,significantly lower than that of ESD group (10. 7%(3/ 28))(χ2 = 6. 37, P = 0. 013). The severe bleeding rate of EMBM group was 3. 1% (1/ 32),which was significantly lower than that in ESD group(14. 3%(4/ 28)) (χ2 = 4. 89, P = 0. 021) . After 6-12 months of follow-up,no recurrence or distant metastasis was found in EMBM or ESD group. Conclusion EMBM and ESD are safe and effective for colorectal submucosal tumors,However the average diameter of the lesion was larger in EMBM group than that in ESD group.

Objective To evaluate the biological effect of tumor necrosis factor-α(TNF-α) on the proliferation and multi-directional differentiation of stem cells from rat apical papilla(SCAP).Methods SCAP was extracted by combining enzyme digestion method with tissue block method.The cells were divided into control group(TNF-α 0 ng/mL) and experimental group(TNF-α 5,10,20,50 ng/mL).The ability of proliferation of SCAP was measured by MTT method.The ability of osteogenic/dentinogenic differentiation of SCAP was measured by alizarin red staining and quantitative real-time PCR.The ability of adipogenic of SCAP was measured by oil red O staining.The expression of vascular related genes of SCAP was measured by quantitative real-time PCR.Results SCAP was consistent with the characteristics of mesenchymal stem cells and possessed the ability of multi-directional differentiation.The MTT results showed that experimental group promoted the proliferation of SCAP in comparison with the control group.The difference was statistically significant(P<0.05),and 10 ng/mL was the optimum concentration.The results of alizarin red staining showed that with the increase of the concentration of TNF-α,the mineralized nodules in the experimental group gradually became smaller,and the number of the formation decreased gradually.The results of quantitative real-time PCR showed that the expression of OC,DMP-1 and DSPP in the experimental group was significantly lower than that of the control group at 3 and 7 days,in which the expression of OC was statistically significant different(P<0.05);at 14 days,the expression of OC,DMP-1 in the experimental group was significantly lower than that of the control group(P<0.05).The result of Oil red O staining showed that with the increase of the concentration of TNF-α,the lipid droplets formation in the experimental group gradually decreased.The result of quantitative real-time PCR showed that the expression of ANGPT1,VEGFA,PECAM-1 in the experimental group was significantly lower than that of the control group(P<0.05).Conclusion TNF-α might promote the proliferation and inhibit the multi-directional differentiation of SCAP.

Objective To investigate the expressions of endogenous cystathionine-γ-1yase (CSE)/ hydrogen sulfide (H2S) and resolvin E1 (RvE1) in patients with ulcerative colitis (UC),and its effect on the pathogenesis of ulcerative colitis.Methods The distribution and expression of CSE proteins in the rectum mucosa in 60 cases of UC and 30 cases of normal control group were detected by Strept Avidin-Biotin Complex (SABC) immunohistochemistry.The average optical density value of CSE was analyzed with an Image Analyzing systems.The expression of CSE mRNA in the rectum mucosa was detected by real-time polymerase chain reaction (RT-PCR).The levels of H2S and RvE1 in sera were detected by spectrophotometry.Results The expressions of CSE proteins in three groups were detected in the rectum mucosa membrane epithelia.The average optical density value of CSE and the expression of CSE mRNA in patients with active UC were higher than that in normal group and remission of UC.The levels of H2S and RyE1 in patients with active UC were significantly higher than that in normal group and remission of UC.Conclusions The abnormal expressions of CSE/H2S and RyE1 in activity of the UC might play an important role in the pathogenesis of UC.

RESUMO

Objective To investigate the prognostic value of prognostic nutritional index (PNI) in patients with non-small cell lung cancer.Methods A total of 179 patients with non small cell lung cancer was enrolled in our hospital from November 2010 to January 2014.All patients were pathologically confirmed to be non small cell lung cancer.The clinical data of patients were collected,and the PNI values of each patient were calculated.The patients were divided into 4 groups according to the patients in the PNI of the patients in the treatment group (PNIQ0-25 group,PNIQ25-50 group,PNIQ50-75 group and PNIQ75-100 group).The survival curve was drawn by Kaplan-Meier method,and the difference of progression free survival (DFS) and overall survival (OS) of each group was compared by Log-rank method.Results (1) compared with PNIQ0-25 group,PNIQ25-50 group,PNIQ50-75 group and PNIQ75-100 group,there was signifi cant difference in age,smoking and KPS score (P < 0.05).(2) the PNIQ0-25 group had a median overall survival of 11.5 months (95% CI:6.6 ~ 15.4),the 3 year survival rate was 6.7%;PNIQ25-50 group had a median overall survival of 12.2 months (95% CI:9.1 ~ 18),the 3 year survival rate was 6.4% in PNIQ50-75 group;the median overall survival was 14.1 the month of (95% CI:8.7 ~ 13.3),the 3 year survival rate was 11.4%,the PNIQ75-100 group had a median overall survival of 15 months (95% CI:12.3 ~ 17.8),the 3 year survival rate was 11.6%.The Log-rank test,the four groups of patients with a significant difference in overall survival (x2 =15.6,P =0.001).(3) the PNIQ0-25 group had a median progres sion free survival was 5 months for (95% CI:4.3 ~5.6),the 3 year progression free survival rate was 4.4%;PNIQ25-50 group had a median progression free survival was 6.4 months for (95% CI:4.7 ~8.1),the 3 year progression free survival rate was 4.3% in the PNIQ50-75 group;the median progression free survival was 7.4 months (95% CI:6 ~ 8.7),the 3 year progression free survival rate was 9.1% in PNIQ75-100 group,the median progression free survival was 8.9 months for (95% CI:6.4 ~ 10.8),the 3 year progression free survival rate was 9.3% by Log-rank test,survival was statistically significant was no difference between the four groups (x2 =26.7,P =0.000).Conclusions PNI has a good application value in the prognosis of patients with non-small cell lung cancer.

Objective To study the expression of endogenous cystathionine-β-synthase (CBS) and hydrogen sulfide in patients with ulcerative colitis (UC), and explore their possible role in the pathogenesis of UC. Methods Thirty patients with active period UC (active period UC group), 30 patients with remission period UC (remission period UC group) and 30 healthy controls (control group) were selected, and SABC method was used to observe the localization of CBS in rectal mucosal tissues. The optical density value of CBS was analyzed with image analysis systems. The relative expression of CBS mRNA in the rectal mucosa tissue was detected by reverse transcriptase-polymerase chain reaction (RT-PCR), and the expression of glyceraldehyde-3-phosphate dehydrogenase (GAPDH) was detected as an internal reference. The expression level of mRNA was detected by semi quantitative analysis with gel imaging analysis system, and the results were expressed as the ratio of the target bands to the GAPDH absorbance. The serum level of hydrogen sulfide was detected. Results The serum level of hydrogen sulfide, optical density value of CBS and relative expression of CBS mRNA of rectal mucosal tissues in active period UC group were significantly higher than those in remission period UC group and control group: (90.13 ± 3.12) μmol/L vs. (50.34 ± 2.34) and (48.13 ± 2.75) μmol/L, 0.433 ± 0.037 vs. 0.295 ± 0.064 and 0.214 ± 0.026, 1.532 ± 0.134 vs. 1.031 ± 0.107 and 0.986 ± 0.067, and there were statistical differences (P 0.05). Conclusions The abnomal expression of CBS and hydrogen sulfide in aactive period UC may play an important role in the pathogenesis of UC.

RESUMO

Salvia miltiorrhiza is a traditional Chinese medicine for the treatment of cardiovascular diseases .Recently, increasing evidence demonstrates that the water-soluble compounds isolated from Salvia miltiorrhiza,including tanshinol and salvianolic acid B, exert a regulatory influence on bone metabolism .The under-lying mechanism of these compounds involves various pathways , such as Wnt/β-catenin, ERK, BMP, OPG/RANKL/RANK and FoxO mediated oxidative stress pathway .This paper reviews pre-vious effects and mechanism of polyphenolic acids in Salvia milt-iorrhiza , which may provide the base for the research and devel-opment of the new agents to treat osteoporosis .

Abstract BACKGROUND: Alveolar bone deficiency wil not meet aesthetic and functional requirements for dental implants. OBJECTIVE:To observe the repair effect of passage 3 autologous bone marrow mesenchymal stem cels (BMSCs) and platelet-rich fibrin (PRF) on alveolar bone defects in rabbits. METHODS:Twenty-seven New Zealand rabbits were randomly divided into BMSCs/PRF group, PRF group and model group (n=9 per group). The left mandible incisors were extracted in al the rabbits under general anesthesia. BMSCs/PRF group was immediately implanted BMSCs/PRF composite into the alveolar socket, PRF group only implanted PRF, and model group implanted nothing. RESULTS AND CONCLUSION: In the model group, the alveolar crest and alveolar mucosa become sunken notably and narrowed. In the BMSCs/PRF and PRF groups, the thickness of alveolar bone wal, alveolar bone width, alveolar bone height difference, and bone mineral density were al increased, especialy in the former group. In addition, the trabecular arrangement was better in the BMSCs/PRF groups than the model and PRF group. Our findings indicate that alveolar socket filing with composite of BMSCs and PRF can achieve preservation of alveolar bone width and height after tooth extraction in rabbits.

BACKGROUND:The periodontal ligament stem cels can promote periodontal tissue regeneration, providing a new way for the treatment of periodontitis. OBJECTIVE:To observe the inflammatory microenvironment effects on the biological properties of periodontal ligament stem cels. METHODS: Periodontal ligament stem cels from healthy controls and patients with periodontitis were primarily cultured by tissue digestion method, purified using limited dilution method, and identified through detection of CD146 and STRO-1. Then, passage 3 cels were taken and denoted as normal control and inflammation groups folowed by osteogenic induction. RESULTS AND CONCLUSION:Purified cels from two sources both expressed STRO-1 and CD146. Periodontal ligament stem cels in the inflammation group showed higher multiplication capacity, but the osteogenesis ability was lower compared with the normal control group. The expressions of Runx2 mRNA and Osterix mRNA were dropped significantly after the stimulus of tumor necrosis factor-α (P < 0.05), but the interleukin-1β and interleukin-6 did not have a significant impact. Tumor necrosis factor-α at 0.1 and 1 μg/L had no significant effects on the expression of Runx2 mRNA, but the expression of Runx2 mRNA was decreased significantly after treatment with 10 μg/L tumor necrosis factor-α (P< 0.05). It is confirmed that the molecular signaling mechanism inside the periodontal ligament stem cels is changed under inflammatory microenvironment, so that the differentiation capacity of cels from the inflammatory sources is lowered. Moreover, tumor necrosis factor-α is one of the key factors and its optimalconcentration is 10 μg/L.

Objective To understand the status of depression and quality of life in the patients with advanced gastric cancer , and to explore their relationship .Methods The depression self rating scale (SDS) and the Chinese Scale of World Health Organiza‐tion Quality of Life Scale‐Brief Form Questionnaire (WHOQOL‐BREF) were adopted to conduct the questionnaire investigation on 131 patients with advanced gastric cancer ,then the results were compared with those of the health population .Results Among 131 patients with advanced gastric cancer ,the prevalence rate of depression was 47 .33% ,and the average SDS score was 45 .99 ± 15 .47 ,which were higher than those of the health population .The scores of quality of life were 61 .61 ± 12 .66 for the physiologic health domain ,59 .54 ± 12 .49 for the mental health domain ,64 .95 ± 14 .16 for the social relation domain and 52 .93 ± 14 .07 for the surrounding environment domain .There was a significant correlation between the depression score with the four domains scores of life quality ,in which the correlation with the mental health domain was strongest (r= -0 .636 ,r2 =0 .405) .Conclusion High depression level and poor quality of life exist in the patients with advanced gastric cancer ,and there is a correlation between them , in which the correlation between depression with the mental health domain is strongest .

<p><b>OBJECTIVE</b>We explored the expressions of the Notch and Wnt signaling pathways and their significance in the repair process of alveolar bone defects by establishing animal models with a composite of autologous bone marrow mesenchymal stem cells (BMSCs) and platelet-rich fibrin (PRF) to repair bone defects in the extraction sockets of rabbits.</p><p><b>METHODS</b>A total of 36 two-month-old male New Zealand white rabbits were randomly divided into four groups, and the left mandibular incisors of all the rabbits were subjected to minimally invasive removalunder general anesthesia. BMSC-PRF compounds, single PRF, and single BMSC were implanted in Groups A, B, and C. No material was implanted in Group D (blank control). The animals were sacrificed at 4, 8 and 12 weeks after surgery, the bone defect was immediately drawn, and the bone specimens underwent surgery after four, eight, and twelve weeks, with three rabbits per time point. The expressions of Notch1 and Wnt3a in the repair process of the bone defect were measured via immunohistochemical and immunofluorescence detection.</p><p><b>RESULTS</b>Immunohistochemistry showed that the expressions of Notch1 and Wnt3a in Groups A, B, and C were higher than that in Group D at the fourth and eighth week after operation (P<0.05). By contrast, the expressions of Notch1 and Wnt3a in Group D were higher than those in Groups A, B, and C at the twelfth week (P<0.05). Immunofluorescence showed that the expressions of both Notch1 and Wnt3a reached their peaks in the new bone cells of the bone defect after four weeks following surgery and gradually disappeared when the bone was repaired completely.</p><p><b>CONCLUSION</b>Notch1 and Wnt3a signaling molecules are expressed in the process of repairing bone defects using BMSC-PRF composites and can accelerate the healing by regulating the proliferation and differentiation of BMSCs. Moreover, the expressions of Notch and Wnt are similar, and a crosstalk between them may exist it.</p>

@#Objective To investigate the factors related with swallowing function after stroke. Methods Stroke patients with dysphagia were reviewed, 296 cases with pseudobulbar paralysis, and 82 cases with bulbar paralysis. Swallowing function was evaluated with videoflu-oroscopic swallowing study (VFSS). The variable of age, sex, Body Mass Index (BMI), scores of National Institutes of Health Stroke Scale (NIHSS), Simplified Fug-Meyer Assessment (FMA) and modified Barthel Index (MBI), intubation, aspiration, length of hospitalization (LOH), and initiation of rehabilitation intervention were analyzed with Logistic regression. Results Age, NIHSS score, intubation, aspira-tion, initiation of rehabilitation intervention were the risk factors for poor outcome of swallowing function in all the patients, while the score of MBI was the protective factor for the pseudobulbar paralysis ones. Conclusion The younger the patients, the less the NIHSS score, no in-tubation, no aspiration, and the earlier the rehabilitation intervened, the more independent the pseudobulbar paralysis patients, the better swallowing function recovered.

Objective In this study,the change of HO-1,RvD1,LXA4 and IL-6 in the patient with ulcerative colitis (UC)was investigated.Effects on the pathogenesis of ulcerative colitis were dis-cussed.Methods The distribution of HO-1 proteins in the colonic tissues in 60 cases of UC and 30 cases of normal control group were detected by SABC immunohistochemistry and perfusion catheter manometer. The slides were then analyzed with an Image Analyzing system to obtain the density of stained proteins.The levels of RvD1,LXA4 and IL-6 in serum by using enzyme linked immunosorbent assay(ELISA).Results The expression of HO-1 in patients with active UC was higher than that in normal group and remission of UC.The levels of RvD1,LXA4 and IL-6 in patients with active UC were significantly higher than that in normal group and remission of UC.The levels of HO-1,RvD1,LXA4 in patients with active UC were posi-tively correlated with IL-6.The levels of HO-1,RvD1,LXA4 in patients with normal group and remission of UC had no relevance with IL-6.Conclusions The expression of HO-1,RvD and LXA4 in activity of the UC may plays an important role in the pathogenesis of gastrointestinal motility disorders such as UC.

@#Objective To explore the effect of quality control circle (QCC) on health education in stroke rehabilitation nursing. Methods Such activities were conducted in the VIP ward of neurological rehabilitation in our hospital as: theme selecting, activity planning, status mastering, cause analyzing, countermeasures formulating and implementing, etc., in accordance with QCC theory from January to April, 2014. Results The satisfaction of stroke patients to health education rose from 80.1% to 91.1% (P<0.001), and the dissatisfaction to guidance of activities of daily living and prevention of complications decreased significantly (P<0.05) after the implementation of QCC program. All the circle members benefited from the improvement of QCC technique, team work, professional knowledge, communication and coordination, activity confidence, duty and honor. Conclusion Carrying out the QCC activities can play a positive role on health education in stroke rehabilitation nursing.

RESUMO

Objective To investigate the effect of intensive education among the families on nutritional status of the stroke patients with dysphagia. Methods 42 stroke patients with dysphagia diagnosed with Standard Swallowing Assessment were enrolled in the study. The pa-tients and their families were randomly divided into intervention group (21 pairs) and control group (21 pairs). The families of the interven-tion group were given the intensive health education, 5~10 minutes each time, once a week for 1 month. The families of the control group were only given routine health education. The two groups of patients received conventional drug and swallowing function training. Hemo-globin (Hb), serum albumin (ALB), contralateral brachial triceps skin fold thickness (TSF) and arm muscle circumference (AMC) were mea-sured 1 month latter. Results Hb, ALB, TSF and AMC in the intervention group were higher than in the control group (P<0.05). Conclusion Intensive education among the families may improve the nutritional status of stroke patients with dysphagia.

BACKGROUND:Bone marrow mesenchymal stem cel s are the ideal cel s for tissue repair. Whether the ability of in vitro proliferation can be enhanced is a key factor to promote tissue repair. OBJECTIVE:To observe the effect of transforming growth factorβ1 on the proliferation of bone marrow mesenchymal stem cel s in vitro. METHODS:Blood samples were taken from the central artery of rabbits to prepare platelet-rich fibrin by centrifugation method which was then placed into fresh DMEM at 37℃for 7, 14, 21, 28 days to col ect exudates. The mass concentrations of transforming growth factor-β1 in the exudates of platelet-rich fibrin were detected. Rabbit bone marrow mesenchymal stem cel s were col ected and cultured in the conditioned medium made by the exudates of platelet-rich fibrin, and the proliferation of cel s was observed. RESULTS AND CONCLUSION:Concentration of transforming growth factorβ1 was increased with time increasing, increased fastest at 21-28 days, and peaked at 28 days. Under the same stimulus concentration, the proliferation of bone marrow mesenchymal stem cel s was reduced at 0-1 day, increased obviously at 1-2 days, and entered into a steady phase at 2-3 days. Under 150 ng/L transforming growth factorβ1, bone marrow mesenchymal stem cel s proliferated fastest. Experimental findings indicate that with the increase of time, the concentration of transforming growth factorβ1 in the exudates of platelet-rich fibrin increase gradual y, and the conditioned media containing different concentrations of transforming growth factorβ1 play different roles in promoting the proliferation of bone marrow mesenchymal stem cel s. Bone marrow mesenchymal stem cel s cultured in the conditioned medium containing 150 ng/L transforming growth factorβ1 for 2-3 days can proliferate fastest.

RESUMO

Objective To investigate the clinical value of multiband mucosectomy (MBM) for the treatment of early esophageal cancer and precancerous lesions.Methods The clinical data of 43 patients with early esophageal cancer or precancerous lesions (including 13 cases of early esophageal cancer and 30 cases of precancerous lesion) who were admitted to the Tengzhou Central People's Hospital from January 2010 to January 2011 were retrospectively analyzed.A total of 51 lesions were confirmed by preoperative endoscopy and mucosal biopsy,and then were treated by MBM.Results A total of 52 MBM procedures were carried out,and 180 lesions were resected.The sizes of the lesions ranged from 10 mm × 8 mm to 25 mm × 23 mm,and the mean operation time was (37 ± 5)minutes.The complete resection rate was 92.2% (47/51).The submucosal layer of 1 patient was invaded by squamous cell carcinoma postoperatively,and the patient was treated by operation;1 patient wth squamous epithelium carcinoma was treated by radiotherapy at 2 weeks after MBM ; the other 2 lesions were treated by argon plasma coagulation (APC).Four lesions with bleeding were detected during 52 MBM procedures,and no esophageal perforation was detected during MBM.Early complications consisted of delayed bleeding (1 patient,cured by APC) and slight esophageal stenosis (1 patient,recovered at postoperative month 6).The accurate rate of pathological examinations was 51.0% (26/51).Forty-three patients were followed up for 1 year,3 patients with tumor recurrence were treated by MBM or APC.No death occurred during the followup.Conclusion MBM is a safe and effective technique for the treatment of early esophageal cancer and precancerous lesions.
