RESUMO

Objective@#To investigate the effect of long-term deep slow-wave sleep deprivation on the gonad axis, sperm abnormality rate, and structure of the testis in male rats and possible mechanisms.@*Methods@#A total of 30 specific pathogen-free male Wistar rats aged 5 weeks were randomly divided into slow-wave sleep deprivation group 1 (SD1 group) , slow-wave sleep and sleep time deprivation group 2 (SD2 group) , and control group, with 10 rats in each group. The flower pot method was used to establish a model of sleep deprivation. In addition to 12-hour sleep deprivation at night, the rats in the SD1 group were given interference once every 24 minutes, and those in the SD2 group were deprived of sleep for 8 minutes every 24 minutes; the rats in the control group were given 12-hour light illumination and then placed in dark environment for 12 hours. All rats were sacrificed by exsanguination from the femoral artery, and the testis, the epididymis, and blood were collected for analysis. Sperm abnormality rate and sperm motility rate were measured, and cauda epididymal sperm counting was performed. ELISA was used to measure the serum levels of testosterone (T) , follicle-stimulating hormone (FSH) , and luteinizing hormone (LH) .@*Results@#Compared with the control group, the SD2 group had a significant increase in organ coefficient of the epididymis (P<0.05) and a significant reduction in sperm motility rate (P<0.05) . There were significant differences between the SD1 group and the SD2 group in the increase in sperm abnormality rate (P<0.05) and the reduction in cauda epididymal sperm count (P<0.05) . The levels of FSH and T tended to increase, and the level of LH tended to decrease. Pathological examination showed degeneration and vacuolization of a small amount of spermatogenic cells in the SD1 group; in the SD2 group, there were significant degeneration, edema, and vacuolization of most spermatogenic cells, some spermatogenic cells were observed in the lumen, and there were no sperms in the lumen.@*Conclusion@#Long-term deep slow-wave sleep deprivation impairs the structure of the testis, affects sperm motility rate and sex hormones, and increases the risk of sperm abnormality.

RESUMO

Objective To explore the effectiveness of the individualized surgical fixation for the treatment of mandibular fractures in children.Methods The individualized mandibular fixation methods were used to treat mandibular fractures in 85 children in Department of Plastic and Reconstructive Surgery in Xijing Hospital from April 2010 to July 2015.Depending on the fracture types and 3D pre simulation with stress and tension band fixation,six methods of treatment were used,including titanium plate,titanium plate-dental arch bars,biodegradable plate-dentalarch bars,biodegradable plate-occlusal splint,occlusal splint,and anchorage devices.Postoperative occlusal relationship,mouth opening and masticatory function were evaluated.Results Fixation of titanium plate was used in 10 cases,titanium plate-dental arch bars in 25 cases,biodegradable plate-dentalarch bars in 20 cases,biodegradable plate-occlusal splint 15 cases,individualized occlusal splint in 3 cases and anchorage devices in 12 cases.The treatment outcomes were good in the combined methods of open reduction with personalized fixation for children with mandibular fractures,and the reduction was satisfactory.Occlusal relationship had a good recovery after operation.Eating and functional training started earlier in these pediatric patients after operation.Conclusions For pediatric mandibular fractures,open reduction and fixation with appropriate methods can lead to favorable anatomic morphology and occlusion relationship.The application of 3D imaging technology in analyzing the fracture and showing tooth germ in the jaw helps the improvement of surgical efficiency and avoidance of unwanted tooth germ injuries.Personalized dental arch bar and splint provide good strength for external fixation or assist fixation on strain band.

Objective To investigate the association between cerebral hemodynamic changes and neonatal hyperbilirubinemia.Methods A total of 67 neonates with hyperbilirubinemia who were admitted to our hospital from January 2014 to November 2015 were enrolled as study group,and another 36 normal healthy neonates were enrolled as control group.The two groups were compared in terms of end-diastolic velocity (Vd),systolic peak velocity (Vs),mean blood flow velocity (Vm),resistancc index (RI),and pulsatility index (PI),and the serum level of bilirubin and systemic symptoms were observed after treatment.The t-test was used for comparison of continuous data between groups,and a Pearson correlation analysis was also performed.Results At the time of enrohnent and on day 3 of treatment,the control group had significantly lower Vd,Vs,and Vm than the study group (before treatment:t =75.873,81.589,64.600,19.834,30.453;day 3 of treatment:t =39.476,55.729,35.274,6.069,9.382,all P <0.001).The study group had improvements in Vd,Vs,Vm,1RI,and PI on day 3 of treatment.On day 5 of treatment,there were no significant differences in hemodynamic parameters between the two groups (all P > 0.05).Serum level of bilirubin was positively correlated with Vd (r =0.387,P < 0.001),Vs (r =0.483,P < 0.001),and Vm (r =0.412,P<0.001) and negatively correlated with RI (r=-0.492,P<0.001) andPI (r=-0.497,P<0.001).Conclusion Serum level of bilirubin interacts with cerebral hemodynamics,and cerebral hemodynamic parameters can provide objective evidence for evaluating disease progession and prognosis of neonatal hyperbilirubinemia.

Objective To observe the promoting effect of hypoxia on proliferation of human bone marrow mesenchymal stem cells (BMMSCs) and maintaining their potential of multi-directional differentiation in vitro.Methods BMMSCs were isolated from bone marrow blood samples of patients with bone fracture, and cultured under hypoxic (group A) or normoxic (group B) conditions.The morphology, proliferation and osteogenic and adipogenic potential of the BMMSCs were observed.Results BMMSCs in the group A showed a long spindle shape and a fish shoal-like distribution, and were well-grown, while the morphology of cells in the group B appeared polygonal or flat.The quantity and growth rate of BMMSCs in the group A were increased compared with the group B (P< 0.05), with an osteogenic and adipogenic potential.Conclusions Hypoxia can promote the proliferation of BMMSCs in vitro and maintain their multi-directional differentiation potential.

Objective Toinvestigatetherelationbetweenserumneuron-specificenolase(NSE), bilirubinandcerebraldysfunction,prognosisafterlarge-arteryatheroscleroticstroke.Methods According to the Trial of Org 10172 in Acute Stroke Treatment (TOAST)criteria,all the 73 patients with large artery atherosclerotic stroke were divided into the test group (41 cases ) and control group (32 cases ) according to the elevated or normal levels of serum NSE and total bilirubin. At the first day of their hospitalization,the National Institutes of Health Stroke Scale (NIHSS)score was conducted,their serum NSE,bilirubin (total bilirubin,direct bilirubin,indirect bilirubin)levels were detected,and were compared with the reevaluation of 7 and 14 days of their hospitalization and reexamination results. The modified Rankin Scale (mRS)was use to assess the recovery of their neurological function at day 30 after onset/admission. The prognosis of the patients was followed up at 1 year after onset/admission. The Kaplan-Meier product-limit method was used to conduct the analysis of the good outcome rate,and the good outcomes of both groups/interlayers (different bilirubin and NSE levels)were tested with Log-rank test. Results (1)The NIHSS scores,the levels of serum bilirubin and NSE at day 1,7,and 14 in the test group were significantly higher than those of a control group (all P<0. 01). The levels of serum bilirubin and NSE at day 7 and 14 were lower than those at day 1. (2)The mRS score at day 30 between the test group and the control group was singnificantly different (Z =3. 286,P =0. 001). (3)At day 1,the CT detection rate of large area cerebral infarction of the test group was significantly higher than that of the control group (56. 1%[n=23]vs. 28. 1%[n=9]). There was significant difference (χ2 =5. 712,P=0. 017). (4)The analysis result of Kaplan-Meier showed that there was no significant difference in its good outcome no matter grouped by the test or by serum NSE level stratification of the patients on admission (the accurateχ2valueswere4.063and4.685respectively,P=0.044and0.030respectively).Conclusion Early high-level serum NSE and hyperbilirubinemia can be used as the indexes of early identification of poor prognosis in patients with large-artery atherosclerotic stroke.

Objective To study the method and effect of subcutaneous pedicle flap based on the orbicularis oculi muscle in the treatment of xanthelasma.Methods According to the situation of xanthelasma and the soft tissues of upper eyelid,two new double-folds eyelid was designed.The soft tissues (including the skin and muscle) between two lines were intended to form a subcutaneous pedicle flap based on the orbicularis oculi muscle.Then,after the excision of xanthelasma from the surface of orbicularis oculi muscle,the flap was transferred to the defected site.Finally,a new double-folds eyelid was made.Results 13 cases with 25 sides were treated with a follow-up period of 3-6 months.All flaps were survived with good functions and the appearance of upper eyelids were satisfactory.Conclusions Subcutaneous pedicle flap based on the orbicularis oculi muscle is a reliable option for the treatment of xanthelasma.It has the advantages of abundant blood supply,convenience and effective.

Objective To investigate the expression of KLF4 in cervical carcinoma and its significance. Methods Immunohistochemistry and tissue microarray were used to examine the expression of KLF4 in 39 cases of carcinoma, 30 cases of normal cervical epithelial tissues and 28 cases of cervical cancer in situ. Results Moderate to strong nuclear staining for KLF4 was found in normal tissues and cervical cancer in situ. Interestingly, KLF4 expression was lost in 16 (P<0.05) of 39 carcinoma cases. However, KLF4 expression was not associated with clinicopathologic parameters, including tumor stage and differentiation. Conclusion Our observations indicate that KLF4 may function as a tumor suppressor in the pathogenesis of cervical cancer.

Objective To investigate the anti-inflammatory/immune modulatory effects of high-expressing membrane bound M-CSF-hematopoietic malignant cells on macrophages. Methods After coculturing RAW264.7 and murine macrophage cell line with Namalwa-M, a cell line stably expressing mM-CSF, and companing with Nainalwa-V, a cell line stably transfected with the empty vector as the control, flow cytometry was used to detect the expression of the marker of alternatively activated macrophages, CD206, and intracellular expression of IL-10, IL-12, IL-6 and TNF-α to study the immunophenotype of RAW264.7; phagocytic assays to investigate their functional activity in vitro. Results RAW264.7 cocultured with Namalwa-M consistently showed high-level expression of CD206, which indicated activities of these macrophage cells were increased. Furthermore, these RAW264.7 expressing high levels of IL-10, TNF-a and low levels of IL-12, IL-6, as determined by intracellular staining were suggested that the phagocytic activity was increased. Functionally, RAW264.7 cocultured with Namalwa-M showed a higher level of phagocytic activity. Conclusion Macrophage generated in vitro after cocultured with mM-CSF-expressing hematopoietic malignant cell line could be transformed into abnormal macrophage with an immunophenotype defined as IL-10-high, IL-12-low, IL-6-low and TNF-α-high.

Objective To develop the PCR-denaturing high performance liquid chromatography (DHPLC) for detection of E. Coli O157: H7. Methods The virulence genes of Shiga-like toxin(SLT) and rfbE were specifically amplified by 2 sets of primers. The target gene fragments of the PCR assay were 224 bp and 499 bp, respectively. Results Analysis of 37 strains demonstrated that this PCR system was specific. The detection limit of the PCR was 4 CFU/ml. Conclusion These results indicated that the multiplex PCR-DHPLC assay can be used for specific and sensitive detection of E. Coli O157:H7.

Objective To study the CT manifestations of delayed traumatic hematomas of the brain and evaluate their diagnostic significance in predicting the delayed traumatic brain hematoma. Methods The manifestations of initial CT studies and follow-up CT examinations of 31 delayed traumatic brain hematomas were analyzed. Another 50 CT studies of head trauma without delayed brain hematomas were included randomly as control. Results The abnormal findings of CT studies of the 31 delayed traumatic brain hematomas included: (1)Decreased density of the local brain parenchyma and disappeared difference between gray and white matter of the same area in 18 cases; (2)Local subarachnoid space hemorrhage in 24 cases; (3)Slight mass effect of local brain parenchyma in 16 cases. (4)Subdural hematoma in 9 cases. The locations of the abnormalities were roughly the same with the delayed hematoma except one local subarachnoid space hemorrhage, which was in the opposite of the delayed hematoma. The appearing rate of those abnormal findings in the control group was low and the difference was statistically significant. Conclusion The decrease of density of local brain parenchyma, the disappeared difference between the gray and white matter, local subarachnoid space hemorrhage, and local swollen of brain presented in the initial CT study of the patient with head trauma should be taken as indicators of delayed hemorrhage of the same area of brain, and it is necessary to do follow-up CT studies to exclude it.

Objective To study the relation between the dissection structure of umbilical cord artery and blood flow indices and fetus′ growth. Methods By color Doppler ultrasound, the diameter of umbilical cord and umbilical artery and umbilical vein was measured to compute surface area. The umbilical cord was divided into three parts: placenta affixing part, floating part and umbilical surrounding part.Their blood flow indices were measured and compared. Results Umbilical cord′s diameter in 21-40 weeks′ gestation was ( 1.25 ? 0.21 ) to ( 1.50 ? 0.18 )cm, surface area was ( 1.26 ? 0.13 ) to ( 1.83 ? 0.47 )cm 2.Umbilical artery′s diameter was ( 0.28 ? 0.06 ) to ( 0.40 ? 0.09 )cm, its surface area was ( 0.06 ? 0.03 ) to ( 0.14 ? 0.06 )cm 2.Umbilical vein′s diameter was ( 0.54 ? 0.11 ) to ( 0.79 ? 0.11 )cm, its surface area was ( 0.25 ? 0.02 ) to ( 0.49 ? 0.15 )cm 2. Before 25 weeks′ gestation, the blood flow indices of umbilical artey of placenta affixing part were lower than those of other two parts(P 0.05 ).Conclusions The diameter and surface area of umbilical and umbilical artery and umbilical vein were growing in pace with gestation, biparietal diameter,frontoccipital diameter, and heand circumference of the fetus,the blood flow indices of umbilical artery goes down continuously.The blood flow indices of umbilical artery of placenta affixing part reflect the fetus′ growth.
