RESUMO

The present prospective study was conducted on 170 patients of lung carcinoma, over a period of two years, to study the clinical features, radiological manifestations and histological types of primary lung carcinoma. These were investigated according to a study protocol which included a detailed history regarding the onset and progress of the disease, smoking habits, detailed examination of the respiratory system, routine laboratory investigations, chest roentgenogram, computed tomography of thorax, fibreoptic bronchoscopy and others. The mean age of the patients was 55.94 years (range 29 to 85 years). Eighty seven percent were males. The smoker to non-smoker ratio was 3.9:1. The average duration of symptoms was three months. Cough was the most common presenting symptom (90%). A history of anti-tubercular treatment was present in 42.5% patients. A mass lesion was the most common radiological finding (30.6%), followed by collapse consolidation in 13.5%. Combination presentation was observed in 43.5% patients. Squamous cell carcinoma presented more commonly as a central mass (75%), while adenocarcinoma as peripheral mass lesion (64.5%). Squamous cell carcinoma was the most common histological type diagnosed (45.3%) although adenocarcinoma was the more common in females (39.1%) and non-smokers (51.4%). In India Squamous cell carcinoma is still the most common cell type. Carcinoma lung is still being misdiagnosed as tuberculosis and many patients are wrongly put on antitubercular treatment. Thus there is a need to create awareness about carcinoma lung among the general practitioners.

RESUMO

Abstract To Study failure of thrombolysis with streptokinase in acute myocardial infarction using E.C.G criteria a prospective study of patients presenting with acute myocardial infarction in G.M.C Jammu for a period of one year. A total of 220 patients who presented with acute myocardial infarction were included in the study . Out of 220 patients 193(87.7%) males and 27 (12.3%) females participated in the study. All the patients underwent thrombolysis with streptokinase. Out of 220 patients who were being thrombolysed with streptokinase, most of them (97; 41.1%) were in the age group of 56-65 years. Out of all the patients who underwent thrombolysis (n=220), thrombolytic failure was observed in 121 (55%) patients. Failure rate was significantly higher in the age group of 56-65 years (88; 72.7%). Significantly higher prevalence of thrombolytic failure with streptokinase was observed among diabetic patients (65.2% ) as compared to non diabetics (52.3%).

RESUMO

Schwannomas are uncommon and often benign neoplasms developing from the nerve sheath of the peripheral nervous system. Thoracic schwannomas most often appear in the posterior mediastinum and costovertebral angle. Pulmonary schwannomas are exceedingly rare and can present a diagnostic challenge.Here we report a case of a solitary right-sided endobronchial schwannoma presenting with progressive respiratory symptoms and discuss the clinical features, histopathological manifestations and diagnostic considerations of this rare tumor.

RESUMO

Seizure is the most common and serious neurological emergency seen in clinical practice. A detailed evaluation of every patient with first onset seizure is mandatory to determine seizure type, precipitating events, underlying etiology. There is no reliable diagnostic test and so its diagnosis and management requires clinical acumen and experience If the event is an epileptic seizure, the seizure type and associated clinical, electroencephalographic (EEG) and neuroimaging findings assist in determining the risk of seizure recurrence and the possible need to begin anticonvulsant therapy.

RESUMO

Urinothorax is a rare cause of pleural effusion, which is mostly seen secondary to obstructive uropathy. We report a patient who had nephrolithasis and underwent percutaneous nephrostomy for treatment of hydroureteronephrosis. The patient developed right-sided pleural effusion, five days after percutaneous nephrostomy, which was later diagnosed as urinothorax. Although rare, urinothorax should be considered in the differential diagnosis of causes of pleural effusion, especially in patients with obstructive uropathy, any form of instrumentation of urinary tract or blunt abdominal trauma.The importance of recognizing this entity lies in the fact that the condition is completely reversible following relief of urinary tract obstruction.

Hemangioma are characterized by hyperplasia of blood vessels, usually veins and capillaries, in a focal area of submucosal connective tissue.We hereby report a case of unilateral lingual hemangiomatosis in a 57 years old female who presented to us with abnormal speech and blueish patches over the tongue.

RESUMO

Mucormycosis is a rare life threatening opportunistic fungal infection caused by fungi of Mycorales order belonging to the Phycomycetes class.Mucormycosis usually develops in the patients with diabetes,prolonged corticosteroid use, hematological malignancies, chronic renal failure and other immunocompromised states. Incidently we here by report a case of young immunocompetent adult without any obious risk factors for Mucormycosis presenting with rhinoorbital swelling.

RESUMO

Isoniazid induced psychosis (IIP) is well reported with varied clinical presentations. Many hypotheses are available to explain the mechanism of (IIP) but lack conclusive evidence in its favour. The current case establishes neuropathy with vitamin B6 deficiency as a possible cause for IIP warranting early clinician's attention as the condition may be potentially serious & life threatening.

The current observational cross-sectional study was undertaken using suspected adverse drug data collection form available under (PvPI) to evaluate comparative ADR profile of DFZ Vs CCS for 3 years in spontaneous reporting system of ADRs in current PvPI. Total number of ADR reports during the study period was 3024, out of which ADRs reports due to CS were 112 accounting for a rate of 3.70%. The rate of total ADR events with CS was 4.11%. Geriatric, urban and female population predominated in contributing ADRs with both CCS and DFZ in the study. Self medication of CCS and DFZ contributed 10.95% and 7.69% of total ADRs. Oral route contributed maximal ADRs. Irrational drug prescription contributed substantially. Maximum ADRs due to CCS and DFZ were moderate, latent, non-serious, type A and were probable followed by possible in nature as per WHO UMC scale. Gastritis, new onset hypertension/ loss of hypertensive control, loss of diabetic control, obesity/overweight, dyslipedemia were common ADRs. Thus, ADRs due to CS is a substantial health problem. ADR profile did not vary although DFZ recorded less ADRs.

Objective: To evaluate effect of iodized oil in crossbred cattle suffering from hyperthermia (heat intolerance syndrome) in Punjab, India. Methods: The present study was conducted in 85 clinical cases of summer hyperthermia in exotic and crossbred cows with rectal temperature of ≥40.56 °C. Haematological examinations of the affected cows (55) showed relatively low haemoglobin, normal differential and total leucocytic count. All the affected cows (85) were treated with three doses of iodized oil (750 mg elemental iodine per dose, 5mL) given at 24 h interval by subcutaneous route in brisket region.Results:coming to normal (38.33 to 38.88 °C) within 5 d of start of treatment. The appetite and milk production was restored by 10th day post treatment.Conclusions: The findings of this study substantiate that subcutaneous administration of Seventy seven (90.58%) cases showed remarkable clinical improvement with temperature three doses of iodized oil given at 24 h interval is effective in decreasing rectal temperature of hyperthermic cattle.
