Objective:To explore the application value of Jiawei Yangyin Jiedu Decoction in the treatment of epidermal growth factor receptor (EGFR)- tyrosine kinase inhibitor (TKI)- related rash of lung cancer, and observe its influence on inflammatory index and immune function of patients, aiming to provide effective treatment for this disease.Methods:From December 2016 to January 2019, 90 patients with EGFR-TKI-related rash belonging to Yin deficiency and toxic type in Traditional Chinese Medicine (TCM) syndromes in Yongkang Traditional Chinese Medicine Hospital of Zhejiang Province were included. The patients were divided into control group and observation group by random digits table method with 45 case each. The patients in control group were treated with mupirocin ointment, and the patients in observation group were treated with Jiawei Yangyin Jiedu Decoction on the basis of the control group. The clinical efficacy was compared after 4 weeks.Results:After treatment, the number of lesions and the number of sites, rash area estimation, rash color and other symptoms in observation group were significantly lower than those in control group: (0.71 ± 0.27) scores vs. (1.36 ± 0.32) scores, (0.68 ± 0.14) scores vs. (1.24 ± 0.27) scores, (0.53 ± 0.17) scores vs. (1.31 ± 0.24) scores, the total effective rate was significantly higher than that in control group: 80.00% (36/45) vs. 28.89% (13/45), and there were statistical differences ( P<0.01). The blood samples of the elbows were taken before and after treatment. The immune function indexes of CD 4+/CD 8+, CD 4+ and CD 3+ after treatment in observation group were significantly higher than those in control group: 1.46 ± 0.20 vs. 1.27 ± 0.23, 0.378 ± 0.042 vs. 0.323 ± 0.031 and 0.573 ± 0.062 vs. 0.518 ± 0.060, and there were statistical differences ( P<0.05). The levels of carcinoembryonic antigen and cancer antigen 125 after treatment in observation group were significantly lower than those in control group: (28.17 ± 8.45) μg/L vs. (35.37 ± 9.73) μg/L and (25.29 ± 6.15) kU/L vs. (36.42 ± 12.06) kU/L, the quality of daily life score was significantly higher than that in control group, and there were statistical differences ( P<0.05). After treatment, the levels of inflammatory markers such as tumor necrosis factor-α and interleukin-1β in observation group were lower than those in control group, and there were statistical differences ( P<0.05). Conclusions:For patients with lung cancer EGFR-TKI-related rash, it is important to treat them with Jiawei Yangyin Jiedu Decoction, which can reduce inflammation, improve immunity and improve rash. It is worth promoting.

The ZNF804A variant rs1344706 has consistently been associated with schizophrenia and plays a role in hippocampal-prefrontal functional connectivity during working memory. Whether the effect exists in the resting state and in patients with schizophrenia remains unclear. In this study, we investigated the ZNF804A polymorphism at rs1344706 in 92 schizophrenic patients and 99 healthy controls of Han Chinese descent, and used resting-state functional magnetic resonance imaging to explore the functional connectivity in the participants. We found a significant main effect of genotype on the resting-state functional connectivity (RSFC) between the hippocampus and the dorsolateral prefrontal cortex (DLPFC) in both schizophrenic patients and healthy controls. The homozygous ZNF804A rs1344706 genotype (AA) conferred a high risk of schizophrenia, and also exhibited significantly decreased resting functional coupling between the left hippocampus and right DLPFC (F(2,165) = 13.43, P < 0.001). The RSFC strength was also correlated with cognitive performance and the severity of psychosis in schizophrenia. The current findings identified the neural impact of the ZNF804A rs1344706 on hippocampal-prefrontal RSFC associated with schizophrenia.

Cross-sectional and longitudinal studies have identified widespread and progressive grey matter volume (GMV) reductions in schizophrenia, especially in the frontal lobe. In this study, we found a progressive GMV decrease in the rostral medial frontal cortex (rMFC, including the anterior cingulate cortex) in the patient group during a 6-week follow-up of 40 patients with schizophrenia and 31 healthy controls well-matched for age, gender, and education. The higher baseline GMV in the rMFC predicted better improvement in the positive score on the Positive and Negative Syndrome Scale (PANSS), and this might be related to the improved reality-monitoring. Besides, a higher baseline GMV in the posterior rMFC predicted better remission of general symptoms, and a lesser GMV reduction in this region was correlated with better remission of negative symptoms, probably associated with ameliorated self-referential processing and social cognition. Besides, a shorter disease course and higher educational level contributed to better improvement in the general psychopathological PANSS score, and a family history was negatively associated with improvement of the negative and total PANSS scores. These phenomena might be important for understanding the neuropathological mechanisms underlying the symptoms of schizophrenia and for making clinical decisions.

Objective:To explore the traits of functional connectivity of resting-state networks in patients of schizophrenia with auditory verbal hallucinations (AVH) by using independent component analysis (ICA).Methods:All patients were met the Diagnostic and Statistical Mannal of Mental Disorders,Fourth Edition,Text Revision (DSM-IV-TR) diagnostic criteria for schizophrenia.Thirty schizophrenia patients with frequent AVH (AVH),24 schizophrenia patients without AVH (non-AVH) and 60 healthy controls (HC) matching with age and gender were analyzed by resting functional magnetic resonance imaging.The AVH were assessed by using Hoffman auditory hallucination scale.By using ICA,auditory network (AUN),language network (LAN),salience network (SAN),right front-parietal network (RFP) and default mode network (DMN) were selected as interesting networks for further analyses.Covariance analysis was used to compare the activity and functional connectivity of multiple resting state networks between three groups,and correlation analysis was used to evaluate the relations between abnormalities of brain and the severity of hallucination.Results:Compared to non-AVH,AVH group showed increased activity in left superior temporal gyrus (STG) and left postcentral gyrus (LPG) in the AUN,and showed decreased activity in right anterior cingulate cortex (ACC) in the SAN (P < 0.05,FDR corrected).The severity of AVH group was associated with activity of the left STG (r =0.43,P <0.05) and the right ACC (r =-0.48,P <0.01).Furthermore,compared to non-AVH group,there were decreased connectivity between LAN and AUN [(-0.29 ±0.21) vs.(-0.16 ± 0.17),P < 0.05,uncorrected],decreased connectivity between SAN and LAN [(-0.09 ± 0.22) vs.(-0.06 ± 0.18),P < 0.05,uncorrected],increased connectivity between SAN and AUN [(0.30 ± 0.18) vs.(0.15 ± 0.24),P < 0.05,uncorrected] in AVH group.Conclusion:There may be aberrant functional connectivities of AUN,LAN and SAN in schizophrenia patients with frequent AVH,and the occurrence of AVH seems to be related to brain areas involved in language production,speech perception and self-monitoring.

Objective To explore the influence of family function on health behaviors of stroke patients and the mediating effects of depression.Methods From September 2011 to April 2012,101 stroke survivors from a certain hospital in Guangzhou were investigated with Health Promoting Lifestyle Profile Ⅱ (HPLP Ⅱ),Family Assessment Device (FAD) and Self-Rating Depression Scale (SDS).Results The mean score of overall health behavior was (2.27±0.36).The mean score of overall family function was (2.18±0.25).The score of overall depression was (37.39±7.94),which was higher than normal people.And the incidence of the stroke survivors' depression state was 29.5%.There was a correlation between family function,depression and health behavior.The depression totally mediated the influence of problem solving on health behaviors.The depression partially mediated the influence of communication,affective responsiveness,behavior control on health behaviors,and their mediating effects were 32.1%,37.4%,26.1%.Conclusions The promotion of family function and the reducing of depression can be regarded as the in-tervention to improve their health behavior.

Objective To study the set-up errors by CBCT in IMRT with two different immobilization techniques for thoracic and abdominal tumors.Methods Sixty patients with thoracic and abdominal tumor were included in this study and separated into study group and the control group.The study group were immobilized with carbon fiber holder,vacuum bag and thermoplastic mask.The control group were immobilized with carbon fiber holder and thermoplastic mask.CBCT scan and auto-match online were regularly performed before the treatment.The setup of left-right(x),superior-inferior(y),anterior-posterior (z) were received.The value of the Mptv was calculated,meanwhile.The grouped t-test of was carried out between these two methods.Results The shift errors in x-,y-,z-dimension of the study group were (0.32 ± 2.58) mm,(-0.40 ± 3.89) mm,(-0.75 ± 2.43) mm.The Mrrv were 5.60 mm,6.08 mm,6.32 mm.The translation set-up errors in x-,y-,z-dimension of the control group were(0.62 ±3.60),(2.44 ± 4.93),(0.66 ±2.85) mm,respectively.The MPrv were 8.07,10.63,6.90 mm,respectively.The t-test value were t =-0.78,-5.11,-4.22,P =0.440,0.000,0.000,respectively.Conclusions The immobilization techniques with carbon fiber holder,vacuum bag and thermoplastic mask would be better than the techniques without the vacuum bag in reducing the setup errors.

RESUMO

Cone-beam CT (CBCT) of Elekta can be used to analyze the setup errors in intensity modulated radiotherapy. A total of 240 patients with head and neck cancer were chosen in the period from October, 2009 to October, 2010. Their CBCT images were obtained after initial setup, and registered to the planning CT images, and then the setup errors on translational directions (X, Y, Z) and on rotational directions (U, V, W) were obtained and analyzed. The results showed that the setup errors on translational directions in Y-axis and Z-axis were larger than in X-axis, and the setup errors on rotational directions in Y-axis was much more obvious than those on the other two directions, which should be taken care during the patient daily setup. It may be concluded that the CBCT system can improve the precision for radiotherapy.

Objective To study the feasibility of dose calculation using kilovoltage X-ray cone-beam CT (KVCBCT) imaging for head-and-neck radiation therapy.Methods 11 patients with nasopharyngeal carcinoma were scanned with KVCBCT to adjust position before treatment, and rescanning images with KVCBCT after correction were input a treatment-planning system.The dose was recalculated by applying the patients′ treatment plans based on planning CT to the KVCBCT images.The dose distributions and dose volume histograms (DVH) of the tumor and critical structures were compared with the original treatment plan.Results The DVH and dose distribution of the plan based on the KVCBCT are compared with that of the planning CT, and they shows a good consistency for the 11 cases.The doses calculated from the planning CT and KVCBCT were compared on the isocenter planes.Using γ analysis with a criterion of 3%/3 mm, 98.0%±1.33% of the points on the isocenter planes in the planning CT and KVCBCT.The difference of the dose to target volume was<1% and to normal structure was<2%.Conclusions This study indicated that CBCT images can be used to make a treatment plan with its individual hounsfield unit-electron density calibration curve.
