RESUMO

Aim: The present study was aimed to analyze the ecological significance of Ulhas River estuary of the western coast of India using the taxonomic, functional and conservation aspects, and also to record an updated data base on the estuarine community structure.Methodology: Ichthyo faunal diversity was assessed by experimental fishing conducted at 3 selected stations using single day dolnets of code end mesh size of 10mm, from September 2017 to August 2018. The diversity of fish species was recorded considering the most relevant taxonomic classification data and further supplemented by the information pertaining to ecological roles played by the individual species (using guild approach) and the IUCN- conservation status. Results: There were 105 species, belonging to 4 classes, 19 orders, 44 families and 75 genera, recorded from Ulhas River Estuary. The class: Actinopterygii was the most diverse taxa, (including 12 orders, 32 families and 55 genera) among the total ichthyofauna reported. Order Perciformes was the most representative order of the class (40 species, 31 genera and 17 families), followed by Decapoda (21species, 8 genera and 6 families). Based on the estuarine usefunctional guild categorization, 51.42 % of the species were marine migrants, followed by amphidromous species (21.9%). Zoobenthivores (29.41%) was found to be the most dominant feeding guild followed by omnivores (19.6%). Based on the IUCN Red List, 53.33% of the fish species observed from Ulhas River Estuary were categorized as “Not Evaluated”, and 7% were “Data Deficient”, signifying the lack of information on biological aspects of the species along Indian waters. Tenualosa toli marked its presence under threatened category (Vulnerable) from Ulhas River Estuary. Interpretation: The abundance of marine migrants as well as the amphidromous species in the estuary signifies the use of this ecosystem for the nursery function of commercially important marine fisheries resources. Increased proportion of “Not Evaluated” and “Data Deficient” categories in the context of high level of anthropogenic stress demands for more updated documentation of the biodiversity and periodic changes in its ecological structure for formulating and implementing effective ecosystem-based management programs for such sensitive estuarine ecosystems.

BACKGROUND & OBJECTIVE: Cardiovascular and other chronic diseases are becoming the major causes of morbidity and mortality in most of the third world countries including India, especially in the southern Indian States, like Kerala, where most of the health indicators match closely with those of any developed country. Various behavioural risk factors (BRF) namely smoking, unhealthy diet, stress at home and work place, consumption of alcohol, sedentary life style, etc., are known to be risk factors for many such diseases. The present study was carried out to estimate the prevalence of various behavioural risk factors for chronic diseases, and to identify their biosocial correlates. METHODS: A cross-sectional study was done in which the data were collected from a sample of 6579 individuals of age 30 to 74 yr, randomly selected following a stratified multi-stage cluster sampling design covering Kerala State. The important factors investigated include various behavioural risk factors, presenting chronic diseases and family histories among close relatives. The data were analysed using both univariate and multivariate analyses. RESULTS: The two major risk factors observed among males were smoking and alcohol consumption. About two fifths (40%) of them were current smokers as well as current users of alcohol (41%). The median age at initiation was 21 yr for both smoking habits and for alcohol consumption. Nearly a quarter of the target population were inactive (23% males and 22% females) based on work and leisure time activities. More than one-fifth of them (23%) reported stress. Obesity was found more among females (33%) than males (17%). Low socio-economic background was found to be a high predictor (high risk group) for habit of smoking, alcohol consumption, stress and unhealthy diet. INTERPRETATION & CONCLUSION: Substantially high levels of the various behavioural risk factors among adults in Kerala suggests an urgent need for adopting healthy life style modifications among the population in general. The increased risk observed among the younger generation for behavioural risk factors such as smoking and alcohol consumption calls for urgent corrective steps and measures for long-term monitoring of all major risk factors as well as the major chronic disease conditions.

The role of immunohistochemistry and electron microscopy in the evaluation of this tumor is stressed. The appropriate terminology to be used and possible etiologic factor are also discussed.

BACKGROUND: Numerous surveys carried out in India report the high prevalence of type 2 diabetes. Such studies have not included the population of Kerala. We estimated the prevalence of type 2 diabetes mellitus in Neyyattinkara taluk, Thiruvananthapuram district, Kerala state. METHODS: All panchayat wards in the taluk were grouped into urban, highland, midland or coastal, and one ward from each stratum was randomly selected for the study. All households were listed and adults, 20 years or older, screened for high (> 110 mg/dl) random blood sugar (RBS) by a glucometer test. Those with high RBS were reassessed by a fasting oral glucose tolerance test (OGTT), consisting of initial examination of venous blood for fasting plasma glucose (FPG) values, administration of 75 g of glucose dissolved in distilled water, and examination of venous blood for postprandial plasma glucose (PPPG) exactly 2 hours after the administration of glucose. Diabetes was diagnosed according to the World Health Organization criteria as either FPG > 139 mg/dl, or PPPG > 199 mg/dl, or both. Impaired glucose tolerance (IGT) was diagnosed if PPPG was 140-199 mg/dl. RESULTS: Out of 4988 eligible subjects, 3899 were available for the study, a response rate of 78.2%. Response was highest in the highland area (86.2%), and lowest in the coastal area (73.6%). The overall crude prevalence rate of type 2 diabetes was 5.9%. It was highest in the urban (12.4%), followed by midland (8.1%), highland (5.8%), and coastal (2.5%) regions. Ageing was associated with greater prevalence of type 2 diabetes in all regions and both sexes. Women showed a higher prevalence in the highland and coastal areas and men in the urban and midland areas. When compared to a population with standard age structure suggested by the World Health Organization for international comparisons, prevalence in the age group 30-64 years was found to be 16.9% in the urban, 10.1% in the midland, 6.8% in the highland and 3.6% in the coastal regions, respectively. Overall age-adjusted prevalence of type 2 diabetes in 30-64-year-olds in Neyyattinkara was 9.2% among men, 7.4% among women, and 8.2% for all persons. There was not much difference in prevalence if the American Diabetes Association criteria for diagnosis were used. The prevalence of impaired glucose tolerance was fairly low in this population. Out of 229 diabetics in the sample, 175 (76.5%) were already diagnosed and under treatment, while our survey identified 54 new diabetics (23.5%). CONCLUSION: Though prevalence of type 2 diabetes is high in this population, the detection rate is also high. However, impaired fasting glucose and impaired glucose tolerance are low. The reasons for this need to be elucidated.

There is a trend towards increase in the incidence of coronary heart disease among Indian population. Also, little information is available on the population distribution of serum lipid components and risk factors for coronary heart disease in Kerala, a state fast turning urban. To study the serum lipid profile and the prevalence of other risk factors for coronary heart disease in the residents of an urban housing settlement in Thiruvananthapuram, fasting blood sample was collected from 206 (64%) residents above the age of 19 years and analysed for plasma glucose and various fractions of serum lipids. A detailed questionnaire on the clinical profile and history of the subjects, and measured weights and heights was also administered. Mean serum total cholesterol was 223.7 +/- 45.3 mg/dL; 223.7 +/- 44.9 mg/dL among males and 223.7 +/- 45.8 mg/dL among females. Mean high-density lipoprotein cholesterol was consistently higher in females in all age groups, while mean low-density lipoprotein cholesterol was higher in males till the age group 40-49 after which the pattern was reversed. Mean total cholesterol in the age range 35-64, after age standardisation, was 229.4 mg/dL. Mean serum total cholesterol was higher in this sample when compared to US population, as well as north and west Indian populations. Thirty-two percent subjects were in the highest risk category with serum cholesterol exceeding 239 mg/dL, while in the US population this fraction constituted only 18 percent. Other risk factors such as high blood pressure, obesity, diabetes, sedentary lifestyle and smoking also had a high prevalence in this population. In this settlement of urban residents in Thiruvananthapuram, serum total cholesterol and low-density lipoprotein cholesterol are high. The causes are likely to be dietary. Combined with the high prevalence of other risk factors such as obesity, hypertension, smoking, diabetes and lack of exercise, this situation demands a preventive programme.

AIM: To induce intrahepatic cholestasis in rats using lomustine 1(2-chloroethyl)-3-cyclohexyl-l-nitrosourea (CCNU). METHODS: Doses of 10 mg, 20 mg and 30 mg/Kg body weight of CCNU were injected intraperitoneally in separate groups of animals. RESULTS: With 10 mg/Kg body weight of CCNU, serum bilirubin levels increased for up to 72 hours and then slowly returned to normal. With a dose of 20 mg/Kg body weight of CCNU, serum bilirubin, AST, ALT and alkaline phosphatase levels increased for 72 hours and then returned to normal over 4-5 weeks. With a dose of 30 mg/Kg body weight peak levels of serum bilirubin were reached on day 17. Pathological studies were carried out after injection of 30 mg/Kg body weight of CCNU. After 72 hours hepatocytes were normal, with minimal nonspecific inflammation and bile duct proliferation. After 16 days, triaditis was observed with deposition of collagen. Focal fibrosis was also noticed. There was no significant abnormality of hepatocytes. After 75 days, hepatocytes showed focal ballooning. Bile duct proliferation was seen invading the parenchyma. Nodules of hepatocytes separated by irregular fibrous bands indicated cirrhosis. CONCLUSIONS: An animal model of intrahepatic cholestasis has been developed using CCNU; this model may be used to assess the utility of hepatobiliary radiopharmaceuticals.

Small cell carcinoma of the gall bladder is very rare. We report a 25-year-old woman with this tumor. These tumors typically occur in elderly women with cholelithiasis; chemotherapy is not known to improve survival.

Kikuchi-Fujimoto lymphadenitis is a distinctive entity which is easily recognised in its classical histology. However, cases which are biopsied in the early stage of the disease, before entering the necrotic phase may be misdiagnosed as non-Hodgkin's lymphoma. Ten cases of Kikuchi-Fujimoto disease which mimicked non-Hodgkin's lymphoma and in which less than 10% of the lymph node was occupied by necrosis were chosen for this study. The patients included 4 males and 6 females with an age range of 15-40 years. Eight patients presented with cervical lymphadenopathy. The pathologic areas ranged from 70% to 95% of the total nodal area. The features that helped distinguish this lesion from non-Hodgkin's lymphoma were: 1) a sprinkling of karyorrhectic debris throughout the node 2) plasmacytoid monocytes as well as "histiocytes" in the company of transformed lymphocytes (immunoblasts) 3) enlarged mottled T-zone areas, and 4) presence of residual lymphoid follicles. Follow up on six patients revealed no evidence of disease.

RESUMO

We report here a murine model of transplantable lymphoblastic leukemia closely simulating human disease in albino mice of ICRC strain. Both male and female mice of this strain developed leukemia with high incidence (approximately 70%) following whole body exposure to 60Co Gamma Rays (Dose: 1.5 Gray/Week x4). The latent period for development of leukemia was six months. In the leukemic mice there is marked leucocytosis with presence of lymphoblasts in peripheral blood. These blasts infiltrate various organs like liver, spleen, kidney, lymph nodes, testes and brain, Neoplastic cells are T lymphoblasts expressing weak Thy 1.2 membrane antigen and found sensitive to anticancer drugs. Salient features of the murine model are described.

RESUMO

We report two cases of squamous carcinoma in which a prominent rhabdoid phenotype was seen. The tumors which occurred in the buccal mucosa of a 70 year old man and the skin of a 85 year old man were positive for cytokeratin and epithelial membrane antigen indicating their epithelial nature. Co-expression of vimentin was also noted. The aggressive nature of the rhabdoid component is confirmed by the fact that the cervical lymph node metastasis in case I was also rhabdoid.

RESUMO

Twenty eight patients of myelodysplastic syndrome (MDS) were treated with low dose cytosine arabinoside to study the effect of this treatment modality. All patients presented with a hemoglobin of less than 12 Gm/dl, 4 (15%) had neutropenia with an absolute neutrophil count of less than 500 x 10(6)/L and 18 (65%) had thrombocytopenia of less than 100 x 10(9)/L. The subtypes according to the bone marrow evaluation included 14 patients of refractory anemia with excess blasts (RAEB), 10 refractory anemia with excess blasts in transformation (RAEB-T), and 4 chronic myelomonocytic leukemia (CMML). Five patients (18%) achieved complete hematological response, 10 (36%) had a partial response and 9 (33%) patients had no response. Four patients died early during treatment due to tumor lysis (1 CMML) and hemorrhage (3 RAEB). Seven patients progressed to acute myeloid leukemia (AML) while on therapy and three progressed to AML after completion of therapy. Five patients died of hemorrhage and 3 of septicaemia after achieving an objective response. The mean duration of follow up in these patient was 8 months (range 1 month-3 years). Only 3 patients of RAEB have survived for greater than 2 years. Our data reveals the short term benefit of this mode of therapy and emphasizes the need to develop newer therapeutic approaches.

Combination chemotherapy consisting of methotrexate with leucovorin rescue, doxorubicin, cyclophosphamide, vincristine, prednisolone and bleomycin (MACOP-B) has been extensively used for the treatment of Non Hodgkins Lymphoma. However, different results have been reported. The aim of this study was to assess the feasibility of administration of this regimen on an out patient basis and to confirm the efficacy of MACOP-B. 51 patients with intermediate--and high--grade lymphoma were treated with this regimen in a single institute study. Numerous clinical features predictive of response and disease free survival were analysed. The Median age was 48 years (range 14-77). Diffuse large cell lymphoma was seen in 65%, diffuse small cleaved in 10% and diffuse mixed in 15%. Eight patients (15%) had Stage I disease, 18 (35%) Stage II, 12 (23%) Stage III and 13 (25%) Stage IV. Complete remission was achieved in 65% of the patients. With a median follow up of 18 months, 40% of the patients are alive at 40 months. Sixty percent of the complete responders are disease free at 40 months. Response rates did not differ significantly for age, sex, stage, histology, bone marrow involvement and extranodal disease. However patients with absence of B' symptoms, non bulky disease at presentation and diffuse large cell histology had a higher percentage of complete remission. Hematological toxicity occurred in 90% and was grade IV in 14% patients. Three patients died of sepsis. Severe mucositis occurred in 40% of the patients. In conclusion, while it is possible to give aggressive chemotherapy at the out patient basis in India we failed to confirm the high response rates as originally reported.

RESUMO

Nutritional status of children in two poor communities was studied using conventional anthropometric techniques. A total of 944 children, of which 246 boys and 242 girls in coastal area and 213 boys and 249 girls in the non-coastal area formed the study group. With measurements of height and weight, the prevalence of various forms of growth retardation was determined in two communities. Inspite of better food intake, the rural coastal children exhibited poorer nutritional status mainly because of environmental deprivation.

We report a case of psammomatous calcification in a carcinoid tumor of the ampulla of Vater. Although the tumor was glandular, its neuroendocrine nature was proved using chromogranin immunostain.

RESUMO

Primary orbital lymphomas are rare. We report nine such cases (4 with DWDL, 3 with DPDL, 1 with DHL and 1 unclassifiable lymphoma). All patients achieved clinical complete remission (CR). Of those who completed treatment more than a year ago, three continue to be in CR at 17, 24 & 25 months and two are lost to follow up.

RESUMO

Alpha-fetoprotein has emerged as a useful diagnostic tool for hepatic tumour and tumours of germ cell origin. However, isolated case reports of association of this tumour marker with tumours of the lung and non-germ cell tumours of the ovary are reported. We present a case of a Rhabdomyosarcoma, a generally non-secretory tumour that showed raised levels of AFP in serum and reacted positively for the same in a PAP technique for AFP. Cross striations were visible in many cells on II & E stained sections.

RESUMO

Six patients of primary lymphoma of breast are presented. All our patients were female with median age of 46 years. Combination chemotherapy was the mainstay of treatment in all the patients. Five patients achieved complete remission of which two relapsed, one had a distant relapse and the other was local. Patient with distant relapse expired after a follow-up of 224 months whereas the one with local recurrence was well controlled with radiotherapy only. The role of combination chemotherapy to decrease the distant relapse and thereby mortality is discussed.

RESUMO

This report documents the occurrence of an extragonadal malignant teratoma on the sole of the foot, in a young male. The patient also developed a groin nodule showing the same histology. The testes, retroperitoneum and mediastinum, which are usual sites of germ cell tumours were all normal, and no other primary could be detected. A teratoma growing in the tip of an extremity is a unique occurrence, biologically difficult to explain, and, which to the best of our knowledge has never been reported in literature.
