RESUMO

Objective:To investigate the specific chest computed tomography(CT) features of corona virus disease 2019 (COVID-19) and to evaluate its clinical diagnostic value. Methods:The clinical data of 35 cases with suspected COVID-19 from Shanghai Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine and Shanghai East Hospital Affiliated to Tongji University from January 1 to February 14, 2020 were retrospectively analyzed. A total of 17 cases with positive results of two times of real time polymerase chain reaction (RT-PCR) for 2019 novel coronavirus (2019-nCoV) were evaluated as the case group, and the remaining 18 cases with negative results of two times of RT-PCR for 2019-nCoV were evaluated as the control group. The features of chest CT images of 35 cases were obtained. The frequencies of four CT imaging indicators including ground glass opacities (GGO), crazy paving, heterogeneous consolidation and mutiple subpleural lesions were analyzed. The sensitivity, specificity, positive predictive values (PPV) and negative predictive value (NPV) for COVID-19 were calculated. Results:In the case group, there were 11 cases with GGO, seven cases with crazy paving, six cases with heterogeneous consolidation, and 16 cases with mutiple subpleural lesions, while in the control group, there were seven cases with GGO, one case with crazy paving, six cases with heterogeneous consolidation, and five cases with mutiple subpleural lesions. When multiple subpleural lesions or any two of the CT imaging indicators were used as the characteristic indicators, the diagnosis efficiencies were better, with the sensitivity, specificity, PPV, NPV and Youden index of 94.12%, 72.22%, 76.19%, 98.86% and 0.66, respectively, and 88.24%, 77.78%, 78.95%, 87.50% and 0.66, respectively. Conclusions:Chest CT indictors are of high clinical diagnostic value for COVID-19. Any two of the four CT indicators (GGO, crazy paving, heterogeneous consolidation and mutiple subpleural lesions) or the single characteristics (mutiple subpleural lesions) are of high diagnostic efficacy.

RESUMO

Objective To investigate the effect of intravenous ultrafine superparamagnetic iron oxide nanoparticles feraheme (generic name:ferumoxytol) on cerebral infarction volume and inflammatory response in mice with permanent middle cerebral artery occlusion. Methods Thirty C57BL/6J mice were divided into sham operation group, saline control group, and feraheme group by the random number table (n =10 in each group).A permanent right middle cerebral artery occlusion model was induced by the modified suture method in the saline control group and the feraheme group, and no suture was inserted into the mice of the sham operation group. The intervention was performed by tail vein injection at 24 h after modeling. The sham operation group and the feraheme group were injected with 18 mg/kg feraheme, and the saline control group was injected with the same volume of normal saline. The neurobehavioral scores were conducted at 24 h (before the feraheme or saline injection) and 48 h (before the MRI exam) after modeling.MRI scans were performed at 48 h after modeling, and the cerebral infarction volume was calculated according to T2-weighted imaging. After the end of the scan, orbital blood was collected for the detection of serum tumor necrosis factor (TNF)-α,interleukin (IL)-1 β,and IL-6 levels. Then, the mice were sacrificed and the brain tissue was taken for HE staining and Ibal immunohistochemical staining. Results There were no significant differences in the infarct volume and neurological function score between the saline control group and the feraheme group. The serum levels of TNF-α,IL-1β,and IL-6 in the saline control group and the feraheme group were significantly higher than those in the sham operation group (P <0.05),but there was no significant difference between the saline control group and the feraheme group. Conclusion Intravenous injection of 18 mg/kg feraheme at 24 h after cerebral ischemia did not affect the infarct volume and inflammatory response, suggesting that this dose of feraheme can be used for molecular imaging studies of inflammatory response after cerebral ischemia.

RESUMO

Objective To investigate cerebral hemodynamic changes in patients with unilateral stenosis of carotid artery by using CTP and CTA,and to analyze the dependability of the degree of artery stenosis and cerebral hemodynamic changes. Methods Fifty patients with unilateral stenosis of carotid artery underwent toshiba aquilion one CTP and CTA.The patients were divided into mod-erate stenosis group, severe stenosis group and occlusion group according to the NASCET criteria. The rCBF,rCBV,rMTT and rTTP were measured ,and compared between stenosis and contralateral hemispheres ,and the dependability were analyzed between the de-gree of unilateral carotid artery stenosis and relative perfusion parameters by using ranking-test.Results (1)There were 20 cases of unilateral carotid artery occlusion,13 severe stenosis,1 7 moderate stenosis.(2 )No statistical significance on rCBF and rCBV was found among the three stenotic groups (P>0.05).There were statistical significances on rMTT and rTTP among the three stenotic groups(P<0.05).(3)There were statistically no correlations between the values of rCBV,rCBF and the degree of artery stenosis. There were statistically positive correlations between the values of rMTT,rTTP and the degree of artery stenosis with the pearson correlations 0.512 and 0.708. Conclusion rMTT and rTTP are more sensitive parameters for evaluation of cerebral hemodynamics and dependability of carotid ar-tery stenosis on the CTP and CTA.These have an important value to guide the clinical treatment.

RESUMO

Objective To investigate the clinical value of 64-slice computed tomography with MinIP and MPR for imaging the bronchus related to a solitary pulmonary lesion (SPL). Methods Seventy-five subjects with solitary pulmonary lesions underwent chest 64-slice CT and their bronchi were analyzed retrospectively. All images of thin-section (0.625 mm) were reconstructed with MPR and MinIP into images of 1, 2, 3, and 5 mm thickness and 1 mm gap in two orthogonal planes along the long axis of bronchus related to the SPL. The image quality of four series of MinIP and MPR images was evaluated in the aspect of bronchus visibility and pulmonary vascular masking. One-way ANOVA with Bonferroni correction and interclass correlation coefficient were used in the statistical analysis. Results (1) The mean scores of display of the bronchi on MinIP images of four series (4.85, 4.77 and 4.84, 4.63 and 4.67, 4.25 and 4.28, in 1, 2, 3, and 5 mm thickness, respectively) and on MPR images of 1 or 2 mm thickness (4.77 and 4.76, 4.04 and 4.27, in 1 and 2 mm thickness, respectively) were good or excellent. MPR images of 1 mm thickness and MinIP images of 1-3 mm thickness showed no significant differences (t=0.318, P> 0.05 for all), but they were superior to MinIP images of 5 mm thickness (t=6.318 and 6.610, P<0.01). MPR images of 2 mm thickness were inferior to MinIP images of 1-3 mm thickness (t=5.003-8.958, P<0.01), but there was no significant difference between MPR images of 2 mm thickness and MinIP images of 5 mm thickness (t=1.794 and 0.3181, all P> 0.05). (2) The effect of suppression of pulmonary vascular markings on MinIP images was better with the increase of slice thickness (F=45.312 and 40.415, P<0.01). The mean scores of MinIP images of 3 mm and 5 mm thickness (4.67 and 4.64, 5.00 and 4.97, for 3 and 5 mm thickness, respectively) were good or excellent, but MinIP images of 2 mm thickness were just acceptable. Conclusion MinIP images of 3 mm thickness may display the bronchus related to SPL more clearly.

RESUMO

Objective To study spiral CT features and pathological basis of nodular goiter ,so that to improve the diagnostic accuracy of nodular goiter. Methods CT findings in 34 patients with nodular goiter confirmed by pathology were retrospectively analyzed and compared with pathological findings. Results Thyroid density in the patients with NG was lower than that in the normal. There was no statistically significant difference in the density between single and multi-NG. Most lesions(28/34) were multiple, round or oval, with different size and smooth edges. The density of majority lesions was lower than nearby normal tissue. Most lesions (27/34) were uniform in density. Central necrosis(5/34)often appeared in the bigger lesions(diameter>3.0 cm). The CT value of the lesions was increased (47.1±42.2) HU after enhancement, but lower than surrounding thyroid tissue. Calcifications in the lesions(6/34)were puncticular, plaque or crustiform. Single or multiple cystic-solid or solid nodules could be seen in pathology, without a continuous capsule in the majority. Some patients (5/13) with single NG shown on imaging study were multi-NG in the pathology. Three cases of diffuse NG showed by CT were malignant in pathology. Conclusion There are certain characteristics for NG on spiral CT. Comprehensive evaluation may improve the accuracy of CT diagnosis.
