Objectives@#The purpose of this study is to investigate the difference in the effects of eye-mask and light-off on sleep status according to a commercial fitness tracker and a sleep diary of psychiatric in-patients in correctional facilities where nocturnal light is compulsory. @*Methods@#This study was conducted over 3 consecutive nights. In-patients of the National Forensic Psychiatric Hospital (n = 29) were assigned random subject numbers and slept as usual in the light-on condition on the first night. The subjects slept with eye-masks in the light-on condition on another night and without an eye-mask in the light-off condition on the other night. Subjects were asked to sleep wearing a commercial fitness tracker and to keep a sleep diary. The order of these changes in bedroom lighting condition on the second and third nights was assigned randomly to participants. @*Results@#In comparison of the sleep variables between the light-on condition and the eye-mask condition, the Wakefullness After Sleep Onset (WASO) was shorter and sleep satisfaction was higher in the latter.(respectively, Z = 3.66, p < 0.017 ; Z = 2.69, p < 0.017) In comparison of the sleep variables between the light-on and light-off conditions, the WASO was shorter and sleep efficiency and sleep satisfaction were higher in the latter (respectively, Z = 2.40, p < 0.017 ; Z = 3.02, p < 0.017 ; Z = 3.88, p < 0.017). However, there were no differences in the sleep variables between the eye-mask condition and the light-off condition. @*Conclusion@#Subjective improvements in sleep variables were noted in sleep diaries of institutionalized psychiatric patients under either the ‘eye-mask’ or ‘light-off’ condition. However, there were no significant differences between the ‘eye-mask’ and ‘light-off’ conditions. Therefore, we suggest that psychiatric patients in correctional facilities use eye-masks when sleeping.

Objectives@#This study compared the demographic and clinical characteristics, including blood testosterone level and intellectual capacity, between pedophilia and other types of paraphilia. @*Methods@#The medical records of 115 subjects with paraphilia who committed sex crimes and were admitted to National Forensic Hospital from January 2009 to December 2019 were analyzed retrospectively. The subjects were divided into two groups: the pedophilia group (n=59) and other paraphilia group (n=56). The demographic and clinical parameters, including intelligence quotient and blood testosterone level, were compared between the two groups. The characteristics of the sexual offenders were compared according to the age of the victims. @*Results@#Among the 115 subjects, 54 (46.9%) were diagnosed with pedophilia. The pedophilia group showed a significantly lower level of testosterone in their blood (t=-2.409, p=0.018), and higher comorbidity of intellectual disability than the paraphilia group (χ 2 =6.962, p=0.008). In addition, paraphilia subjects who had sexually offended victims under 18 years of age showed a higher frequency of pedophilia (67.2%, p=0.001), and a higher frequency of intellectual disability than those with adult victims (χ2 =3.893, p=0.048). @*Conclusion@#In this study, pedophilia subjects showed lower blood testosterone levels and higher comorbidity of intellectual disability than those with other types of paraphilia. Further study will be needed to prevent sexual offending based on these findings.

Objective@#As the population of the elderly increases, the prevalence of dementia and other mental diseases of the elderly is also increasing. Dementia is a typical elderly mental illness. Some people with dementia may cause a crime due to symptoms of mental behavior. The purpose of this study is to examine the characteristics of criminal behavior of various elderly mental patients. @*Methods@#From January 1, 2010 to December 31, 2019, the court selected 65 years of age or older from among the criminal psychiatric appraisal cases commissioned by the court. A total of 205 medical records and mental assessments were reviewed retrospectively. All records were prepared through psychiatric interviews, clinical psychological examinations, and other examinations, and the information compiled was viewed in several aspects, including demographic factors, crime characteristics, and psychiatric opinions, at the level of technical analysis. @*Results@#There was a statistically significant difference in the judgment of criminal liability according to the degree of cogni-tive impairment (p<0.001) in the elderly mentally ill, and a significant difference in criminal behavior depending on the type ofmental illness (p<0.001). @*Conclusion@#Not only dementia but also other elderly mental illnesses are related to violent crime. In particular, paranoid symptoms and alcohol abuse in older adults’ mental illness are related to the type of criminal behavior. In an aging society, appropriate therapeutic intervention is needed to prevent criminal behavior of the elderly mentally ill.

Objective@#The incidence of bystander cardiopulmonary resuscitation in out-of-hospital cardiac arrest has increased rapidly over the past 10 years. On the other hand, automated external defibrillators (AEDs) are still only used in a minority of cases. This study investigated the feasibility of mobile video call guidance to facilitate AED use for laypeople. @*Methods@#Ninety laypersons were randomized into three groups: mobile video call guided, voice call guided, and nonguided. The participants were exposed to a simulated cardiac arrest requiring AED use and guided by video call, voice call, or not. The simulation experiments were saved as a video clip, and other researchers blinded to simulation assessed the performance according to a prespecified checklist after the simulations. The performance score and analyzed time intervals from AED arrival to defibrillation in the three groups were compared. @*Results@#The basic characteristics were similar in the three groups. Performance scores in the checklist for using AEDs were higher in the mobile video call guided group, particularly in a category of ‘power on AED’ and ‘correctly attaches pads’ than non-guided groups. The performance scores in the category of ‘safely delivers a shock and resume compression’ were also higher in the mobile video call group. On the other hand, the time interval to defibrillation was significantly longer in the mobile video call group. @*Conclusion@#This study showed that mobile video call guidance might be an alternative method for laypeople to facilitate AED use, but further well-designed research will be needed.

RESUMO

PURPOSE: This research was conducted to examine the successful aging and identify factors influencing successful aging in the Korean elderly. METHODS: The subjects were 4,507 elders and data were collected from the 4th Korean Longitudinal Study of Ageing in 2012. The data were analyzed using frequencies, weighted percentage, chi2 test, and logistic regression. RESULTS: The results of this study showed that 19.6% of the participants were categorized to successful aging. The domains of the absence of disease and disability and maintaining physical and mental functioning showed a high rate of achievement, while the domain of active engagement with life showed a low rate of achievement compared to others. Successful aging was influenced by gender, age, subjective economic status, subjective health status, and family support, particularly spouse's support. CONCLUSION: Intervention programs to increase the prevalence of successful aging are required. Especially programs for strengthening spouse's support and enhancing subjective health status are required.

Growth factor-stimulated phospholipase D (PLD) catalyzes the hydrolysis of phosphatidylcholine (PC), generating phosphatidic acid (PA) which may act as a second messenger during cell proliferation and survival. Therefore, PLD is believed to play an important role in tumorigenesis. In this study, a potential mechanism for PLD-mediated tumorigenesis was explored. Ectopic expression of PLD1 or PLD2 in human glioma U87 cells increased the expression of hypoxia-inducible factor-1alpha (HIF-1alpha) protein. PLD-induced HIF-1 activation led to the secretion of vascular endothelial growth factor (VEGF), a HIF-1 target gene involved in tumorigenesis. PLD induction of HIF-1alpha was significantly attenuated by 1-butanol which blocks PA production by PLD, and PA per se was able to elevate HIF-1alpha protein level. Inhibition of mTOR, a PA-responsive kinase, reduced the levels of HIF-1alpha and VEGF in PLD-overexpressed cells. Epidermal growth factor activated PLD and increased the levels of HIF-1alpha and VEGF in U87 cells. A specific PLD inhibitor abolished expression of HIF-1alpha and secretion of VEGF. PLD may utilize HIF-1-VEGF pathway for PLD-mediated tumor cell proliferation and survival.

PURPOSE: The purpose of this study is to survey general and clinical characteristics of a group of relapsed patients with pulmonary tuberculosis, who are registered at a public health center in order to provide basic data for treatment of a patient's relapse, follow-up examination after the first treatment and health education. SUBJECT: Among 68 relapsed patients with pulmonary tuberculosis registered at two public health centers in Seoul from Jan, 1, 2002 to Dec. 31, 2002, 50 patients were selected as research subjects because they were contactable by telephone and they agreed to be a research subject. METHOD: The research carried out the structured questionnaire survey by telephone and surveyed research subjects' general and clinical characteristics, diseases except tuberculosis, drinking and smoking habit and knowledge about pulmonary tuberculosis treatment. Furthermore, the results of x-ray, whether a cavity existed or not, bacteriologic test, culture examination and drug resistance examination were examined and interval to relapse and history of dose attitude were surveyed. RESULTS: Among relapsed tuberculosis patients, male patients were 60% and they were more than women(40%). Patients over 60 were 42% most. In their occupation, 30% of them were unemployed and 50% of patients surveyed had monthly income below one million won. According to the findings in radiographic extent, most 50% of them were minimal, 44% were moderately advanced and 2% were far advanced. Patients who had cavity were 30%. According to the result in bacteriologic test, 54% showed positive smear pulmonary tuberculosis and 50.0% positive culture. According to the result in drug resistance, 24% showed drug resistance against one drug and 76% showed sensitive response. 46% had experienced medication side effects. According to interval to a relapse, 32% were relapsed less than 2 years, 14% less than 3-5 years and 54% more than 5 years. Average interval before a relapse were 11.6 years(11.6+/-11.15). There was a difference between their past regimen and their present regimen(P=0.001). The attitude towards taking a medication was assessed and they showed higher compliance with medication at present than in the past(P=0.001). In a case of smoking rate during treatment, the rate of non-smokers, who had been smokers, were higher(P=0.047). CONCLUSION: As a result, in order to prevent the disease from being relapsed and to increase the rate of treatment effectiveness, we need to make patients recognize the importance of regular follow-up examination and we should control thoroughly the patients. Furthermore, we need to provide them with more health education not to stop taking a medication earlier.
