RESUMO

Spontaneous fistula formation between the ureter and duodenum is extremely rare. The underlying cause of fistula between upper urinary tract and GI system is tuberculosis, chronic inflammation, carcinoma and GI diseases. Renal pelvis adenocarcinoma is also rare. We report a case of ureteroduodenal fistula associated with adenocarcinoma of the renal pelvis in 65-year old female with a brief review of literature.

RESUMO

Male infertility occurs after spinal cord injury (SCI). However, little is known about the effect of SCI on male reproductive function. We evaluated objective assessment of spermatogenesis of spinal cord injured rats by quantitative micrometry and DNA flow cytometry analysis at various time intervals. SCI was produce(B in 30 rats by 710 spinal cord transection. Controls received similar surgery without transaction. From 10 randomly selected seminiferous tubules per subject the mean number of mature spermatids, Sertoli cells, tubular diameter and tubular wall thickness were determined in both groups. The percentage of haploid, diploid and tetraploid cells were determined of each subject by DNA (low cytometry, Two way analysis of variance (ANOVA) was performed. In the SCI rats the mean number of spermatid per tubule was significantly lower (p=0.01) than that of controls. The mean number of Sertoli cells per tubule was not different from that of controls. Mean of spermatid to Sertoli cell ratio per seminiferous tubule in SCI rats was significantly lower than in controls (p=0.048). No significant differences of tubulr wall thickness, diameter and percentage of haploid cells were noted between both groups. These results suggest that the effect of SCI on spermatogenesis is associated with inadequacy of spermatid formation and maturation delay of spermatid.

RESUMO

Neuroblastoma originate in neural crest and is one of the most frequent tumor next to leukemia and brain tumor in child. The tumor is most highly malignant tumor due to early metastasis at diagnosis. It could be diagnosed by various diagnostic modalities such as roentgenologic examination, laboratory data and confirmed by pathologic diagnosis. Management of neuroblastoma is surgery, radiotherapy and chemotherapy. We experienced a case of adrenal neuroblastoma at age of 20-month male child and report with a brief review of literature

The bone scan in patients with prostate cancer remains to be the most sensitive method to assess the bone metastasis. But it is nonspecific and shows similar finding in either benign or malignant disease. Bone mineral densitometry allows for the quantification of total body bone mass. So we evaluated the efficacy of the measurement of the bone mineral density with dual-energy X-ray absorptiometry in patients with prostate cancer who had showed hot spot on the bone scan. Of the 15 patients 7 cases( 47% ) showed increased bone mineral density. To evaluate the efficacy of bone mineral densitometry, we analyze the 11 patients whose bone scan show hot spot on lumbar spine. Among them 2 cases( 18%) show osteoblastic finding on the bone x-ray, but 5 cases(45%) show increased density and 2 cases show decreased density on the bone mineral densitometry. These results show that the bone mineral densitometry is more valuable than bone x-ray in the detection of abnormal bone density. Therefore we suggest that bone mineral densitometry can be used as a helpful method of bone scan to detect the metastatic bone lesion in the advanced prostatic cancer.

We compared the prophylactic efficacy of intravesical BCG with thiotepa instillation after complete removal of superficial bladder cancer. Forty patients received BCG and 32 received thiotepa. The mean followup period was 29 months in BCG group, and 34 months in thiotepa group. The overall recurrence rate was 35% in BCG group compared with 53% in thiotepa group, showing difference statistically(p0.05). The overall recurrence index per 100 patient-months was also lower for the BCG versus the thiotepa group(1.21 versus 1.56, p>0.05). One patient in the BCG group and 4 in the thiotepa group had recurrent tumors with progression in stage. Side effects were irritative voiding symptoms(100%), fever and chills(10%), and inguinal lymphadenitis(2.5%) in BCG group, while irritative voiding symptoms(25%), fever and chills(6.3%), and myelosuppression (6.3%) in thiotepa group. Our results suggest that BCG is significantly superior to thiotepa in reducing bladder tumor recurrence and in retarding tumor progression.

RESUMO

Ectopic thoracic kidney is an extremely rare developmental anomaly. Most cases are discovered incidentally on a routine chest roentgenogram. Since the ectopic thoracic kidney may be mistaken for a space-occupying lesion originating above the diaphragm, it is of clinical and diagnostic importance to the thoracic surgeon. We report a case of ectopic thoracic kidney that was diagnosed by chest roentgenogram, intravenous pyelogram, computerized tomogram, and aortogram.

RESUMO

Vesicovaginal fistula is a distressing complication which may follow total hysterectomy, prolonged labor, chemical cauterization due to uterine prolapse. radiation therapy, and cervix cancer. The management of vesicovaginal fistula still remains controversial in regard to the type of approach and timing of repair. A review of experiences with vesicovaginal fistula repair was done on 39 cases admitted to the Department of Urology, Chonnam University Hospital, during the period from January, 1977 to December. 1991. The most prevalent age group was the forties to fifties (25/39 cases, 64.1%). The most common location of vesicovaginal fistula was the trigone (19/39 cases, 48.7%). The highest range of size of vesicovaginal fistula was that below 1.0 cm (24,39 cases, 61.5%).Of 39 cases. 14 occurred after total hysterectomy (35.9%). 13 chemical cauterization (33.3%). 8 prolonged labor(20.5%). 2 radiation therapy (5.1%). 1 Cesarean section (2.6%), and 1 Dilatation & Curettage (2.6%). The success rate of the initial repair was 88.5% (23,26 cases) by transvaginal repair. 83.3% (5/6 cases) by transperineal transvesical repair. 60.0% (3,5 cases) by suprapubic transvesical repair. 100.0% (1/1 case) by colpotleisis, and l00.0% (1/1 case) by ileal conduit, respectively. The transvaginal repair was most simple, and required the least time of average 96 minutes. Of six cases failed in the initial repair, one case was cured with the second repair and two cases with the third repair, with overall cure rate of 92.3% (36/39 cases).
