RESUMO

Further understanding of male human papillomavirus (HPV) infection is necessary to prevent infection in men, as well as transmission to women. In our current study, we investigated patterns of HPV infection and genotype distributions in male genital warts using the Anyplex II HPV28 Detection kit. We reviewed the medical records of 80 male patients who presented to 5 neighborhood clinics in Ulsan, Korea, for the treatment of genital warts between April 2014 and January 2015. All patients underwent HPV genotyping. The prevalence and characteristics of HPV infection were analyzed, and the patterns of HPV infection according to age were assessed. Among the study patients, 13 (16.3%) were negative for HPV infection, 46 (57.3%) were infected with low-risk HPV, and 21 (26.3%) were infected with high-risk HPV. Patients with multiple HPV infection were more likely to have high-risk HPV infection (P = 0.001). The prevalence of HPV infection was much higher in samples obtained by tissue excision due to a definite lesion (P = 0.001). There were no differences in high-risk HPV infection (P = 0.459), multiple HPV infection (P = 0.185), and recurrence at diagnosis (P = 0.178) according to age. HPV-6 and HPV-11 were the most common type overall (39.7% and 13.8%, respectively). HPV-16 and HPV-18 were the most common high-risk infections (both 3.4%). HPV infection is not only commonly encountered in male genital warts, but is also accompanied by high-risk HPV and multiple infections.

PURPOSE: The aim of this study is to evaluate the sequential histologic changes and spermatogenesis of the testis after extracorporeal shock wave treatment (ESWT). MATERIALS AND METHODS: ESWT to right testis was applied on total 48 immature, 5-week-old Sprague Dawley rats. The rats were sacrficed immediately, at 1 day, 3 days, 1 week, 2 weeks and 4 weeks after ESWT, and each group consisted of 8 rats including 2 rats as control (without ESWT). Histologic changes of both testes were examined, and serial three or more parafin sections were obtained, and immunohistochemical stains for bcl-2 and Hsp 70 and TUNEL method for apoptosis were performed. RESULTS: Hemorrhage and destruction of seminiferous tubules which appeared immediately after ESWT were maximal at 1 day after ESWT and were seen absorption and atrophy at 1 week after ESWT. Even regenerating tubules were found at 2 weeks after ESWT, no significant histologic difference between both testes experimental and control groups were seen except for some regenerating tubules. The right testicular tissues, remote from the ESWT-targeted area, showed preserved spermatogenesis since the age of 5 weeks 3 days, and was presented no significant difference between experimental and control groups. Hsp 70 and bcl-2 expression and apoptosis were maximal at 1 day after ESWT and persisted to 3 day after ESWT. Weak and focal expression were noted at 1 week after ESWT, and mostly disappeared at 2 weeks after. CONCLUSIONS: Spermatogenesis of right testis except the targeted area was not diffrent compared with that of left testis and control group. Hsp-70 expressin, which limits tissue destruction after injury, was immediately appeared for defense mechanism, and hemorrhage and tissue destruction following ESWT seemed to be due to apoptosis controlled by bcl-2 expression.

Suprapubic cystostomy by trocar is a simple, safe and useful method for urinary diversion in acute urinary retention. But rarely, severe complications occur during or after suprapubic cystostomy such as bowel perforation, through and through perforation of the bladder, rectal injury and severe hematuria. We studied the complications of suprapubic cystostomy retrospectively and compared 19 patients who performed ultrasonographic examination with 30 patients without ultrasonographic examination. The complications in 30 patients without preliminary ultrasonographic examination were initial failure in 4 cases (13.3%), extravesical Foley indwelling in 2 cases (6.7%), puncture site closed to the pubic bone in 2 cases (6.7%), spontaneous bladder rupture in 1 case (3.3%) and bowel injury in 1 case (3.3%). But, the complication in 19 patients with preliminary ultrasonographic examination was only 1 retried case (5.3%) because of extravesical Foley indwelling. As above results, the brief preliminary ultrasonographic examination of the bladder is a very useful method with minimal risk during suprapubic cystostomy by trocar.

RESUMO

Vesical dome tumors are classified, according to their origin, as either vesical or urachal. A case of vesical dome tumor was diagnosed as an urachal carcinoma preoperatively because CT scan and cystoscopic findings were suitable to the diagnostic criteria of urachal carcinoma, but the transurethral biopsy result was transitional cell carcinoma (TCC). The final histopathologic report after extended partial cystectomy revealed that the tumor was originated from vesical mucosa and an intact urachal remnant was found around the resected mass. In a tumor situated on the midline of bladder dome, a transurethral biopsy before curative operation is essential.

The ultrasonographic features in 26 cases of blunt testicular trauma were reviewed. Ultrasonography diagnosed correctly in 11 surgically proven cases of testicular rupture. There were three false-positive cases, two with a large scrotal hematocele and one with epididymis rupture. there were no false-negative cases. The sensitivity and specificity of ultrasonographic diagnosis were 100% and 80% respectively. The positive and negative predictive values were 78. 6% and 100% respectively. Therefore, the negative diagnosis by ultrasonography can warrant conservative therapy.

We designed this study to investigate the effects of adenosine on penile erection in Sprague- Dawley rats, in vivo. This was performed by simultaneous monitoring intracavernosal pressure( ICP) and systemic blood pressure with UROCOMPACT 6000 plus( WIEST, Germany) after intracavernosal injection of adenosine, papaverine and adenosine plus papaverine. Increment of maximal ICP was observed in the adenosine treated-group (0.015 to 0.15mg, n=30) in dose dependent manner, i.e., 15.8+/-0.95cmH2O with 0.015mg adenosine to 44.6+/-2.72cmH2O with 0.15mg adenosine, in mean ICP(n=l0, P<0.05). A similar finding was observed in the papaverine treated-group (0.015 to 0.15mg, n =30) in dose dependent manner. i.e., 28.1+/-2.88cmH2O with 0.015mg papaverine to 62.3+/-4.93cmH2O with 0.15mg papaverine, in mean ICP (n=10, P<0.05). The additive effect of adenosine plus papaverine treated-group( 0.015 + 0.015 to 0.15 +0.l5mg, n=30) was to enhance the results obtained by adenosine or papaverine alone in dose dependent manner. i.e., 50.7+/-1.71cmH2O with 0.015mg + 0.015mg papaverine to 79.5+/-5.63cmH2O with 0.15mg adenosine + 0.15mg papaverine. in mean ICP(n= 10, P<0.05). These results suggest that adenosine plays an important role on penile erection in rats. Adenosine may be useful for the future management of impotence, although the clinical effect remains to be investigated.

RESUMO

Nephrogenic adenoma is rare, especially in the ureter. It is thought to represent a metaplastic phenomenon forming the tubules which resemble renal tubules. The cause is unknown but may be associated with chronic irritations such as surgical trauma, incarcerated calculi and infection. We report a case of nephrogenic adenoma of the ureter in a 44-year-old woman who bad been suffered from the left flank pain due to left ureteral stone with giant hydronephrosis.

RESUMO

Priapism is a well recognized complication of some oral medications. Those most commonly cited are: antipsychotic drugs, especially the phenolhiazine: trazodone and chlorpromazine :and the antihyperlensive drugs hydralazine, guanethidine and prazocin. We report a case or priapism associated with the use of chlorpromazine who experienced 2 episodes or priapism and never experience priapism after cessation of the drug.

RESUMO

Renal cell carcinoma has been recognized for several years as a relatively common cause of paraneoplastic syndromes. Fever, polycythemia, hypercalcemia, amyloidosis, abnormal liver function, Cushing's syndrome and neuropathy have been ostm frequently reported in association with renal cell carcinoma, but leukemoid reactions have been reported only rarely with this tumor. Recently we experienced a leukemoid reaction in a 64-year old female patient who presented with an advanced case of renal cell carcinoma. According to the literature the mortality is 100% with leukemoid reactions in renal cell carcinoma, and indeed this patient expired. We therefore, consider this reaction as a late manifestation of a bizzare presentation of renal cell carcinoma.

RESUMO

We report a case of vesico-sigmoido-appendiceal fistula from unknown origin in a 29 years old female who was presented with the chief complaints of fecaluria and bladder irritation symptoms The diagnostic procedure such as excretory urography, cystography, barium enema, cystoscopy and proctoscopy were performed, and the vesico-recto-appendiceal or vesico-sigmoido-appendiceal fistula was suspected. On operation, the tip of appendix was communicated with the sigmoid fistulous tract and opened into the bladder in a common pathway. The patient underwent appendectomy, sigmoid fistulectomy, and partial cystectomy and the postoperative course was uneventful.
