RESUMO

PURPOSE: To analyze the clinical and radiological differences of posterior cruciate-retained Total Knee Arthroplasty (TKA) patients with those of posterior cruciate-sacrificed TKA patients. MATERIALS AND METHODS: The number of cases used in this study was 83 knees (51 pts) in group I and 30 knees (24 pts) in group II. Patient's average age was 57 years in group I and 59 years in group II. The follow-up period ranged from 1 year to 3 years in both groups. RESULTS: The ROM of knee was 84.6 degree preoperatively and 109 degree postoperatively in group I, and 80.6 degree and 110 degree in group II. Preoperative flexion contracture was 11.0 degree in group I and 22.3 degree in group II, and was decreased in both groups. Preoperative tibiofemoral angle (TFA) was varus 3.4 degree in group I and varus 3.9 degree in group II. Postoperatively, it was corrected as valgus 6.0 degree in group I and valgus 6.2 degree in group II. The Knee Rating Scale improved from 56.6 to 89.7 points in group I and 50.7 to 88 points in group II. CONCLUSIONS: According to clinical and radiological assessments there were no significant differences between the two groups.

RESUMO

The prime objective in treating the intertrochanteric fracture is to achieve bony stability at the fracture site using an appropriate fixation nail plate to maintain a stabilized position. Since the introduction of the Smith-Petersen nail, numerous internal fixation devices have been developed. Complications after surgical treatment of intertrochanteric fractures of the femur though rarely resulting in non-unions or aseptic necrosis of the femoral head, varus deformities or delayed unions are frequent. Recently a compression hip screw is popular because it can provide more secure internal fixation. The authors treated 41 cases of intertrochanteric fractures of the femur during a 5. 5 year period from January, 1975 to June, 1980. 1. Of 41 cases of intertrochanteric fractures, 9 cases were stable fractures, and 32 cases were unstable. 2. Of 4I cases of intertrochanteric fractures, 9 were treated conservatively, and 32 were treated with open reduction and internal fixation. Of the fixation devices, compression hip screws were used in 17 cases, Jewett nail plates in 12 cases, Mclaughlin plate in 2 cases, and a Judet plate in a case. 3. Of the 32 cases treated by open reduction, reduction with medial displacement by the Dimoe- Hughston method was used in 10 cases, 3 of which were fixed with compression hip screws and 7 with Jewett nails. The remaining 22 were anatomically reduced. 4. The average time until weight bearing in conservatively treated patients began was 17. 7 weeks but in the surgically treated patients weight bearing started after 5. 5 weeks, and in patients treated with a compression hip screw patient conld bear weight 3. 7 weeks after surgery. 5. The average fracture union time was 16. 6 weeks in conservatively treated patienta, 16. 3 weeks in operatively treated patients (14. 6 weeks in compression hip screw fixation group, 18 weeks in Jewett nail, 21 weeks in Mclaughlin plate,16 weeks in Judet plate). 6. There were complications in 8 cases; 3 in the conservatively treatment group, and 5 in the internal fixation group. Of 5 internal fixation cases, 2 complications were caused by technical error. 7, After comparison of the result of treatment obtained by 4 different types of iaternal fixation devices, our conclusion is that compression hip screw is the device of choice for treatment of intertrochanteric fractures.

RESUMO

The Scalenus anticus syndrome is a definite clinical entity and the result of pressure upon the component parts of the brachial plexus and the subclavian artery by the scalenus anticus, scalenus medius, scalenus minimus muscle and the first rib. This case, a fifty one years old male, complained of pain, tingling sensation in the right shoulder and arm. Roentgenograms showed no specific finding. Symptom was dramatically relieved by the scalenotomy. Six years later he had no complaints and no evidence of recurrence.

RESUMO

Hemophilia is a sex-linked recessive hereditary bleeding disorder occurring only in the male and transmitted by the female. This disease is characterized by a bleeding tendency due to prolonged coagulation time causing by deficiency of one of three plasma factors, such as anti-hemophilie globulin(A. H. G.), plasma thromboplastin component(P. T. C.) and plasma thromboplastin antecedent (P. T. A.) for first phase of coagulation process. The majority of hemophilia, 74% is due to a deficiency of A. H. G., 15% to a deficiency of P. T. C. and remaining 11% to a deficiency of P. T. A. as outlined in the literature. This case, 18 years old male, is complained of repeated hemorrhagic manifestations, residual deformity with stiffness of the hip and knee joints, and limping following minor trauma has developed since early childhood. Radiologically, the hip and knee joints were involved, showing the findings of typical hemophilic arthropathy. Laboratory finding showed markedly prolonged coagulation time. Diagnosis was confirmed to be plasma thromboplastin antecedent deficiency homophilia.

RESUMO

A case of pigmented villonodular synovitis developed in the right ankle joint is reported. The patient was a 25 years old house wife who had complained marked swelling and tenderness over the anterolateral aspect of the right ankle for 2 years. Roentgenological studies showed marked swelling of the ankle joint and erosion of the neck of the talus. Synovectomy of the affected joint was performed. The tumor mass, measuring about 7cm×5cm×2cm in size, was yellowish brown in colour, elastic and soft in consistency, and contained several well encapsulated nodules.
