RESUMO

EC-18 (monoacetyldiacylglyceride) stimulates T cell production of IL-2, IL-4, IL-12, IFN-gamma, and GM-CSF in vitro. To study the effects of these cytokines stimulated by EC-18 on cancer cells, we applied hamster biliary cancer model, a difficult cancer to treat. Cancer (KIGB-5) cells were given intravenously to produce hematogenous metastatic lung lesions which were treated with EC-18 at 10, 25, and 50 mg/kg/day respectively. The fourth group was untreated control. At 4th, 8th, and 12th week the lungs were examined. EC-18 treated groups showed only a few microscopic lung lesions and no evidence of metastatic lesion with highest dose whereas widespread gross lung lesions were observed in untreated control. To investigate whether the anti-tumor effect of EC-18 is associated with suppression of tumor cell Toll-like receptor 4 (TLR-4) expression in addition to stimulation of the immune cells, KIGB-5 cells were exposed to LPS with or without EC-18. TLR-4 mRNA and protein expression, measured by reverse transcriptase PCR (RT-PCR), real-time quantitative PCR and western blot analysis, showed suppression of TLR-4 expression in KIGB-5 cells treated with EC-18 compared with control. In conclusion, EC-18 has a significant anti-tumor effect in this experimental model of biliary cancer suggesting potential for clinical application to this difficult cancer.

A subcapsular liver hematoma (SLH) is a relatively common lesion in fetuses and neonates. Although an SLH ruptures rarely, it may be life threatening. We report on a term neonate with a delayed rupture of an SLH that occurred on day 7 of life. The infant had been resuscitated with intubation, positive pressure ventilation, and chest compression at birth because of meconium-associated perinatal depression. The SLH was diagnosed by abdominal ultrasonography and paracentesis, and the ruptured SLH was treated operatively. After intensive medical and surgical management, the infant was discharged healthy on day 27 of life. A newborn infant presenting with the sudden onset of extreme shock and pallor associated with abdominal distension should undergo differential diagnosis for SLH and a clinical evaluation concurrent with fluid resuscitation and timely surgery.

Pulmonary hypoplasia(PH) commonly occurs in association with oligohydramnios and other congenital anomalies, especially congenital diaphragmatic hernia (CDH). Pulmonary hypoplasia is an important factor, as persistent pulmonary hypertension, in the prognosis of CDH. In some reports, there is a decrement of pulmonary surfactant in PH accompanying CDH. Recently, there are some reports that exogenous pulmonary surfactant therapy is effective in experimental animal model and neonatal respiratory distress with PH. We report a case of a 5 day-old male neonate, who had shown dyspnea and diagnosed as left pulmonary hypoplasia accompanying CDH. The CDH was surgically treated and the ipsilateral PH, with intratracheal administration of exogenous pulmonary surfactant postoperatively. After exogenous pulmonary surfactant application, the left lung volume was increased on chest roentgenogram and lung perfusion scan findings, and there was an improvement in oxygenation and clinical manifestations. We suggest that postoperative exogenous pulmonary surfactant replacement therapy is effective in the case of PH and further trials are needed to clarify the optimal dose and timing of supplementation of surfactant for treatment of infants with PH accompanying CDH.

Fine needle aspiration biopsy cytology is a widely recognized and useful technique which can provide diagnosis in lesions of the head and neck, enabling appropriate management plans for individual patient to be made. Fifty one fine needle aspirates from salivary gland masses were examined. Four aspirates(8%) were inadequate for examination. Of the remaning 47 samples, 42 cases(82%) were benign lesions which consist of 30 pleomorphic adenoma(58%), 7 inflammatory lesion(14%) 4 Warthin's tumor (8%) and 1 benign lesion(2%). Two cases(4%) were atypical lesions. Three case(6%) were malignant lesions consisting of 2 adenoid cystic carcinomas(4%) and 1 mucoepidermoid carcinoma(2%). The cytologic diagnoses were compared with the subsequent histologic diagnosis of surgical resected specimen in 24 cases. 19 cases of 21 aspirates from benign tumors were correctly diagnosed by fine needle aspiration cytology, with a specificity of 90%. All 3 aspriates from the 3 patients with malignant tumor were correctly diagnosed by fine needle aspiration cytology, with a sensitivity at 100%. Overall acurracy was 88%. Diagnostic error was encountered in adenoid cystic carcinoma, mucoepidermoid carcinoma and warthin's tumor. Correct histologic diagnosis was made in 86% of benign tumors(84% for pleomorphic adenoma and 100% for Warthin's tumor) and in 100% of malignant tumors.
