Objectives@#:The purpose of this study was to quantitatively assess the degree of Graves’ Disease affecting mental health through the results of multiphasic military personality inventory (MPI) in 19-year-old men. @*Methods@#:We collected and analyzed the results of the MPI for 592 healthy subjects and 148 Graves’ disease patients among the 19-year-old men who visited the Military Manpower Administration from February 2008 to January 2010. @*Results@#:Among demographic characteristics, both Graves’ disease and control group are 19-year-old men. Educational level, socioeconomic status, parental relationship, and BMI were not statistically different between the two groups (χ2 =0.089, p=0.766; χ2 =5.090, p=0.278; χ 2 =5.988, p=0.112 ; t=-0.635, p=0.526). In the MPI, among validity scales, the score of the faking-good subscale was significantly lower in the Graves’ group (t=3.507, p<0.001), but in the faking-bad and infrequency subscales scores were not significantly different between two groups (t=-1.700, p=0.090; t=-0.519, p=0.604). Among the neurosis scale, anxiety, depression, somatization, and personality disor-der subscale scores were all significantly higher in Graves’ disease group (t=-3.323, p<0.001; t=-4.210, p<0.001; t=-6.202, p<0.001; t=-2.872, p<0.01). Among the psychopath scale, each score of schizophrenia and paranoia subscales were not statistically different between the two groups (t=-0.158, p=0.874; t=-0.846, p=0.398). @*Conclusions@#:According to MPI result, we can confirm that Graves’ disease patients in 19-year-old-men may appear features such as anxiety, depression, somatization and personality tendency (introversion or impulsivity) than the control group.

Background@#It is getting important to improve the oral health status of the elderly because oral health status may affect their health status of the whole body. In this respect, we aimed to explore the association of oral health status and behavior factors with self-rated health status by sex. @*Methods@#Using the data from the 7th Korea National Health and Nutrition Examination Survey for health surveys and oral examinations (2016–2018), we analyzed a total of 3,070 people aged 65 or older (men: 1,329; women: 1,741). Our dependent variable, self-rated health status, was divided into two groups: not good (bad and very bad) and good (very good, good, and fair), whereas our independent variables of interest were oral health status and behavior factors. In addition to descriptive analysis and the Rao-Scott chi-square test, reflecting survey characteristics, we conducted hierarchical multivariable logistic regression analyses adjusted for socio-demographics and health status and behavior factors. All analyses were stratified by sex. @*Results@#The proportion of people having ‘not good’ self-rated health was 36.5% in women but 24.5% in men. In a model adjusted for all covariates, the self-rated health status showed significant association with the self-rated oral health status. For example, in men, the risk of having ‘not good’ self-rated health was high in people having ‘poor’ (odds ratio [OR], 5.31; 95% confidence interval [CI], 2.34–12.03) self-rated oral health status and in those having ‘fair’ (OR, 4.03; 95% CI, 1.68–9.70) in comparison with those having ‘good’ self-rated oral health status. Dental status regarding speaking difficulty seemed to be very important in influencing self-rated health status. For instance, in women, compared to people having ‘no discomfort’ speaking difficulty, the risk of having ‘not good’ self-rated health was high in people having ‘not bad’ (OR, 1.60; 95% CI, 1.14–2.24) and ‘discomfort’ (OR, 1.79; 95% CI, 1.30–2.47) speaking difficulty. The covariates significantly associated with the risk of having ‘not good’ self-rated health were: physical activity, chronic disease, stress, and body mass index in both sexes; health insurance type and drinking only in men; and economic activity only in women. @*Conclusion@#Oral health status and behavioral factors were associated with self-rated health status among the elderly, differently by sex. This suggests that public health policies toward better health in the elderly should take their oral health status and oral health behaviors into account in a sex-specific way

Background@#Metabolic syndrome has been known as a risk of cardiovascular disease. Meanwhile, high sensitivity C-reactive protein (hs-CRP) is used as a predictor of cardiovascular disease. In this paper, we aimed to investigate the association between hs-CRP and metabolic syndrome.Method: A total of 7,633 were chosen as the study population from the 7th Korea National Health and Nutrition Examination Survey dataset (2016–2017). Our dependent variable was whether an individual had metabolic syndrome or not, and the independent variable of interest was hs-CRP which was categorized into three groups. The chi-square tests and hierarchical logistic regression analyses reflecting survey characteristics were conducted. All analyses were stratified by gender. @*Results@#According to the adjusted model with all covariates, compared to individuals having the low risk of hs-CRP, those having its average risk were more likely to have metabolic syndrome in men (odds ratio [OR], 1.41; 95% confidence interval [CI], 1.12–1.76) and women (OR, 1.69; 95% CI, 1.33–2.16). Individuals having the high risk was not significantly different in men; however, they were more likely to have metabolic syndrome in women (OR, 2.03; 95% CI, 1.28–3.23). @*Conclusion@#In an upcoming aging society, it is important to reduce the risk of metabolic syndrome to improve population health. This study suggests that hs-CRP may be used as a marker of the risk of metabolic syndrome in a gender-specific way, thereby contributing to enhancing awareness of the risk of metabolic syndrome among the general public.

Oral sulfate solution (OSS) is an emerging cleansing agent for bowel preparation. However, data comparing OSS to other conventional bowel preparations in Asian patients are limited. Therefore, the objective of this study was to compare the efficacy and tolerability of OSS to ascorbic acid plus polyethylene glycol (AA + PEG) in Asian patients. Methods: This was a prospective, randomized, parallel, investigator-blind study performed in two university hospitals in Korea. Bowel preparation efficacy was evaluated using both the Ottawa Bowel Preparation Scale (OBPS) and Boston Bowel Preparation Scale (BBPS). Results: Among 173 patients, 86 received OSS while 87 received AA + PEG for bowel preparation. Total OBPS score was 2.80 ± 2.48 in the OSS group and 4.49 ± 3.08 in the AA + PEG group, indicating significantly (p < 0.001) better efficacy with OSS. Total BBPS was higher in the OSS group (7.43 ± 1.49 vs. 6.51 ± 1.76, p < 0.001), indicating superior bowel preparation quality with OSS. Preparation-related adverse events were generally acceptable. Patients receiving OSS had more nausea (1.92 ± 0.94 vs. 1.54 ± 0.76, p = 0.004) and abdominal cramping (1.45 ± 0.78 vs. 1.17 ± 0.51, p = 0.006) than those receiving AA + PEG. However, overall satisfaction and taste were similar between the two groups. Conclusions: OSS had a non-inferior bowel cleansing efficacy than AA + PEG regardless of colon segment.

OBJECTIVE: This study is a retrospective cost-benefit analysis of cervical anterior interbody fusion and cervical artificial disc replacement, which are the main surgical methods to treat degenerative cervical disc disease. METHODS: We analyzed 156 patients who underwent anterior cervical disc fusion and cervical artificial disc replacement from January 1, 2008 to December 31, 2009, diagnosed with degenerative cervical disc disorder. In this study, the costs and benefits were analyzed by using quality adjusted life year (QALY) as the outcome index for patients undergoing surgery, and a Markov model was used for the analysis. Only direct medical costs were included in the analysis; indirect medical costs were excluded. Data were analyzed with TreeAge Pro 2015™ (TreeAge Software, Inc, Williamstown, MA, USA). RESULTS: Patients who underwent cervical anterior fusion had a total cost of KRW 2501807/USD 2357 over 5 years and obtained a utility of 3.72 QALY. Patients who underwent cervical artificial disc replacement received 4.18 QALY for a total of KRW 3685949/USD 3473 over 5 years. The cumulative cost-effectiveness ratio of cervical spine replacement surgery was KRW 2549511/QALY (USD 2402/QALY), which was lower than the general Korean payment standard. CONCLUSION: Both cervical anterior fusion and cervical artificial disc replacement are cost-effective treatments for patients with degenerative cervical disc disease. Cervical artificial disc replacement may be an effective alternative to obtain more benefits.

OBJECTIVES@#The purpose of this study was to quantitatively assess the degree of myasthenia gravis (MG) affecting mental health through the results of multiphasic military personality inventory in late adolescence.@*METHODS@#We collected and analyzed the results of the military personality inventory for healthy controls and MG patients among the 19-year-old men who visited the Military Manpower Administration from February 2007 to January 2010. Military personality inventory invented for Korean military test has similar system to Minnesota multiphasic personality inventory.@*RESULTS@#Among validity scales, each score of faking good, faking bad and infrequency subscales showed no difference between healthy control and MG groups, respectively (t=−0.51, p=0.607 ; t=0.11, p=0.913 ; t=1.41, p=0.158). Among neurosis scales, the score of somatization subscale was higher (t=2.29, p=0.023) in MG group. Among psychopath scales, the score of schizophrenia subscale was lower in MG group (t=−2.38, p=0.018).@*CONCLUSIONS@#According to MPI results, we can confirm that MG patients in late adolescence may be more concerned with their somatic symptoms than the control group, and that they may be more likely to abide by the regulation and be conventional in their behavior than the control group.

BACKGROUND@#As South Korea is becoming an aged society very rapidly, the increase in osteoarthritis prevalence raises various public health issues in this country. This study aimed to explore the socio-demographic factors associated with osteoarthritis in the current Korean elderlies.@*METHODS@#Using the National Survey of Korean Elderly data (2014, 2017), we analyzed 20,326 elderlies (males, 8,248; females, 12,078) and conducted multivariate logistic regression analyses by sex. The dependent variable is whether a participant was diagnosed with osteoarthritis or not. Independent variables of interest is socio-demographic factors such as age, marital status, household type, residential area, household income, religion, the longest job, the number of close relatives, the number of close friends, and survey year. Control variables are various health behavioral factors and disease-related factors.@*RESULTS@#Prevalence of osteoarthritis was 19.2% in male elderlies and 47.0% in female elderlies. According to the results of the multivariate logistic regression adjusted for all studied control variables, the odds ratio (OR) for having osteoarthritis of under-elementary school graduates (their counterpart was college graduate group) was 1.81 (95% confidence interval [CI], 1.29–2.52) in males and 1.67 (95% CI, 1.12–2.47) in females. The OR of those having a job in agriculture & forestry fishery as their longest job (their counterpart was those who had never participated in labor force during their lifetime) was 5.07 (95% CI, 1.46–17.58) in males and 1.49 (95% CI, 1.27–1.74) in females. In males, the second-low quartile group in household yearly income (their counterpart was the highest quartile group) had the OR of 1.22 (95% CI, 0.98–1.53). In females, the OR of those having a religion of Buddhism (their counterpart was those having no religion) was 1.20 (95% CI, 1.07–1.35) and the OR of those who had no relatives (their counterpart was those having more than 3 close relatives) was 1.31 (95% CI, 1.10–1.56).@*CONCLUSION@#This study found that in Korean elderlies, education, the longest job, household income, religion, and the number of close relatives are associated with their suffering from osteoarthritis. Further study and designing appropriate interventions are needed to alleviate current and future individual and socioeconomic burdens of osteoarthritis in an aged society like South Korea.

OBJECTIVE: We investigated the prevalence and risk factors of physically abusive behaviors (PhAB) and psychologically abusive behaviors (PsAB) towards people with dementia (PWD) in family caregivers, and compared their prevalences between East Asian and Western countries. METHODS: We estimated the prevalence and risk factors of PhAB and PsAB in 467 Korean pairs of community-dwelling PWD and their primary family caregivers. We evaluated abusive behaviors using the Modified Conflict Tactics Scale. In addition, we compared the prevalence of abusive behaviors between Asian and Western countries through a meta-analysis on 12 studies including the current one. RESULTS: More than a half of the caregivers reported PsAB and about one out of seven caregivers admitted PhAB within past three months. PsAB and PhAB were slightly more prevalent in East Asian countries than in Western countries. Non-Alzheimer type and moderate to severe behavioral and psychological symptoms of dementia were associated with the risk of PhAB but not with the risk of PsAB. Severe care burden and low income were associated with the risk of PhAB and PsAB. CONCLUSION: PhAB and PsAB were as prevalent in the family caregivers of PWD in Asian countries including Korea as in Western countries. Prevention strategies should be implemented according to the type of abusive behaviors.

OBJECTIVES: The aim of this study was to identify the association between scoliosis and psychopathological effects using the Military Personality Inventory (MPI). METHODS: A retrospective cross-sectional study was conducted of 19-yr-old examinees admitted to the Military Manpower Administration in Korea from February 2007-January 2010. The authors compared the profiles of MPI of 3004 scoliosis cases with the profiles of MPI of 11920 normal controls. The scoliosis cases were classified into two groups according to severity, including mild to moderate, and severe. RESULTS: There was a significant difference between the two groups based on the validity scales. The neurosis scales, including anxiety, depression, somatization and personality disorder, were significantly different between the groups. Concerning the anxiety, depression and somatization subscales, the scores of the mild to moderate group and the severe group were higher than those of the control group. Regarding the personality disorder subscale, the mild to moderate group was higher than the control group. For the paranoid subscale, there was a significant difference between the groups and the mild to moderate group had a higher score than the control group. Furthermore, the partial eta square was 0.018, 0.019, and 0.018 for anxiety, depression, and somatization respectively, which suggests that scoliosis was significantly associated with symptoms of anxiety, depression, and somatization. CONCLUSION: Young men with scoliosis tended to have higher scores concerning the MPI than young men without scoliosis. In particular, the scores were higher for anxiety, depression, and somatization symptoms. This result shows that scoliosis may have psychopathological effects on young men.

RESUMO

An inverted hyperplastic polyp (IHP) found in stomach is rare and characterized by downward growth of hyperplastic mucosal component into the submucosa. Because of such characteristic, IHP can be misdiagnosed as subepithelial tumor or malignant tumor. In fact, adenocarcinoma was reported to have coexisted with gastric IHP in several previous reports. Because only 18 cases on gastric IHP have been reported in English and Korean literature until now, pathogenesis and clinical features of gastric IHP and correlation with adenocarcinoma have not been clearly established. Herein, we report a case of gastric IHP which was initially misdiagnosed as gastrointestinal stromal tumor and resected using endoscopic submucosal dissection. Literature review of previously published case reports on gastric IHP is also presented.

OBJECTIVES: The aim of this study was to identify associations between pneumo- or hemo-thorax presence and psychological distress in young males by using the Military Multiphasic Personality Inventory (MPI) test. METHODS: A retrospective cross-sectional study was conducted on data from 19-year-old examinees that were admitted to the Military Manpower Administration in Korea between February 2009 and January 2010. A total of 1955 young males were enrolled in this study. Among them, a normal volunteer control group (n=1561) included individuals without a pneumo- or hemo-thorax. The pneumo- or hemo-thorax group (n=394) included individuals with a pneumo- or hemo-thorax. The latter group was divided into two subgroups : Group A (treated with conservative care or chest tube insertion, n=341) and Group B (treated with wedge resection, n=53). RESULTS: We compared each of three groups (Control group, Group A, Group B) by using covariance analysis. The somatization subscale score within the neurosis category was significantly higher in Group A (p<0.001) than in the Control group, and there was a tendency toward a higher somatization score in Group B than in the Control group (p=0.073). The other score categories (validity scale ; anxiety, depression, and personality disorder subscales for neurosis scale ; and psychopath scale) showed no significant differences among the three tested groups. CONCLUSION: Conservative care or chest tube insertion group was associated with higher somatization symptom scores than that in the control group. The wedge resection group had a tendency toward a higher somatization score than that in the control group. The result indicate that individuals with a history of pneumo- or hemo-thorax may be more concerned about their body shape and/or general condition than those without such a history. Supportive intervention and psychiatric education approaches may be useful in relieving somatic distress.

RESUMO

An esophagomediastinal fistula is rare complication of nontuberculous mycobacterium infection. Here, we report the case of a patient with advanced acquired immunodeficiency syndrome who presented with a fever, cough, and dyspnea, and was eventually diagnosed with nontuberculous mycobacterium infection. Computed tomography revealed multiple lymphadenopathy with an esophagomediastinal fistula. The patient was treated with anti-mycobacterial medications and endoscopic fistula closure.

Objective personality tests, such as the Minnesota Multiphasic Personality Inventory (MMPI), might be more sensitive to reflect subclinical personality and be more state-dependent in an individual's lifetime, so they are good scales to predict the psychological distress regarding certain states. The aim of this study was to identify the specific pattern between body mass index (BMI) and psychological distress using the objective personality test. For this study, we investigated BMI and the Korean Military Multiphasic Personality Inventory (MPI). A retrospective cross-sectional study was conducted with 19-yr-old examinees who were admitted to the Military Manpower Administration in Korea from February 2007 to January 2010. Of 1,088,107 examinees, we enrolled 771,408 subjects who were psychologically apparent healthy possible-military-service groups. Afterwards, we reviewed and analyzed directly measured BMI and MPI results. In terms of the validity scales, the faking-good subscale showed an inverted U-shaped association, and faking-bad and infrequency subscales showed a U-shaped association with BMI groups. In terms of the neurosis scales, all clinical subscales (anxiety, depression, somatization, and personality disorder) also showed a U-shaped association with BMI groups. For the psychopath scales, the schizophrenia subscale showed a U-shaped association, and the paranoia subscale showed a near-positive correlation with BMI. In conclusion, a specific U-shaped pattern was observed between BMI and the MPI in 19-yr-old men in Korea. Underweight and obesity are related to psychological distress, so supportive advice and education are needed to them.

RESUMO

Although the nasogastric tube (NGT) is widely used in critically ill patients, most intensivists do not give much thought to it or its possible complications. NGT syndrome is a rare but fatal complication characterized by throat pain and vocal cord paralysis in the presence of NGT. Recently, we experienced a case of NGT syndrome developed in an 86-year-old female twelve days after NGT insertion. We immediately removed the NGT and secured the airway by tracheostomy. She was treated successfully with an intravenous antibiotic, steroid and proton pump inhibitor and the syndrome did not recur after reinsertion of the NGT.

RESUMO

Notch signaling is a key regulator of neuronal fate during embryonic development, but its function in the adult brain is still largely unknown. Mind bomb-2 (Mib2) is an essential positive regulator of the Notch pathway, which acts in the Notch signal-sending cells. Therefore, genetic deletion of Mib2 in the mouse brain might help understand Notch signaling-mediated cell-cell interactions between neurons and their physiological function. Here we show that deletion of Mib2 in the mouse brain results in impaired hippocampal spatial memory and contextual fear memory. Accordingly, we found impaired hippocampal synaptic plasticity in Mib2 knock-out (KO) mice; however, basal synaptic transmission did not change at the Schaffer collateral-CA1 synapses. Using western blot analysis, we found that the level of cleaved Notch1 was lower in Mib2 KO mice than in wild type (WT) littermates after mild foot shock. Taken together, these data suggest that Mib2 plays a critical role in synaptic plasticity and spatial memory through the Notch signaling pathway.
