RESUMO

Twenty cases of the rheumatoid arthritis was carried out the histopathological observation from the affected synovial membrane at the knee joint by the Cops biopsy needle, at the Department of Orthopedic Surgery, Chosun University Hospital. The authors researched the difference to compare with the histological finding and its clinical features. The following results were obtained. 1. The average age was 36.9 years old and the sex ratio between the male and female was 1:2.7. 2. The erythrocyte sedimentation rate and protein amont in the synovial fluid was elevated than the normal value. 3. The most common clinical sign was the morning stiffness with multiple joint pain and moderate swelling, tenderess in the involved joint. 4. The test of rheumatoid factors revealed positive about 70% in the serum and 100% in the joint fluid. 5. The characteristic histological finding on the light microscopical examination was the hyperplasia of surface lining cells, deposition of the fibrin with the fibrinoid necrosis, and vascular hypertrophic changes of the blood vessel in all of those cases. 6. The histological finding had no difference to compare with the clinical course.

RESUMO

The cases of 128 humeral fractures were evaluated at the Department of Orthopedic Surgery, Chosun University Hospital, during the six year period from January 1974 to December 1979 and the following results were obtained. 1. The high incidence of the humeral fracture was found under the age of 30 years and the sex ratio between the male and female was 3.6:1. 2. The most common causes of injuries were fallen down on the ground (36.7%) and the nexts were traffic accidents (34.4%). 3. Among the 128 cases of fractures, humeral shaft fractures were 33 cases (32%) under the age of 40 years and 47 cases (67.1%) of supracondylar fractures under the age of 20 years and 6 cases (54.5%) of neck fractures over the age of 60 years were noted. 4. The shapes of humeral fractures found out transverse fracture in 31 cases (65.9%) and the short oblique or spiral fracture in 35 cases (71.4%). 13 cases were combined with the open would to the fracture site (10.2%). 5. In the Correlation between the age and the union rate, sound union was more rapid under the age of 20 years than the aged person. Delayed unions were revealed 5 cases in transverse fractures, 2 cases in spiral fractures and 2 cases of nonunions were experienced on both segmental and transverse fracture of the shaft (1.6%). 6. The radial nerve paralysis revealed 5 cases (10%) at the shaft fracture and 2 cases (4.3%) Volkmanns ischemic contracture, 2 cases (4.3%) of cubitus varus, 2 cases (4.3%) of cubitus valgus were experienced on the complicution in there 47 cases of supracondylar fractures.

Two hundred and eighty six cases of the fracture of the tibial shaft treated at Orthopedic Department of the Chosun University Hospital during the period from January 1974 to December, 1978. The following results were obtained. 1. Most common cases of the tibial shaft fractures were traffic accidents (66.1%). 2. The incidence of trauma was high in the young man, most frequent in the 2nd and 3rd decade (43.0%). 3. The evaluation was done according to the classification of Ellis. Among 286 cases of the fractures of tibial shaft, 139 being mild severity, 70 moderate severity and 77 major severity. 4. The group of mild severity at the fracture of upper 1/3 of tibial shaft was shown the best prognosis and the average healing period was 9.3 weeks. 5. The wound infections in the open fractures of tibial shaft were revealed 37 cases (35%) and 8 cases of chronic osteomyelitis (8%). 6. Of 54 cases, we experienced 38 cases of delayed union (13.3%), 16 cases of non-union (5.6%) and the highest rate of delayed or non-union was shown in the group of major severity at the fracture of lower 1/3 of tibiai shaft. 7. The open reduction by the compression plating reduced the time of healing period and delayed the time of healing period by the intramedullary nailing.

31 cases of the trochanteric fractures were treated at the Department of Orthopedic Surgery, Chosun University Hospital, from Jan. 1975 to Dec. 1978 and the following results were obtained. 1. The sex ratio was 3:2 (male to female). 18 cases were in man, with the peak age at the 4th decade and 5th decade and 13 cases were in woman over the 6th decade. 2. The most common causes of the trochanteric fractures were traffic accident or fall in man and slipping in woman. 3. The features of the trochanteric fractures were mostly unstable type (60%) and 16 cases in the right hip (52%) and 15 cases in the left hip (48%). 4. Dimon-Hughston method and Wayne-Country method were used to the unstable fractures and anatomical reduction to the stable fracture was maintained by inserting the S-P nail, Jewett nail and Mclaughlin nail. 5. Post-operative cast immobilization was applled until clinical union was oftained on the fracture site and early weight bearlng was carried out for the acceleration of the bony union. 6. Complications were found in 50% of the group treated by conservative method, while it occurred in 21.7% of the group treated surgically.

RESUMO

Congenital pseudarthrosis of the tibia is a condition most often combined with cafe-au-lait spots and neurofibromatosis. It is notoriously difficult to attain a sound bony union. Compression plating and screw fixation with massive bone graft resulted in successful union in our case.

RESUMO

The clincal experience of thirteen patients with proven acute osteomyelitis under twelve years of age treated by surgical exprolation such as immediate decompression followed by continuous suction-irrigation. Five patients of them who had short duration from the onset improved while three patients who had long duration from the onset had persisting inflammatory change of the bone in spite of the regimen. The causating organisms were mostly staphylococcus aureus in bacterial cultures. The combination of gentamycin, minocin and penclox were the antibiotics of choice. Early diagnosis and early operation is advocated to attain good results in the treatment of acute osteomyelitis in the children.

RESUMO

Two cases of tendon transfer for paralysis of the radial nerve due to trauma in the humerus have been operated by transferation of foream flexors such as flexor carpi radialis, pronator teres and palmaris longus at the Department of Orthopedic Surgery, Chosun University Hospiatl. The one case was a open fracture at midshaft of the humerus with cut down the radial nerve in the distal third of the humerus and with contaminated severe muscle injuries. The other case was radial nerve injury at the level of supracondylar area of humerus by the wound of deep laceration. The operative procedure carried out two month later after neurorrhpy of the radial nerve. However, the result of these cases were exhibited very good motor and sensory function of the radial nerve during two year follow up study. A review of literatures is included.

RESUMO

A case of disseminated bone tuberculosis was reported, observed in a 23 year old male who visited with chronic ulcer on the midportion of the left upper arm and painful, tender swelling at the proximal part of the left tibia. Brief review was done regarding its pathogenesis and controversy of its nomenclature.

RESUMO

Anterior interosseous nerve paralysis observed in a 13-year-old boy with supracondylar fracture of the left humerus was reported. During the conservative management at a bone setter of the fracture, the gradual loss of pinch action of the thumb and index finger became conspicuous, and with operative reduction of the fracture and release of the fibrous compression and adhesion to the nerve at the ulnar head of the pronator teres, the paralysis could be relieved.
