RESUMO

In order to provide efficient medical care to atrial fibrillation patients in the community, the Huamu Community Health Service Center in association with its medical consortium, Renji Hospital have developed a novel atrial fibrillation management system. With the collaboration of general practitioners and specialist team from the tertiary hospital, a special clinic for atrial fibrillation has been set up in the community health service center, which is based on the internet technology and the medical consortium platform. This article introduces the development of this novel system and the initial outcome of the measures, to provide a reference for the management of atrial fibrillation patients in the community.

OBJECTIVE@#To systematically evaluate the protective effects of Humulus lupulus L. extract (HLE) on osteoporosis mice.@*METHODS@#In vivo experiment, a total of 35 12-week-old female ICR mice were equally divided into 5 groups: the sham control group (sham); the ovariectomy with vehicle group (OVX); the OVX with estradiol valerate [EV, 0.2 mg/(kg•d)] the OVX with low- or high-dose HLE groups [HLE, 1 g/(kg•d) and 3 g/(kg•d)], 7 in each group. Treatment began 1 week after the ovariectomized surgery and lasted for 12 weeks. Bone mass and trabecular bone mircoarchitecture were evaluated by micro computed tomography, and bone turnover markers in serum were evaluated using enzyme-linked immunosorbent assay (ELISA) kits. In vitro experiment, osteoblasts and osteoclasts were treated with HLE at doses of 0, 4, 20 and 100 µg/mL. Biomarkers for bone formation in osteoblasts and bone resorption in osteoclasts were analyzed.@*RESULTS@#Compared with the OVX group, HLE exerted bone protective effects by the increase of estradiol (P<0.05), the improvement of cancellous bone structure, bone mineral density (P<0.01) and the reduction of serum alkaline phosphatase (ALP), tartrate resistant acid phosphatase (TRAP), bone gla-protein, c-terminal telopeptides of type I collagen (CTX-I) and deoxypyridinoline levels (P<0.01 for all). In vitro experiment, compared with the control group, HLE at 20 µg/mL promoted the cell proliferation (P<0.01), and increased the expression of bone morphogenetic protein-2 and osteopontin levels in osteoblasts (both P<0.05). HLE at 100 µg/mL increased the osteoblastic ALP activities, and HLE at all dose enhanced the extracellular matrix mineralization (both P<0.01). Furthermore, compared with the control group, HLE at 20 µg/mL and 100 µg/mL inhibited osteoclastic TRAP activity (P<0.01), and reduced the expression of matrix metalloproteinase-9 and cathepsin K (both P<0.05).@*CONCLUSION@#HLE may protect against bone loss, and have potentials in the treatment of osteoporosis.

RESUMO

Objective To explore the effects of lupulone (LUP) and humulone (HUM) in Humulus lupulus L. on osteoblasts and osteoclasts of rats. Methods Osteoblasts and osteoclasts isolated from 24-h-old Wistar rats were studied and divided into control group, LUP-treated low (10-15 mol/L)-, medium (10-14mol/L)-and high (10-13 mol/L)-dose groups, and HUM-treated low (10-15 mol/L)-, medium (10-14 mol/L)-and high (10-13mol/L)-dose groups. After drug treatment, the proliferation, differentiation and bone mineralization of osteoblasts were determined by MTT assay, alkaline phosphatase (ALP) activity assay and alizarin red staining, respectively. Osteoclasts were counted and tartrate-resistant acid phosphatase (TRAP) activity was measured to evaluate the effects of LUP and HUM on the activity of osteoclasts. Osteocalcin (OCN) levels were measured by kit assay, and the expression levels of bone formation related proteins osteopontin (OPN), bone sialoprotein (BSP), bone morphogenetic protein 2 (BMP-2), bone resorption related proteins cathepsin K (CK) and matrix metalloproteinase 9 (MMP-9) were measured by Western blotting analysis to evaluate the effects of LUP and HUM on bone metabolism. Results At the osteoblast level, LUP at dosages of 10-15 and 10-14 mol/L could significantly promote the cell proliferation (P0.05). LUP at dosages of 10-14 and 10-13 mol/L could significantly improve ALP activity and bone mineralization (P0.05, P0.01). LUP at dosage of 10-13 mol/L could significantly induce the expression of OCN (P0.01). Furthermore, LUP at dosages of 10-14 and 10-13 mol/L could significantly increase the expression of BSP and BMP-2 (P0.05). HUM at dosages of 10-15-10-13 mol/L could also significantly promote the osteoblastic proliferation, ALP activity and bone mineralization (P0.01), and could significantly increase the expression of OCN and OPN (P0.05, P0.01). Additionally, HUM at dosages of 10-14 and 10-13 mol/L could significantly increase the expression of BSP and BMP-2 (P0.05). At the osteoclast level, both LUP and HUM at dosages of 10-15-10-13 mol/L could significantly reduce the number of osteoclasts (P0.01) and could significantly inhibit the expression of CK (P0.05, P0.01). HUM at dosages of 10-15-10-13 mol/L could also significantly inhibit the expression of MMP-9 (P0.05, P0.01). Conclusion This study preliminarily clarifies that LUP and HUM can prevent bone loss by promoting bone formation and inhibiting bone resorption, which provides a new reference for the development of osteoporosis drugs.

Tissue culture seedlings of Bletilla striata were treated with MeJA, SA and two kinds of endophytic fungi in order to study the effects of those treatments on the physiology and total phenols content. The method of tissue culture was used to culture seeds into seedlings, and then different treatments were applied on them to observe and measure the changes of physiology and total phenols content. We find that the growth of seedlings treated with SA was poor, which treated with 40 μmol•L⁻¹ MeJA, 50 mL•L⁻¹ Hypocrea koningii and 10 mL•L⁻¹ Trichoderma koningiopsis showed better. The activity of SOD, POD and CAT was at a high level under SA treatment of each concentration. The activity of SOD and POD increased as the rise of MeJA concentration, while CAT was highest at 80 μmol•L⁻¹. The activity of SOD and POD increased with the increasing of the concentration of H. koningii treatment, while CAT reached the highest at 1 mL•L⁻¹. The activity of SOD, POD and CAT increased first and then declined with the concentration of T. koningiopsis increasing, and the highest activity was at 10 mL•L⁻¹. The contents of MDA, soluble protein and proline were increased more or less under the four treatments. The content of polysaccharide was at a high level under 60 μmol•L⁻¹ of MeJA. The total phenols content was at a high level under 40 μmol•L⁻¹ of MeJA, 60 μmol•L⁻¹ of SA, 1 mL•L⁻¹ of H. koningii and 10 mL•L⁻¹ of T. koningiopsis. The results indicated that the addition of exogenous MeJA, SA and endophytic fungi under certain concentrations could improve the resistance of B. striata and increase the content of total phenols at some degree and the trearment of MeJA, H. koningii and T. koningiopsis could promote the growth of seedlings under certain concentrations.

<p><b>OBJECTIVE</b>To explore the expression of p-p38 MAPK protein and the number of astrocytes expressing p-p38 MAPK in CA1 hippocampus in rats during the induction of brain ischemic tolerance induced by intermittent hypobaric hypoxia (IH) preconditioning.</p><p><b>METHODS</b>Thirty healthy adult male Wistar rats were randomly divided into 6 groups (n = 5 in each group): sham 0 min group, IH + sham 0 min group, sham 7 d group, IH + sham 7 d group, Ischemia (Is) 7 d group, and IH + Is 7 d group. Neuropathological evaluation was performed by thionine staining in CA1 hippocampus in rats. The expression of p-p38 MAPK in CA1 hippocampus was observed by immunohistochemical staining. And the number of astrocytes expressing p-p38 MAPK was observed by immunofluorescent double labeling.</p><p><b>RESULTS</b>The results showed that IH preconditioning induced brain ischemic tolerance successfully. At the same time, IH preconditioning obviously up-regulated the expression of p-p38 MAPK protein in CA1 hippocampus, and also increased the number of astrocytes expressing p-p38 MAPK.</p><p><b>CONCLUSION</b>It might be concluded that IH preconditioning induced brain ischemic tolerance by up-regulating the expression of p-p38 MAPK protein in pyramidal neurones and astrocytes.</p>

Objective To investigate acute effects of anaerobic exercises by a bicycle ergometer on arterial elastic modulus and local hemodynamics in human common carotid arteries with different genders. Methods Nine male and eight female healthy young volunteers at the age of 20-30 year-old successively underwent four groups of exercise trainings with the same workload by an anaerobic bicycle ergometer. The waveforms of arterial diameter and center-line blood velocity were measured in the right common carotid artery using a color Ultrasonic Doppler for each group when at rest and right after exercise training. The heart rate, systolic and diastolic blood pressures were simultaneously measured in brachial artery using an automatic electronic sphygmomanometer. All the measured data were analyzed based upon the principle of classic hemodynamics. The arterial elastic modulus and local hemodynamic parameters, including pressure-strain elastic modulus, flow rate, circumferential strain, wall shear stress and oscillatory shear index (OSI), were then calculated. Results The heart rate and arterial elastic modulus increased after exercises; with the accumulative exercises, in one cardiac cycle, the maximum and mean center-line velocity and flow rate increased while the minimum velocity and flow rate decreased; the systolic and mean blood pressure increased while diastolic blood pressure exhibited no significant change; no significant change could be found in the circumferential strain; the maximum and mean shear stress increased significantly while the minimum shear stress reduced; the oscillatory shear index also increased. Conclusions The anaerobic exercises by a bicycle ergometer may increase the arterial elastic modulus and induce significant effects on local hemodynamic parameters in common carotid arteries for young volunteers with different genders at the age of 20 30 year old. The results in this study could provide useful hemodynamic information for regulation of cerebrovascular function by anaerobic exercises.

<p><b>OBJECTIVE</b>To investigate the interface bond and thermal compatibility between Mark II machining ceramic and Vita VM9 veneering porcelain.</p><p><b>METHODS</b>A bar shaped specimen (30 mm x 15 mm x 1 mm in size) of Mark II block was prepared, with 0.5 mm-deep notch (vertical to the long axis of specimen) at the middle of the bottom surface. The upper surface was veneered with 0.3 mm VM9 dentin base porcelain. Then the specimen was fractured from the notching site and the fracture surface was examined under scanning electron microscope (SEM) and electron microprobe analyzer (EMPA) with electron beam of 1 microm in diameter. Another ten specimens (30 mm x 15 mm x 1.5 mm in size) were fabricated and the temperature of thermal shock resistance were tested.</p><p><b>RESULTS</b>SEM observation showed tight bond between these two materials and EMPA results showed penetration of Al element from Mark II block into veneering porcelain and Ca element from veneering porcelain into Mark II block occurred after sintering baking. The average temperature of thermal shock resistance for specimens in this study was (194.0+/-10.3) degrees C. Cracks were mainly distributed in veneering porcelain.</p><p><b>CONCLUSION</b>Chemical bond exists between the Mark II machining ceramic and Vita VM9 veneering porcelain, and there is good thermal compatibility between them.</p>

<p><b>OBJECTIVE</b>To evaluate the effects of all-ceramic veneers in clinic fabricated by computer aided design/computer aided manufacturing (CAD/CAM) system and veneered with Vita VM9.</p><p><b>METHODS</b>54 all-ceramic veneers were made for 12 patients. The patients were divided into three groups: Tetracycline staining group, fluorosis group and devitalization group. The color of patients' teeth was checked before and after laminates with Shade Eye. All-ceramic veneers was checked on the white background and abutment background. The value of L*, a* and b* were calculated to compare the color difference among 3 groups. The color, fitness and fracture of all-ceramic veneers were checked in clinic after restoration every 3 months.</p><p><b>RESULTS</b>All-ceramic veneers as a replacement for discolored teeth showed good appearance. The substructures and veneers showed significant color difference between white background and abutment background in tetracycline staining group and devitalization group, but there was no significant color difference in fluorosis group. All veneers had good esthetic effect, excellent marginal fit, good gingival and adjacent condition. The fracture was not found in clinic.</p><p><b>CONCLUSION</b>The effects of all-ceramic veneers were perfect in color, fitness and stability to fracture. Fluorosis teeth is the best indication for all-ceramic veneers. To tetracycline teeth and devital teeth, the fundic porcelain with deep color should be chosen, or an opaquing material be applied as fundus in all-ceramic veneers repairing.</p>
