RESUMO

To evaluate the different complaints and presentations, such as emergency visits, after penetrating keratoplasty [PKP], ways of management, and the outcome of these visits. This prospective study included a total of 60 consecutive emergency visits by 55 post-PKP patients at the eye clinic in King Hussein Medical Center, Amman, Jordan between January 2007 and January 2008. Fifty-five patients were included [66.7%] with a preoperative diagnosis of keratoconus. Pain and gritty sensation were the main presenting symptoms; loose irritating sutures [26.7%], and graft rejection [25%] were the most common diagnoses. Fourteen patients [23%] were admitted to the hospital for either re-suturing or intensive treatment, the graft survived in all patients, while the visual acuity was preserved in 96.3%. Proper postoperative care is critical for a successful penetrating keratoplasty; early intervention of sight threatening complications increases the chance of graft survival and best-obtained vision

RESUMO

To report the pattern of various causes of decreased visual acuity [VA], within the age group of young [18-25 years] healthy adults in Jordan. We carried out this retrospective study on the records of 16550 candidates examined from June 2004 to June 2005 by the Medical Committee of Employment, Royal Medical Services in Amman, Jordan. All candidates received an ophthalmic examination as well as medical and ENT examination. We divided the candidates with visual acuity of less than 6/12 in either eye into 3 groups. The most important cause of decreased VA in the 18-25 years age group was refractive errors followed by amblyopia, which represents the major ophthalmic cause of employment rejection in a wide range of occupations in adult life. This study emphasize the need for early visual screening for refractive errors and anisometropic amblyopia in early primary schools, and every effort must be made to achieve the best possible acuity in young patients with amblyopia

RESUMO

This prospective study was conducted to evaluate the effectiveness of contract trans-scleral diode laser cyclophotocoagulation [TSCPC] for Neovascular glaucoma.20 eyes of nineteen patients with Neovascular glaucoma on maximum medical treatment were consecutively treated by TSCPC. Success rate was defined as IOP of 5-25 mmHg in eyes of VA> hand movement [HM]; or relief of pain in eyes with VA= HM including blind eyes [NPL] and reduction of eye drops used in all eyes TSCPC treatment was performed with the center of the proble 1.5 mm behind the limbus, 28-30 pulses were applied for 270 degrees around the cornea.Results:19 patients were followed up for 1 year after initial treatment. The overall success was bout 74% within one year 30 laser procedure was performed; mean 1.55 per patient. As for complications, there was no phthisis bulbi, persistent hypotonia, or sympathetic ophthalmia; however hyphema and moderate inflammatory reaction each occurred in 15%.Trans- scleral diode laser cyclo- photocoagulation is an effective method for the treatment of Neovascular glaucoma

RESUMO

Ocular pseudoexfoliation [PXF] has been recently considered as a systemic disease affecting other organs as well as the eye. This prospective study is to assess the relationship between PXF and sensorineural hearing loss. Patients attending a general ophthalmic clinic at King Hussein Medical Center, Amman, Jordan from [March 2002 through to March 2003] who were found to have ocular PXF on routine ophthalmic examination were referred to the Audiometric Department. Pure tone hearing threshold was measured at 1, 2, 3 kHz for each ear and was compared with International Standard [ISO 7029] median age associated hearing loss at 1, 2, 3 kHz [AAHL]. Forty-one patients were studied of whom 24 were males [58.5%]; the mean age of the male patients was 78-years while that of the female group was 72-years. All patients had PXF affecting at least one eye, 16 patients [39%] had bilateral PXF. Overall [72-years] of 36 patients [87%] had a higher hearing threshold level [HTL] at 1, 2, 3 kHz [HTL [1, 2, 3] than the ISO 7029 median AAHL 1, 2, 3 which included [44 ears] of 22 patients in the male group [87%] and [28 ears] of 14 patients in the female group 82%. Approximately 26.8% of patients had glaucoma, however; there was no correlation between glaucoma and sensorineural hearing threshold level. The majority of patients with ocular PXF had sensorineural hearing loss compared to age-matched controls. Thus, there is increasing evidence that PXF is a systemic disease

This study aims at examining the change of corneal curvature as a function of time following cataract surgery, the effect of the suture material used and effect of selective suture removal on final astigmatism. Patients and 38 patients who underwent Extra capsular cataract extraction and Intra Ocular Lens implantation at Queen Alia Military Hospital, with astigmatism of more than 3 Diopters, were followed up closely for 6 months with keratometric readings. Selective suture removal bet ween 8-10 weeks was performed and final astigmatism and stabilization of refraction was studied. Mean Keratometric reading difference 1st week post-operative accounted for 8.25 Diopters 79.5 degree for the V.S suture group [12 patients], while it accounted for 7.5 Diopters at 84 degree for the 10.o Nylon suture grup [26 patients]. The final mean Keratometric reading difference at 6 months in the V.S suture group accounted for 1.79 Diopters at 131.6 degree, while in the 10.o Nylon suture group it accounted for 1.6 Diopters at 121 degree. 90% of the patients achieved 6/6-6/12 corrected VA at 6 months final refraction. Astigmatism with the rule [WTR] was found in all patients 1 week post-op. There is a gradual decay of WTR astigmatism during the 1st 2 months period towards against the rule [ATR] astigmatism Selective suture removal at 8-10 weeks post-operatively allows accurate adjustment of astigmatism

Few cases were reported following coronary artery bypass surgery. A 63-year-old man, who is both diabetic and hypertensive, underwent coronary artery bypass graft complicated postoperatively by sudden visual loss in his right eye. The diagnosis was non-arteritic anterior ischemic optic neuropathy. Possible predisposing factors were crowded disc and internal carotid artery stenosis
