RESUMO

ObjectiveTo investigate the association between the risk of increase in total cholesterol (TC) and the risk of cholelithiasis by using bidirectional Mendelian randomization (MR). MethodsThe open gwas public database was used to obtain the single nucleotide polymorphism data associated with TC and cholelithiasis, and a secondary data analysis was performed for all summary data of genome-wide association studies. The genetic loci closely associated with TC or cholelithiasis were selected as exposure or outcome variables, and the bidirectional MR analysis was performed using the methods such as Egger regression, Weighted median, IVW random effects model, and IVW fixed effects model, with odds ratio (OR) values for evaluating the causal relationship between TC and cholelithiasis. ResultsWith TC as the exposure and cholelithiasis as the outcome, TC-cholelithiasis had an overall OR value of 0.91 (95% confidence interval [CI]: 0.85‍ ‍—‍ ‍0.97) before elimination of heterogeneity and 0.93 (95%CI: 0.89‍ ‍—‍ ‍0.97) after elimination of heterogeneity. With cholelithiasis as the exposure and TC as the outcome, TC-cholelithiasis had an overall OR value of 0.20 (95%CI: 0.06‍ ‍—‍ ‍0.65) before elimination of heterogeneity and 0.28 (95%CI: 0.10‍ ‍—‍ ‍0.83) after elimination of heterogeneity. There was a bidirectional causal relationship between genetically predicted TC and cholelithiasis. ConclusionThis study confirms the bidirectional causal relationship between TC and cholelithiasis. The risk of cholelithiasis decreases with the increase in alleles associated with the elevation of TC level; on the contrary, the risk of elevated TC level decreases with the increase in alleles associated with the onset of cholelithiasis.

Objective:To investigate the neurobiochemical metabolites of caudate nucleus and thalamus in patients with obsessive-compulsive disorder and their relationship with obsessive-compulsive symptoms.Methods:From April 2019 to January 2022 in Beijing Anding Hospital, totally 25 untreated patients with obsessive-compulsive disorder were recruited, and 20 healthy controls matched with gender, age and educational background were recruited for the study.The maps of neurobiochemical metabolites of patients and normal controls were collected by hydrogen proton magnetic resonance spectroscopy.With bilateral caudate nucleus and thalamus as brain regions of interest.The relative concentrations of N-acetylaspartic acid (NAA), glutamic acid (Glu) and γ-aminobutyric acid (GABA) were fitted by LCModel software.At the same time, the clinical symptoms of patients were evaluated with Yale-Brown obsessive-compulsive scale (Y-BOCS) and Hamilton anxiety scale (HAMA). SPSS 20.0 software was used for statistical analysis.Independent double sample t-test was used to compare the differences of different nerve biochemical metabolite concentrations between patients with obsessive-compulsive disorders and healthy controls.Pearson correlation analysis was used to explore the correlation between biochemical metabolite concentrations and clinical symptoms. Results:The Glu concentration in the left thalamus of patients with obsessive-compulsive disorder (3.97±0.41) was higher than that of the control group (3.66±0.55)( t=-2.11, P<0.05), while the NAA concentration was (4.87±0.47)lower than that of the control group (5.15±0.44)( t=2.05, P<0.05). The GABA concentrations in the right caudate nucleus (0.50±0.18) and thalamus (0.80±0.19) were lower than those in the control group ((0.63±0.23), (0.96±0.24))( t=2.08, 2.36, both P<0.05). Pearson correlation analysis showed that the Glu concentration in the left caudate nucleus of patients with obsessive-compulsive disorder was positively correlated with the total score of Y-BOCS( r=0.46, P<0.05). Spearman correlation analysis showed that Glu concentration in the right caudate nucleus was positively correlated with the total score of HAMA in patients with obsessive-compulsive disorder ( r=0.46, P<0.05). Conclusion:NAA, Glu and GABA metabolism in caudate nucleus and thalamus are abnormal in patients with obsessive-compulsive disorder, and Glu concentration is positively correlated with the severity of obsessive-compulsive and anxiety symptoms.

Objective:To explore the efficacy of subcutaneous injection of granulocyte-macrophage colony-stimulating factor (GM-CSF) in preventing invasive fungal disease (IFD) in patients with multiple myeloma (MM).Methods:The clinical data of 222 patients who were admitted to the Second Hospital of Harbin Medical University from January 2015 to June 2021 were retrospectively analyzed. The patients was given GM-CSF (3-5 μg·kg -1·d -1, GM-CSF group) or granulocyte colony-stimulating factor (G-CSF, 2-5 μg·kg -1·d -1, G-CSF group) when neutrophils (ANC) ≤1.5×10 9/L after induction chemotherapy. Patients were discontinued when white blood cell count (WBC) ≥10.0×10 9/L. The incidence of IFD (including confirmed, clinical and proposed diagnosis) and breakthrough invasive fungal infections was compared between the two groups. Results:The incidence of IFD was 8.1% (18/222) in all patients. The incidence of IFD was 3.5% (3/85) and 10.9% (15/137) in the GM-CSF and G-CSF groups, respectively, and the difference between the two groups was statistically significant ( χ2 = 3.88, P = 0.049). In 9 patients of GM-CSF group receiving fungal infection prophylaxis and in 15 patients of G-CSF group receiving fungal infection prophylaxis, the incidence of breakthrough invasive fungal infections was 0 and 7 cases, respectively, and the difference between the two groups was statistically significant ( P = 0.022). Conclusions:GM-CSF application in MM patients can reduce the incidence of IFD and breakthrough invasive fungal infections.

RESUMO

Hepatocellular carcinoma (HCC) is a major cause of cancer-related death, and surgical resection remains an important method for radical treatment, but it is urgently needed to solve the problem of high postoperative recurrence rate. Neoadjuvant therapy can reduce the high recurrence rate after surgery, and there are little benefits from neoadjuvant therapy for HCC due to a lack of effective treatment methods in the past. At present, combination therapy based on immune checkpoint inhibitors has a relatively high response rate and has thus changed the treatment landscape for patients with advanced HCC. This urges investigators to reexamine the neoadjuvant treatment strategies for HCC, and it is expected that neoadjuvant therapy can provide new opportunities, reduce the postoperative recurrence rate, and improve the survival rate after treatment. This article discusses the current status and prospects of neoadjuvant therapy for HCC and related hot topics, so as to provide more ideas for exploring neoadjuvant therapy for HCC.

Objective:To investigate the clinical efficacy of high-frequency repetitive transcranial magnetic stimulation combined with electrical stimulation in patients with dysphagia after stroke.Methods:Seventy-two stroke patients with dysphagia who received treatment in The First Hospital of Jiaxing from February 2019 to February 2020 were included in this study. They were randomly assigned to receive either electrical stimulation (control group, n = 36) or high-frequency repetitive transcranial magnetic stimulation combined with electrical stimulation (observation group, n = 36) for 2 weeks. Changes in serum brain-derived neurotrophic factor, neuron-specific enolase, and vascular endothelial growth factor levels after treatment relative to before treatment were observed in each group. Scores of the swallowing function assessment scale, neurological deficit, and quality of life were compared between the control and observation groups. Results:After treatment, serum levels of brain-derived neurotrophic factor and vascular endothelial growth factor in the observation group were (7.98 ± 1.14) μg/L, (168.78 ± 10.28) ng/L, respectively, which were significantly higher than those in the control group [(5.80 ± 1.10) μg/L, (110.34 ± 10.47) ng/L, t = 8.26, 23.90, both P < 0.01]. Serum neuron-specific enolase level was significantly lower in the observation group than in the control group [(7.57 ± 1.17) μg/L vs. (10.66 ± 1.30) μg/L, t = 10.60, P < 0.001). The scores of swallowing function assessment scale and neurological deficits in the observation group were (2.47 ± 1.16) points and (7.03 ± 1.14) points, respectively, which were significantly lower than those in the control group [(5.75 ± 1.10) points, (9.66 ± 1.20) points, t = 12.31, 9.53, both P < 0.001]. Total effective rate [97.22% (35/36) vs. 77.78% (28/36)] and the score of swallowing quality of life questionnaire [(160.40 ± 8.32) points vs. (146.74 ± 8.10) points] were significantly higher in the observation group than in the control group ( χ2 = 4.57, P = 0.03, t = 7.25, P < 0.001). Conclusion:High-frequency repetitive transcranial magnetic stimulation combined with electrical stimulation can greatly improve neurological function and swallowing function of stroke patients with dysphagia. The combined therapy is of certain clinical value and innovation.

Objective:To investigate the level of trimethylamine N-oxide (TMAO), one of gut metabolites, in patients undergoing maintenance hemodialysis (MHD) accompanied by congestive heart failure (HF) and its influencing factors.Methods:Those patients of 18-75 years old who received three or more times of hemodialysis sessions per week for three months or longer during Nov 2018 and Mar 2019 were enrolled. Those attended health checkup at the same time without obvious kidney abnormality served as non-kidney disease controls. Serum TMAO concentrations were measured using high-performance liquid chromatography electrospray ionization-tandem mass spectrometry (HPLC-ESI-MS/MS). The levels of TMAO were compared between patients on hemodialysis and controls, between those with heart failure and without heart failure using logrithmically transformed TMAO (lnTMAO). Linear regression analysis was performed to investigate factors influencing TMAO levels.Results:A total of 195 patients undergoing MHD and 40 controls were enrolled. Among them, 30 hemodialysis cases (15.4%) manifested as heart failure symptoms and/or left ventricular ejection fraction less than 50%. Males accounted for 67.2% in patients on hemodialysis and 37.5% in controls ( χ2=12.426, P<0.001) respectively, while the median ages in both groups were 62.0(48.0, 71.0), 45.0(33.3, 55.0) years old respectively ( Z=5.685, P<0.001). TMAO concentrations were significantly higher in patients on hemodialysis than controls [5.54(3.84, 8.91) mg/L vs 0.17(0.11, 0.30) mg/L, after log transformed, t=21.687, P<0.001]. However, there was no statistically significant difference between those with heat failure and those without in male [63.3% vs 67.9%, χ2=0.238, P=0.626], age [64.5(56.8, 71.0) years old vs 61.0(47.0, 72.0) years old, Z=0.894, P=0.372] and TMAO [5.17(3.30, 9.46) mg/L vs 5.57(3.87, 8.95) mg/L, after log transformed, t=-1.537, P=0.135]. Multivariate linear regression analysis demonstrated that in all the participants, serum urea was the main risk factor for TMAO [standardized coefficient ( SB)=0.483]. lnTMAO=0.078×[serum urea(mmol/L)]+0.001×[serum creatinine (μmol/L)]-0.002×[serum uric acid (μmol/L)]-0.003×[platelet (×10 9/L)]+0.014×[age (years old)]+0.344 (if diabetic)-1.266. While in those undergoing MHD, ultrafiltration volume had the most significant effect on TMAO levels ( SB=0.279). lnTMAO=0.249×[ultrafiltration volume(L)]+0.059×[serum albumin (g/L)]+0.008×[age (years old)-0.526 (if heart failure existed)-1.865. Conclusions:MHD patients have gut dysbiosis, while those hemodialysis patients accompanied by heart failure may have peculiar gut microbiota which induces lower serum TMAO levels than those without heart failure after adjusting for multiple related factors. Serum TMAO levels may be associated with ultrafiltration volume and nutrition status etc.

The prediction of survival time for advanced cancer patients undergoing palliative care has important clinical and social value. The prediction of survival time of advanced cancer patients includes clinical prediction and statistical prediction. Due to the late start of palliative medicine in China, it is particularly important to evaluate the widely used survival prediction tools in clinical practice. In this paper, we will review the common survival prediction tools of advanced cancer patients from the perspective of Western and Traditional Chinese Medicine,to provide reference for development and application of a survival prediction system in China.

Objective:To explore the effect of nutrition intervention on patients with cervical cancer undergoing chemoradiotherapy.Methods:From October 2016 to October 2018, 60 patients with cervical squamous cell carcinoma confirmed by pathology and treated with radiotherapy and chemotherapy in the First Hospital of Shanxi Medical University were selected. The subjects were randomly divided into the control group (nutrition education + dietary guidance only, 30 cases) and the intervention group (nutrition intervention according to nutrition assessment results, 30 cases) by random number table method. The nutrition assessment for patients was performed with patient-generated subjective global assessment (PG-SGA) scale. The changes of nutrition status, quality of life, incidence of adverse reactions, interruption status of radiotherapy and chemotherapy, and hematological indexes between the two groups in the treatment process were compared.Results:The incidence of moderate and severe malnutrition in the control group and the intervention group in the treatment process was 90% (27/30) and 50% (15/30), respectively, and the difference was statistically significant ( P < 0.01). When the dose of radiotherapy reached 40 Gy, the quality of life of patients in the intervention group was better than that in the control group in some functional fields, symptom fields and overall health status, and the differences were statistically significant (all P < 0.05). The incidence of acute gastrointestinal reaction in the control group and the intervention group was 96.7% (29/30) and 83.3% (25/30), respectively, and the incidence of bone marrow suppression was 96.7% (29/30) and 76.7% (23/30), respectively, and there were statistical differences in the incidence rates of different degrees of acute gastrointestinal reaction and bone marrow suppression between the two groups (both P < 0.05). The interruption rate of radiotherapy in the control group and the intervention group was 26.7% (8/30) and 6.7% (2/30), respectively, and the interruption rate of chemotherapy was 56.0% (14/25) and 26.9% (7/26), respectively, and the differences between the two groups were statistically significant (both P < 0.05). The cycles of chemotherapy completed in the intervention group were more than those in the control group [(4.38±1.10) cycles vs. (3.44±1.58) cycles]. The differences of peripheral blood red blood cells count, hemoglobin, albumin, and total protein between the control group and the intervention group before and after treatment were statistically significant (all P < 0.05), and the decreased values in the intervention group were lower than those in the control group. Conclusion:Nutrition intervention can effectively improve the nutrition status and quality of life of patients with cervical cancer undergoing chemoradiotherapy, reduce the incidence of adverse reactions, chemoradiotherapy interruption rate and decline degree of hematological indexes.

Objective To investigate the expression of oxytocin ( OXT ) and oxytocin receptor (OXTR) in the prefrontal cortex of postpartum depression (PPD) rats induced by restraint stress during pregnancy and to observe the antidepressant effect of oxytocin and its analogue capitoxin and its mechanism. Methods Twenty-four adult female SD rats of SPF grade were randomly divided into control group,PPD +saline group,PPD + oxytocin group and PPD + captopril group with 6 rats in each group. Rats were subjec-ted to restraint stress for 2 hours every day on the 8th to 21st day of pregnancy to establish PPD model. While the rats in control group were not given any treatment. Rats in PPD + saline,PPD + oxytocin and PPD +captopril were injected bilaterally into prefrontal cortex (PFC) at 10 days postpartum (1 μl/side),oxytocin (30 ng/side) and captopril (45 ng/side) respectively once a day for 5 days. The depressive behaviors of rats were detected by sugar-water preference experiment. Rats were sacrificed 18 days after delivery. The ex-pression of OXT was detected by ELISA method,OXTR by Western blot,Iba-1 by immunofluorescence,and IL-1β,IL-6 and TNF-α by qRT-PCR. Results (1) The sucrose consumption of the PPD + saline group ((67. 1±10. 4)%) was significantly lower than that of the control group((92. 6± 3. 9)%,t=-5. 31,P<0. 01). (2) The expression of oxytocin in prefrontal cortex in PPD group was significantly lower than that in control group ((0. 03±0. 01) ng/mg) vs (0. 08 +0. 05) ng/mg,t=-2. 67,P<0. 05). However,there was no significant difference in the expression of oxytocin receptor between PPD group and control group ((0. 90 ±0. 06) vs (0. 90±0. 05),t=0. 709,P=0. 517). (3) The sucrose consumption of PPD+saline group de-creased than that of control group((65. 6±16. 9)% vs (91. 5±3. 5)%,t=3. 35,P<0. 001). Compared with PPD+saline group,the sucrose consumption of PPD+oxytocin group ((81. 8±8. 4)%) and PPD+carbetocin group ((78. 4±9. 4)%) increased(t=1. 98,1. 68,both P<0. 05). (4) The expression of Iba-1 in the pre-frontal lobe of PPD + saline group was higher than that of control group ((1. 15±0. 05) vs (1. 04 +0. 06), t=3. 50,P<0. 01). Compared with PPD + saline group,the expression of Iba-1 in PPD + oxytocin group (1. 03±0. 06) and in PPD + captopril group (1. 00±0. 02) were lower (t=-3. 50,-6. 55,both P<0. 01). (5) The expression of inflammatory factors IL-1β mRNA (1. 0±0. 1),IL-6 mRNA (1. 1±0. 1) and TNF-α mRNA (1. 7±0. 4) in the prefrontal cortex of rats in the PPD group were higher than that in the control group (IL-1β mRNA (0. 7± 0. 3),IL-6 mRNA (0. 9± 0. 1),TNF-α mRNA ( 1. 1± 0. 3),t=1. 92,3. 19, 2. 43 respectively,all P<0. 05). The expression of inflammatory factors IL-1β,IL-6 and TNF-α mRNA of the PPD+oxytocin group(IL-1β mRNA (0. 6±0. 1),IL-6 mRNA (0. 9±0. 1),TNF-α mRNA (1. 2±0. 4) )and the PPD+carbetocin group ( IL-1β mRNA ( 0. 7± 0. 1),IL-6 mRNA ( 0. 9 ± 0. 1),TNF-α mRNA ( 1. 0 ± 0. 2))in the prefrontal cortex were lower than that in the PPD group(t=-3. 17,-2. 78,-1. 84,t=-2. 76,-2. 40,-2. 94 respectively,all P<0. 05). Conclusion Oxytocin and capitoxin injected into prefrontal cortex can effectively improve depression-like behaviors in PPD model rats. Activation of microglia and decrease of inflammatory factors in prefrontal cortex may be the potential antidepressant mechanism.

Objective To investigate the value of real-time fluorescent quantitative polymerase chain reaction (RT-qPCR) in the detection of Streptococcus agalactiae in early screening of Streptococcus agalactiae in pregnant women in the third trimester, and to study the effect of Streptococcus agalactiae infection on them.Methods A retrospective analysis of 5 855pregnant women with vaginal discharge and rectal secretions from January 2017to January 2018in our hospital were performed to detect Streptococcus agalactiae by RT-qPCR.Meanwhile, 100vaginal non-vaginosis women in the third trimester were selected as the negative group and100healthy women as the control group.The vaginal micro-environment-related indexes (Lactobacilli content, flora diversity and cleanliness) of three groups were analyzed.Results The total carrier rate of Streptococcus agalactiae of the third trimester from January 2017to January 2018in our hospital was 9.6%, of which vaginal carriage rate was 2.4%, rectum carriage rate was 7.2%, Streptococcus agalactiae positive group (GradeⅡ-Ⅲ), rates of population diversity (GradeⅡ-Ⅲ) cleanliness (Ⅰdegree) and microenvironment imbalance were 30.1%, 36.4%, 25.9%and 76.2%respectively, while those in the negative group were 58.0%, 55.0%, 61.0%and 63.0%and those in the control group were 87.0%, 91.0%, 83.0%, 24.0%.The positive rate of Streptococcus agalactiae in each group was significantly higher than that in the control group (P<0.05).The differences of all the indexes between the negative group and the control group were also statistically significant (P<0.05).Conclusion The carrying rate of Streptococcus agalactiae in third trimester pregnancy in our hospital was within the normal range reported in the literature.The detection of Streptococcus agalactiae by RT-qPCR could be effectively screened and monitored.Carrying Streptococcus agalactiae might increase the risk of vaginal infection in third trimester women.

Rheumatic diseases such as rheumatoid arthritis and Sjogren′s syndrome can be associated with a wide range of peripheral neuropathy. The diagnosis and treatment are more difficult in rheumatic diseases combined with peripheral neuropathy due to the complex etiology and clinical manifestations. This article reviews the recent progress in the diagnosis and treatment of rheumatic diseases-related peripheral neuropathy.

Objective To investigate the clinical manifestations,laboratory tests,and image modalities on rheumatoid arthritis (RA) accompanied with atlanto-axial joint involvement.Methods Sixty-four cases of RA accompanied with cervical vertebra involvement were divided into 2groups by lesion location.Twenty-two cases were RA accompanied with atlanto-axial joint involvement,while 42 cases were RA with cervical vertebra other than atlanto-axial joint involvement.The age,course,clinical manifestations and the lab results were compared between the two groups by t test,Chi-square test and Fisher's exact probability.The X-ray,CT and magnetic resonance imaging (MRI) for cervical vertebra were analyzed in RA with atlanto-axial joint involvement.Results Compared with non-atlantoaxial cervical group,the disease course [(15±10) years vs (8±9) years,t=3.030,P=0.004],upper cervical vertebra pain (73% vs 7%,x2=29.75,P<0.01),lower cervical vertebra pain (9% vs 40%,x2=6.813,P=0.009),cervical activity limitation (68% vs 14%,x2=19.023,P<0.01),upper cervical vertebra pressing pain (100% vs 7%,22=52.297,P<0.01),lower cervical vertebra pressing pain (9% vs 60%,x 2=15.056,P<0.01) and erythrocyte sedimentation rate (ESR) [(73±34) mm/1 h vs (53±37) am/1 h,t=2.039,P=0.046)] were significantly different in atlantoaxial joint lesions group.CT combined with MRI exams had high diagnostic..value in RA with atlanto-axial joint involvement.CT scan had the conformed diagnostic value.Sixteen cases were positive (73%) by MRI scan,while 3 cases (14%) by X-ray.Conclusion Timely CT scan and/or MRI scan for RA patients with neck pain in upper cervical vertebra,long disease course,cervical activity limitation and high ESR are helpful for early diagnosis of atlanto-axial joint involvement.

Objective To explore the feasibility of quantitative assessment of pancreatic perfusion using dynamic contrastenhanced MRI (DCE-MRI).Methods Totally 68 healthy volunteers were divided into youth,middle and old groups according to ages.All volunteers underwent pancreas DCE-MRI examination.Images were transmitted to Research-DCE MRI Tool workstation to calculate the quantitative parameters,including volume transfer constant (Ktrans),interstitium-toplasma rate constant (Kep),interstitial volume (Ve) and plasma volume (Vp).Independent sample t test and one-way ANOVA test were used to evaluate the differences of pancreatic perfusion.Results There were no significant differences of Ktrans,Kep, Ve and Vp between male and female;Ve in old group was higher than that in youth and middle groups (P =0.036,0.001);Vp of pancreatic head was higher than that of pancreatic body and tail (P=0.011,0.023).Conclusion DCE-MRI can be applied to provide a reliable quantitative assessment of pancreatic perfusion noninvasively.The parameters of DCE-MRI of pancreatic perfusion are independent of gender but vary with age and pancreatic sites.

Objective To explore the expression level of Ki67 and its correlation with clinicopathological types and prognosis of patients with breast cancer.Methods Clinical data of 93 patients with breast cancer and 20 patients with benign breast diseases were collected from Jan.2013 to Dec.2015,and the correlation between Ki67 and pathological parameters and laboratory parameters were analyzed.Results Among the 93 patients with breast cancer,the Ki67 index was 1% to 90%,with median of 21.65%.Ki67 index was correlated with tumor grade,tumor size and human epidermalgrowth factor receptor-2(HER2,P0.05).Ki67 index of patients with triple negative breast cancer,patients with non-triple negative breast cancer and patients with benign breast diseases were 30.15%,20.26% and 4.67%,which were with significant difference(P<0.04).Ki67 index was also related to the breast cancer subtypes(P<0.05),and serum potassium and hematocrit levels of patients with breast cancer(P<0.05).Conclusion Ki67 index could be used for the diagnosis,treatment and prognosis of patients with breast cancer.

Objective To analyze CT findings of insulinoma,and to summarize the causes of missed diagnosis of nontypical insuli-noma.Methods Clinical and CT manifestations of 18 patients with 18 insulinomas were analyzed retrospectively which were proved by surgery and pathology,and the causes of the missed diagnosis of nontypical insulinoma were also summarized.Results 10 patients with 10 insulinoma underwent CT plain scan with isodensity in 9 and slightly lower density in 1 with thread-like capsule.Other 18 patients underwent enhanced CT scan,10 of whom showed obvious enhancement in arterial phase with isodentisy in 6 and slightly higher density in 4 in portal phase,and isodensity in 10 in delayed phase.4 lesions showed mild-to-moderate enhancement in arterial phase with slightly higher density than normal pancreas in 2 and isodentisy in 2 in portal phase,and slightly higher density than nor-mal pancreas in 1 and similar density to pancreas in 3 in delayed phase.In portal phase,the enhanced degree in 8 was similar to the pancreas,and that in 6 was slightly higher or higher than that of pancreas.In delayed phase,13 were similar to the pancreas and other 1 was higher than that.3 of 18 lesions were easily missed,and 4 lesions with missed diagnosis showed isodensity on plain CT and en-hanced CT,and were further detected by other imaging methods.Conclusion Multiphase enhancement CT scanning can be used as the first choice for the insulinoma.

Objective To evaluate the value of DWI and ADC value in monitoring the chemotherapy response of advanced gastric carcinoma dynamically.Methods 42 advanced gastric carcinoma patients who were confirmed by histopathology underwent T2 WI and DWI examinations at pre-chemotherapy,post-chemotherapy 3 d,7 d,30 d and 60 d respectively.The longest diameters of tumor pre-chemotherapy and post-chemotherapy 60 d were measured on axial T2 WI,meanwhile the ADC values at different time points were calculated.The mean ADC value among pre-and post-chemotherapy of each group (PR and SD)was compared.Results The ADC value of PR group increased gradually.The mean ADC value before therapy was statistically lower than those at differ-ent time points post-chemotherapy (P < 0.05).The ADC value of SD group increased gradually from pre-chemotherapy to post-chemotherapy 30 d,and then the ADC value decreased at post-chemotherapy 60 d.The differences of the mean ADC values in differ-ent time points were statistically significant (P < 0.05).Conclusion DWI and ADC value can dynamically,quantitatively and early detect and monitor the chemotherapy response of advanced gastric carcinoma.

<p><b>OBJECTIVE</b>To screen novel miRNAs targeting EZH2 3' untranslated region (UTR) in recombinational MCF-7 breast cancer cells over-expressing EZH2 3' UTR and quantitative analyze the expressions of the screened miRNA in breast cancer cells and tissues.</p><p><b>METHODS</b>A lentiviral library was transfected into the recombinant cell line MCF-7. The cells were screened with cytotoxic agents before extraction of the genome for amplification of the miRNA precursors using PCR. The screened miRNAs were identified with sequence analysis and their expressions were analyzed quantitatively with real-time PCR in breast cancer cells and tissues.</p><p><b>RESULTS</b>Seven miRNAs were screened from the recombinant MCF-7 cells, namely miR-15b, miR-16-2, miR-181b2, miR-217, miR-224, miR-329-1, and miR-487b, all of which failed to be predicted by bioinformatics software. Real-time PCR showed that miR-217, miR-329-1, and miR-487b were over-expressed in MCF-7 cells, and the expression of miR-15b and miR-16-2 was significantly increased in cancer tissues compared with the adjacent tissues (P<0.05).</p><p><b>CONCLUSION</b>Novel targeted miRNAs that can not be predicted by bioinformatics software were successfully screened from MCF-7 breast cancer cells over-expressing EZH2 3' UTR. These miRNAs are expressed differentially between normal breast cells and breast cancer tissues.</p>

Objective To investigate the distribution and variation trend of whole-blood Ca,Cu,Zn,Fe and Mg concentrations of children in fengxian district,Shanghai,and provide a reference for prevention of elements deficiency in children.Methods 2 219 children in outpatient department of pediatrics were enrolled in the study,who reccieved the tests for five kinds of elements(Ca, Mg,Fe,Cu,Zn)in whole blood by using atomic absorption spectrometer.The children were divided into 5 age groups:infancy group (1-0.05).Differences among different age groups were statistically significant(P <0.05).The Average concentrations of Zn displayed increasing tendency of different degrees in different groups of children,other elements displayed no obvious variation trend.Conclusion Children′s Zn,Fe and Ca deficiencies in Fengxian District,Shanghai are in different degrees,regular determina-tion of calcium and trace elements should be performed and take supplementary measures as soon as possible.

Objective To investigate the risk factors of premature atherosclerotic three-vessel coronary artery dis?ease (CHD), and the association between single nucleotide polymorphism (SNP) of CD36 rs3211956, rs7755 and premature CHD. Methods Patient with premature three-vessel coronary artery disease (n=102) which were confirmed by consecutive coronary angiogram (lesion group) and patients (n=72) without CHD (control group) were enrolled in the study. Conventional CHD risk factors were compared between the two groups as well as SNPs of CD36 rs3211956 and rs7755 to disclose inde?pendent risk factor for CHD, which were measured by mass spectrometry. Results Among the conventional CHD risk fac?tors, male, HBP, high LDL-C, low HDL-C were independent risk factors of premature CHD. The GT genotype proportion of rs3211956 was significantly lower (χ2=8.042,P=0.005) in the lesion group than that in control group while the TT genotype proportion is significantly higher in lesion group compared with that in control group (χ2=6.191,P=0.014). Patients with the TT genotype have higher score of BMI than patients with GG or GT genotype (P=0.037). The G allele proportion of rs7755 in the lesion group was significantly higher than control group (χ2=3.636, P=0.047). Patients of the GG genotype have higher scores of BMI but lower level of HDL-C than those with AA or AG genotype (P<0.001). Logistic regression analysis re?vealed that after excluding a number of confounding factors, GG and TT genotype of rs3211956 and GG and GA genotype of rs7755 were respectively one of the independent risk factors for premature CHD. Conclusion The SNPs of CD36 rs7755 and rs3211956 may be the independent risk factors of premature coronary heart disease and might affect the the onset of CHD by affectting BMI and HDL-C.

The objective of this work was to observe the side effects on the peripheral blood treated with 131I in children and adolescents with differentiated thyroid carcinoma (DTC). The levels of white blood cells (WBC), neutrophil, HGB (hemoglobin), RBC (red blood cells) and the PLT (platelet) were measured by automatic hemocyte analyzer in 31 young enrolled patients (< or = 20 years old) before 131I therapy, 1 month and 6 months after 131I therapy, and at the end of follow up. The results of 131I treatment at the before and after the treatment were compared. The level of PLT decreased obviously (P < 0.01), WBC, RBC and HGB decreased (P < 0.05), neutrophil increased slightly (P > 0.05) 1 month after 131I therapy. The level of WBC, neutrophil, RBC, HGB and PLT were near to the level of before treatment after 6 months (P > 0.05) of 131I therapy. At the end of follow up, the components of the peripheral blood in the high dose group (7.4GBq-11.1GBq) were not significantly different from those at the beginning. The results showed that, there was only a transient impact on the peripheral blood by the 131I therapy in the children and adolescents with differentiated thyroid carcinoma.
