RESUMO

Oral mucositis (OM) is a common complication during hematopoietic stem cell transplantation. When OM occurs, with the destruction of oral mucosal barrier function, the formation of oral bacterial environment is accelerated, which has a negative impact on the life quality and clinical outcomes of patients. OM can be prevented by keeping the oral cavity clean, using the mouthwash properly, and limiting the delivery of cytotoxic drugs to oral tissues. This review describes the research progress on the prevention and local intervention of OM in patients with hematopoietic stem cell transplantation, in order to provide ideas for the selection of clinical intervention and management strategies on OM, and to provide references for exploring effective management methods for OM.

RESUMO

Aim To predict the potential mechanism of ophiopogonin D (OPD) against pulmonary fibrosis by network pharmacology, and further verify it by experiment in vivo. Methods This study found that ophiopogon was the most frequently used drug in the treatment of pulmonary fibrosis with deficiency of Qi and Yin through data mining. In order to explore its material basis, network pharmacology analysis was carried out. A model of pulmonary fibrosis was established by bleomycin, and different concentrations of ophiopogonin D were administered to verify the results of the pharmacological network. Results Firstly, through network pharmacology analysis, it was found that mitophagy might be the potential target for ophiopogon to exert anti-pulmonary fibrosis effect. Meanwhile, network topology analysis showed that OPD had the greatest relationship with mitophagy. Animal experiments showed that OPD could relieve pulmonary fibrosis and reduce collagen deposition in mice. At the same time, the detection of mitophagy related proteins showed that the compound could increase the expression of PINK1 and Parkin proteins, reduce the content of P62 protein in lung tissue, and reduce the intracellular ROS level. Conclusions OPD can improve mitochondrial function and play an anti-pulmonary fibrosis role by promoting PINKl/Parkin dependent mitophagy in lung tissue.

RESUMO

Ligustrum lucidum is a woody perennial plant of genus Ligustrum in family Oleaceae. Its dried fruit has high medicinal value. In this study, the authors evaluated the variability and species identification efficiency of three specific DAN barcodes(rbcL-accD, ycf1a, ycf1b) and four general DAN barcodes(matK, rbcL, trnH-psbA, ITS2) for a rapid and accurate molecular identification of Ligustrum species. The results revealed that matK, rbcL, trnH-psbA, ITS2 and ycf1a were inefficient for identifying the Ligustrum species, and a large number of insertions and deletions were observed in rbcL-accD sequence, which was thus unsuitable for development as specific barcode. The ycf1b-2 barcode had DNA barcoding gap and high success rate of PCR amplification and DNA sequencing, which was the most suitable DNA barcode for L. lucidum identification and achieved an accurate result. In addition, to optimize the DNA extraction experiment, the authors extracted and analyzed the DNA of the exocarp, mesocarp, endocarp and seed of L. lucidum fruit. It was found that seed was the most effective part for DNA extraction, where DNAs of high concentration and quality were obtained, meeting the needs of species identification. In this study, the experimental method for DNA extraction of L. lucidum was optimized, and the seed was determined as the optimal part for DNA extraction and ycf1b-2 was the specific DNA barcode for L. lucidum identification. This study laid a foundation for the market regulation of L. lucidum.

RESUMO

Objective@#Most acute promyelocytic leukemia cases are characterized by the PML-RARa fusion oncogene and low white cell counts in peripheral blood.@*Methods@#Based on the frequent overexpression of miR-125-family miRNAs in acute promyelocytic leukemia, we examined the consequence of this phenomenon by using an inducible mouse model overexpressing human miR-125b.@*Results@#MiR-125b expression significantly accelerates PML-RARa-induced leukemogenesis, with the resultant induced leukemia being partially dependent on continued miR-125b overexpression. Interestingly, miR-125b expression led to low peripheral white cell counts to bone marrow blast percentage ratio, confirming the clinical observation in acute promyelocytic leukemia patients.@*Conclusion@#This study suggests that dysregulated miR-125b expression is actively involved in disease progression and pathophysiology of acute promyelocytic leukemia, indicating that targeting miR-125b may represent a new therapeutic option for acute promyelocytic leukemia.

RESUMO

Ligustri Lucidi Fructus, the sun-dried mature fruit of Ligustrum lucidum, is cool, plain, sweet, and bitter, which can be used as both food and medicine, with the effects of improving vision, blacking hair, and tonifying liver and kidney. It takes effect slowly. However, little is known about the genetic information of the medicinal plant and it is still a challenge to distinguish Ligustrum species. In this study, the complete chloroplast genome of L. lucidum was obtained by genome skimming and then compared with that of five other Ligustrum species, which had been reported. This study aims to evaluate the interspecific variation of chloroplast genome within the genus and develop molecular markers for species identification of the genus. The result showed that the chloroplast genome of L. lucidum was 162 162 bp with a circular quadripartite structure of two single-copy regions separated by a pair of inverted repeats. The Ligustrum chloroplast genomes were conserved with small interspecific difference. Comparative analysis of six Ligustrum chloroplast genomes revealed three variable regions(rbcL-accD, ycf1a, and ycf1b), and ycf1a and ycf1b can be used as the species-specific DNA barcode for Ligustrum. Phylogeny analysis provided the best resolution of Ligustrum and supported that L. lucidum was sister to L. gracile. This study clarified the genetic diversity of L. lucidum from provenance, which can serve as a reference for further analysis of pharmacological differences and breeding of excellent varieties with stable drug effects.

RESUMO

Objective To evaluate whether patients aged over 70 years old is an independent predictor for prolonging the waiting time, and to know whether the population can be prioritized in the emergency grading system. Methods A retrospective analysis was conducted. All the clinical data of adult visitors to the emergency department of the Third Affiliated Hospital of Sun Yat-sen University from January 1st in 2016 to December 31st in 2017 were collected. All the patients received four emergency first aid triages and treated according to the emergency classification results. The relevant data of selected patients were retrieved from the hospital information system (HIS) including gender, age, triage level, triage department, outpatient service and initial diagnosis time. According to the conclusion of pre-phase analysis that the patients over 65 years old was not associated with waiting time, all subjects were divided into ≥ 70 years old group and < 70 years old group for further analysis. The waiting time was defined as the difference in registration time and the initial diagnosis time, and the patients were divided into two groups according to whether it exceeded the target waiting time (TWT). TWT was considered as follows according to each emergency triage level: level Ⅰ instantly, level Ⅱ10 minutes, level Ⅲ 30 minutes, level Ⅳ 60 minutes (subacute patients) or 2-4 hours (non-urgent patients). The multivariable Logistic regression analysis was conducted to determine whether the factor that aged over 70 years old was an independent predictor for prolonging waiting time over TWT.Results There were 272989 adult visits recorded totally, of which obstetrics and gynecology department visits, incomplete personnel information, invalid diagnosis (or no diagnosis), triage levelⅠ, abnormal waiting time (the waiting time exceeded 99% of the average waiting time of all samples) records were excluded. Finally, 219299 emergency patients were enrolled, with 102218 male and 117081 female, which showed an average age of (35.86±15.81) years, 208082 patients (94.9%) in < 70 years old group, and 11217 (5.1%) in ≥ 70 years old group. Totally 191588 patients (87.4%) were diagnosed within the TWT, and the number was 86.5% (9701/11217) in the ≥ 70 years old group. Multivariate Logistic regression analysis indicated that age under 70 years old was independently associated with an exceeded TWT [adjusted odds ratio (OR) = 1.106, 95% confidence interval (95%CI) = 1.041-1.176,P = 0.001]. Meanwhile, visits during the peak period or low triage level was also a risk factor of exceeded TWT. According to the Logistic regression analysis of different departments and removing other factors, in the emergency internal medicine with the most elderly patients, the probability of exceeded TWT in ≥ 70 years old group was lessened (adjustedOR = 1.203, 95%CI = 1.121-1.291,P < 0.001).Conclusion In the emergency pre-grading system, the elderly aged (≥ 70 years old) was more likely to see a doctor in TWT than those under 70 years old, so they were easy to get priority treatment.

RESUMO

Notch gene encodes a class of highly conserved cell surface receptors that determine the fate of cells in embryonic development and mature tissue, which is an important pathway of communication between adjacent cells, and then regulates of cell development, proliferation and apoptosis. The occurrence and progression of multiple tumors are associated with abnormal Notch signaling pathways. For different tumors, it is important significance for anti-tumor therapy to effectively regulate the Notch participants and target molecules.

RESUMO

Antiepileptic drugs and anti-infective drugs are prescribed at the same time in many cases. The clinically important interactions between them may lead to adverse reaction or failure of treatment. We summarize these clinically important interactions with case reports and mechanism of action. The combinations include macrolides and carbamazepine, carbapenems and valproic acid, ciprofloxacin and carbamazepine, phenytoin, triazole antifungal agents and carbamazepine, phenytoin, and phenobarbital, antituberculosis drugs and phenytoin, diazepam, carbamazepine and oxcarbazepine, anthelmintic drugs and phenytoin, carbamazepine and phenobarbital. While new antiepileptic drugs used in China like oxcarbazepine, lamotrigine, topiramate and levetiracetam have little clinically important interaction with anti-infective drugs.

RESUMO

Currently, clinical antiviral therapies for chronic hepatitis B virus (HBV) infection can efficiently control HBV replication. However, it remains difficult to achieve HBsAg clearance and sustains off-therapy, that is the functional cure of chronic hepatitis B ( CHB) in most of patients.Host immune responses play critical roles in HBV clearance and HBV infection control by activating innate and adaptive immune responses, resulting in improving the functional cure rate of patients with CHB.This article reviews the recent advances in immunology studies related to functional cure of CHB.

RESUMO

OBJECTIVE:To investigate the characteristic and regularity of antineoplastics-induced ADR,and to provide refer-ence for safety use of antineoplastics in clinic. METHODS:In retrospective study,309 cases of antineoplastics-induced ADR col-lected from a special hospital during Jan.-Dec. 2015 were summarized and analyzed statistically. RESULTS:Among 309 ADR cas-es,female (58.25%) was more than male (41.75%). The patients aged more than 40 years old occupied the highest proportion (78.64%). Main route of administration was intravenous dripping(94.82%). There were 225 cases of severe ADR(72.82%). To-tal 268 cases were improved after drug withdrawal or treatment(86.73%). A total of 24 kinds of antineoplastics were involved in 309 ADR cases;most of ADR were induced by docetaxel, followed by cisplatin, cyclophosphamide. Most common organs/system involved in ADR was hemopoietic system (64.24%),main clinical manifestation was myelosuppression. CONCLUSIONS:It is necessary to strengthen ADR monitoring, prevent, discover and treat antineoplastics-induced ADR timely,in order to reduce the harm to the patients and ensure the safety and effectiveness of drug treatment.

RESUMO

Objective: To study the pharmacokinetics of phosphatidylserine (PS)-containing curcumin-loaded nanostructured lipid carriers (Cur-mNLC), curcumin-loaded nanostructured lipid carriers without PS (Cur-NLC), and curcumin solutions in SD rats. Methods: Blood samples were collected from the orbit of rats at different periods of time after they were ip injected with Cur-solution, Cur-NLC, and Cur-mNLC at 10 mg/kg (corresponding to Cur), then Cur content in plasma was determined by UPLC-MS/MS, and the pharmacokinetic parameters were calculated by DAS software. Results: The pharmacokinetic parameters of Cur-solution, Cur-NLC, and Cur-mNLC were respectively calculated as follows: Cmax (29.453±1.146), (20.045±0.818), (15.865±0.409) μg/L; MRT(0-∞) (18.196±1.456), (18.196±1.456), (89.252±12.049) h. The AUC(0-∞) of Cur-mNLC was 2.58 and 1.48 times larger than those of Cur-solution and Cur-NLC. Conclusion: The Cmax of Cur-NLC and Cur-mNLC are lower than that of Cur-solution, but the MRT(0-∞) of Cur-NLC and Cur-mNLC are longer than that of Cur-solution. Compared with Cur-NLC, Cur-mNLC has better sustained-release behaviors, which contributes to its superior bioavailability.

RESUMO

<p><b>PURPOSE</b>Traumatic optic neuropathy (TON) is a serious complication of head trauma with the incidence rate of 0.5%-5%. The aim of this study was to investigate the therapeutic efficacy of endoscopic decompression of the optic canal for optic nerve injuries.</p><p><b>METHODS</b>In this study, 11 patients treated in our hospital from January 2009 to January 2015 with the visual loss resulting from TON were retrospectively reviewed for preoperative vision, visual evoked potential (VEP) scan, surgical approach, postoperative visual acuity, complications, and follow-up results.</p><p><b>RESULTS</b>All these patients received endoscopic decompression of the optic canal. At the 3-month follow- up, the visual acuity improvement rate of the 11 patients was 45.5%. The vision acuity of 2 cases improved from hand movement to 0.08 and 0.3 after operation. Another patient's vision acuity returned to 0.05 compared to light sensation preoperatively. Two cases had finger counting before surgery but they had a vision acuity of 0.4 and light sensation respectively after surgery. However, the other 6 cases' vision did not improve after surgery.</p><p><b>CONCLUSION</b>Endoscopic decompression of the optic canal is an effective way to cure TON. VEP could be used as an important reference for preoperative and prognosis evaluation. Operative time after trauma is only a relative condition that may affect the therapeutic effect of optic canal decompression. Poor results of this procedure may be related to the severity of the optic nerve injury.</p>
