RESUMO

ObjectiveTo systematically evaluate the safety of Tianzhi granules used in the treatment of mild-to-moderate vascular dementia. MethodA randomized, double-blind, double-simulated, positive drug/placebo parallel controlled multi-center phase Ⅳ clinical trial and an open multi-center phase Ⅳ clinical trial of Tianzhi granules in the treatment of mild-to-moderate vascular dementia were conducted. Safety data of 1 492 patients were included and analyzed according to inclusion and exclusion criteria. The main evaluation measures were the incidence rate of adverse events/adverse reactions, laboratory indicators, vital signs, and electrocardiogram (ECG) results. ResultA total of six adverse events possibly related to the test drug occurred in 520 patients of the double-blind trial, and the symptoms were all mild and recovered. The incidence of adverse events was not statistically different among Tianzhi granules, donepezil, and placebo groups. Nine adverse events possibly related to the test drug were observed in 972 patients of the open trial, and the symptoms were mild and recovered. Laboratory tests (blood routine, urine routine, liver function, kidney function, and coagulation) and vital signs were compared before treatment (baseline) and after treatment of 12 and 24 weeks, respectively. There was no statistical significance in the main indicators before and after treatment. In the double-blinded trial, there was no significant difference in safety indicators between different groups before and after treatment. The most frequent adverse reaction was gastrointestinal discomfort, with an incidence rate of 6.64‰. ConclusionAdverse reactions occasionally occur in patients using Tianzhi granules, and it is safe to use Tianzhi granules to treat mild-to-moderate vascular dementia clinically.

BACKGROUND:In recent years, increasing research emphasizes the puncture position of the foramen ovale in skul , but most of the positin methods require a higher personal experience of surgeons and lack of individualized quantitative parameters. OBJECTIVE:To establish a visualized digital model of the foramen ovale in skul , explore the reasonable puncture path and puncture depth of percutaneous treatment of foramen ovale puncture for trigeminal neuralgia, and develop the individualized treatment of trigeminal neuralgia. METHODS:Head CT images from healthy adult male volunteers were obtained and were input into three-dimensional reconstruction software MIMICS 10.01, the three-dimensional visualized models of the skul and skin were established. Using the models, the puncture path of the foramen ovale was designed and the preliminary model of the puncture locator was plotted. RESULTS AND CONCLUSION:The three-dimensional visualized digital model of the foramen ovale puncture path was established with CT scan images by using MIMICS software, which provides reliable anatomical data for clinical teaching and lays the groundwork for the simulation of puncture surgery. On the three-dimensional models, the“needle points”,“target points”, and“midpoint”were determined, and the triangle consisted of the three points was regarded as“positioning plane”. Using these parameters, the positioning instrument is characterized by simple structure, convenient operation, high positioning precision and short period of exposure to radiation, it simulates the foramen ovale puncture needle depth and needle direction in a precise and individualized manner.

Objective To determine the distribution profile of apocrine sweat glands in axillary region of patients with axillary osmidrosis,and to compare their distribution at different sites.Methods Fifteen patients with axillary osmidrosis were enrolled in this study from September to December 2010.All the patients underwent surgical removal of apocrine sweat glands under direct vision.Full-thickness skin tissue measuring 2 mm in width was excised down to the axillary superficial fascia at the incisional surgical sites from five patients.Five points,which were at the center of axillary region (point 1),1 cm away from the center of axillary region (point 2),1 cm inside the edge of axillary hair-bearing area (point 3),the edge of axillary hair-bearing area (point 4),and 1 cm outside the edge of axillary hair-bearing area (point 5),were marked,and dark red,rough granular subcutaneous tissue was obtained at these points in 10 patients with axillary osmidrosis.Results The secretory portion of apocrine sweat glands was mainly distributed in the reticular dennis and superficial subcutaneous adipose tissue,but no apocrine sweat glands were obs erred in the epidermis,dermal papilla or axillary superficial fascia.The distribution profile of apocrine sweat glands was consistent with that of axillary hairs.There were numerous apocrine sweat glands in the center of axillary region,but only a small number at the edge of axillary hair-bearing area,and no apocrine sweat gland was observed at 1 cm outside the edge.The percentage of apocrine sweat gland area at point 1-5 was 74.1%,46.6%,25.3%,2.1%,and 0 respectively,with significant differences between point 1 and 2 (t--29.78,P< 0.01),point 2 and 3 (t--9.76,P< 0.01),point 3 and 4 (t =20.83,P< 0.01),but not between point 4 and 5 (t =1.96,P > 0.05).Conclusions During the surgical treatment of axillary osmidrosis,the removal of apocrine sweat glands should be extended to the reticular dennis and superficial subcutaneous adipose tissue in depth and the edge of axillary hair-bearing area in width,and there is no need to blindly increase the extent of removal.

BACKGROUND:Recently, scholars al over the world have made numerous studies on three-dimensional visualization model of reconstructing maxil ofacial bone tissue, skin, and muscle in the ordinary PC. However, few studies concerned the three-dimensional visualization model of constructing maxil ofacial bone tissue, skin, muscle and blood vessels. OBJECTIVE:To establish a three-dimensional visualization model of the complete maxil ofacial region using an ordinary PC based on spiral CT, MRI data, and three-dimensional reconstruction software. METHODS:One healthy male adult was chosen as the object in this study. CT and MRI were performed respectively to acquire the DICOM standardized image of the sample. Al CT and MRI images were imported to Mimics to construct three-dimensional visualization models of the bones in the maxil ofacial region, some masseter muscles, trigeminal cistern, internal carotid artery and internal jugular vein. Three-dimensional model of the bones in the maxil ofacial region was chosen as the reference. Models of muscles, blood vessels and trigeminal cistern established by MRI images were imported, and registration in space of three-dimensional model was performed. At last, three-dimensional visualization model of the complete maxil ofacial region was acquired. RESULTS AND CONCLUSION:The integrated three-dimensional models of the bones, muscles, skin, trigeminal cistern and blood vessels in the maxil ofacial region established accurately reflected the complex anatomic structure of maxil ofacial region, provided reliable anatomic data for clinical diagnosis and laid a good foundation for sham operations in the future.

RESUMO

OBJECTIVE To understand the pathogens and their drugs resistance in general surgery department and provide rational suggestion of antibiotics use for clinic treatment.METHODS A total of 158 cases with nosocomial infection among the general surgery department inpatients from Jun 2006 to Oct 2008 were retrospectively analyzed and summarized.RESULTS The common nosocomial infection sites were the lower respiratory tract,gastrointestinal tract,urinary tract and surgical sites.The G-bacilli of nosocomial infections in turn were Escherichia coli(18.02%),Acinetobacter baumannii(11.26%),Pseudomonas aeruginosa(7.66%),and Klebsiella pneumoniae(4.50%).The main G+cocci were Staphylococcus aureus(21.62%),Enterococcus faecium(5.86%)and E.faecalis(3.15%)in turn.In G-bacilli,the sensitivity to imipenem was the highest from 58.82% to 100.00%.The sensitivity to amikacin were more than 70.00% except A.baumannii,and to sulbactam/cefoperazone were more than 50.00% except Pseudomonas aeruginosa.In G+ cocci,the sensitivity to vancomycin of S.aureus and E.faecium was 100.00% and 84.62%.CONCLUSIONS Investigating the pathogens and their drug resistance in general surgery department is very important to prevent and control nosocomial infections.

RESUMO

Objective To determine the pharmacokinetic interaction between cefalor and bromhexine in healthy Chinese volunteers. Methods Twelve subjects received a cefaclor (CEF) treatment, a bromhexine (BHX) treatment, and a co-treatment of CEF and BHX with a 3 × 3 Latin square design. The wash-out time between periods was 14 days. The plasma and urine drug concentrations of CEF and BHX were detected by HPLC-UV and LC/MS, respectively. Results All the 12 volunteers completed the study. There were no significant differences in AUC0-t and Cmax of CEF in logarithm between the single administration group of CEF and the co-administration group of CEF with BHX. Two one sided t-test showed that CEF was bioequivalent in the 2 groups. There were no significant differences in tmax, MRT, t1/2, and Clr between the 2 groups. Vd/F was significantly lower in the single CEF group than in the co-administration group of CEF and BHX. There were no significant differences of AUC0-t and Cmax of BHX in logarithm between the single administration group of BHX and the co-administration group of BHX with CEF. Two one sided t-test showed that BHX was bioequivalent in the 2 groups. There were no significant differences in tmax, MRT, t1/2, Vd/F, and Clr between the 2 groups. Conclusion There is no significant pharmacokinetic parameter change in the drug absorption, metabolism, and excretion, but Va/F of CEF significant increases in the co-administration of CEF with BHX. The co-administration of CEF and BHX has no adverse drug interaction. The increase of Vd/F may be a favorable drug interaction, which may be the mechanism of the synergistic effect of the 2 drugs.

RESUMO

Purpose:To investigate the relationship between the expression tansforming growth factor ?(TGF ?) and the differentiation, invasion of colorectal adenocarcinoma.Methods:To observe the expression of transforming growth factor ? and transforming growth factor ? receptor in 68 samples of colorectal adenocarcinoma with immunohistochemical technique. Results:The cells with positive reaction of TGF ? antibody were scattered in the stroma. The cells with infiltrative growth had positive TGF ? staining.Conclusions:It was indicated that TGF ? might play an important role in the invasion and metastasis of colorectal adenocarcinoma.

RESUMO

A batch affinily adsorption method with heparin-sepharose 4B gel has been described for preparation and purification of antithrombin Ⅲ from plasma. The me- thod is suitable for batch preparation. The processes of each batch are completed only in 3 days. The recovery of activity was 22.4-42.3% from 20 liters of plasma. By fur- ther purification, the recovery of activity and antigen was 26.3% and 20.3% respec- tively, the specific activity was 9.9U/mg. The purified AT-Ⅲ showed a single band on disc gel electrophoresis, and the levels of purity, biologic activity and the recov- ery of activity are similar to those of for- eign reports.
