RESUMO

AIM:To assess the evolving burden of cataracts in China from 1990 to 2019.METHODS: Data on disease burden related to cataracts in China were retrieved from the Global Burden of Disease(GBD)2019 study based on large public databases. Utilizing data from the GBD 2019 study, we extracted information on cataract-related disease burden in China from extensive public databases. Analysis of prevalence and disability-adjusted life years(DALYs)associated with cataracts in China was conducted based on GBD 2019 findings. The variable characteristics of age-standardized prevalence rates(ASPR)and age-standardized DALYs rates(ASDR)in China and its neighboring countries were also explored.RESULTS: Between 1990 and 2019, the number of prevalent cases of blindness and vision loss caused by cataracts in China increased by 223.54%, and the corresponding DALYs raised by 142.14%. Over the past 30 years, females exhibited higher age-standardized prevalence and DALYs rates compared to males. Meanwhile, individuals aged 65 to 84 years were found to be more susceptible to cataracts than other age groups. Compared with neighboring countries, China ranked from the 9th position in 1990(867.09, 95%UI: 761.36 to 975.42, per 100 000 population)to the 11th in 2019(991.56, 95%UI: 861.52 to 1131.04, per 100 000 population)in ASPR, while from the 9th in 1990(65.85, 95%UI: 46.39 to 89.41, per 100 000 population)to the 10th position in 2019(59.16, 95%UI: 41.70 to 80.15, per 100 000 population)in ASDR. However, on a global scale, China maintained relatively low ASDR and ASPR for cataracts in 2019.CONCLUSION: The study highlights a substantial rise in the prevalence and DALYs associated with blindness and vision loss due to cataracts from 1990 to 2019 in China, and underscores the urgent need for increased early screening of cataracts, particularly among the elderly and females.

RESUMO

One case of knee infection after anterior cruciate ligament reconstruction caused by the gram-positive anaerobic bacterium Finegoldia magna was reported. The patient was admitted to hospital due to fever and knee joint swelling and pain after anterior cruciate ligament reconstruction. Through medical history, physical examination, imaging examination and next-generation sequencing, it was confirmed that the infection was caused by Finegoldia magna. Through literature review, 37 literatures on infectious diseases caused by Finegoldia magna was retrieved and analyzed, and the identification points of anaerobic bacteria, the application of second-generation sequencing technology and the treatment status of infection after anterior cruciate ligament reconstruction were reviewed. The incidence of infection after arthroscopic anterior cruciate ligament reconstruction is low, while anaerobic infection is even more rare and difficult to culture. The next-generation sequencing can be used to assist the diagnosis. On the basis of giving priority to the preservation of the reconstructed ligament, the combined use of arthroscopic debridement, irrigation and sensitive antibiotics is the main treatment method.

RESUMO

Background Dentists are a high-risk population of work-related musculoskeletal disorders (WMSDs), where the body part with the highest prevalence is the neck. Objective To analyze potential influencing factors of neck pain among dentists, and explore a prediction model of neck pain in dentists. Methods Dentists from different hospitals in Fuzhou were selected as study subjects by stratified cluster sampling according to hospital characteristics (dental hospitals, general hospitals, and dental clinics). The basic information, presentation of WMSDs, and its influencing factors were investigated by using the Chinese version of Musculoskeletal Disorders Questionnaire and the Subjective Workload Assessment Technique. A total of 655 questionnaires were collected, of which 603 were valid, with an effective rate of 92.1%. Multiple logistic regression was used to analyze potential influencing factors of neck pain in dentists. A prediction model of neck pain of dentists was constructed by using neural network model, and the prediction efficiency of the model was evaluated. Results The neck was the body part with the highest prevalence (43.8%, 264/603) of WMSDs among dentists. The results of multiple logistic regression analysis showed that female (OR=2.709, 95%CI: 1.852-3.962, P <0.001), working age of 10-<20 years (versus <10 years, OR=3.836, 95%CI: 2.471-5.957, P<0.001), keeping head up or down for a long time (OR=8.492, 95%CI: 2.203-32.731, P=0.002), holding head sideways for a long time (OR=2.210, 95%CI: 1.376-3.550, P<0.001), maintaining the same sitting spot for a long time (OR=4.336, 95%CI: 2.192-8.579, P<0.001), and psychological load value ≥70 (versus <40, OR=1.901, 95%CI: 1.038-3.480, P=0.037) increased the risk of neck pain in dentists. Sufficient operating space (OR=0.507, 95%CI: 0.302-0.850, P=0.010) and doing some exercise during work break (OR=0.670, 95%CI: 0.453-0.991, P=0.045) reduced the risk of reporting neck pain among dentists. A neural network prediction model of dentists' neck pain was constructed with 1 hidden layer and 6 hidden layer neurons. The percentage of correct prediction of training set was 89.6%, and the percentage of correct prediction of test set was 83.9%. The order of importance of the independent variables included in the model were working age, holding head sideways for a long time, psychological load, etc. The result of neural network model of neck pain among dentists showed that the area under the curve of receiver operator characteristic (ROC) was 0.940 (95%CI: 0.922-0.958, P<0.001). When the maximum diagnostic value was determined by the ROC curve, the sensitivity was 84.8%, the specificity was 91.2%, and the Youden Index was 0.760. Conclusion Neck pain of dentists is affected by many factors, such as individual factors (gender and working age), ergonomic factors (keeping various postures and operations for a long time, operating space, etc.), psychological factors (different levels of psychological load) and so on. The neural network model can be used as a prediction tool to explore the risk of reporting neck pain among dentists.

RESUMO

OBJECTIVE To evaluate the quality of total flavonoids of Lamiophlomis rotata from different producing areas. METHODS Total flavonoids of S1-S15 batches of L. rotata from different producing areas were extracted by percolation and purified by polyamide column. The content of total flavonoids was determined by UV spectrophotometry, and its purity was calculated. HPLC fingerprint chromatograms and control fingerprint of total flavonoids from 15 batches of L. rotata were established with Similarity Evaluation System of Chromatographic Fingerprint of TCM (2012 version). Their similarities were analyzed. Cluster analysis, principal component analysis and orthogonal partial least squares-discriminant analysis were used to evaluate the quality of total flavonoids from 15 batches of L. rotata, and the main components which affected the quality of total flavonoids were analyzed. RESULTS The purities of total flavonoids from 15 batches of L. rotata were 55.82%-94.12%, with an average value of 77.72%; a total of five common peaks were identified in the fingerprint, and No. 3 peak was luteolin; the similarities between the fingerprint of each batch of samples and the control fingerprint were 0.925-1.000. By cluster analysis, S1 and S3-S9 were clustered into the first class, which were samples from Qinghai Province and Tibet Autonomous Region. S14 and S15 were clustered into the second class, which were samples from Yunnan Province. S10-S13 were clustered into the third class, which were all samples from Sichuan Province. S2 was clustered into the fourth class. The principal component analysis showed that the qualities of samples from the first and fourth classes were better; peaks 2, 3 and 5 were identified as the main components that caused the differences among different batches of samples by orthogonal partial least squares-discriminant analysis. CONCLUSIONS The qualities of total flavonoids of L. rotata from Qinghai Province and Tibet Autonomous Region are better.

RESUMO

Objective:To observe any effect of transcranial direct current stimulation (tDCS) on the speed and accuracy of picture naming and on the phonological fluency of aphasic stroke survivors.Methods:Twenty-four stroke survivors with aphasia were randomly divided into an observation group and a control group, each of 12. In addition to language training and picture naming training, the observation group received 20 minutes of tDCS over the left dorsolateral prefrontal cortex (DLPFC) daily, 5 days a week for 2 weeks. The control group was given sham stimulation. Before and after the 2 weeks of treatment, both groups were tested for picture naming and phonological fluency. Results:Significant improvement in the number of accurately pronounced high-frequency words and in reaction time was observed in both groups, but the observation group′s average reaction time for high-frequency words was significantly shorter than the control group′s average. The observation group′s average reaction time for low-frequency words had also improved significantly. After the 2 weeks of treatment, the phonological fluency of the observation group was significantly better than before the treatment and better than that of the control group. Conclusions:tDCS applied over the left DLPFC of stroke survivors with aphasia can promote lexical retrieval and strengthen their executive and control functioning.

RESUMO

Objective:To assess the impact of kidney damage on the progression and prognosis of coronavirus disease 2019 (COVID-19) patients. Methods:The databases including CNKI, Wanfang Chinese Academic Journal Database, Pubmed, EMBASE and Cochrane databases were searched. The data retrieval period was from December 2019 to June 2020. During this period, RevMan 5.2 was used to extract and analyze data from the literature. Results:In this study, 15 articles, covering 11 448 COVID-19-related cases, were selected from the target databases. Meta-analysis results indicated that, for COVID-19 patients with acute kidney injury (AKI), the proportion of COVID-19 patients with AKI who received intensive care unit (ICU) treatment was significantly higher than that of patients without AKI ( OR=10.83, 95% CI 9.43-12.45, Z=33.53, P<0.001). Among them, the Asian group of COVID-19 patients with AKI accounted for 3.4% of all COVID-19 patients. The Asian group of COVID-19 patients with AKI who received ICU treatment accounted for 74.1% of all COVID-19 patients with AKI. Meta-analysis showed that the proportion of COVID-19 patients with AKI in the Asian group receiving ICU treatment was significantly higher than that of COVID-19 patients without AKI ( OR=18.66, 95% CI 9.85-35.34, Z=8.98, P<0.001). COVID-19 patients with AKI in the European and American groups accounted for 36.5% of all COVID-19 patients, and the European and American groups of COVID-19 patients with AKI who received ICU treatment accounted for 53.3% of all COVID-19 patients with AKI. Meta-analysis showed that the proportion of COVID-19 patients with AKI in the European and American groups receiving ICU treatment was significantly higher than that of COVID-19 patients without AKI ( OR=10.58, 95% CI 9.18-12.21, Z=32.40, P<0.001). The death risk of COVID-19 patients with AKI was significantly higher than that of patients without AKI ( OR=56.46, 95% CI 15.86-200.96, Z=6.23, P<0.001). COVID-19 patients with renal impairment had a higher risk of worsening and death than those without renal impairment ( OR=6.40, 95% CI 4.14-9.90, Z=8.36, P<0.001). Subgroup analysis showed COVID-19 patients with positive urine protein had a higher risk of worsening and death than those without positive urine protein ( OR=6.27, 95% CI 3.88-10.14, Z=7.49, P<0.001), and COVID-19 patients with elevated serum creatinine had a higher risk of worsening and death than those without elevated serum creatinine ( OR=7.13, 95% CI 2.58-19.72, Z=3.79, P<0.001). Conclusions:Patients with COVID-19 combined with AKI or only with positive urine protein and elevated serum creatinine are the risk factors for aggravation and even death. It is recommended that, in the diagnosis and treatment of COVID-19 patients, an optimization plan should be adopted to avoid or reduce the burden on the kidneys, meanwhile the patients' kidney damage should be paid closely attention to for protection and treatment in the early stage to avoid the occurrence of AKI. For patients who already suffered from AKI, replacement therapy, which would prevent these patient's condition from getting worse or even death, should be promptly implemented to alleviate the impact of kidney damage.

RESUMO

ObjectiveTo investigate the efficacy and safety of elbasvir/grazoprevir in patients with genotype 1 hepatitis C in the real world. MethodsA total of 35 patients with hepatitis C who received elbasvir/grazoprevir treatment in Guangzhou Eighth People’s Hospital, Guangdong Provincial Hospital of Traditional Chinese Medicine, and Guangdong General Hospital from August 2018 to March 2019 were enrolled, treated for 12 weeks, and then followed up for 12 weeks after drug withdrawal. The patients were observed in terms of sustained virologic response at week 12 after drug withdrawal (SVR12), biochemical response, and incidence rate of adverse events during treatment and follow-up. The Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between groups, and the Mann-Whitney U test was used for further comparison between two groups; the chi-square test was used for comparison of categorical data between groups. A logistic regression analysis was used to investigate the risk factors for virologic response in patients with hepatitis C. ResultsAmong the 35 patients with HCV infection, 97.1% (34/35) had genotype 1b HCV and 2.9% (1/35) had genotype 1a HCV; of all patients, 28 (80%) were non-cirrhotic patients with chronic hepatitis C and 7 (20%) had compensated liver cirrhosis. At the end of treatment, the virologic response rate of 100% (28/28) and SVR12 was 94.74% (18/19). In addition, age, sex, baseline HCV RNA load, previous treatment history, presence or absence of liver cirrhosis, renal function, and presence or absence of other diseases did not affect the treatment outcome (all P>0.05). There were significant changes in the levels of alanine aminotransferase, aspartate aminotransferase, gamma-glutamyl transpeptidase, and albumin from baseline to the end of 12-week treatment (Z=-7.131, -6.797, -3.060, and -2.875, all P<0.05). No patient experienced drug withdrawal during treatment. ConclusionThis study confirms that elbasvir/grazoprevir has good efficacy and safety in the treatment of hepatitis C in domestic real-world studies.

RESUMO

Objective To investigate the effect of rosiglitazone on the epithelial-mesenchymal transition process of renal tubular epithelial cell induced by transforming growth factor-β(TGF-β). Methods The human renal tubular epithelial cell line HK-2 was cultured in vitro and treated with TGF-β at the presence or absence of rosiglitazone, and the morphological changes of HK-2 cells were observed. Then PPARγ,SMAD family number 2/ 3,E-cadherin and Vimentin were detected by western blot, and the mRNA expression of PPARγ, E-cadherin, Vimentin, zinc finger transcription factor Snail and Slug were measured by realtime quantitative PCR. Finally, dual luciferase reporter assay was performed to detect the effect of TGF-β and rosiglitazone on the tran-scription activity of E-cadherin promoter. Results TGF-β could induce the EMT process of HK-2, including morphological changes such as longer pseudopod and broader cell space. TGF-β also activated the SMAD2/ 3 signaling pathway, with expression changes of E-cadherin and Vimentin. Rosiglitazone reversed the morphological changes of HK-2 cells induced by TGF-β, with shor-ter pseudopod and narrower cell space. Rosiglitazone also inhibited the mRNA and protein expressions of Vimentin, and restored the mRNA and protein expression of E-cadherin by up-regulating PPARγ. The activity of E-cadherin promoter was enhanced under the treatment of rosiglitazone. Conclusion Rosiglitazone can antagonize the EMT process of renal tubular epithelial cell induced by TGF-β, through the activation of PPARγ and stimulating transcription and protein expression of E-cadherin.

RESUMO

Objective To prospectively evaluate the risk factors for the decline of residual renal function (RRF) in new peritoneal dialysis (PD) patients. Methods A total of 84 new PD patients in our PD center were included in this study.Clinical comprehensive assessment were made, and regression models was established to analyze the relationship between the decline of RRF and clinical indicators, which included the rate of peritonitis, systolic pressure, diastolic pressure, urine volume,24 h urinary protein, serum albumin,C-reactive protein(CRP),history of diabetes mellitus, and the use of angiotensin converting enzyme inhibitor (ACEI) or angiotensin receptor blockers (ARB) drugs, cardiac functional grading, sodium and water retention and biochemical indicators. The primary outcome was defined as two consecutive urine volume ≤ 100 ml/24 h.Results The mean follow-up time was (11.7± 1.1) months, primary outcome occurred in 20 patients, accounting for 23.8%,and their average period progressed to the primary outcome was (10.5±2.0) months. The 20 patients had higher ultrafiltration volume [(551.6±328.2) ml vs (294.1±288.0) ml,P=0.001],higher systolic blood pressure [(145.2±16.5) mmHg vs (136.0±13.8) mmHg,P=0.016],worse cardiac functional grading [(1.7±0.8) vs (1.3±0.4),P=0.000],more serious water-sodium retention [(1.0±0.7) vs (0.6±0.5),P=0.012],higher peritonitis rates (35.0% vs 4.7%,P=0.000),lower Kt/V [(1.7 ± 0.4) vs (2.0 ± 0.3),P=0.003],lower hemoglobin levels [(89.0± 14.9) g/L vs (99.5± 17.8) g/L,P=0.020],higher C-reactive protein levels [(19.4±34.4) mg/L vs(8.7±12.6) mg/L,P=0.017],higher Scr levels [(1 004.6±291.1) μmol/L vs (753.1±254.3) μ mol/L,P=0.000],lower serum calcium levels[(1.86±0.1) mmol/UL vs (2.02±0.2) mmol/L,P=0.000],higher serum phosphorus [(2.1±0.6) mmol/L vs (1.6±0.4) mmol/L,P=0.001] and higher calcium phosphorus product [(3.8±1.1) mmol2/L2 vs (3.1±0.8) mmol2/L2,P=0.010] as compared with those of the patients without the primary outcome.Based on the results of multivariable Cox regression analysis, ultrafiltration volume, cardiac functional grading,peritonitis,Kt/V and serum phosphorus level contributed to the decline of RRF significantly. Conclusion The higher Kt/V in PD patients plays a protective role, the higher ultrafiltration volume, the worse heart function, the more peritonitis rate and higher serum phosphorus predict more rapid declination of RRF.
