RESUMO

Ovarian cancer has the highest mortality among the three major gynecological malignancies. Peritoneal metastasis, intestinal obstruction and then death is the main outcome pattern of advanced ovarian cancer. Traditional Chinese medicine maintenance treatment of advanced ovarian cancer is effective, but the theoretical basis needs to be further improved. Under the guidance of the membranous Sanjiao theory and combined with clinical experience, this paper traces the origin, focuses the location of membranous Sanjiao of ovarian cancer peritoneal metastasis and the role of membranous Sanjiao dysfunction in the occurrence and metastasis of ovarian cancer. Besides, the paper puts forward that the main strategies for the maintenance treatment of traditional Chinese medicine on ovarian cancer are ordering the Qi movement of Sanjiao, regulating and harmonizing Qi and blood, and clearing away the latent toxin. From the perspective of membranous Sanjiao theory, the theory enriches the traditional Chinese medicine theory of ovarian cancer and provides new ideas for the treatment of ovarian cancer.

RESUMO

Objective To evaluate the risk of recurrence or metastasis of breast cancer by LNG-IUS via meta-analysis. Methods We searched literature in the PubMed, Web of Science, Cochrane, EMBASE, CBM, CNKI, VIP, and WanFang database.The retrieval period was from January 2014 to October 2021.Data extraction and quality evaluation were conducted for the included randomized controlled study (RCT) to analyze whether LNG-IUS can increase the risk of recurrence or metastasis of breast cancer. Results A total of 1309 Chinese and English studies were retrieved; 5 RCTs were included in this study, and 446 patients were enrolled.The combined total effect value in the fixed-effect model with RD (95%CI)0.03(-0.03-0.08), Z=1.05, P=0.29) indicated that the LNG-IUS+tamoxifen group did not increase the risk of recurrence or metastasis of breast cancer compared with the tamoxifen group.The funnel diagram was basically symmetrical, suggesting that the publication bias was small. Conclusion Wearing the LNG-IUS does not increase the risk of relapse or metastasis of breast cancer significantly, and it provides certain safety for patients with breast cancer.

Objective@#To observe the effect of traditional Chinese medicine hot-ironing therapy at Shenque point on cancerous abdominal pain in advanced malignant tumors.@*Methods@#Forty patients with cancerous abdominal pain who met the inclusion criteria were randomly divided into two groups, 20 in each group.The control group was treated with traditional Chinese medicine hot-ironing analgesic package at Shenque point, and the treatment group was treated with abdominal massage therapy on the basis of the control group. Both groups were treated continuously for 7 days. The degree of abdominal pain was assessed using a digital assessment scale, and the quality of life was assessed using a quality of life assessment scale. The time to remission of cancerous abdominal pain was observed during the treatment period.@*Results@#After treatment, the NRS scores on the 3rd day (1.9 ± 0.6 vs. 2.4 ± 0.3, t=3.333), 5th day (1.0 ± 0.3 vs. 1.6 ± 0.5, t=4.602), 7th day (1.0 ± 0.6 vs. 2.4 ± 0.7, t=6.791), in the treatment group were significantly lower than those in the control group (P<0.01). The remission time of the treatment group was longer than that of the control group on the 1st, 3rd, 5th and 7th day after treatment, but there was no significant difference between the groups (t values were 0.243, 1.374, 0.185, 0.7284, P>0.05). The body function score in the treatment group (88.6 ± 13.7 vs. 77.0 ± 19.4, t=2.184) was significantly higher than that in the control group (P<0.05).@*Conclusions@#The combination of traditional Chinese medicine hot-ironing package combined with pushing therapy can alleviate the pain level and improve the quality of life of patients with cancerous abdominal pain.

Objective To study the effects ofYiqi Huoxue Jiedu Recipe on immunomodulatory of patients with advanced ovarian cancer.Methods It was performed in a single-arm, open-label study with 40 patients with advanced epithelial ovarian cancer. All patients were treated withYiqi Huoxue Jiedu Recipe lasting for 4 weeks (200 mL, twice a day). The main endpoint was the quantities of CD4+CD25+ cells, CD8+CD28- regulatory T cells and CD8+CD28+ cytotoxic T cells measured by flow cytometry, and the accessory endpoints were variations of symptoms which included pain, fatigue, and numbness etc.Results The quantities of CD4+CD25+ and CD8+CD28- cells significantly decreased compared with those of pre-treatment (P<0.05); the quantity of CD8+CD28+ cell increased significantly (P<0.05). Except for pain, significant improvement showed in fatigue, tinnitus, numbness, poor appetite, abdominal discomfort, insomnia, diarrhea and abnormal urinate (P<0.05).ConclusionYiqi Huoxue JieduRecipe is proved to be effective in regulating immunosuppression advanced ovarian cancer and improving patients’ life quality.

Objective To investigate the effect of Shugan-Yishen formula on Tamoxifen-resistant MCF-7 cell line(LCC9) through HER2-ERK/MAPK-ERα pathway. Methods Four serums were prepared with the method of serum pharmacology:the serum of Shugan-Yishen formula, Shugan-Yishen formula plus Tamoxifen(TAM), blank and TAM alone. After LCC-9 cells were affected in the above-mentioed serums in vitro, the inhibition rate and the level of mRNA as well as the protein expression of HER2, ERα, ERK/MAPK, p-ERK/MAPK were detected. Results MTT showed that TAM alone group、Shugan-Yishen formula group and the Shugan-Yishen formula plus TAM group had higher inhibition rates at 48th hour, the inhibition rates was(6.61±0.129)%, (47.43±2.24)%, (54.19±3.364)%, compared to the blank group, Shugan-Yishen formula group and the Shugan-Yishen formula plus TAM group had a stronger inhibition on LCC9 at 48th hour (P0.05). RT-PCR result showed that compared to the blank group, the expression levels of ESR1 mRNA of group of Shugan Yishen formula plus TAM werehigher by and large at 48th hour (P0.05). Conclusions ①The combination of Shugan-Yishen formula with tamoxifen can inhibit the LCC9 cells. The inhibition effect reaches the peak at 48th hour. ②The mechanism of Shugan-Yishen formula plus TAM combination effects on human breast cancer TAM-resistant cell line LCC9 may improve the sensitivity of ERαby inhibiting the expression of HER2, ERK/MAPK.

Objective To provide solid data for TCM treatment and research of triple negative breast cancer (TNBC) by investigating its syndromes distribution.Methods 180 patients diagnosed of TNBC in Department of Oncology,Guang'anmen Hospital from Janurary 2007 to November 2012 participated in this clinical trial.Eight principals syndrome differentiation,and Qi,Blood and viscal syndrome differentiation were used.According to the syndromes diagnosis standard,the differentiation of one single syndrome firstly and then multi-syndrome were conducted and related variables were analyzed finally Results 404 symptoms presented in all 180 patients.Qi deficiency syndrome (35.89%) and blood stasis syndrome (32.18%) showed statistical difference when compared with blood deficiency syndrome (3.22%),yin deficiency syndrome (4.95%),yang deficiency syndrome (3.96%),spleen-dampness syndrome (3.47%),qi stagnation syndrome (7.43%),toxic heat syndrome (4.95%) and dampness-heat syndrome (1.49%) (P<0.01,x2=124.709,116.007,127.813,126.913,105.411,116.007,140.015; 118.273,105.447,117.630,120.445,97.411,105.447,110.273respectively).92.22% syndroms were multi-syndrome including 23.89% bi-syndrome,32.22% tri-syndrome and 25.56% qua-syndrome.Conclusion The syndromes of TNBC were complicated but with predominant Qi dificency and Blood stasis; single syndrome identification should be put in the first place and then combining them together may be practical approaches; Replenishing Qi and activating blood circulation might be breaking through in TNBC treatment.

Objective To evaluate three-dimentional (3D) reconstruction technique and methylene blue staining in precise anatomic hepatectomy.Methods The clinical data of 12 patients with hepatocellular carcinoma who were admitted to the Chinese PLA General Hospital from February 2009 to August 2011 were retrospectively analyzed.The 3D reconstruction of the liver tumor and intrahepatic vessels were done based on the computed tomography data and magnetic resonance imaging data.The portal vein supplying the tumor and its anatomic relationship with adjacent vessels were evaluated.Precise anatomic hepatectomy was performed guided by sustained methylene blue staining.Results The accurate rate of 3D model of the portal triad was 12/12.The shape of target segments observed after methylene blue staining was consistent with the results of 3D evaluation.Two patients received hemihepatectomy,3 received lobectomy,5 received monosegmentectomy or subsegmentectomy,2 received multisegmentectomy.The mean tumor diameter,operation time,blood loss,postoperative hospital stay and complication rate were 5.6cm (2.5-16.0 cm),(150±24)minutes,(236±25)ml,(10±3)days and 2/12,respectively.After a median follow-up of 14 months,tumor recurrence was found in 2 patients,and 1 of them died of tumor progression.Conclusions The 3D reconstruction may contribute to precise evaluation of the anatomic relationship between the tumor and its adjacent vessels.The 3D technique combined with sustained methylene blue staining may significantly improve the accuracy of anatomic hepatectomy.

Objective Observing the effect of Shuganyishen formula on hormone-dependant metastatic breast cancer and the mechanisms of anti-resistance and endocrine therapy. Methods 226 ITT cases were divided into two cohorts by treatment. 126 cases were treated with the combination of Shuganyishen formula and standard western medicine including endocrine therapy, and 100 cases were treated with western medicine alone. The end points were time to progress (TTP) and overall survival and quality of life (QOF). The effect of Shuganyishen formula on growth of TAM-resistance cell line in vitro was observed. Results There were statistical significances of median TTP between combination and western medicine in Luminal A (ER+ /PR+, HER2-) as well as Luminal B (ER+/PR+, HER2+), but no significances of OS; The extent and incidence of hot flush, sweat and syndromes related to bone were decreased significantly in combination treated group. Shuganyishen formula could reverse resistance to TAM. Conclusion Patients with LuminalA and LuminalB metastatic breast cancer benefit from Shuganyishen formula, which maybe related with its reverse resistance to endocrine therapy.

Objective The prospective study was trying to explore the altering tendency of plasma concentration of E2, P and PAS after TCM intervention for post-surgery breast cancer patient who received endocrine therapy (tamoxifen).Methods By randomization, parallel, coincidence measures and the lab. technologies of AXSYM registered at Abbott lab. in the USA, plasma concentration of E2, P and FAS were tested before and after treatment. Results The results of biochemical test of plasma E2, P and FAS concentration of the two arms showed no significance difference before and after the intervention,indicating the side effects relating to TAM had nothing to do with plasma E2, P and FAS level. Conclusion It could be predicable that there must be some other by-pass ways initiating breast cancer rather than the way directly altering the level of plasma concentration of E2, P and FAS.

Objective Infected pancreatic necrosis is a serious complication of necrotizing pancreatitis. A method of minimally-invasive retroperitoneal infected pancreatic necrosectomy using percutaneous nephroscope was evaluated. Methods 21 patients with acute pancreatitis were treated in our hospital from June 2008 to August 2009. Among 13 patients who developed infected pancreatic necrosis, 6 underwent percutaneous catheter drainage by CT guidance. Then retroperitoneal infected pancreatic necrostectomy using percutaneous nephroscope along the sinus tract were performed after drainage for 5-36 d. Results In these 6 patients, 3 received percutaneous nephroscopic treatment one time, 2 two times and 1 three times. There were no operative mortality and morbidity except that 1 patient developed pseudocyst 6 months after operation. Conclusion Retroperitoneal pancreatic necrosectomy by percutaneous nephroscope is a safe, feasible, minimally-invasive and efficient method for treating infected pancreatic necrosis when the indication and occasion are suitable. This method would be a valid therapeutic option for treating necrotizing pancreatitis. However, further evaluation is necessary.

Objective To investigate the clinical value of lasting methylene blue staining in precise hepatectomy.Methods The clinical data of 21 patients with liver cancer who received precise hepatectomy after methylene blue staining at General Hospital of PLA from February to August in 2009 were retrospectively analyzed.After the hepatic pedicle Was dissected,methylene blue WaS injected into the portal vein,and then the hepatic pedicle was ligated.Parenchymal division is initiated along the line of devascularization demarcated on Glisson capsule.Results The success rate of methylene blue staining Was 100%.Methylene blue retained in the parenchyma for(80±23)minutes.Right hepatectomy was performed on 2 patients,left hepatectomy on 1,right posterior lobectomy on 2,right anterior lobectomy on 3,left lateral lobectomy on 1,segmentectomy of segment Ⅷon 2,segmentectomy of segment Ⅶ on 3,segmentectomy of segment Ⅵ on 1,segmentectomy of segment Ⅳ on 2 and combined segmentectomy on 4.The mean volume of blood loss,incidence of postoperative complications and postoperative hospital stay were(236±6)ml,14%(3/21)and(12±3)days.Conclusions Ligation of hepatic pedicle after methylene blue injection has the advantages of high success rate and lasting staining of parenchyma of liver.Especially,this staining method contributes to improve the precision of hepatectomy by guiding the segment selection during parenchyma transection.

<p><b>OBJECTIVE</b>To study the effects of the traditional Chinese drugs, Shuganyishen, on Ishikawa cell line.</p><p><b>METHOD</b>Rat blood serum was prepared by using Chinese drugs serum pharmacology method, in which, Shuganyishen was contained or not contained. Ishikawa cells were subjected into five groups: (1) normal cells group (NCG), (2) rat blood serum without Shuganyishen group (NSGG) as negative control, (3) Shuganyishen group (SGG), (4) Cisplatin group (DDPG) as positive control, and (5) Shuganyishen with Cisplatin group (S&CG). MTT assay was used to evaluate the inhibition effect on cell growth in each group. The mRNA expression of estrogen receptor alpha and beta (ERalpha and ERbeta) in each group was detected by RT-PCR.</p><p><b>RESULT</b>Effective inhibition of cell growth was found in the groups of SGG and S&CG, respectively (P < 0.05), which had no significant difference from positive control by MTT. The mRNA expression of ERbeta was slightly going up, while the expression of ERalpha was hardly effected in SGG by RT-PCR. Interestingly in S&CG, the mRNA expression of ERalpha was significantly down-regulated (P < 0.05), but the mRNA expression of ERbeta remained no change. As the positive control, the mRNA expression of ERalpha and ERbeta was significantly down-regulated in DDPG (P < 0.5).</p><p><b>CONCLUSION</b>Traditional Chinese drugs, Shuganyishen, may inhibit Ishikawa cell growth and had no effects on the expression of ERalpha, furthermore, can reverse the inhibition of ERbeta expression by Cisplatin.</p>

Phytoestrogens (PEs) and estrogen receptorβ(ERβ) have close relationship with breast cancer. PEs preferentially binds to ERβ rather than ERa. Although there are still some arguments, it provides a new though for researching the mechanism of Phytoestrogen-containing herbs on breast cancer and exploring the essence of TCM theory.

Objective By previous overview of patients treated in our hospital, this study highlights on the intervention effects of TCM treating principle of regulating liver Qi and tonifying kidney Yin on the adverse reaction of TAM on breast cancer patients in post surgery. Methods Perspective study with the measures of randomization, and paralleling and synchronization control was conducted in the research. Results symptoms such as flush, insomnia, night sweat, palpitation, depression and heat sensation in the chest, palms and soles were improved significantly; while manifestations such as nausea, vomiting, irregular menstruation, bitter taste and dry mouth showed no significant changes. QOL changed significantly. Conclusion The application of treating principle of regulating liver Qi and tonifying kidney Yin is effective in reducing side effects caused by TAM. This result further improved the theory of TCM that the nature of liver is active, although the liver belongs to Yin in physique and but Yang in function.

Objective To evaluate the feasibility of radical liver resection following downstaging of hepatocellular carcinoma (HCC) by transcatheter arterial chemoembolization (TACE). Methods Eleven patients with HCC underwent radical liver resection after downstaging of HCC by TACE from January 1991 to June 2002, then the clinical features, intraoperative blood loss, postoperative complications and survival time were studied. Results All patients received the radical liver resection successfully, and the mean intraoperative blood loss and operative time of all the patients Was 1336.4ml (110-3200ml) and 272.8 minutes (210-390 minutes), respectively. Each of the 6 patients had an amount of blood loss more than 1000ml. The postoperative complications included bile leakage (3 patients) and ascites (1 patient), with total incidence of 36%. The median survival time was 61.5 months, and the number of patients survived for 1, 2, 3 years was 8, 7, 6, respectively. Conclusion Radical liver resection after downstaging of HCC by TACE is safe and feasible.
