OBJECTIVES@#Congenital birth defects are the main source of disease burden among children under 5 years old in China. This study aims to compare the trends in disease burden of different congenital birth defects among Chinese children under 5 years old from 1990 to 2019, and to provide a scientific basis for strengthening the comprehensive prevention and control of birth defects.@*METHODS@#Based on data from the Global Burden Disease (GBD) in 2019, the incidence mortality rate, and disability-adjusted life years (DALYs) rate of congenital birth defects among Chinese children under 5 years old from 1990 to 2019 were selected as evaluation indicators. The Joinpoint regression model was used to analyze the trends in disease burden of different types with congenital birth defects over three decades. The study also compared the differences in disease burden of congenital birth defects among children under 5 years old by gender.@*RESULTS@#Compared to 1990, the DALYs rates of congenital heart anomalies (1 931.91/100 000), digestive congenital anomalies (364.63/100 000), neural tube defects (277.20/100 000), congenital musculoskeletal and limb anomalies (133.33/100 000), and Down syndrome (128.22/100 000) in children under 5 years old in China in 2019 were decreased 70.78%, 71.61%, 86.21%, 36.84% and 73.65%, respectively. From 1990 to 2019, the mortality rates and DALYs rates of different congenital birth defects showed an overall downward trend, but the incidence of digestive congenital anomalies and Down syndrome showed an upward trend after 2005 and 2001, respectively. Except for congenital musculoskeletal and limb anomalies, incidence of the remaining categories of birth defects were higher in boys than that in girls.@*CONCLUSIONS@#The disease burden of congenital birth defects in children under 5 years old in China is decreased substantially from 1990 to 2019, but the burden of congenital heart anomalies is still serious and the incidence of some birth defect diseases is on the rise, and it is still crucial to strengthen the prevention and treatment for birth defects in children and propose targeted measures according to their gender characteristics.

Objective@#To investigate the clinical efficacy and adverse effects of stereotactic body radiotherapy (SBRT) in the treatment of T2N0M0 non-small cell lung cancer (NSCLC) patients.@*Methods@#By retrospectively analyzing the clinical data of 30 inoperable patients with stage T2N0M0 NSCLC treated by SBRT, the overall survival, progression-free survival, cancer-specific survival and adverse effects were determined.@*Results@#The median follow-up was 18.4 months. The 1-, 2-, and 3-year overall survival rates were 92.2%, 92.2% and 80.6%, respectively. The corresponding cause-specific survivals were 95.7%, 95.7% and 83.7%. The progression-free survivals were 70.2%, 54.1% and 40.6%. The local control rates were 100%, 94.4%, and 94.4%. The regional controls were 84.2%, 72.1%, and 54.1%; and distant controls were 84.6%, 72.4% and 64.3%, respectively. Twenty patients (66.7%) developed symptoms of grade 1 radiation-related toxicities: dyspnea, chest pain, fatigue, cough, esophagitis, or pneumonia. Among these, 5 patients suffered grade ≥2 radiation pneumonitis, and one patient experienced grade 4 radiation pneumonitis.@*Conclusions@#SBRT was efficient and safe for patients with inoperable T2N0M0 NSCLC, imposing tolerable toxicities.

Objective To compare FibroTouch, serum hepatic fibrosis index and aspartate aminotransferase-to-platelet ratio index (APRI) in diagnosis of liver fibrosis and early cirrhosis for patients with chronic HBV infection with mild abnormal liver function.Methods One hundred and seven chronic HBV infected patients with alanine transaminase (ALT)≤80 U/L who underwent liver biopsy in Mengchao Hepatobiliary Hospital of Fujian Medical University from January 2014 to January 2017 were enrolled in the study.According to the liver biopsy pathology , the patients were divided into mild liver fibrosis group (S1, n=26 ), significant fibrosis group ( S2-3, n =34 ) and early cirrhosis group ( S4, n =47 ).The differentiations of liver stiffness measurement (LSM), Laminin(LN), Collagen Type ⅢN-peptide(PⅢP N-P), Collagen Type IV(CⅣ), Hyaluronic acid ( HA) and APRI were compared among the three groups , and their correlations with fibrosis were analyzed with Spearman rank correlation analysis .Logistic regression analysis was performed to establish a multi-index model for predicting the degree of liver fibrosis , and the receiver operating characteristic (ROC) curves were drawn to compare and analyze the predictive value for different stages of liver fibrosis.Results The LSM in the significant fibrosis group was higher than that in the mild liver fibrosis group (Z=-2.611, P<0.01).The levels of PⅢP N-P,CIV,LN and HA in early cirrhosis group were higher than those in mild liver fibrosis group (Z=-4.026,-4.205,-2.938 and-3.156, all P<0.01).LSM and APRI in early liver cirrhosis group was higher than that in mild liver fibrosis group(Z=-5.789 and -4.308, both P<0.01) and significant liver fibrosis group (Z=-3.460 and -3.498, both P<0.01).PLT in early liver cirrhosis group was lower than that in mild liver fibrosis group and significant liver fibrosis group (Z=-4.533 and -3.206, both P<0.01).The correlation analysis showed that the values of LSM , CIV, PⅢP N-P and APRI were positively correlated with the stage of liver fibrosis (r=0.606, 0.418, 0.396 and 0.470, respectively; all P<0.01).There was no significant difference in the efficacy of each single index in predicting hepatic fibrosis S≥2 and S=4 (all P>0.05). The prediction of liver fibrosis models were established with Logistic regression analysis .The model 1 Logit (P)=-3.810+0.385×LSM+0.052×CIV was used for predicting liver fibrosis S≥2; the area under the receiver operating characteristic curve ( AUC) was 0.839, with specificity of 96.0%, and the diagnostic efficacy was superior to PⅢP N-P, CⅣand APRI(Z=2.579, 2.145 and 2.219, all P<0.05), but there was no significant difference compared with LSM ( Z =1.001, P >0.05).The model 2 Logit ( P)=-4.291+0.105×LSM+0.028×CIV+2.682×APRI was used for predicting liver fibrosis S =4; its AUC was 0.865 with sensitivity of 82.9%and specificity of 81.8%, and the diagnostic efficacy was superior to LSM, PⅢP N-P, CⅣand APRI (Z=2.309, 2.485, 2.403 and 2.103, all P<0.05).Conclusions For chronic HBV infected patients with mild abnormal liver function , FibroTouch can be selected as a noninvasive method to predict liver fibrosis S≥2.The diagnostic efficacy of the prediction model 2, based on the combination of LSM, CIV and APRI is superior to each single index , which can improve the prediction level of early cirrhosis.

Objective: To detect the effects of melatonin (MLT) combined with radiotherapy on the proliferation and apoptosis of non-small cell lung cancer (NSCLC) H1299 cells, and to explore the role of MLT in regulating the radiosensitivity of H1299 cells. Methods: The H1299 cells were divided into control group, MLT group, irradiation (IR) group and IR+MLT group. The H1299 cells in control group didn't receive any treatment, the H1299 cells in MLT group were treated with different concentrations of MLT (100 and 500 μmol · L-1), the H1299 cells in IR group were exposed to 6 Gy137 Cs γ-ray, and the H1299 cells in IR+MLT group were treated with different concentrations of MLT (100 and 500 μmol · L-1) and exposed to 6 Gy137 Cs γ-ray. Colony formation assay was used to detect the clone numbers of H1299 cells in various groups; Flow cytometry was applied to detect the apoptotic rates of H1299 cells at 24 and 48 h after irradiation; Western blotting method was performed to detect the phosphorylation levels of nuclear factor kappa B (NF-KB) protein in the H1299 cells in various groups. Results: The colony formation assay results showed that compared with IR group, the clone number of H1299 cells in IR+MLT group was significantly decreased (i=7. 234, P 0. 05). The Western blotting results showed that the phosphorylation level of NF-κB protein in the H1299 cells in IR+MLT group was lower than that in IR group. Conclusion: MLT combined with radiotherapy has obviously inhibitory effect on the human H1299 cells and can increase the radiosensitivity of H1299 cells.

Objective To explore the effects of both pre-gestational BMI and gestational weight gain (GWG) on the birth weight of neonates.Methods A total of 5 395 pregnant women were selected from the Southwest areas of China (Sichuan/Yunnan/Guizhou) and were divided into groups as pre-gestational underweight,normal weight,overweight and obesity,according to the WHO Recommendation on BMI Classification.Guidelines on Pregnancy weight were adopted from the Institute of Medicine to confirm the accuracy of GWG.Multinomial logistic regression model was used to assess the associations between pregestational BMI and GWG,on the birth weight of the neonates.Results After adjusting for related confounders,low pre-gestational BMI appeared as a risk factor for SGA (OR=1.91,95%CI:1.47-2.50),and was also associated with the decreased risk of LGA (OR=0.55,95%CI:0.47-0.66).Inadequate GWG was both associated with the increased risk of delivering SGA (OR=1.57,95%CI:1.21-2.03) and the decreased risk of LGA (OR=0.48,95%CI:0.41-0.57).Pre-gestational overweight/obesity (OR=1.85,95%CI:1.58-2.17) and excessive GWG (OR=1.87,95%CI:1.67-2.11) were both positively associated with the risks on LGA.Data from the stratified analysis indicated that inadequate GWG was positively associated with the risk of SGA among underweight or normal weight women (all P<0.05),but not with those overweight/obese women.Conclusions Pre-gestational BMI and GWG were important influencing factors on the birth weight of neonates.Health education programs for pregnant women should be intensified and gestational weight gain should also be reasonably under control.

Objective To explore the mechanism of notch signaling pathway in the treatment of cervical cancer with notch receptor blocker(DAPT).Methods After treatment of Me180 cells with different concentrations of DAPT combined with curcumin-photodynamic therapy (PDT),3-(4,5 Dimethylthiazlo-yl)-2,5 Dimethylthiazol-2-yl)-2,5 dipheny tetrazo lium bromide(MTT)assay was used to detect the effect on the proliferation of Me180 cells,annexin V-FITC/PI combined with flow cytometry was used to detect the effect on apoptosis,and real-time fluorescence quantitative PCR and Western blot were used to detect the mRNA and protein expressions of Notch1 gene respectively.Results Both DAPT(1.0 μmol/L)and curcumin-PDT groups could inhibit the proliferation and induce apoptosis of cervical cancer cell line Me180,and the synergistic effect of the two groups was significant (P<0.05 or<0.01).DAPT and curcumin-PDT group reduced the mRNA expression level of Notch1 in cervical cancer Me180 cells with inhibition ratio of 40.0% and 32.3% respectively.DAPT and curcumin-PDT group inhibited the protein expressions of Notch1,nuclear factor-kappa B(NF-kB),vascular endothelial growth factor (VEGF),and the synergistic effect of the two groups was statistically significant.Conclusions Both DAPT and curcumin-PDT may effectively block the conduction of Notch signaling pathway,which is associated with down-regulation of the expression of Notch1 and NF-kappa B.Notch signaling pathway may be one of the targets of curcumin photodynamic therapy.

Objective:To explore the curative effect and safety of febuxosta for the patients with primary gout complicated with mild to moderate renal insufficiency.Methods:60 male patients with primary gout complicated with mild to moderate renal insufficiency were enrolled in the affiliated Hospital of integrated traditional Chinese and Westem Medicine,Nanjing University of Chinese Medicine from January 2015 to August 2016 and randomly divided into two groups,the control group (n=30) was treated by allopurinol,and the study group (n=30) was treated by febuxosta treatment.The blood uric acid (BUA) and hepatorenal function were compared in the treatment period,and the incidence of adverse reactions and the recurrence of gout in post-treatment were compared.Results:At 1,2 and 3 month after treatment,the serum level of BUN in the study group was significantly lower than that of the control group (P<0.05).The renal function of all patients were significantly improved compared with pre-treatment (P<0.05),but the no significant difference was found between two groups at the same time(P>0.05).The control group showed the adverse reactions with limb discomfort,drowsiness,nausea,abdominal distension,diarrhea and itch of skin,and the study group showed limb discomfort,drowsiness and abdominal distension,and one patients presenting allergic skin rash and discontinuation of allopurinol.The total incidence of adverse reactions in the study group was significantly lower than that of the control group (P<0.05).Conclusions:Febuxosta could effectively and safely decrease the blood uric acid levels and improve the renal function of patients with primary gout complicated with mild to moderate renal insufficiency.

Objective To investigate the effect of PCIA with low-dose fentanyl on plasma endothelin (ET) after craniotomy. Methods 47 cases of selected craniotomy were divided randomly into two groups :experimental group (26cases) and control group (21cases) . Patients in experimental group were treated with PCIA (fentanyl 15ug/kg +ondansetron 8 mg+100 mL NS) but patients in control group were not given PCIA. Then HR,MBP, VAS, ET and side-effects were observed and compared between two groups before treatment and 0, 2, 4, 8, 12, 24, 48 h after treatment.Results In experimental group, HR was lower at 2, 4, 8, 12, 24 and 48 h after treatment than control group. MBP was lower at 0 and 2h after treatment in experimental group than control group. Plasma levels of ET were lower at 8, 24 and 48h after treatment in experimental group than control group. There were significant differences in VAS scores at 2,4,8,12,24 and 48 h after treatment between two groups. There was no significant differences in side reactions including consciousness, respiratory depression, vomiting and sedation between two groups. The incidence of nausea was higher in experimental group than control group. Conclusion PCIA with low-dose fentanyl after craniotomy has good analgesic effect and few side reactions, can reduce the formation of plasma endothelin, and then alleviate brain damage.

Objective To analyze Chinese oncology papers of 2012 included by Science Citation Index (SCI) and to facilitate domestic oncology researchers to better master the foreign periodical information and publish research results.Methods Chinese oncology papers included by SCI in 2011 and 2012 were retrieved,by using SCI-Expanded database.The periodical distribution,subject distribution,organization distribution,fund and citations were analyzed.Results In 2011,6570 papers were included by SCI,while 9226 were included in 2012,with the increase rate of 40.426%.The major language was English,and the most frequent literature type was article.The amount of source journals increased from 668 in 2011 to 749 in 2012.PLoS One published the most amount of literature both in 2011 and in 2012.The periodical with the highest impact factor (IF) in 2011 was Nature Genetics (IF =35.209),while in 2012 was Nature (IF =38.597).The university with the most amount of published literature was Sun Yat-sen University (653,7.078%),and the corresponding medical institution was PLA General Hospital (113,0.932%) and the corresponding research institute was Chinese Academy of Medical Sciences (316,3.425%).The most funded project was National Natural Science Foundation with the rate of 46.339% (1949/4206) in 2011 and 40.000% (3290/8225) in 2012.The total citation frequency was 36 569 in 2011 and 18 945 in 2012.Conclusion Over the past two years,Chinese oncology papers included by SCI make great progress,in terms of quantity and quality.English is the main written language,and there are more source journals but less journals with high IF.

Objective To analyze Chinese oncology papers included by Chinese biomedical literature database (CBM) and science citation index (SCI),and to comprehensively and objectively reflect the current situation of Chinese oncology literatures published in domestic and international journals.Methods Chinese oncology papers were retrieved by using CBM and SCI.A comparison analysis was made from the respects of subject distribution,district distribution,organization distribution and high citation situation.Results 67 071 papers of 2012 were retrieved from CBM database,while 9226 papers were indexed by SCI.In terms of district distribution,the first three districts were Jiangsu (6707),Guangdong (4897) and Beijing (2214) in CBM database,while in SCI database,they were Beijing (2214),Shanghai (2008) and Guangdong (1393).The institute with the most amount of literature is university,respectively 9725 and 5755.Moreover,the mostly funded project is National Natural Science Foundation (4710,35.38% ; 3290,35.68%).Conclusion The 2012 Chinese oncology literatures published in domestic journals has advantage with respect to quantity,compared with the international published journals.However,there exists a bigger difference in terms of quality.Through the comparative analysis,the medical researchers in our country will have some implications in the research orientation.

Lobaplatin is the third generation platinum drug.Many studies on pharmacodynamics have confirmed that lobaplatin can block the cell cycle and induce cell apoptosis,and thus inhibits tumor cell proliferation effectively.Lobaplatin is not cross-resistant to cisplatin.Besides,it has good curative effect to various kinds of cancer patients with mild adverse reactions,and the patients are well tolerable to it.

Objective To analyze the articles of Journal of Intentational Oncology published from 2000 to 2009, and offer reference for the edition and publication of the journal. Methods Bibliometric analysis was used to analyze articles published in Journal of International Oncology from 2000 to 2009, including the quantity analysis, ration of fund-aided articles and authors analysis. Results 2 551 pieces of articles were published,including 182 pieces of original articals, 2 198 pieces reviews and 171 pieces of abstracts, and the density was 0. 3 piece per page. Fund-aided articles were 536 pieces and the ration of fund-aided articles was 0. 21. Fundaided articles and the ration were rising in these ten years. The authors distributed widely and came from 29 province, autonomous region or municipality under the direct control of the Central Government. Author cooperation degree was 1.6 in general, 1.4 in reviews and 4.3 in original articls. Conclusion Articles in this journal are from various sources, and the ratio of fund-aided articles is high. Most original articles have high author cooperation degree. Journal of International Oncology has a very high utilization value in professional population.

Objective To elucidate the expression of gankyrin in human gastric cancer cells and it's role in nimesulide induced apoptosis. Methods Four human gastric cancer cell lines including MKN28 (well differentiated), AGS (poorly differentiated), MKN45 (poorly differentiated), and SGC7901(moderately differentiated) were cultured and treated with nimesulide. Nimesulide induced growth inhibition and apoptosis of the cells were detected by methyl thiazolyl tetrazolium assay, and confirmed by flow cytometry. The expressions of gankyrin gene and protein were further assessed by real-time PCR and Western blotting. Results Gankyrin mRNA and protein were detected in all four human gastric cancer cell lines. The proliferations of AGS and SGC7901 cell lines were significantly suppressed by nimesulide in a time-dose dependent manner. When treated with 400 μmol/L of nimesulide for 48 hours, the significant apoptosis was found in AGS cells (23.30%±2.50%) and SGC7901 cells (16.80%±1.55% ) in comparison with controls (0.57%±0.19% and 0.88%± 0.17%, respectively, all P values <0.01). Apoptosis of AGS cells induced by nimesulide was accompanied by a considerably decreased gankyrin expression that was more significant at 24 hours (0.0035±0.0014) and 36 hours (0.0980±0.0160) in comparison with controls (0.4690±0.1190, all P values<0.01). Conclusion Gankyrin expresses in human gastric cancer cell lines and may be involved in nimesulide induced apoptosis of AGS cells.

Objective To explore the safety of the elderly donors in living related donor kidney transplantation. Methods Forty-five elderly donors (51 - 78 years,study group) who underwent ne-phrectomy for living related donor kidney transplantation from April 1993 to December 2007 were retrospectively investigated. Clinical data including serum creatinine (SCr), glomerular filtration rate (GFR) in pre-and post-operation, operation complications and hospital stay time were analyzed and compared with the control group(62 cases, the donors age were younger than 50 years). Results The operations of all living donors were successful. The SCr and GFR in pre-operation were (82.16 ± 10.86)μmol/L, (85. 82±6.26)ml/min(study group)and (78. 66±10. 41)μmol/L, (88. 74±9. 44) ml/min (control group) respectively. There were no significant differences in SCr and GFR between the groups at different time points (P>0. 05). The average hospitalization time was 9 days in study group and 8 days in control group. There were no severe perioperative complications and no renal function failure was found in long-term following-up in study group. Conclusions Age is not the absolute contraindication of donor for living related donor kidney transplantation. The preoperative evaluation and careful operation can ensure the safety of elderly donors.
