RESUMO

OBJECTIVE@#To compare the clinical efficacy of micro steel plate and Kirschner needle oblique and transverse internal fixation of adjacent metacarpal bone in the treatment of metacarpal diaphyseal oblique fracture.@*METHODS@#Fifty-nine patients with metacarpal diaphyseal oblique fractures admitted between January 2018 and September 2021 were selected as the study subjects and divided into the observation group (29 cases) and the control group (30 cases) based on different internal fixation methods. The observation group was treated with Kirschner wire oblique and transverse internal fixation of adjacent metacarpal bones, while the control group was treated with micro steel plate internal fixation. Postoperative complications, operation time, incision length, fracture healing time, treatment cost, and metacarpophalangeal function were compared between the two groups.@*RESULTS@#No incision or Kirschner wire infections occurred in the 59 patients, except for one in the observation group. No fixation loosening, rupture, or loss of fracture reduction occurred in any of the patients. The operation time and incision length in the observation group were (20.5±4.2) min and (1.6±0.2) cm, respectively, which were significantly shorter than those in the control group (30.8±5.6) min and (4.3±0.8) cm (P<0.05). The treatment cost and fracture healing time in the observation group were (3 804.5±300.8) yuan and (7.2±1.1) weeks, respectively, which were significantly lower than those in the control group (9 906.9±860.6) yuan and (9.3±1.7) weeks (P<0.05). The excellent and good rate of metacarpophalangeal joint function in the observation group was significantly higher than that in the control group at 1, 2, and 3 months after operation (P<0.05), but there was no significant difference between the two groups at 6 months after operation (P>0.05).@*CONCLUSION@#Micro steel plate internal fixation and Kirschner wire oblique and transverse internal fixation of adjacent metacarpal bones are both viable surgical methods for treating metacarpal diaphyseal oblique fractures. However, the latter has the advantages of causing less surgical trauma, shorter operation time, better fracture healing, lower cost of fixation materials, and no need for secondary incision and removal of internal fixation.

OBJECTIVE@#To study the comprehensive impact of scar and maxillary expansion combined with protraction on the development of maxilla with cleft lip and palate after repair operation.@*METHODS@#In the original finite element model of the maxilla with cleft palate, a finite element model of the maxilla with cleft lip and palate was established by using Boolean operation in ANSYS. Scar force after cleft lip and palate repair and maxillary expansion force combined with protraction were added simultaneously to process the stress analysis.@*RESULTS@#Maxillary deformation occurred in the three-dimensional direction. The comparison of displacements was as follows: X-axis>Z-axis>Y-axis.@*CONCLUSIONS@#Maxillary growth is significantly inhibited in the three-dimensional direction under the comprehensive impact of scar and maxillary expansion combined with protraction after repair operation, especially transverse and sagittal growth.

OBJECTIVE@#To investigate clinical effect of open reduction and internal fixation in treating proximal humeral fracture by using long head of biceps tendon cutted-off and re-fixation.@*METHODS@#From January 2014 to January 2017, 50 patients with proximal humeral fractures were treated by open reduction and internal fixation. According to whether long head of biceps tendon cutted-off and re-fixation, patients were divided into control group and treatment group, 25 patients in each group. There were 13 males and 12 females with an average of (73.8±4.5) years old in control group, treated with conventional open reduction and internal fixation. There were 15 males and 10 females, with an average of (74.1±5.4) years old in treatment group, treated by using long head of biceps tendon cutted-off and re-fixation on the basis of control group. Fracture healing time and complications after operation were compared between two groups, VAS score was used to evaluate relieve degree of pain, and Neer functional score at 1, 3, 6, 12 months after operation was applied to access clinical efficacy.@*RESULTS@#All patients were followed up for 12 to 24 months with an average of (14.5±3.6) months. There was no statistical difference in following-up between tow groups. Postoperative VAS score at 1 week between treatment group and control group were 2.92±1.10 and 5.88±0.90 respectively, and had significant difference(0.05). According to postoperative Neer score at 12 months, 9 patients got excellent results, 14 moderate and 2 poor in treatment group; while 4 patients got excellent results, 18 moderate and 3 poor in control group; but there was no difference between two groups. Fracture were healed without postoperative complications between two groups.@*CONCLUSIONS@#Open reduction internal fixation with in treating proximal humeral fracture by using long head of biceps tendon cutted-off and re-fixation could reduce pain, speed up early recovery of joint function, is worthy of further promotion.

Objective: To observe the clinical effects of the combination of auxiliary exercise instrument and systematic activity for preventing the formation of postoperative deep venous thrombosis (DVT) of artificial joint replacement. Methods: 217 patients who once underwent replacements of hip joint and knee joint were randomly divided into 3 groups as random method. The patients of A group (72 cases) received auxiliary exercise instrument, and those of B group (73 cases) received systematic activity, and those of C group(72 cases) received combinative treatment of auxiliary exercise instrument and systematic activity. And all of these patients received corresponding nursing scheme to prevent postoperative DVT of artificial joint replacement, respectively. And the symptoms of them at pre-operation and 12d of post-operation were observed, respectively, and the changes of plasma D- dimer(D-D), prothrombin time activity percentage (PTA), fibrinogen (FB), activated partial thromboplastin time (APTT), prothrombin time (PT) and international normalized ratio (INR) also were detected and compared. Results: At the postoperative 1st d, 3th d, 7th d, 12th d, the limb diameter above ankle at 5 cm of patients of C group was significantly less than that of other two group (F=4.066, F=4.256, F=3.452, F=4.678, P<0.05), respectively. However the differences of limb diameter above patella at 10cm of patients among three groups were no significant. And surface temperature above ankle at 5 cm of the C group at 1st d, 3th d, 7th d and 12d were significantly lower than those of other two groups (F=4.659, F=4.325, F=3.196, F=4.754, P<0.05), respectively. And the numbers of gastrocnemius Neuhof symptom and gastrocnemius Homans symptom of C group were significantly lower than that of other two groups (x2=13.317, x2=8.517, P<0.05), respectively. At the 7th d, the plasma D-D of C group was significantly lower than other two groups (F=5.063, P<0.05), while the differences of other plasma indicators among the three groups were no significant. Conclusions: Auxiliary exercise instrument combined with systematic activity has better therapeutic effect for preventing postoperative DVT of artificial joint replacement and it is worthy to be popularized in clinical practice.

<p><b>OBJECTIVE</b>To investigate the causes and operations for entrapment syndrome of posterior interosseous nerve caused by elbow cyst.</p><p><b>METHODS</b>Form March 2005 to March 2012,5 patients with entrapment syndrome of posterior interosseous nerve caused by elbow cyst were treated with surgical excision and neurolysis including 3 males and 2 females with an average age of 50.4 years old ranging from 35 to 60 years old. The course was from 3 to 10 months with an average of 6.3 months. The main clinical symptoms were pain on the outside of the forearm. The extension muscle power of the metacarpophalangeal joints at the fingers and the wrist had decreased. The EMC showed neurogenic damage or nerve conduction slowing down. According to the functional neurological evaluation standard of British Medical Research Institute ,the motion and sensory function after nerve injury was assessed.</p><p><b>RESULTS</b>Patients were followed up for 3 to 15 months with an average of 8.5 months. Wound healing in patients was good after the operations. There were no significant complications. Three patients were excellent (M4S3+) and 2 patients were good (M3S3). The hand joint function of 5 patients recovered well after operation.</p><p><b>CONCLUSION</b>The surgical excision and neurolysis for treatment of entrapment syndrome of posterior interosseous nerve caused by elbow ganglions cyst can remove the entrapment syndrome of the posterior interosseous nerve thoroughly ,promote the neurological function recovery. According to the functional evaluation standard ,operations achieved better therapeutic effect.</p>

<p><b>OBJECTIVE</b>Left ventricular remodeling (LVR) following myocardial infarction (MI) is a key pathophysiological process in which MI develops into heart failure. The exact mechanism of LVR remains unclear. We performed differential proteomic analysis on the myocardia of rats with LVR after MI, to explore the mechanism of ventricular remodeling after MI.</p><p><b>METHODS</b>In the LVR group (n = 12), after the anterior descending coronary artery was ligated, the rats were fed for four weeks before the LVR models were established. Rats in the sham-operated group (n = 11) underwent thread-drawing without ligation. The hemodynamic parameters, pathological findings, and proteomics were compared between the two groups.</p><p><b>RESULTS</b>In the LVR group, the left ventricular end-diastolic pressure increased, the maximal left ventricular pressure increase/decrease ratio decreased significantly, and the left ventricular systolic pressure decreased. H-E staining and Masson staining of cardiac muscle tissues of the LVR group showed myocytolysis, disarray, and collagen proliferation. Twenty-one differentially expressed proteins were detected by proteomic analysis. We validated two proteins using western blot analysis. The differentially expressed proteins could be divided into six categories: energy metabolism-related proteins, cytoskeletal proteins, protein synthesis-related proteins, channel proteins, anti-oxidation- related proteins, and immune-related proteins.</p><p><b>CONCLUSION</b>These differentially expressed proteins might play key roles in LVR following MI.</p>

<p><b>OBJECTIVE</b>To investigate the expression and pathobiological implications of angiotensin II type 1 receptor (AT1R) in development of myocardial fibrosis of rats.</p><p><b>METHODS</b>Rat myocardial necrosis model was established using isoproterenol injection (15 mg/kg). Rat serum aspartate transaminase (AST), lactate dehydrogenase (LDH), creatine kinase (CK) and creatine kinase isoenzymes (CK-MB) were detected by MD-100 automatic biochemical analyzer. Masson staining was used to evaluate the morphological changes. The expression of AT1R protein was determined by immunohistochemistry and its mRNA expression was analyzed by RT-PCR. The expression of collage type I and III was determined by immunohistochemistry.</p><p><b>RESULTS</b>Serum LDH, CK and CK-MB reached their peaks at 4 h (chi2 = 16.90, P < 0.05), and AST achieved its peak in 6 h (chi2 = 16.90, P < 0.05). AT1R mRNA expression was increased 2 - 12 h after isoproterenol injection, but no statistical significance (P > 0.05) was observed comparing with the control. However, a significant AT1R mRNA increase was present at 24 h and decreased gradually after 48 h, and back to the control level after 3 weeks. Protein expression of AT1R increased proportionally with the severity of the fibrosis.</p><p><b>CONCLUSIONS</b>AT1R mRNA and protein expressions increase significantly during myocardial ischemia, and is closely correlated with the fibrosis. These findings indicate that AT1R may play an important role in the pathogenesis of myocardial fibrosis.</p>

<p><b>OBJECTIVE</b>To investigate the ameliorative effect of ginseng glycopeptide on cross-linking of rat tail tendon collagen.</p><p><b>METHOD</b>ELISA was used to determine the inhibitory effect of ginseng glycopeptide on cross-linking of rat tail tendon collagen in vitro. After ginseng glycopeptide was intraperitoneally administrated to streptozotocin-induced diabetic rats for 12 weeks, the acid solubility, limited pepsin degradation properties and solubility in SDS-2-mercaptoethanol of the rat tail tendon collagen were determined, and the effect of ginseng glycopeptide on the tail tendon collagen cross-linking was evaluated.</p><p><b>RESULT</b>Ginseng glycopeptide inhibited significantly the cross-linking of rat tail tendon collagen in vitro. The solubility of the tail tendon collagen (in acid, pepsin and SDS-2-mercaptoethanol) was markedly decreased in diabetic rats and ginseng glycopeptide-treated diabetic rats had significantly an increase in the collagen solubility in the above-mentioned solutions, suggesting that ginseng glycopeptide decreased severity of the collagen cross-linking.</p><p><b>CONCLUSION</b>Ginsengglycopeptide exhibits an significantly ameliorative effect on cross-linking of rat tail tendon collagen.</p>
