RESUMO

Objective:With the in-depth study of complement dysregulation, glomerulonephritis with dominant C3 has received increasing attention, with a variety of pathologic types and large differences in symptoms and prognosis between pathologic types. This study analyzes the clinical, pathological, and prognostic characteristics of different pathological types of glomerulonephritis with dominant C3,aiming to avoid misdiagnosis and missed diagnoses. Methods:The clinical, pathological, and follow-up data of 52 patients diagnosed as glomerulonephritis with dominant C3 by renal biopsy from June 2013 to October 2022 were retrospectively analyzed.According to the clinical feature and results of pathology,15 patients with post-infectious glomerulonephritis(PIGN)and 37 patients with of non-infectious glomerulonephritis(N-PIGN)were classified.N-PIGN subgroup analysis was performed, and 16 patients were assigned into a C3-alone-deposition group and 21 in a C3-dominant-deposition group,or 27 in a C3 glomerulopathy(C3G)group and 10 in a non-C3 nephropathy(N-C3G)group. Results:The PIGN group had lower creatinine values(84.60 μmol/L vs 179.62 μmol/L,P= 0.001),lower complement C3 values(0.36 g/L vs 0.74 g/L,P<0.001)at biopsy, and less severe pathological chronic lesions compared with the N-PIGN group. In the N-PIGN subgroup analysis, the C3-dominant-deposition group had higher creatinine values(235.30 μmol/L vs 106.70 μmol/L,P=0.004)and higher 24-hour urine protein values(4 025.62 mg vs 1 981.11 mg,P=0.037)than the C3-alone-deposition group. The prognosis of kidney in the PIGN group(P=0.049),the C3-alone-deposition group(P=0.017),and the C3G group(P=0.018)was better than that in the N-PIGN group, the C3-dominant-deposition group, and the N-C3G group, respectively. Conclusion:Glomerulonephritis with dominant C3 covers a variety of pathological types, and PIGN needs to be excluded before diagnosing C3G because of considerable overlap with atypical PIGN and C3G;in addition, the deposition of C1q complement under fluorescence microscope may indicate poor renal prognosis, and relevant diagnosis, treatment, and follow-up should be strengthened.

RESUMO

BACKGROUND:With the innovation of examination technique, the number of patients with spinal metastases in different stages is increasing year by year.Percutaneous vertebroplasty is an important treatment for spinal metastases;however, there is no report on the biomechanical effect in different stages and different activities after operation. OBJECTIVE:To simulate thoracic T10 bone stress and displacement of the different locations of the tumor metastasis based on the three-dimensional finite element model. METHODS:According to thoracic three-dimensional CT images of a 30-year-old healthy male,Mimics software was used to construct a three-dimensional geometric model of thoracic vertebrae(T9-T11),including ribs, ligaments and intervertebral discs.Three-dimensional models of T9-T11 vertebral bodies and different parts of the posterior thoracic vertebrae invaded by thoracic metastatic tumors were simulated, including the control group with intact vertebral structure, unilateral metastasis involving the vertebral body area(experimental group 1),unilateral metastasis involving the vertebral body and pedicle area(experimental group 2),unilateral metastasis involving the vertebral body, pedicle and transverse process area(experimental group 3),and bilateral metastasis involving the vertebral body, pedicle and transverse process area(experimental group 4).Abaqus software was used to create a three-dimensional finite element model. The von Mises stress distribution and the displacement of the model were analyzed under the loading condition, buckling condition, extension condition, and rotation condition. RESULTS AND CONCLUSION:(1)In the study of the maximum total displacement of loading points in different experimental groups under loading, flexion, extension, and rotation conditions, with the increase of metastatic tumor invasion site and invasion surface, the total displacement of loading points increased, and the overall stiffness decreased, especially the total displacement of loading points in experimental group 4 was the largest.(2)Under flexion condition, the maximum Von Mises stress value increased significantly after vertebral body and pedicle destruction, while the maximum Von Mises stress value was almost unchanged when the thoracocostal joint destruction was added.(3)On the basis of finite element analysis and simulation of bone tumor model, the elements in the bone cement region were set as a single set,and the bone cement region was set as the corresponding material properties to simulate bone cement filling. The results showed that the maximum total displacement under loading, flexion, extension, and rotation conditions was less than that of each experimental group.(4)The maximum stress values of the simulated percutaneous vertebroplasty patients in the loading, flexion, extension and rotation conditions were significantly lower than those of the femoral model.(5)It is concluded that the three-dimensional finite element model based on thoracic T9-T11 conducive to the biomechanics characteristics of thoracic vertebrae tumor metastasis, and on the basis of the thoracic vertebrae tumor metastasis model can accurately simulate load point after percutaneous vertebral body under different conditions of total displacement and the maximum Von Mises stress situation.

RESUMO

【Objective】 To analyze the data of non-small cell lung cancer (NSCLC) gene chip using the bioinformatics method, screen differential expression genes (DEGs), and explore the biomarkers related to the prognosis of NSCLC so as to provide a new target for the treatment of NSCLC. 【Methods】 The NSCLC gene chip data were downloaded from the GEO database and the common DEGs in the two datasets were screened by GEO2R tool and FunRich3.1.3 software. The DAVID database was used in GO analysis and KEGG analysis of the DEGs. The protein-protein interaction (PPI) network was constructed using the STRING database; Cytoscape 3.8.0 software was used to select the top 20 hub genes. Then Kaplan-Meier plotter was used to analyze the prognosis of the identified hub genes, and multiple external databases were used to verify the expressions of the hub genes and their relationship with prognosis. 【Results】 A total of 159 intersect DEGs were screened from the two datasets. A total of 20 hub genes were identified via PPI network. Survival analysis and validation results from multiple external databases showed that SPP1 was highly expressed in NSCLC tumor tissues and was significantly correlated with the patients’ poor prognosis (P<0.05). The subgroup analysis showed that SPP1 might cause the poor prognosis by affecting lymph node metastasis. 【Conclusion】 SPP1 may be a biomarker for evaluating the prognosis of NSCLC patients, providing a new idea for the targeted therapy of NSCLC.

RESUMO

Objective:To analyze the epidemiological characteristics of hepatitis B (HB) in Huzhou from 2004 to 2017 in order to provide theoretical basis for the prevention of HB. Methods:Descriptive epidemiologic methods were used to analyze the data of HB cases in Huzhou city, 2004-2017. Results:A total of 16 685 HB cases were reported in Huzhou from 2004 to 2017. The average annual incidence of HB was 41.58/105. Incidence tended to decrease (χ2=5 094.667, P<0.01). The incidence of HB among males was higher than that among females (χ2=2 619.143,P<0.01), and the incidence rate among males and females both tended to decrease (male: χ2= 3 408.813, P<0.01;female: χ2=1 716.388, P<0.01). Of the reported case of HB, 83.0% involved individuals between 20 and 60 years of age. Most of the patients were farmers, accounting for 52.41% of the total cases. The top three districts reporting high case numbers were Changxing (4 584 cases (27.47%)), Wuxing (4 356 cases (26.11%)), Anji (3 192 cases(19.13%)), while the top county/district of the annual incidence was Changxing (50.10/105). Conclusion:Incidence of HB kept decreasing between 2004 and 2017 in Huzhou. Adults are the key population vulnerable to HB virus. To further reduce the incidence of HB, it is necessary to strengthen the prevention of HB and HB vaccination rate among high-risk groups.

RESUMO

Objective:To assess the diagnostic value of whole-lesion intravoxel incoherent motion diffusion weighted imaging (IVIM-DWI) in differentiating benign and malignant thyroid nodules based on turbo spin echo (TSE) sequence. Methods:Totally 57 patients with thyroid nodules diagnosed by pathology were prospectively collected at the Affiliated Lianyungang Hospital of Xuzhou Medical University from March 2019 to December 2019. A total of 62 nodules were included for analysis, including 27 benign nodules and 35 malignant nodules. All patients underwent routine MRI scan and IVIM-DWI based on TSE sequence before surgery. The regions of interest were drawn along the edge of the lesion on multiple consecutive slices to cover the entire nodule, and the apparent diffusion coefficient (ADC), diffusivity (D), pseudo-diffusivity (D *) and quantitation of perfusion fraction (f) values were measured, and the mean value of the multilayer measurement was recorded for further analysis. The differences of the parameters between benign and malignant thyroid nodules were analyzed using independent samples ttest (normal distribution) and Mann-Whitney U test (non-normal distribution). The diagnostic efficacy of parameters with statistical significance was evaluated using the receiver operating characteristics (ROC) curve. Results:The ADC, D and D *values in the malignant nodules were lower than those in benign nodules, the differences were statistically significant ( t=3.821, P=0.001; t=2.034, P=0.046).There were no statistical differences of D * and f values ( t=-1.170, P=0.247; Z=-0.559, P=0.577).The optimal cut-off values of ADC and D were 1.90×10 -3mm 2/s and 1.10×10 -3mm 2/s, with both sensitivity of 81.5%, specificity of 73.5% and 61.8%, respectively. The area under ROC curve of ADC and D were 0.782 and 0.688, and the statistical difference was found ( Z=2.098, P=0.040). Conclusion:The whole-lesion IVIM parameters (ADC and D) based on the TSE sequence provide imaging features for preoperative differentiation of benign and malignant nodules of the thyroid gland.

RESUMO

Retinoblastoma (RB) is the most common intraocular malignant tumor in early childhood, which has an adverse effect not only on the eyesight, but also on patients' life. Uveal melanoma (UM) is an another common intraocular malignant tumor in adults, nearly half of the patients will have metastasis of tumor with a low survival rate. The long non-coding RNAs (lncRNAs) are non-coding RNA with a length greater than 200 nt. More and more studies have confirmed that lncRNAs are involved in the proliferation, migration and invasion of tumors by regulating the gene expression at various levels, which makes lncRNAs become the novel target in the therapy of intraocular malignant tumors. This article reviewed studies of the roles and mechanisms of lncRNAs in RB and UM.

RESUMO

Objective To learn the salt intake level of residents with hypertension in rural community and the influencing factors of salt restriction behavior. Methods We used random cluster sampling method to extract two rural communities. A total of 200 residents with hypertension or high - risk of hypertension were selected as the subjects. We conducted general information questionnaire, hypertension knowledge, attitudes and behavior questionnaire,3 - day salt intake survey and urine sodium detection to evaluate the level of sodium intake. We used multivariate logistic regression equation modeling to predict influencing factors of salt restriction behavior. Results After the completion of the investigation,M (QR )for daily sodium intake of 173 cases in the intervention group was 6. 72(5. 18)g,and the main route were through the salt, monosodium glutamate ,soy sauce, pickles. Daily 24 hours urinary sodium excretion amount was 90. 10(62. 28) mmol / 24 h and 152 cases(87. 86% )of respondents had salt - restriction - spoon, and 108 cases(71. 05% )use the salt- restriction - spoon, but only 53 cases(49. 07% )used the spoon correctly. The frequency of spoon for salt restriction and sodium intake was negatively correlated(P ﹤ 0. 05),whether spoon for salt restriction was used correctly and 24 - hour urinary sodium excretion was negatively correlated( P ﹤ 0 . 0 5 ). By multivariate logistic regression analysis ,those people who had high level of the average annual household income(OR = 2. 75,95% CI:1. 16 - 6. 53),identified 6 g of salt a day(OR = 5. 43,95% CI:1. 22 - 24. 07),regular consumption of vegetables(OR = 9. 35,95% CI:1. 16 - 75. 01) and initiative to take measures to control salt( OR = 5. 05,95% CI:1. 19 - 21. 45)were more likely to use salt -restricted spoons. Residents of drinking(OR = 0. 13,95% CI:0. 02 - 0. 84)did not tend to use salt - restricted spoons. Conclusion For people with high NaCl intake and no restriction behavior, the level of health knowledge, especially the knowledge of sodium salt, should be improved and the good dietary habits including salt - limited support tools and correct methods should be promoted .

RESUMO

Objective@#To explore the feasibility and effectiveness of "one-puncture one-needle" transrectal ultrasound (TRUS)-guided prostate biopsy in the prevention of postoperative infections.@*METHODS@#We retrospectively analyzed the clinical data about "one-puncture one-needle" (the observation group) and "one-person one-needle" (the control group) TRUS-guided prostate biopsy performed in the Second People's Hospital of Guangdong Province from January 2005 to December 2015, and compared the incidence rates of puncture-related infection between the two strategies. By "one-puncture one-needle", one needle was used for one biopsy puncture, while by "one-person one-needle", one needle was used for all biopsy punctures in one patient and the needle was sterilized with iodophor after each puncture.@*RESULTS@#Totally, 120 patients received 6+1-core or 12+1-core "one-person one-needle" and 466 underwent 12+1-core "one-puncture one-needle" TRUS-guided prostate biopsy. There were no statistically significant differences between the two groups of patients in age, the prostate volume, the serum PSA level, or the detection rate of prostate cancer (P >0.05). Compared with the control group, the observation group showed remarkably lower incidence rates of puncture-related urinary tract infection (7.5% vs 0.9%, P <0.05), fever (5.0% vs 1.1%, P <0.05), bacteriuria (2.5% vs 0.2%, P <0.05), and total infections (16.7% vs 2.6%, P<0.05) postoperatively. Two cases of bacteremia or sepsis were found in each of the groups, with no significant difference between the two.@*CONCLUSIONS@#"One-puncture one-needle" TRUS-guided prostate biopsy can effectively prevent puncture-related infections.
