RESUMO

Objective How to improve the operational safety of foraminoplasty has become a hot spot in present clinical research. This study was to observe the clinical effect of minimally invasive treatment of lumbar disc herniation by percutaneous transforaminal endoscopic discectomy (PTED) combined with dynamically assisted visualized intervertebral foraminoplasty (VIVF).Methods Totally, 61 patients with lumbar disc herniation underwent PTED combined with dynamically assisted VIVF in Jiangsu Provincial Hospital of Integrated Traditional Chinese and Western Medicine from January to November 2017. We evaluated the clinical effects using the Visual Analogue Scale (VAS), Oswestry Disability Index (ODI) and modified Macnab Criteria.Results The VAS scores of the patients were significantly lower at 3 days, 3 months and 6 months after surgery than the baseline (1.10±0.60, 1.03±0.26 and 1.07±0.31 vs 7.64±1.11, P<0.05), and so were the ODIs (2.10±0.54, 1.30±0.49 and 1.23±0.46 vs 34.46±3.57, P<0.05). The excellence rate of treatment 96.72% (59/61). None of the patients experienced such postoperative complications as nerve root injury, spinal injury, and dural matter, and no recurrence was observed.Conclusion PTED combined with dynamically assisted VIVF is safe and effective for the treatment of lumbar disc herniation.

<p><b>BACKGROUND</b>Although the role of fibrinogen as a predictor of acute myocardial infarction (MI) has been well-established, the association of genetic polymorphisms in the fibrinogen gene with MI is still controversial. This study was conducted to elucidate the association between the genetic polymorphisms of the fibrinogen beta-chain (FGB) gene and MI in Chinese Han population.</p><p><b>METHODS</b>The occurrence of 3 common polymorphisms (i.e., -455G/A, R448K and 8558C/G) in a case-control study including 508 patients with MI and 503 healthy controls was investigated. Results Analyses of single polymorphisms showed that individuals carrying the rare alleles for the 3 polymorphisms were significantly associated with a decreased risk of MI. Logistic regression analysis indicated that R448K remained independently associated with MI after adjustment for environmental risk factors (adjusted odds ratio(OR) = 0.71 for KK/RK versus RR, P = 0.023). The three polymorphisms were found to be in strong linkage disequilibrium. Haplotype analyses showed that the A-K-G haplotype (-455A, 448K, 8558G) was associated with a protective effect against MI. Compared with the common haplotype G-R-C, the adjusted OR for A-K-G was 0.68 (95% CI, 0.51-0.90; P = 0.006).</p><p><b>CONCLUSION</b>These data indicate that individuals carrying the FGB 448K allele may be protective against having MI in this population.</p>

<p><b>OBJECTIVE</b>To investigate the association between atrial fibrillation and the single nucleotide polymorphism (SNP) of slow delayed rectifier K(+) channel (I(Ks)) genes in Han nationality Chinese.</p><p><b>METHODS</b>Three hundred and eighty of Han nationality Chinese (142 atrial fibrillation, 120 in-hospital and 118 out-hospital control) were enrolled in this study. Asian specific non-synonymous SNPs of KCNQ1 P448R, KCNQ1 R519H, KCNQ1 G643S, KCNE1 G38S and KCNE1 D85N were genotyped by restriction fragment length polymorphism analysis. A newly cloned KCNE4 gene was also screened for any possible SNP.</p><p><b>RESULTS</b>The minor allele frequency of KCNQ1 P448R, KCNQ1 R519H, KCNQ1 G643S, KCNE1 G38S and KCNE1 D85N in out-hospital subjects was 0.079, 0, 0.042, 0.317 and 0.004, respectively. None of these SNPs was relationed with any atrial fibrillation phenotype. A KCNE4 E145D was discovered and proven statistically to relation significantly to atrial fibrillation by logistic regression analysis (OR = 1.66, P = 0.044). The minor allele frequency of KCNE4 E145D was as high as 0.271 in out-hospital subjects.</p><p><b>CONCLUSIONS</b>None of the SNPs of KCNQ1 P448R, KCNQ1 R519H, KCNQ1 G643S, KCNE1 G38S and KCNE1 D85N was associated with atrial fibrillation phenotype, but KCNE4 E145D may relation to atrial fibrillation. The effect of KCNE4 E145D variation on the function of I(Ks) channel is to be determined.</p>

<p><b>AIM</b>To determine whether bombesin prevents IFN-alpha-induced fever and it's possible mechanism.</p><p><b>METHODS</b>Effects of BN on changes in body temperature and arginine vasopressin(AVP) content in the ventral septal area(VSA) and hypothalamus were measured in the rats following intracerebroventricular (ICV) injection of IFN-alpha.</p><p><b>RESULTS</b>(1) IFN-alpha produced a dose-dependent rise in colonic temperature simultaneously with increase in AVP content in the VSA in the rats. (2) BN produced a dose-dependent hypothermia and significantly elevated AVP content in the VSA in rats. (3) BN injected intracerebroventricularly at 30 min after IFN-alpha prevented the increase in colonic temperature which recovered to the control level as well as AVP content in the VSA in rats at 150 min.</p><p><b>CONCLUSION</b>AVP in the VSA may play a role in IFN-alpha-induced fever. AVP in the VSA may play a partial role in the BN antipyretic action and hypothermic action.</p>
