RESUMO

<p><b>BACKGROUND</b>Short-term outcomes of the Wallis system in the treatment of lumbar degenerative disease (LDD) have been shown to be effective, whereas there is a paucity of studies on the mid-long-term effects of the treatment of the Wallis system. This study was to evaluate the mid-long-term effects of the Wallis dynamic stabilization system in the treatment of LDD.</p><p><b>METHODS</b>A total of 26 patients who received the treatment of the Wallis system between February 2008 and January 2009 were included in the study, with 14 patients (Group 1) with L4/5 disc herniation and 12 patients (Group 2) with L5/S1 disc herniation and L4/5 intervertebral disc degeneration (IDD). Visual analog scale (VAS) and Oswestry Disability Index (ODI) were used to evaluate the clinical outcomes and lumbar X-rays and MRI were obtained to observe imaging changes before and after operation.</p><p><b>RESULTS</b>The mean follow-up period was (63.50±2.12) months. The mean ODI and VAS scores decreased obviously three months and five years after operation (P < 0.05). In Groups 1 and 2, L4/5 Cobb angle and range of motion (ROM) decreased and L4/5 posterior disc height increased at the last follow-up (P < 0.05). There were no statistically significant changes in L4/5 anterior disc height and L3/4 University of California at Los Angeles grading before and after operation. There was no statistically significant change in Pfirrmann grading system of L4/5 IDD in Group 2 before and after operation. Adjacent segment degeneration at the last follow-up was found in two patients (2/26, 7.69%) and Modic changes in L4/5 endplates were detected in one patient (1/26, 3.85%).</p><p><b>CONCLUSIONS</b>The mid-long-term effects of the Wallis system in the treatment of LDD were satisfied. The Wallis system, as a dynamic stabilization system, which can preserve some ROM of the fixed segment, sustain the lumbar stabilization, and prevent adjacent segment disease and fixed segment degeneration, is an effective instrument to treat LDD.</p>

@#Objective To identify the role of paravertebral muscles in the pathogenesis of scoliosis.MethodsParavertebral muscles were gotten from the 37 patients(12 congenital scoliosis patients and 25 idiopathic scoliosis patients) during the operations.Cryostat sections were cut by 10 μm nd stained with H&E,m-GT,NADH-TR,ATPase.ResultsMyogenic changes,incuding muscle fibrosis,fiber necrosis,etc,were common in paravertebral muscles of scoliosis patients,however regenerating fibers were quite rare.Diffuse fibrosis and remarkablely disorganized fiber directions presented in most of congenital scoliosis patients,while focal fibrosis without necrosis in most of idiopathic scoliosis patients.Neurogenic changes were found in one congenital scoliosis patient and 4 idiopathic scoliosis patients,however four of the five patients had undergone orthopedics.Thickened capsule wall of muscle spindles and connective tissue infiltration in muscle spindles were found in both kinds of scoliosis.ConclusionThere are some differences on pathological changes of paravertebral muscles between congenital scoliosis and idiopathic scoliosis,which indicates that paravertebral muscles may play a special role in the pathogenesis of idiopathic scoliosis.

0.05)indicating no significant loss of correction.The length of the growth of instrumented spine was 13.3 mm.No sever complications in the series. Conclusion The PRSS which dispenses without bony fusion is a safe and an effective instrumen for management of juvenile scoliosis.It provides and maintains desirable scoliosis correction in one stage procedure,while allowing spinal growth.

@#ObjectiveTo analyze the classifications of the abnormalities of cervical curvature and its treatments with manipulation. Methods209 patients with cervical lordosis were analyzed. The abnormalities of the cervical curvature were classified as over-curvature (>45°), hypo-curvature (0°~30°), the straightened cervix (0°), backward curvature(0°~―20°), multiple curvatures (the cervical curvature showed "S" shaped). The C_5 subluxation were restored with manipulation in patients with hypo-curvature and straightened cervix, or other vertebra subluxation in multiple curvatures. Results1~4 yeared follow-up, 127 cases were excellently well being, 59 were well being, 23 were improved.ConclusionManipulation is effective on abnormalities of cervical curvature.

@#目的探讨应用非手术方法治疗老年患者腰椎管狭窄症的疗效。方法对74例老年(>60岁)腰椎管狭窄的患者采用手法矫正椎体的假性滑移、药物治疗、适当的腰背肌功能锻炼综合治疗。随访至少2年。采用自定标准评定疗效。结果优(无疼痛及间歇性跛行,无功能限制,不用止痛剂)34例,良(偶然疼痛及间歇性跛行,轻度功能受限,偶尔使用止痛剂)27例,一般(中度的疼痛及间歇性跛行,中度功能限制,无运动,经常使用止痛剂)11例,差(中度每天疼痛及间歇性跛行,或偶尔严重疼痛,每天日常生活活动受限,应用强止痛剂)2例。结论对老年腰椎管狭窄症采取综合的非手术方法能取得一定疗效。

RESUMO

Objective:Research on establishing a better experimental arthritis rat model of cold-dampness arthraligia on rheumatoid arthritis(RA) research.Methods:In order to induce arthritis,the rat's feet were injected with different concentrations of type Ⅱ collagen mixed with Freund's adjuvant;measure the feet swelling degrees of rats,then calculated swelling rate;detect the level of serum sialic acid(SA);observ the synovial histopathology and ultrastructural of rats.Results:The rat joints swell of the group with low concentration of type Ⅱ collagen and completed Freund's adjuvant was rapid and severe,while that of the group with high concentration of type Ⅱ collagen and incompleted Freund's adjuvant was moderate and lasting.The level of serum SA of model groups was higher than the normal group.The histopathology changes of rat's synovium conform to RA.Conclusion:The group injected with high concentration of type Ⅱ collagen and incompleted Freund's adjuvant is the ideal RA model.It is belonged to the symptom of cold-dampness arthralgia,compared with this model,the AA approaches the symptom of heat-dampness arthralgia.
