RESUMO

Traditional Chinese Medicine (TCM) can inhibit the proliferation of leukemia cells and induce apoptosis. The signaling pathways involved mainly include PI3K/Akt pathway, MAPK signaling pathway, JAK-STAT pathway, Wnt pathway and mitochondrial pathway. Among them, the mitochondrial apoptotic pathway is affected by many key apoptotic pathways, which plays a terminal role in promoting apoptosis. At present, there is a great need for the systematic and comprehensive research on various signaling pathways and intermolecular interactions.

RESUMO

Objective Under noninvasive positive pressure ventilation, in view of inconvenient operations in human airway, and avoiding the interference of spontaneous breathing and the necessary leakage flow, the method for dynamic online monitoring of the respiratory resistance (R) and compliance (C) was studied.Methods At the end of expiration, when the exhaled flow was 0, relative to the expiration positive airway pressure (EPAP), a step-dropped pressure was produced with the amplitude Δp and hold time Δt. Under this dropped pressure, a short-time discharged flow was generated from the lung, and the discharged flow was applied to calculate the R and C. In addition, a respiratory model was developed in MATLAB to simulate the breaths of normal adult, acute respiratory distress syndrome (ARDS) patient and chronic obstructive pulmonary disease (COPD) patient. A serial of simulation experiments were carried out for obtaining data and verification.Results The calculated R and C from simulation for normal adult, ARDS patient, COPD patient, deviated from the actual value by 1.60% and -1.60%, 1.21% and -1.19%, -12.53% and 14.32%, respectively.Conclusions The proposed algorithm is practicable and feasible for calculating the R and C. The simulation results are beneficial for studying and realizing the intelligent ventilation and proportional assist ventilation in respirator.

RESUMO

Objective To analyze and compare the curative effect of thoracic laparoscopy combined with resection of esophageal carcinoma and conventional three incision resection of esophageal carcinoma .Methods From January 2012 to January 2016,sixty-two patients received esophageal cancer resection in Shanxi Provincial Cancer Hospital were selected in the research.The patients were divided into laparoscopic group (28 cases) and traditional group(34 cases) according to surgical methods ,and they were treaded with thoracic laparoscopy combined with resection of esophageal carcinoma and three incision resection of esophageal carcinoma ,respectively.The amount of blood loss , the number of lymph node dissection ,the postoperative 1d flow,the ventilator use of 24h and the number of tracheotomy were recorded.The time of hospitalization,the time of closed drainage and the incidence of postoperative complications were recorded and compared between the two groups at discharge time.Results The operative time,amount of blood loss,drainage volume,drainage time and hospitalization time of the laparoscopic group were (3.22 ±0.64)h,(218.20 ± 45.42)mL,(235.67 ±42.15)mL,(3.32 ±1.81)d,(14.28 ±2.53)d,respectively,which were significantly lower than those of the traditional group [(4.52 ±1.11)h,(407.42 ±50.06)mL,(321.71 ±37.82)mL,(5.16 ±1.78)d, (17.35 ±4.38)d,t=5.764,15.438,8.465,4.020,3.448,all P<0.05].The incidence rates of pulmonary infection and atelectasis in the laparoscopic group were 3.57% and 0.00%,respectively,which were significantly lower than those in the traditional group (23.53% and 17.65%,χ2=4.929,5.471,all P<0.05).The ventilator -assisted breathing rate and postoperative tracheotomy rate of the laparoscopic group were both 0.00%,which were significantly lower than those of the traditional group (23.53%and 14.71%,χ2=7.564,4.479,all P<0.05).Conclusion It is safe and reliable to complete lymph node dissection and surgical treatment of thoracic laparoscopy combined with resection of esophageal carcinoma,and compared with conventional three incision resection of esophageal carcinoma it has the advantages of small trauma,low incidence rate of complication and fast recovery,which is worthy of further promotion.

Objective To compare the clinical outcomes of different coracoclavicular ligament (CCL) reconstruction combined with hook plate fixation in the treatment of acromioclavicular joint (ACJ) dislocation of Rookwood type Ⅲ-Ⅴ. Methods A retrospective study was conducted to evaluate the outcomes of 76 patients with ACJ dislocation of Rockwood type Ⅲ-Ⅴwho had been treated with CCL reconstruction using iliotibial tract fascia graft or palmaris longus tendon in addition to hook plate fixation from January 2012 to March 2016. The patients were divided into 2 groups according to their grafts used in CCL reconstruction:group A (39 cases using iliotibial tract fascia graft ) and group B (37 cases using palmaris longus tendon iliotibial tract fascia graft). They were firstly treated with CCL reconstruction followed by hook plate fixation. The acromioclavicular and coracoclavicular distances were measured on the postoperative anteroposterior radiographs of the injured shoulders. The outcomes were assessed at the final follow-ups according to Constant-Murley shoulder score and Karlsson criteria. Results The acromioclavicular distances and coracoclavicular distances measured in group A at 12 months after operation were significantly smaller than those measured in group B: (4.7 ± 0.7) mm vs. (5.4 ± 1.3) mm, (7.5 ± 1.5) mm vs. (8.5 ± 1.6) mm, P<0.05. At 18 months after operation, the Constant-Murley shoulder scores (92.2 ± 7.3) and Karlsson excellent to good rate 82.1%(32/39) in group A were similar to those in group B (P > 0.05). The total incidence of complications in group A and there showed no statistical difference compared with that of group B (P>0.05). Conclusions In the treatment of ACJ dislocation of Rookwood type Ⅲ-Ⅴ, CCL reconstruction using iliotibial tract fascia graft may lead to better radiographic outcomes than compared with that using palmaris longus tendon, though the 2 grafts lead to similar functional recovery of the injured shoulders.

Objectives To compare clinical manifestations and electrophysiological features in patients with chron?ic inflammatory demyelinating polyneuropathy (CIDP) and Type-I Charcot Marie Tooth Disease (CMT-I) for guiding dif?ferential diagnosis. Methods Data including clinical manifestations and electrophysiological indexes was collected from thirty-one CIDP cases and 28 CMT-I cases. Correlation analysis was used to assess the association of the severity of electrophysiology with the severity of clinical symptoms. Results There were statistically significant differences in onset site, sensory dysfunction, foot deformity and cerebrospinal fluid protein between these two groups (P0.05). Conclusions Differential diagnoses of CIDP and CMT-I can be made based on clinical manifestations and electro?physiological features.

Objective To analyze the outcome of patients receiving intrauterine insemination with husband sperm,in order to evaluate the effect of relative factors on pregnancy rate after intrauterine insemination. Methods Ninety-eight infertile couples who received intrauterine insemination in the Affiliated Hospital of Nantong University from March 2013 to May 2014 were selected as our subjects and 181 cycles were included. The information including clinical factors including maternal age,infertile time,infertile causes, ovulation induction protocol,time of insemination and postwash total motitle sperm(TMS)and pregnancy rate were recorded. Results (1)Totally 26 patients received clinical pregnancies,and clinical pregnancy rate(CPR) was 14. 36% per cycle. With age increase pregnancy rate decreased( χ2 = 1. 654 9,P = 0. 647).(2)The pregnancy rate of the patients was the same within the infertile time( χ2 = 1. 588 5,P = 0. 662).(3)The pregnancy rate of the patients with secondary infertility was lower than that of the patients with primary infertility,but there was no significant difference(χ2 = 0. 923 3,P = 0. 337).(4)The pregnancy rate of ovulation induction cycles was lower than that of nature cycles,but there was no significant difference(χ2 = 2. 222 0,P= 0. 136).(5)Postwash TMS was showed the same trend(χ2 = 0. 643 4,P = 0. 422). Conclusion In terms of intrauterine insemination with husband sperm,age,infertile time,infertile types,ovulation induction protocol and posrwash TMS can affect pregnancy rate,and the effects of various factors should be considered comprehensively in the process of therapy.

<p><b>OBJECTIVE</b>To investigate the clinical and genetics characteristics of patients with monosomal karyotype acute myeloid leukemia (MK-AML).</p><p><b>METHODS</b>The karyotypes of 3743 patients with newly-diagnosed de novo AML were analyzed, which had identified 153 cases with MK-AML, for whom the clinical and genetics characteristics were analyzed.</p><p><b>RESULTS</b>There were 2056 patients (54.9%) among all patients. A total of 153 patients fulfilling the criteria for MK-AML were identified, which comprised 93 males and 60 females, with a median age of 54. The median white blood cell count on presentation was 4.4×10 (9)/L. One hundred and forty-five cases (94.8%) have fulfilled the criteria for complex karyotype (≥ 3 chromosomal abnormalities). Although the monosomy could be found with all autosomes, chromosome 7 has been most frequently involved (38.56%, 59/153).</p><p><b>CONCLUSION</b>MK-AML is a distinct cytogenetic subtype of AML. Monosomy 7 is frequently detected among MK-AML patients. The monosomal karyotype is common among elder patients with AML.</p>

Objective: To observe effect of square dance exercises on symptom scores in menopausal women with coronary heart disease (CHD). Methods: A total of 39 menopausal CHD patients participated in square dance exercises, exercise intensity was 47.2%HRmax~58.1%HRmax(60~120 min/d), 5~7 times/week, three months, and they were regard as dance group. Control group consisted of 42 CHD women of same age living in the same district, who didn’t take part in exercise. After enrollment, these two groups received assessments of Seattle angina questionnaire (SAQ) and frequent physical and mental symptoms of menopausal women self-rating scale (Kupperman self-rating scale) within 3d and after three months. Results: There were no significant difference in score of each item and total score of SAQ in first assessment between two groups (P>0.05 all). Compared with first assessment and control group after three months, there were significant increase in scores of most items and total score of SAQ [(66.40±15.16) scores vs. (67.18±13.93) scores vs. (85.15±16.30) scores] in dance exercises group, P0.05 all). Compared with first assessment and control group after three months, there were significant decrease in scores of most items and total score of Kupperman scale [(29.40±6.51) scores vs. (29.73±6.26) scores vs. (23.65±6.29) scores] in dance exercises group, P<0.01~0.05. Conclusion: Square dance exercises may significantly decrease angina pectoris symptom and menopausal physical and mental symptoms in menopausal women with coronary heart disease.

Objective To detect the asymptomatic cardiovascular changes in children with adenoid hypertrophy.Methods One hundred and twenty children with adenoid hypertrophy underwent both chest X-ray and echocardiography before adenoidectomy,and echocardiography 6 months after operation.Results No child showed an increase in the cardiothoracic ratio on X-ray.But preoperative echocardiography showed an increase in pulmonary artery pressure [(22.6 ±3.6) mm Hg,1 mm Hg =0.133 kPa],a decrease in E/A (1.01 ± 0.17),and an increase in right ventricular end-diastolic diameters [(1.88 ± 0.18) cm].While after operation,pulmonary artery pressure decreased to(17.1 ± 3.2) mm Hg,E/A increased to 1.25 ± 0.12,and right ventricular end-diastolic diameters decreased to (1.67 ± 0.11) cm.Each index change before and after operation was statistically significant(P < 0.05).Conclusion Adenoid hypertrophy can result in clinically asymptomatic cardiopulmonary changes.Early diagnosis and treatment of this disease can prevent serious cardiopulmonary complications.

Objective To investigate effects of combined use of glycogen synthase kinase (GSK) -3 inhibitor and fructose-1,6-diphosphate (FDP) on liver trauma in rats. Methods After crea-tion of liver trauma model in 49 Sprague-Dawley rats, 42 rats were randomly divided into control group (NaCl group), FDP group and FGI Group (FDP and GSK-3 inhibitor in combination group). Then, each group was randomly subdivided into pre-ischemia group and 4-hour reperfusion group on account of time point when animals were sacrificed before and after iachemia. The other seven rats set as sham operation group (SH group) were sacrificed at 4-hour reperfusion time point. The AST and ALT levels in hlood and glycogen content, SOD vitality and MDA content in liver tissues were determined. Results At pre-is-chemia time point, liver glycogen content in three groups was in order of control group < FDP group < FGI group (P <0.01). At 4-hour reperfusion time point, blood ALT and AST levels in four groups were in order of control group > FDP group > FGI group > SH group (P < 0.01), while SOD vitality in liver tissues of four groups was in order of control group < FDP group < FGI group < SH group (P < 0.01) and MDA content in four groups was in order of control group > FDP group > FGI group > SH group (P < 0.01). Conclusions Combined use of FDP and GSK-3 inhibitor can enhance the protective effect of FDP on liver rupture, as may relate to the mechanism that GSK-3 inhibitor can effectively enhance glycogen synthesis of FDP as substrate before liver ischemia so that the liver glycogen storage is increased in a short period of time and hence post-traumatic warm ischemia-reperfusion injury is alleviated in the liver of rats.

BACKGROUND: Autologous bone, bone substitute materials and guided bone regeneration (GBR) technique can repair jaw defects, but the absorption speed of bone substitute materials and GBR membrane are faster than the formation speed of new bone, therefore, it affects the volume and shape of new bone.OBJECTIVE: To investigate the role of personalized prefabricated titanium template, autologous bone and nano-hydroxyapatite on restoration of maxillary defect in rabbit.METHODS: A total of 18 rabbits were randomly divided into two groups, and maxillary alveolar defect with 10 mm length and 5 mm high was created. The template was implanted in both two groups, and fastened with titanium screws. Autologous and nano-hydroxyapatite were placed into the defect in experimental group; neither autologous bone nor bone substitute materials were implanted into the defect in control group. New bone formation, X-ray findings, and histological changes with HE stain were carded out 4, 8 and 12 weeks postoperatively.RESULTS AND CONCLUSION: The quality of new bone in experimental group was batter than that in control group 4 weeks postoperatively, but the quality of new bone was almost the same 8 and 12 weeks postoperatively. By paired t-test, there was significant difference in new bone density between the experimental group and the control group 4 .weeks after operation (P<0.01), but there was no significant difference in new bone density between the experimental group and the control group 8 and 12 weeks after operation (P > 0.05). Autologous bone and nano-hydroxyapatite can restore the defect of maxillary alveola.Personalized prefabricated titanium template can play an important role of screen membrane and external scaffold in new bone formation, and remain shape of new bone.

Objective To develop a device for stretch-induced injury to cultured cells and measure the injury parameters.It makes possible to observe the changes of cell configuration and function directly after injury. Methods The device was developed based on aero-dynamical principle,and consisted of mini-type air compressor,electromagnetic valve,pressure transmitter,PGA amplifier,A/D converter and computer.The computer can accurately control the injury parameters.A new method and device were first introduced to measure the deformation rate and deformation degree of silastic membrance in the stretch induced cultured cell injury process.The device controlled by the computer was used to cause the astrocyte injury.The degree of cell injury was assessed qualitatively by electron microscopy,and quantitatively by lactate dehydrogenase(LDH) enzyme release and trypan blue staining.Results The experiment results showed that the injury of different levels could be reproduced by our device under the injury parameters we set.Conclusion The injury device and the measuring system meet the requirements of our design.The results suggested that the new device and the method were much better than those reported in foreign literature.It has the advantages of simple and convenient manipulation,high precision and real-time processing.It can be used for the cell-level experiment research.

<p><b>OBJECTIVE</b>To investigate the histopathological changes in the liver and other organs after impact injury.</p><p><b>METHODS</b>The rabbits were impacted with a BIM-IV biological impacting machine at the xiphoid process. The severity of liver injury was graded and scored through gross anatomy. At the same time, the pathological changes in the liver, heart, and lung were observed by light and electron microscopes.</p><p><b>RESULTS</b>Light microscopy showed that the pathological changes in the liver were: 1) loss of normal structure, hemorrhage and distortion of hepatic lobules; 2) cloudy swelling, degeneration, vacuolation and necrosis of liver cells; 3) infiltration of neutrophils. The lungs were injured and there were liver cell emboli in the small pulmonary arteries. Electron microscopy showed that the ultrastructure of the liver cells was severely damaged and the cells had significant features of necrosis.</p><p><b>CONCLUSIONS</b>The major pathomorphological changes in the liver after impact injury are hemorrhage and necrosis. They may be complicated by exfoliation of liver cells to hepatic sinusoids. These cells circulate with the blood to form emboli in the pulmonary blood vessels.</p>

Several specialized branches are inclusive in the department of trauma surgery,and the instruction of medical ethics during the practice of trauma surgery surely contributes to raise the integrated diathesis and capability for medical students.Common ethical issues in trauma surgery practice include ethics of operative principles,conflict between trauma sufferers and accident-makers,privacy preservation,decision making,and the principles of clinical pharmaceutical trials,etc.It is believed that advantageous principles and independence principles should be overall carried out during the instructional course of medical ethics education.

Objective To establish an HPLC method for the determination of schisanhenol in plasma and to study the pharmacokinetics of schisanhenol in rats.Methods After sedimentation by methanol,plasma samples were then prepared based on a liquid-liquid extraction by ether.The extracted samples were analyzed by liquid chromatography.Schisanhenol was eluted on Eclipse XDB-C18 Agilent(250 mm?4.6 mm,5 ?m) column,using a mobile phase of acetonitrile-H2O(65∶35),and detected at 254 nm.The plasma concentration of schisanhenol in rats was determined after iv administration of 18 mg/kg,and the data were processed with the pharmacokinetic software 3P87.Results Calibration curves were linear over 0.1—2.5 ?g/mL (r2=0.999) and the LOD was 10 ng/mL.The recoveries of schisanhenol from plasma were between 88%—110%,and the RSD values of intra-day and interday assay were below 15%.After iv administration at 18 mg/kg,the schisanhenol concentration-time curve confirmed in a two-compartment model and the pharmacokinetic parameters of t1/2?,t1/2?,V,AUC,MRT were(0.22?0.11) h,(1.19?0.22) h,(12.81?2.91) L/kg,(1.32?0.19) ?g/mL/h,(1.51?0.24) h,respectively.Conclusion A reliable HPLC-DAD method is developed for the determination of schisanhenol in rat plasma and it is applicable to the in vivo analysis.

OBJECTIVE:To survey the dosage range,clinical effect and safety of hypertonic sodium chloride hydroxylethyl starch 40 injection(HSS40) on anti-hemorrhagic shock METHODS:The changes of blood pressure and laboratory examination indices were detected after HSS40 was infused into the patients who suffered from hemorrhagic shock RESULTS:HSS40 could rapidly raise the blood pressure in a dose below 500ml and the effective rate could reach 100% In part of patients,the levels of serum Cl- and Na+ increased temporarily and restored after 24 hours CONCLUSION:HSS40 was safe and effective in doses of 80ml~500ml The clinical doses of 300ml~500ml are recommended
