RESUMO

Objective To propose a two-way fluid-structure interaction (FSI) method based on real patients with carotid artery stenosis, and analyze the hemodynamic parameters of carotid plaques with different types at the lesion as well as deformation and stress changes of the plaque itself. Methods Three-dimensional ( 3D) modeling was performed based on computed tomography angiography ( CTA) data of patients with moderate carotid artery stenosis. The carotid artery wall model and plaque model were separated, and transient fluid structure coupling calculation was performed. The situation from early stage of carotid atherosclerosis to formation of the plaque was simulated. The plaque types were divided into thickened plaques, lipid plaques, mixed plaques and calcified plaques, among which thickened plaques were regarded as non-plaque conditions for representing the thickening of vascular intima-media. The stenotic carotid arteries with different plaque types were compared and analyzed. Results The plaques with different types had little effect on the overall blood flow, but the wall shear stress of lipid plaques at the lesion was lower than that of other plaques. With thickened plaques as a control, concurrence of the plaque would inhibit artery expansion, and lipid plaques were the most obvious. Calcified plaques had the highest average plaque structure stress, while lipid plaques had the lowest average plaque structure stress. Conclusions The method proposed in this study can analyze fluid area and solid area at the same time. The results can contribute to better understanding the influence of different plaque types on carotid artery diseases.

Objective:To study magnetic resonance imaging (MRI) in differential diagnosis between benign and malignant gallbladder diseases.Methods:A retrospective study was conducted on 115 patients with gallbladder diseases managed at Nankai Hospital from August 2017 to August 2019. There were 50 males and 65 females, with the age of (57±12) years. Patients who underwent MRI and surgery were studied and the surgical histopathological examination was used as the gold standard. The MRI imaging findings in differentiating between benign and malignant gallbladder diseases were evaluated.Results:Of 115 patients, histopathological examination showed 70 patients had benign lesions, which included 24 gallbladder polyps, 19 adenomyomatosis of gallbladder, 18 gallstones or bile duct stones, and 9 gallbladder adenomas. Of the 45 patients with malignancies, there were 37 gallbladder adenocarcinomas and 8 squamous cell carcinomas of gallbladder. MRI examination on these 115 patients showed 64 benign and 42 malignant lesions. The overall compliance rate was 92.2% (106/115). The sensitivity, specificity, positive predictive and negative predictive value of MRI in the differential diagnosis between benign and malignant gallbladder diseases were 93.3% (42/45), 91.4% (64/70), 87.5% (42/48) and 95.5% (64/67), respectively. The Kappa value of MRI compared with the gold standard was 0.838, showing good consistency.Conclusion:MRI was useful, in the differential diagnosis between benign and malignant gallbladder diseases.

RESUMO

Objective: To explore deep vein thrombosis (DVT) occurrence in patients after right heart catheterization and the effect of anticoagulant therapy. Methods: A total of 171 consecutive patients with electrophysiological study (EPS) and/or radiofrequency catheter ablation (RFCA) in our hospital from 2015-01 to 2016-05 were enrolled. All patients had supra-ventricular tachycardia and completed a venous surgery, they were randomly divided into 2 groups: Anticoagulation group,n=87 and Non-anticoagulation group,n=84. Lower extremity vascular Doppler ultrasonography was performed at (24-48) h post-operation to compare the incidence of DVT between 2 groups. Results: There were 13/171 patients were excluded for not completing post-operative lower extremity vascular Doppler ultrasonography including 9 patients in Anticoagulation group and 4 in Non-anticoagulation group. 158 patients finished post-operative examination and follow-up study. Anticoagulation group had 7/78 (8.97%) patients suffered from DVT, Non-anticoagulation group had 41/80 (51.3%) patients suffered from DVT,P<0.001. Conclusion: The incidence of DVT was higher after right heart catheterization without anticoagulation; heparin treatment may reduce DVT occurrence in relevant patients.

RESUMO

Objective To study the curative effect and practicability of traditional anterior cervical surgery for cervical spondylosis.Methods retrospective lyunaly2ed,104 cage8 of anterior cervical sugery from June 1997 tO October 2005 Among them,there were 17 patients with radculopathy,31patients with cervical myelopathy,56 patients with combination of myelopathy and radieulopathy.all cases were treated by traditional anterior trephiement to excise prominent uncleus+interbedy fusion with auto-ilium graft.Results The resuhs were evaluated by Odom.The cases were all followed up.13 excellent,68 good,23 fair,0 poor.Excellent/good rate is 77.9%.The average medical coat of one patient is 4200 RMB.Conclusion The traditional Anterior cervical surgery for cervical mydopathy is safe、effective、practical.
