Objective:To analyze the clinical characteristics of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccine breakthrough infections in children, and to provide reference basis for the SARS-CoV-2 vaccination in children.Methods:A total of 97 children aged 3 to 14 years and diagnosed with coronavirus disease 2019 (COVID-19) admitted to Xi′an People′s Hospital (Xi′an Fourth Hospital) from December 27, 2021 to February 7, 2022 were included. According to the COVID-19 vaccination status, the enrolled children were divided into unvaccinated group, partially vaccinated group and fully vaccinated group, and the clinical data of the children in the three groups were collected and compared. Chi-square test, two independent sample t-test and Kruskal-Wallis H test were used for statistical analysis. Results:Totally 97 children including 49 males and 48 females were enrolled, with 87(89.7%) children of mild type, 10(10.3%) children of common type, and no severe or critical case. The proportions of unvaccinated, partially vaccinated and fully vaccinated preschool-aged children (3 to 6 years old) were 56.5%(13/23), 30.8%(12/39) and 17.1%(6/35), respectively, while those of school-aged children (7 to 14 years old) were 43.5%(10/23), 69.2%(27/39) and 82.9%(29/35), respectively. The vaccination proportion in preschool-aged children was significantly lower than that in school-age children ( χ2=9.94, P=0.007). The proportion of the children with fever in fully vaccinated group was 17.1%(6/35), which was lower than that in unvaccinated group (43.5%, 10/23), and the difference was statistically significant ( χ2=4.82, P=0.028). The cycle threshold (Ct) values of the open reading frame ( ORF)1 ab gene in the unvaccinated, partially vaccinated and fully vaccinated groups were 33.77(26.87, 36.58), 35.23 (33.45, 38.57) and 37.12 (34.91, 39.39), respectively, and there was a statistically significant difference among the groups ( H=7.76, P=0.021). The Ct values of the nucleocapsid protein ( N) gene in the three groups were 32.26(25.85, 36.18), 35.12(33.18, 37.96) and 37.26(34.27, 39.24), respectively, and the difference among the groups was statistically significant ( H=7.84, P=0.020). The Ct values of ORF1 ab gene and N gene in fully vaccinated group were higher than those in unvaccinated group, and the differences were statistically significant ( Z=-2.69, P=0.007 and Z=-2.39, P=0.017, respectively). The duration of viral shedding in fully vaccinated children was (9.9±4.1) d, which was shorter than that in unvaccinated children ((12.8±3.7) d), and the difference was statistically significant ( t=2.72, P=0.009). Conclusions:The majority of children with breakthrough infections with SARS-CoV-2 are mild. Vaccination may effectively shorten the duration of viral shedding. And fully vaccination is associated with mild clinical symptoms and lower serum viral load compared to unvaccinated children.

OBJECTIVE@#Hypertension is a low-grade inflammation state of the disease and was easily complicated by kidneys' inflammatory response. Mangiferin (MGF), a pharmacologically active compound in various plants including Mangifera indica, has a strong anti-inflammatory activity. However, the effects of MGF on renal inflammatory injury in spontaneously hypertensive rats (SHRs) remain unclear. The purpose of this study was to investigate the protective effects and mechanisms of MGF on renal inflammatory injury in SHRs.@*METHODS@#MGF was used in SHRs at the doses of 10, 20, 40 mg/kg/d for 8 weeks consecutively. The blood and urine were collected for assessment of renal function. Renal tissues were collected for histological, immunohistochemistry, ELISA, Western blot and real time reverse transcription PCR (RT-PCR) analysis.@*RESULTS@#The results showed that the levels of interleukin 6 (IL-6), tumor necrosis factor-α (TNF-α), monocyte chemoattractant protein-1 (MCP-1) and recombinant chemokine C-C-Motif receptor 2 (CCR2) were increased in SHRs, meanwhile, the level of IL-10 was decreased in SHR. Treatment of MGF inhibited the expression of IL-6, TNF-α, MCP-1 and CCR2, and promoted the expression of IL-10. Furthermore, the content of blood urea nitrogen (BUN) and serum uric acid (SUA) was significantly increased in the model group, and treatment of MGF had no obvious effects on these parameters at all dose levels.@*CONCLUSION@#Our study proved that the kidneys of SHRs had significant inflammatory injury, and MGF had the protective effects on renal inflammatory injury in SHRs; The protective mechanism may be mediated partly by the MCP-1/CCR2 signaling pathway. Thus, it is a potential new drug for the treatment of hypertension.

Objective:To study the correlation of serum monokine induced by IFN-γ(Mig)and interleukin9(IL-9) levels to HBV pre-C/BCP mutation and their influences on the response to interferon therapy in patients with HBeAg-positive chronic hepatitis B(CHB).Methods:A total of 110 cases of HBeAg-positive CHB patients in the People′s Hospital of Jianyang City were retrospectively analyzed from February 2015 to February 2018, and all received pegylated-IFN-αtherapy.Before treatment, the gene fragment of HBV pre-C/BCP region was amplified and the patients were divided into two groups: wild type group (61 cases) and mutant group (49 cases). The difference of serum MIG, IL-9 and antiviral treatment effect between the two groups were compared.Results:The serum IL-9 of mutant group before treatment ((10.8±1.9) ng/L)was significantly higher than that of wild group((8.3±1.7) ng/L) ( P<0.05). The complete response rate of mutation group was 46.9% (23/49) after 48 weeks of treatment, which was significantly higher than that of wild group(16.4% (10/61)) (χ 2=7.083, P=0.018). After 24 weeks of treatment, the serum Mig of patients with HBsAg decrease>1 lg 10 in mutation group before treatment was significantly higher than HBsAg decrease≤1 lg 10 ((129.2±37.9) ng/L vs.(91.5±25.7) ng/L)( P<0.05). The serum IL-9 in the mutant group were(8.8±1.9) ng/L and (8.9±1.6) ng/L at 12 and 24 weeks after treatment respectively, which were significantly lower than that before treatment (all P<0.05). Conclusion:The high expression of IL-9 in serum of CHB and HBeAg positive patients before treatment is closely related to the variation of HBV pre-C/BCP region, and the expression of IL-9 is not related to the antiviral effect of interferon.The high expression of MIG in patients before treatment may be more beneficial to the antiviral effect of interferon in patients with HBV pre-C/BCP region variation.

RESUMO

To search for potent drugs against non-small-cell lung cancer(NSCLC),a series of hybrids(9a-9e, 10a-10e and 11a-11e﹚ from anilinopyrimidines and diazeniumdiolates were designed and synthesized.The MTT assay was employed to evaluate their antiproliferative activity against H1975 cells harboring epithelial growth factor receptor(EGFR)L858R/T790M mutation.The results showed that compounds 9a-9e displayed remarkable inhibitory activity on H1975 cells.Among these compounds, the most potent was compound 9b(IC50=0.65 μmol/L),which was superior to the positive control gefitinib.Additionally,molecular docking study indicated that 9b could bind with EGFR T790M by forming hydrogen bond, electrostatic interactions, et al, suggesting that compound 9b may be a potential anti-NSCLC agent for further investigation.

Objective To understand the situation of college students' awareness about innovative credit, analyze the influencing factors, in order to provide a scientific basis for constructing innovation credits and promoting the double strategy. Method The undergraduates of a medical university who were still at school in April 2015 were selected, and by using a stratified cluster sampling method, 450 students were chosen to be conducted by self-administered questionnaire survey to understand the cognitive status of in-novation of credit system in college students. Chi square test was used to compare the passing rates of students in different demographic characteristics, and the relationship between scores was analyzed by linear regression analysis. Result A total of 439 valid questionnaires were collected , and the total scores of innovative knowledge, attitudes and behaviors of 439 undergraduates were 15.95% (70/439), 36.44% (160/439) and 32.80%(144/439) respectively. In the part of knowledge innovation credits, girls' passing rate was higher than boys ( χ2=4.010,P=0.045. In the part of attitude, high grade group students had more positive attitude than the low grade group of students ( χ2=6.227,P=0.0013). In the part of behavior; higher grade students with innovative credits had higher pass rates than those in lower grades (chi, 2=7.781, P=0.005), and boys had a higher rate of passing than girls ( χ2=6.658,P=0.010). The total score of knowledge was positively linear with the total score of attitude and behavior. Conclusion College Students' awareness of innovation credit rate is low, but the innovative attitude and behavior is positive. The higher the awareness rate is conducive to the cultivation of attitude and behavior, so it is necessary in medical colleges to carry out innovation of credit and promote the cultivation of innovative consciousness.

Objective To establish an HPLC method to determine the related substances of metolazone and valsartan in com?pound metolazone tablets. Methods An Agilent Eclipse SB-C18 column (4.6 mm × 250 mm,5 μm) was used with 0.01 mol/L KH2PO4 buffer(pH=3.5)-acetonitrile as the mobile phase with gradient elution at a flow rate of 1.0 ml/min. The column temperature was 30℃ and the detection wavelength was 237 nm. Injection volume was 20 μl. Results Metolazone,valsartan and related sub?stance B of valsartan were separated completely. The calibration curves were linear within the range of 3-30μg/ml for metolazone, 0.1-2.0μg/ml for valsartan and 0.08-2.0μg/ml for related substane B of valsartan. The average recoveries were 102.97%,100.81%and 100.44%,respectively. The repeatability and intermediate precision met with requirements. The test solution was stable within 24 h. Conclusion The method is specific,sensitive,accurate and reliable,thereby can be used for the determination of metolazone and valsartan related substances in compound metolazone tablets.

Objective To observe the therapeutic effect of intravitreal lucentis injections combined with laser photocoagulation for Coats disease.Methods This was a retrospective case series study.From August 2012 to December 2015,18 cases (18 eyes) were diagnosed as Coats disease with macular edema by FFA and OCT.All patients were treated at baseline using intravitreal lucentis injections.One week later,photocoagulation was performed by the guiding of FFA.Follow-up treatment was performed as necessary.The primary BCVA and central macular thickness (CMT) was measured postoperatively.The follow-up time was 16-24 months (mean 18.8 months).Results The mean number of lucentis injections was 2.6,and the mean number of laser treatment sessions was 1.8.The mean preoperative BCVA was (0.81 ±0.28) LogMAR,while the final visit was (0.76 ± 0.37) LogMAR (P =0.396).In 3 patients (16.7 %),BCVA had improved by more than 3 lines,and 13 patients (72.2%)showed stable BCVA (changes within 2 lines of visual acuity),while in 2 patients,BCVA decreased by more than 3 lines (11.1%).The mean CMT improved significantly from (341.11 ±67.97) μm preoperatively to (277.83 ±51.59) μm at the fmal visit (P =0.030).Final BCVA was significantly correlated with preoperative BCVA (r =0.817,P <0.001).The final BCVA of patients with subfoveal hard exudates preoperatively was significantly worse than that of patients without such exudates (P =0.044).Conclusion Intravitreal lucentis injection combined with laser photocoagulation may be an effective treatment for Coats disease.It may stablize the visual acuity,decrease the CMT of Coats disease.The final BCVA is associated with preoperative BCVA and occurrence of subfoveal hard exudates.

AIM To observe the protective effects of mangiferin on the inflammatory injury and expression of the inflammatory factor in the cerebral tissue of spontaneously hypertensive rats and on MCP-1/CCR2 signal pathway.METHODS Forty spontaneously hypertensive rats were randomly divided into model,benazepril [10 mg/(kg · d)] and mangiferin high,middle and low dose [40,20,10 mg/(kg · d)] groups and other eight rats of same week age served as control group.After consecutive intragastric administration for eight weeks,morphology of the rats' cerebral tissue was observed;their levels of ICAM-1,IL-6 and TNF-α in cerebral tissue were determined by ELISA;their expressions of MCP-1 and CCR2 protein in brain tissue of rats were detected by immunohistochemistry and Western blot and the detection of mRNA expressions of MCP-1 and CCR2 in cerebral tissue of rats were carried out by RT-PCR.RESULTS Compared with the model group,the blood pressure of mangiferin in each dosage group decreased slightly,but there was no significant statistical difference.In the control group and the model group,there was no obvious morphological change in the cerebral tissue.The morphology of rats in the benazepril group,each dose of mangiferin group were all normal.The contents of IL-6,TNF-α,ICAM-1 and MCP1,CCR2 protein and mRNA expression were significantly decreased in the cerebral tissues of spontaneously hypertensive rats.CONCLUSION Mangiferin has obvious anti-inflammatory effects on inflammatory reaction in spontaneously hypertensive rats,its mechanism may be related to inhibiting the expression of MCP/CCR2 signaling pathway.

Objective To establish an in vitro accelerated release method of triptorelin acetate microspheres with good in vi-tro/in vivo correlation(IVIVC). Methods The in vivo release of triptorelin acetate from microspheres was obtained by residual method in rats. Influences of pH value,concentration of ethanol,temperature,rotation speed and concentration of antiseptic on the in vitro accel-erated release were studied,then the correlation between in vitro accelerated release and in vivo release of the microspheres was estab-lished by adjusting the release conditions. Results The in vitro accelerated release medium of triptorelin acetate microspheres composed of 15%ethanol solution(containing 0.06%Tween 80 and 0.1%benzalkonium chloride)at 55℃with rotating rate of 200 r/min. The cumulative release of triptorelin acetate from microspheres was 87.35%at 30 h under accelerated release condition,equivalent to in vivo release for 30 days. The established in vitro accelerated release had a good correlation with that of in vivo(y=0.8845x+12.4510, R2=0.9938). Conclusion The in vitro accelerated release of triptorelin acetate microspheres could correlate well with in vivo release and has a potential application in rapid and effective evaluation of triptorelin acetate microspheres.

Objective To explore the association between adiponectin rs2241766、rs1501299 polymorphisms and gene-environment interaction and the risk of colorectal cancer (CRC).Methods Four hundred CRC patients confirmed by histopathology and 400 healthy controls were recruited in this study.Cases and controls were matched on age and gender.A well-designed questionnaire was used to collect the information of demography,lifestyle and dietary habit on the 400 case-control pairs.Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was applied to detect the adiponectin rs2241766 and rs1501299 genotypes.Results Data from conditional logistic regression analysis showed that those carrying TG + GG genotype on rs2241766 having an increased risk compared to those that carrying TT genotype (OR=1.354,95%CI:1.004-1.827),and those that carrying GT + TT genotype on rs 1501299 having an decreased risk when compared to those that carrying GG genotype (OR=0.680,95% CI:0.501-0.923),after adjusted by factors as CRC family history,BMI,sedentary time,red meat consumption,and tea-drinking habit.Data from generalized multifactor dimensionality reduction showed that the gene-environment interaction among rs2241766,rs1501299 and red meat consumption on the risk of CRC might be significant (P=0.001).A significant dosage effect with an increasing number of risk genotypes was observed as the risk of CRC increased (x2=8.458,P=0.004).Conclusion Both adiponectin rs2241766 and rs1501299 were associated with CRC risk and the two SNPs might have worked together with red meat consumption in affecting the CRC risk.

Objective To study the effects of enriched rehabilitative training on the expression of microtu-bule associated protein 2 (MAP-2) and synaptophysin (SYN), and to explore its relationship with brain plasticity. Methods Seventy-seven male Wistar rats weighing 160 to 200 g were randomly divided into an ischemia + enriched rehabilitation group (IE, n=36), an ischemia + standard rehabilitation group(IS, n=8), a sham ischemia + en-riched rehabilitation group (SE, n=21) and a sham ischemia + standard rehabilitation group (SS, n=12). Rats in the ischemia groups had their middle cerebral artery sutured for two hours before reperfusion, while those in the sham groups had a similar operation without occlusion. The enriched groups were given enriched rehabilitative train-ing, while the standard groups were left without any training. Behavioral tests, including the acrobatic performance, were administered once daily 2 days after operation, and SP staining of MAP2 and SYN were used to observe the func-tional recovery and brain plasticity changes among the groups at 1,7, 14, 21 and 28 days after the operations. Re-sults Acrobatic performance times reduced gradually. Bederson scores were significantly better in the IE than in the IS group by the 28th day after the operation). There was no significant difference between IE and IS groups in a foot fault test). The expression of MAP-2 and SYN around the infarct and in the hippocampus decreased significantly at first), then recovered gradually. The expression of MAP-2 and SYN in the IE group was significantly higher than that in the IS group at various time points of observation). Conclusion Enriched rehabilitative training can improve functional recovery and the expression of MAP-2 and SYN after brain ischemia, and the functional enhancement may attribute to the brain plasticity.

Objective: To observe the effects of high-dose aprotinin and low-dose aprotinin on the inflammatory response induced by cardiopulmonary bypass (CPB). Methods: Thirty-two patients who underwent heart valve replacement were randomized in double-blind fashion into three groups: control group (n=6), low-dose group (n=13) and high-dose group (n=13). Blood samples were taken from radial artery at three times intervals: before CPB, at the end of CPB and 2 hours after termination of CPB. Neutrophil CD11b integrin expression, plasma level of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were measured. Results: The high-dose group demonstrated no significant change in neutrophil CD11b expression and in plasma level of TNF-α, but a significant decrease in plasma level of IL-6. However, the low-dose group only demonstrated a lower CD11b expression and a lower TNF-α level at 2 h after CPB termination. Conclusion: Aprotinin has a dose-reponse effect. High-dose aprotinin is more effective in the reduction of inflammatory response induced by cardiopulmonary bypass.

To evaluate the feasibility of delivering NO through N_2O path in anesthesia machine,NO at 800 ppm was delivered through nitrous oxide path in anesthesia machine SIEMENS Ventilator 710 or NARKOMED 2B,to ventilate a test lung. The minute volume of ventilation (MV) was set from 2 to 6L/min, and NO concentration from 10 to 80 ppm. NO and NO_2 concentrations were measured by hemiluminescence and electro-chemical fuel cell technique. SIEMENS Ventilator 710:Non-rebreathing model,its inspiratory limb was prolonged to 7 meters,the NO level was adjusted at 80ppm before passing through soda lime, NO and NO_2 samples were taken before absorber,after absorber, 1, 2,4 and 6 meter site in inspiratory limb. NARKOMED 2B:Oxygen flows were at the rates epual to the MV mentioned above, NO concentration was at 10,20,40,60 and 80 ppm,the inspiratory gas mixture was sampled immediately passing through absorber, NO_2 levels were less than 5.0ppm in all conditions listed above. With SIEMENS Ventilator 710, the highest level of NO_2 was 3.45 ppm before absorber and 2.43 ppm after absorber,with NARKOMED 2B,that was 3.6 ppm. RESUMO

This study was designed to establish methods to measure partition coefficients of inhaled anesthetics at 1 atmosphere and 37 C by using syringe-flask double equilibration technique. Olive oil/gas (O/G), saline/gas (S/G), and water/gas (W/G) partition coefficients of isoflurane were 90.20?2.01, 0.6256?0.109, and 0. 741?0.025, respectively. Even though O/G was 150 folds of S/G,coefficients of variation for both O/G and S/G were less than 3%. The results indicate that the established double-equilibration technique has a very high repeatability and is applicable to wide ranges of inhaled anesthetics' solubilities.

This study was undertaken to compare the effects of acute hemodilution (AH) induced separately by three solutions on blood properties and hemodynamics. Sixty-six patients, ASA grade Ⅰ to Ⅱ, aged 18 to 54 years, having open heart surgery with fentanyl-pancuronium anesthesia, were acted as subjects. During the induction of AH, blood 10-12ml?kg~(-1) was drawn from internal jugular vein in each subject, as Ringer's lactated injection (RL) at equivalent volume was infused intravenously, afterwards, all subjects were randomly allocated to being intravenously infused with RL at a dose of two times as many as the blood volume drawn (BVD) (group RL), Haemaccel 35 (R35) (group H35) or hydroxyethyl starch (Hes) (group Hes) at a dose equal to BVD, respectively. All patients were observed for 30 mins following the status of AH. The results showed that plasma colloid osmotic pressure was descended markedly in these three groups, but more significantly in group RL immediately after induction of AH, and was kept at low level in group H35 and RL and continued to decrease in group Hes during AH. The hemodynamic values of these three groups were within normal range during whole procedures. Prothrombin time and activated partial thrombin time were prolonged in each group, but within normal range. No anaphylactic reactions occured in all subjects. It is suggested that H35 can be applied more safe to acute moderate hemodilution in clinical anesthesia.

Objective To study the effect of Buyang Huanwu Decoction (BHD) on adhesion molecules in vascular endothelial cells (VECs) of blood stasis rats. Methods Expression of ICAM-1, VCAM-1, PECAM-1, iNOS in vascular endothelial cells of blood stasis rats receiving different doses of BHD was observed by immunohistochemical method and RT-PCR.Results The expression of ICAM-1, VCAM-1, PECAM-1, iNOS was increased in model group, while was decreased by BHD. There was an increase tendency on the expression of molecules and significant dose-effect relationship along with the decrease of the BHD doses. Conclusion The increased expression of adhesion molecules in vascular endothelial cells of blood stasis rats could be reduced by BHD in a dose dependent manner.

AIM: To research the effect of serum from patients with blood stasis syndrome(BSS) associated with hypertension disease on vWF、TM、EPCR excreted by ECV-304 and Tanshinone Ⅱ_A's intervention. METHODS: Cultivated human umbilical vein endothelial cells(CRL-1730) were divided into four groups,according to different serum of patients with BSS associated with hypertension disease,BSS(model) group,non-blood stasis group,health group and control group.The incubation time was 24 h.Tanshinone Ⅱ_A was of different concentrations(40、20、10、5 ?g/mL),all Tanshinone Ⅱ_A groups' cells were incubated by patients' serum and Tanshinone Ⅱ_A for 24 h.vWF,TM and EPCR were assayed by enzyme-linked immunosorbent assay(ELISA). RESUMO

AIM: To study the effects of Shenghua Decoction on the expression of adhesion molecules in vascular endothelial cells (VECs) of blood-stasis rats. METHODS: The expression of VCAM-1, ICAM-1, PECAM-1, iNOS and their corresponding mRNA in VECs were assayed by immunohistochemistry, image analysis and RT-PCR under the function of different Shenghua Decoction doses (high, middle, low). RESULTS: Immunohistochemistry and image analysis indicated: the expression of VCAM-1, ICAM-1, PECAM-1 and iNOS were higher in the model group than in the control group, Shenghua Decoction could decrease the expression of VCAM-1, ICAM-1, PECAM-1 and iNOS in the model group, moreover as the dose decreased, the expression of these molecules increase, it was of dose-effect relation. CONCLUSION: Shenghua Decoction can downregulate the expression of adhesion molecules in vascular endothelial cells of blood-stasis rats, and the dose-effect relation is obvious.

AIM: To study the effects of xijiao dihuang decoction, a Chinese medicine, on the expression of adhesion molecules in vascular endothelial cells (VECs) of adrenine and hypothermia-treated rats. METHODS: The expression of VCAM-1, ICAM-1, PECAM-1, iNOS and their corresponding mRNA in VECs was assayed by immunohistochemistry analysis and RT-PCR, respectively. RESULTS: Immunohistochemistry analysis showed the expression of VCAM-1, ICAM-1, PECAM-1 and iNOS are higher in adrenine and hypothermia-treated rats than that in controls, xijiao dihuang decoction decreased the expression of VCAM-1, ICAM-1, PECAM-1 and iNOS in a dose-dependent manner in adrenine and hypothermia-treated rats. CONCLUSION: Xijiao dihuang decoction can down-regulate the expression of adhesion molecules in vascular endothelial cells of adrenine and hypothermia-treated rats.
