RESUMO

Objective To investigate the effects and mechanism of Interleukin-33 (IL-33) mediated proliferation and differentiation of pulmonary myofibroblasts (MFbs) in pulmonary fibrosis (PF). Methods C57BL/6 mice were randomly divided into four groups: a control group, a bleomycin (BLM) group, a BLM combined with IL-33 group and a BLM combined with anti-IL-33 antibody group, 12 mice in each group. The PF model was induced by intratracheal injection of BLM (5000 U/kg). The degrees of fibrosis were examined using HE and Masson staining. ELISA was used to measure the plasma levels of IL-33. Immunohistochemical staining was used to measure the expression of alpha smooth muscle actin (α-SMA) in lung tissue. Primary pulmonary fibroblasts were isolated and cultured from lung tissues of mice. The cells were divided into four groups: a control group, an IL-33 group, an IL-33 combined with dimethyl sulfoxide (DMSO) group and an IL-33 combined with pyrrolidine dithiocarbamate (PDTC) group. The cells were treated with DMSO or PDTC for 1 hour and then with IL-33 for 48 hours. Cell proliferation was measured by 5-ethynyl-2'-deoxyuridine (EdU) assay and cell cycle was measured by flow cytometry. TranswellTM assay was used to analyze cell migration. Real-time quantitative PCR was used to measure the expression of collagen type I (Col1), Col3 and α-SMA mRNA. The protein levels of IL-33, Col1, Col3, α-SMA, eukaryotic initiation factor 3a (eIF3a), phosphorylated IκBα (p-IκBα) (total lysate), p-NF-κB p65(total lysate) and NF-κB p65 (nucleus) were measured by Western blot analysis. Results In vivo, compared with the control group, the expressions of IL-33, p-IκBα (total lysate), p-NF-κB p65 (total lysate), NF-κB p65(nucleus), eIF3a, α-SMA, Col1 and Col3 in the BLM group significantly increased. Compared with the BLM group, the expressions of p-IκBα (total lysate), p-NF-κB p65 (total lysate), NF-κB p65 (nucleus), eIF3a, α-SMA, Col1 and Col3 in the IL-33 group increased further and the PF was further aggravated. But the effect of anti-IL-33 antibody was just opposite to that of IL-33. In vitro, IL-33 markedly induced the proliferation and migration of pulmonary fibroblasts, and significantly up-regulated the expression of p-IκBα (total lysate), p-NF-κB p65(total lysate), NF-κB p65 (nucleus), eIF3a, α-SMA, Col1 and Col3. But all these effects of IL-33 were reversed by pyrrolidine dithiocarbamate. Conclusion The results suggest that IL-33 may promote the expression of eIF3a by activating NF-κB signaling pathway, thus inducing the proliferation and differentiation of MFbs and promoting the occurrence and development of PF.

RESUMO

Objective:To explore the effect of follow-up care based on the knowledge attitude belief practice theory on postoperative care quality and readmission rate for children with critical congenital heart disease (CCHD).Methods:From June 2018 to October 2020, 76 children with CCHD admitted to this hospital were selected and divided into 38 cases in the control group and 38 cases in the study group according to the random number table method. The control group was given routine follow-up care after discharge, and the study group was given follow-up care based on the knowledge attitude belief practice theory besides the routine follow-up care for the control group after discharge. The readmission rates during the follow-up period were compared between the two groups, and the quality of care and self-efficacy (GSES) of caregivers were compared between the two groups at discharge, 6 months after discharge. Results:During the follow-up period, the readmission rate of the study group was 2.63% (1/38) lower than that of the control group 18.42% (7/38) ( P<0.05); 6 months after discharge, the study group had higher scores of care quality in postoperative feeding, surgical incision care, medication after discharge, prevention of complications, and critical illness recognition than those of the control group [(10.56±1.12) vs (9.78±1.03), (10.92±1.05) vs (9.96±0.98), (10.62±1.32) vs (9.35±1.02), (10.95±1.03) vs (9.97±0.99), (11.05±0.80) vs (9.23±0.75), (10.56±1.35) vs (8.95±0.99)] (all P<0.05); 6 months after discharge, the caregiver′s GSES score of the study group was higher than that of the control group ( P<0.05). Conclusion:Follow-up care based on the knowledge attitude belief practice theory can improve the quality of postoperative care for children with CCHD, reduce the readmission rate, and improve the self-efficacy of caregivers.

RESUMO

BACKGROUND@#Little is known about the effects of environmental cobalt exposure on insulin resistance (IR) in the general adult population. We investigated the association between cobalt concentration and IR.@*METHODS@#A total of 1281 subjects aged more than 20 years with complete blood cobalt data were identified from the National Health and Nutrition Examination Survey (NHANES) 2015-2016 cycle. Blood cobalt levels were analyzed for their association with IR among all populations and subgroups by sex. Regression coefficients and 95% confidence intervals (CIs) of blood cobalt concentrations in association with fasting glucose, insulin and homeostatic model assessment of insulin resistance (HOMA-IR) were estimated using multivariate linear regression after adjusting for age, sex, ethnicity, alcohol consumption, body mass index, education level, and household income. A multivariate generalized linear regression analysis was further carried out to explore the association between cobalt exposure and IR.@*RESULTS@#A negative association between blood cobalt concentration (coefficient = - 0.125, 95% CI - 0.234, - 0.015; P = 0.026) and HOMA-IR in female adults in the age- and sex-adjusted model was observed. However, no associations with HOMA-IR, fasting glucose, or insulin were found in the overall population. In the generalized linear models, participants with the lowest cobalt levels had a 2.74% (95% CI 0.04%, 5.50%) increase in HOMA-IR (P for trend = 0.031) compared with subjects with the highest cobalt levels. Restricted cubic spline regression suggested that a non-linear relationship may exist between blood cobalt and HOMA-IR.@*CONCLUSIONS@#These results provide epidemiological evidence that low levels of blood cobalt are negatively associated with HOMA-IR in female adults.

RESUMO

Objective To analyze the clinical features, diagnosis and treatment of the primary omental torsion (POT).Method The clinical data of 12 patients with POT admitted to our hospital from May 2010 to May 2015 were retrospectively reviewed. The clinical manifestations, laboratory investigation, medical image, treatment and outcomes of patients were analyzed. Results Twelve cases were all males with median age of 50.The POT was diagnosed during surgical exploration and confirmed pathologically after operation.Right abdominal pain was the main complaint in 11 cases, and pain migration to right lower quadrant in 1 case.In physical examination, the local peritonitis signs were elicited, including right quadrant tenderness and rebound tenderness at same location. All cases had mal temperature except one presenting low fever of 37.5 ℃.White blood cell counts was 12×109/L in one case, and(4-10)109/L in other 11 cases.Abdominal solid mass was found in one case by ultrasonic scan, measured 7 cm×5 cm with unclear surrounding boundary.Mesenteric fat opacity and dropsy were shown in 10 cases on CT plain scan, while thickness and effusion of hepatic flexure of colon was found in one case;however, none of them were suggested as POT by these two image study before operation. All patients were gnosed as localized peritonitis or appendicitis clinically before emergent surgical exploration. The necrotic omentum was resected and all patients recovered smoothly. There were no complications in all cases during one-year follow-up.The CT images were reviewed after operation, which indicated that the mesentery of dropsy and the thickened hepatic flexure of colon should be the imaging signs of omental torsion;and also 9 of 12 cases had whirlpools sign. Conclusion The preoperative diagnosis of POT is difficulty, the typical whirlpools sign of abdominal mass on CT imaging is of highly value in diagnosis. The surgical removal of omental infarction is effective and the prognosis is good.

RESUMO

Objective To explore the safety, feasibility, and techniques of hand-assisted laparoscopic splenectomy (HSLS) for massive splenomegaly. Methods Between January 2005 and December 2006, 40 patients with massive splenomegaly owning to portal hypertension were treated with HSLS or open splenectomy (OS). The two groups were comparable in age, sex, hepatic function by Child classification, and size of the spleen. Results No serious complications occurred in both the groups. Compared with the OS group, patients in the HALS group had more blood loss [ (312?61) ml vs (235?105) ml, t=2.583, P=0.014], longer operation time [(95?20) min vs (73?16) min, t=3.832, P=0.000], earlier recovery of intestinal function [(48?1) h vs (98?1) h, t=-153.093, P=0.000], and shorter postoperative hospital stay [(6?2) d vs (10?2) d, t=-6.124, P=0.000]. Conclusions HSLS is feasible and safe for patients with massive splenomegaly. Despite a longer operation time, the procedure is superior in postoperative recovery.
