RESUMO

Objective To understand the application value of spacial clusters detection of human schistosomiasis epidemic,based on small scale level in heavy mountainous and hilly endemic areas, Songzi County,Hubei Province,China. Methods Positive results of human serological detection antibody titer equal or above 80,and positive schistosomiasis cases of fecal examination from 2016 to 2018 as research object in Songzi County.The flexible irregular space scan statistics was used to analyse the spatial clustering analysis of human schistosomiasis epidemic in the heavy mountainous and hilly endemic areas, setting parameter K=2 ,K=6 or K=10, respectively,based on small scale of village level in Songzi County. Results There was none positive schistosomiasis cases of fecal examination in Songzi County from 2016 to 2018.The number of human serological detection antibody titer equal or above 80 were  74, 206, 83, from 2016 to 2018,respectively.There was spatial clusters of positive of human serological detection antibody titer equal or above 80 for schistosomiasis cases in the county from 2016-2018. Areas of flexible irregular space scan statistic in cluster detection changing with the change of different K values. Under different parameters of flexible irregular space scan statistic results show that the most likely cluster of 40 endemic villages in three towns,inculding Laocheng town,Chendian town and Wangjiaqiao town were the prominent. Conclusion There are spatial clusters of human schistosomiasis based on small scale of village level by flexible irregular space scan statistic in mountainous and hilly endemic areas, Songzi County,Hubei Province.Therefore,the monitoring and control of schistosomiasis should be consolidated in the future,in order to achieve schistosomiasis elimination in Hubei Province at an early date.

Objective To analyze the spatial clustering of human schistosomiasis at the village level in key counties in Hubei Province, to provide scientific evidence for formulating strategies for human schistosomiasis prevention and control in the next stage. Methods Gong'an County and Jiangling County in Hubei Province were selected as representative counties for this study. A town or village was set up as a research unit. Schistosomiasis cases of a positive fecal examination in 2015 and cases with positive detection for schistosomiasis serological antibody titer equal or above 80 in 2016-2018 were selected as research subjects in these two counties. The Kulldorff circular scan statistic was used for the spatial clustering analysis of human schistosomiasis infection status in the population. Results There was spatial clustering of positive schistosomiasis cases of fecal examination, at the level of a town or village in both counties in 2015. There was spatial clustering of positive human serological antibody detection at the level of town or village from 2016-2018. Eighty-six endemic villages in five towns in the northeast of Gong'an County, along the Yangtze river,including Mahaokou Town, Zhakou Town, Yangjiachang Town, Jiazhuyuan Town and Douhuti Town, were the most prominent. There was no spatial clustering of positive results of human serological antibody detection at the town and village level in Jiangling County, in2017, while there was spatial clustering of human serological antibody detection in 2016 and 2018,respectively. Fifty-seven endemic villages in two towns (Puji Town and Xionghe Town) in the southwest of Jiangling County, along the Yangtze river,were the most prominent. Conclusion There were spatial clustering of human schistosomiasis epidemic at village level both in Gong'an County and Jiangling County, Hubei Province. Compared with the previous studies, there was a trend of shrink and decline of clustering areas. Therefore, the current situation of the epidemic has put forward higher requirements for the implementation of precise prevention and control in the progress of schistosomiasis elimination work in various epidemic areas.

Objective To systematically evaluate the completion and implementation effect of the medium-long-term program for schistosomiasis control in Tianmen City, and to provide references for formulating the next-stage control plan. Methods According to "National Medium-long-term Plan for Prevention and Control of Schistosomiasis (2004-2015) Final Evaluation Scheme", data on population, livestock, and infection status of snails in Tianmen City from 2004-2019 was retrospectively collected database was established with Excel. SPSS 19.0 software was used for statistical analysis to evaluate the achievement of goals, and task completion and implementation effects. SaTScan was used to detect the aggregation area of schistosomiasis cases. Results From 2004 to 2019, Tianmen City completed the planned tasks. All 23 villages and towns in the epidemic area reached the transmission interruption standard. In 2004, the infection rate of schistosomiasis among the residents was lower than 0.01%, then decreased to 0 in 2015, with no acute cases occurring for 11 consecutive years. In 2004, the infection rate of farm cattle in the city was 3.5%, and it then decreased to 0 in 2011. The area of snails increased from 2.4735 million square meters in 2004 to 5.132 million square meters in 2010, and no infectious snails was detected after 2009. The awareness rate of schistosomiasis knowledge and correct behavior formation rate of primary and secondary school students and housewives in the city exceeded 95%. Through spatial distribution analysis, schistosomiasis cases in Tianmen City showed a certain aggregation. Conclusion The medium-long-term program for the prevention and control of schistosomiasis in Tianmen City from 2004 to 2019 achieved the expected results. The overall epidemic situation declined year by year, but schistosomiasis control still faces many challenges.

Objective To improve the information level of schistosomiasis control by using the related functions of Corel-DRAW software. Methods Combining with the requirement of schistosomiasis control,the sketch map was drawn according to the linear element drawing,the geometric drawing,the color rendering and the text adding. Results The schistosomiasis epi-demic sketch map and the Oncomelania hupensis snail distribution sketch map at all levels were produced in CorelDRAW soft-ware. Conclusion The sketch map drawing in CorelDRAW software is beautiful and standardizing,and it can improve the lev-el of information management.

Objective To investigate the water infectivity in key water regions,and analyze the transmission risk of schisto-somiasis in suspicious and high-risk environments in Hubei Province in 2016. Methods Schistosome-endemic areas of the Yangtze River,the Hanbei River and the Fu River were chosen as the surveillance and forecast sites. The water infectivity was detected by using the sentinel mice during the flood season. The infection status of residents in the villages around the surveil-lance sites and the activities of human beings and domestic animals were surveyed. The emergency response system was initiated when the water infectivity areas were detected. Results From May to June,among the ten surveillance sites of the first batch, two positive spots with infected mice were found,accounting for 20%. All the recovered mice(totally 200)were dissected:five mice were infected,with a total infection rate of 2.5%. Totally five schistosome adult worms were collected,with mean worm bur-den of 1 worm per infected mouse. The site with cercariae-infected water body started the emergency response and no epidemic occurred. From August to September,among the eight surveillance sites of the second batch,the recovered mice(totally 160) were dissected,and no infected sentinel mice were found. Conclusion The sentinel mice method plays an important role in an-alyzing the schistosomiasis transmission risk in Hubei Province.

Objective To analyze the Oncomelania hupensis snail situation in schistosomiasis surveillance sites in Hubei Province from 2009 to 2014. Methods From 2009 to 2014,207 schistosomiasis endemic villages from 13 cities were selected as the surveillance sites in Hubei Province,where the surveillance of snail situation was performed,and the data about the in?dexes of snail situation during the 6 years were collected and analyzed statistically. Results From 2009 to 2014,totally 80 963.47 hm2 of snail areas were investigated,and 45 309.77 hm2 of area with snails as well as 364.93 hm2 of area with infected snails were found out,and 1 646 125 snails were captured. In the 207 surveillance sites,the density of living snails was reduced from 0.493 3 snail/0.1 m2 in 2009 to 0.339 3 snail/0.1 m2 in 2014,with a decline rate of 31.22%;the density of infected snails was reduced from 0.000 7 snail/0.1 m2 in 2009 to 0 in 2014;the infection rate of snails was reduced from 0.14%in 2009 to 0 in 2014. The infection rates of snails in the third?rank villages,endemic areas in inner embankment,ditch environment and weed environment were relatively higher. Conclusions The snail situation of Hubei Province has been declined steadily. However, the environment of snail breeding has not been changed completely. In order to consolidate the control achievements further ,the measures including mollusciciding and snail control projects should be strengthened.

Objective To evaluate the effect of wild feces detection in Oncomelania hupensis environments on the surveil?lance of infection source of schistosomiasis,and find the weakness in schistosomiasis control in Hubei Province,so as to put for?ward the targeted strategies and measures. Methods Four environments with O. hupensis snails in endemic areas of Hubei Provinces,where human and livestock often haunted,were selected according to the river systems,namely the Juzhanghe River beach in Jingzhou City,Changjiang River beach in Jiayu County,Hanbei River beach in Tianmen City,and Changshou River beach in Zhongxiang City,then the snail survey and wild feces detection were implemented in the selected environments. Re?sults There were O. hupensis snails,livestock,wild feces of cattle or sheep as well as positive cattle feces found in all the 4 environments,and the positive rate of schistosome miracidium incubation was 47.62%. Conclusion The schistosome miracidi?um positive rate of wild feces of cattle is high in the environments with snails in endemic areas of Hubei Province ,which has high risk for schistosomiasis transmission.

Objective To grasp the distribution and epidemiology of confirmed cases of schistosomiasis in Hubei Province, so as to provide the evidence for promoting the prevention and control work. Methods The confirmed cases of schistosomiasis in Hubei Province from 2010 to 2014 were epidemiologically investigated,and the prevalence characteristics and main influenc?ing factors were analyzed. Results A total of 10 102 confirmed cases from 2010 to 2014 were surveyed. There were 1 062 local infected patients,accounting for 10.51%and including 354 repeated infections and 17 newly infected. There were 290 foreign?infected patients,accounting for 2.87%,with 206 repeated infection cases and 84 newly infected. There were 8 750 historical patients,including 2 229 patients who leaked the former schistosomiasis investigations,accounting for 22.06%;570 patients missed treatment,accounting for 5.64%;3 640 patients were treated with non?standard therapy,accounting for 36.03%;2 311 patients were treated with poor medication efficacy,accounting for 22.88%. The multivariate non?conditional Logistic regres?sion,targeting at confirmed cases in 2014,showed that,for the leaking investigations,the potential risk factors included the age,educational level,and latrine renovation(b>0,OR>1),the protective factors were the times of previous treatment,cat?tle feeding in villager team,and Oncomelania hupensis snails in surroundings(b0,OR>1),and the educational level and snails in the own field were protective factors(b<0,OR<1). Conclusion The epidemiological investigation on the confirmed cases of schistosomiasis could grasp the epidemic factors so as to improve the management and carry out the scientific control.

Objective To analyze the surveillance results of schistosomiasis and understand the transmission situation in Hu?bei Province in 2014. Methods According to the Scheme of Schistosomiasis Surveillance in Hubei Province(the 2009 edi?tion),207 endemic sites were selected,where the schistosome infections of residents and livestock,and the distribution of On?comelania hupensis snails were investigated. Results In the 207 sites,there were 27 sites without positive results of the blood tests and 129 sites without positive results of the stool tests. The infection rates of residents and cattle were 0.17%and 0,respec?tively. The density of living snails was 0.339 3 snails/0.1m2,the rate of living snails was 10.12%,and the infection rate of snails was 0. The infection rate of residents in the inner embankment type was 0.22%,higher than the rates in the other types. In differ?ent age groups,the infection rate in the 50-59 years age group was higher than the rates in other age groups. The infection rates of residents in farmer and fisherman were higher than those in the other occupation groups. The infection rate of floating people was 0,and no acute schistosomiasis patients were found. Conclusion The endemic situation of schistosomiasis has decreased obviously in Hubei Province,and the whole province has achieved the criteria for transmission control of schistosomiasis.

Objective To analyze the epidemic situation and spatial distribution of schistosomiasis in Hubei province.Methods Data on schistosomiasis endemic in humans were retrospectively collected at the county level from 2008 to 2012.Data on endemic schistosomiasis were matched to Geographic Information System (GIS) and geospatial databases to constitute the spatial database for analysis on schistosomiasis.Spatial aggregation of Schistosoma (S.)japonicum infection was analyzed by global spatial autocorrelation,using the Moran' s I index and local spatial autocorrelation of the Getis-Ord Gi* index.Results Both the number of schistosomiasis patients and the rate of infection on S.japonicum decreased from 2008 to 2012.Results from the global autocorrelation analysis on the prevalence of human S.japonicum infection for 5 years were statistically significant (Moran' s I>0,P<0.01).From local autocorrelation analysis,data showed that the number of highly aggregated areas ranged from eight to eleven within the five-year period.The highly aggregated areas were mainly distributed in eight counties,namely Jingzhou district,Shashi district,Jiangling county,Gongan county,Shishou city,Jianli county,Honghu city,and Chibi city.Conclusion Spatial agglomerations were present in the distribution of human S.japonicum infection from 2008 to 2012.Highly cluster areas were mainly distributed in the Yangtze River Basin of Jianghan Plain area.

Objective To study the endemic trend of schistosomiasis japonica in Hubei Province,so as to provide the theo?retical basis for surveillance and forecasting of schistosomiasis. Methods The time?series auto regression integrated moving av?erage(ARIMA)model was applied to fit the infection rate of residents of Hubei Province from 1987 to 2013,and to predict the shot?term trend of infection rate. Results The actual values of infection rate of residents were all in the 95%confidence inter?nals of value predicted by the ARIMA model. The prediction showed that the infection rate of residents of Hubei Province would continue to decrease slowly. Conclusion The time?series ARIMA model has good prediction accuracy,and could be used for the short?term forecasting of schistosomiasis.

Objective To evaluate the effect of comprehensive schistosomiasis control measures based on infection source control in Hanchuan City marshland and lake endemic regions . Methods The data of comprehensive schistosomiasis control in Hanchuan City from 2004 to 2013 were collected and the change trends of the Oncomelania hupensis snail status and the in?fection situation of human and livestock were analyzed to evaluate the control effect. Results After the implementation of the comprehensive measures the infection rates of residents and cattle decreased from 6.38%and 8.11%in 2004 to 0.16%and 0 in 2013 respectively. There were no acute schistosomiasis patients since 2007 no new infection cases since 2011 and no infected snails since 2012. Compared with 2004 the occurrence rate of frames with living snails and the average density of living snails decreased by 56.78%and 68.35% respectively but the snail area and susceptible area increased by 0.62%and 7.10% respec?tively. In 2013 all the 367 endemic villages in 26 townships reached the criteria of transmission controlled. Conclusions The comprehensive schistosomiasis control measures based on infection source control can control the schistosomiasis transmission effectively in marshland and lake endemic regions. When consolidating the achievement the snail area in inner embankment should be compressed to prevent the schistosome re?infection in human and livestock.

Objective To understand the dynamic schistosomiasis situation in Hubei Province so as to provide the evidence for formulating the effective control strategy. Methods According to the Scheme of Schistosomiasis Surveillance in Hubei Province the 2009 edition 207 endemic villages were selected and investigated for the schistosome infections of residents and livestock and the distribution of Oncomelania hupensis snails. Results The average infection rates of residents and cattle were 0.35% and 0.15% respectively and the endemic situation of fishermen and farmers were relatively serious 0.44% and 0.42% respectively . The density of living snails was 0.30 snail/0.1m2 but no infected snails were found. Conclusion The en-demic situation of schistosomiasis in Hubei Province was stable in 2013 but the efforts for the infectious source control still should be strengthened.

Objective To assess the schistosomiasis endemic situation in the national surveillance sites in Hubei Province, so as to provide the evidence for the prevention and control of schistosomiasis. Methods According to the national surveillance protocol,a longitudinal surveillance of endemic situation of schistosomiasis was carried out in 16 national surveillance sites from 2005 to 2010. Results In general,the positive rates of IHA,Kato-Katz technique and infection rates of Schistosoma japonicum decreased from 15.67%,10.93%and 1.71%in 2005 to 10.48%,8.54%and 0.90%in 2010,and declined by 33.12%,22.70%and 47.95%,respectively. The infection rates of S. japonicum of the male were higher than that of the female,and the peak infec-tion rates were in the groups aged above 30 years. The endemic situation of fishermen and farmers were relatively serious. The in-fection rates of S. japonicum in cattle decreased from 11.69%in 2005 to 1.41%in 2010,and declined by 88.01%(χ2 =298.79, P<0.001). The areas with infected Oncomelania hupensis snails,the densities of living snails,the rates of infected snails and the densities of infected snails decreased by 90.88%,61.66%,80.00%and 92.00%,respectively. Conclusion The schistosomiasis endemic situation in the national surveillance sites in Hubei Province mitigates in 6 years,but the prevention is still a very daunt-ing task.

Objective To understand the change of hydrological regime in the Yangtze River and schistosomiasis endemic situation after the implementation of Three Gorges Project in Hubei Province. Methods The data of hydrological regime of the Yangtze River,schistosomiasis epidemic situation,and Oncomelania hupensis snail status were collected and analyzed in Hu-bei Province from 2002 to 2012. Results After the implementation of the Three Gorges Project in 2003,the water level of the Yangtze River slightly rose from January to March,the average water level in May and August both reduced compared with that in 2002,and the time of water withdrawal was moved up. The endemic situation of schistosomiasis showed a decline tendency in areas along the Yangtze River in Hubei Province. The positive rate of serological tests in the residents and the Schistosoma ja-ponicum infection rate in bovine decreased by 2.97%and 92.64%respectively. The numbers of human and bovine infected with S. japonicum decreased by 48.35%and 94.48%,respectively. Conclusion After the implementation of the Three Gorges Proj-ect,the schistosomiasis endemic situation shows a decline tendency in the areas along the Yangtze River in Hubei Province ,but the long-term monitoring still need to be carried out.

RESUMO

Objective: To find out the pathomechanism of low back and leg pain related to intervertebral disc. Methods: The nucleus pulposus of coccygeal vertebral was transplanted to the cavum epidurale of rats to establish the non-compressive model with transplanted nucleus pulposus. The evoke potentials and morphology of nerve roots were observed. Results: Even without mechanical compression, rats transplanted with nucleus pulposus resulted in significant harm to evoked potential and morphology of cauda equina. Conclusion: The biomechanical and/or immunologic inflammatory effect of nucleus pulposus can result in nerve roots injury and is an important factor in the pathogenesis of low back and leg pain.
