RESUMO

ObjectiveTo develop and validate a predictive model to individually predict the risk of patients with stroke in the eICU Collaborative Research Database for early clinical identification and intervention. MethodIndividual patient data (200 859 cases) from a national multicenter cohort study (eICU database) were selected, and the patients with stroke in neurological diseases (9 037 cases) were selected for statistical analysis. The main outcome was hospital mortality. The Glasgow Coma scale (GCS) was used to divide all patients with stroke into stroke in meridian and stroke in viscera (GCS≤14 for stroke in viscera and GCS=15 for stroke in meridian). The patients were then divided into a training set and a test set according to 7∶3, respectively, to evaluate the differences in hospital mortality between the two groups. The multivariate logistic regression was used to analyze the related factors affecting the prognosis of the two groups, and a predictive model was established. Receiver operator characteristic (ROC) curves were used to assess the discrimination of the predictive model. ResultThe predictive model based on 9 037 patients with stroke was established. The predictors of the stroke in meridian (4 475 cases) included pulmonary infection, mechanical ventilation, acute physiology, and chronic health status scoring system Ⅳ (APACHE Ⅳ) score. The predictors of the stroke in viscera (4 562 cases) included anticoagulation therapy (AT), mechanical ventilation, acute physiology, and APACHE Ⅳ score. According to the predictors, the predictive models of the stroke in meridian and the stroke in viscera were constructed, respectively. The areas under the curve (AUC) of ROC of the training set and the test set of the predictive models of the stroke in meridian were 0.845 [95% confidence interval (CI) (0.811, 0.879)] and 0.807 [95% CI (0.751, 0.863)], respectively. The areas under the ROC curve of the training set and test set of the predictive models of the stroke in viscera were 0.799 [95% CI (0.781, 0.817)] and 0.805 [95% CI (0.778, 0.832)], respectively. The AUC of the predictive model of the training set and the test set were both above 0.7. ConclusionThe model established in this study can conveniently, directly, and accurately predict the hospital mortality risk of patients with stroke. Physicians and other healthcare professionals can use this predictive approach to provide early care planning and clinical interventions for patients with stroke during their hospital stay.

Objective:To establish the quality standard for Xiaotang Zhike pills.Methods:Atractylodis Rhizoma,Puerariae Lobatae Radix and Scrophulariae Radix in the formula were qualitatively identified by TLC. HPLC was used for the determination of puerarin in Xiaotang Zhike pills. The analysis was performed on a Wondasil C18-WR(250mm × 4.6 mm,5 μm)column with methanol-water(25:75) as the mobile phase at a flow rate of 1.0 ml·min-1. The detection wavelength was set at 250 nm, the column temperature was 40℃ and the sample size was 10 μl. Results:TLC showed good specificity without any interference for the identification of atrctylodin,puerarin and harpagoside. The calibration curve of puerarin was in good linearity over the range of 41.20-618.00 ng(r = 0.999 9). The average recovery was 100.16%(RSD = 2.91%). Conclusion:The established methods in the study are simple,feasible and reproducible,which are suitable for the quality control of Xiaotang Zhike pills.

The processing of traditional Chinese medicine(TCM) is a traditional pharmaceutical technology,which meets the development demand of the treatment based on syndrome differentiation of TCM. It is one of the important means to guarantee the quality of Chinese medicine and improve the clinical efficacy of TCM. Medical institutions of traditional Chinese medicine should take the advantages and characteristics of traditional Chinese medicine as the foundation, vigorously carry out processing technology for traditional Chinese medicine, rich clinical drug varieties of traditional Chinese medicine, and develop such characteristics of TCM as treatment based on syndrome differentiation and individualized drug treatment. The processing of temporary prescriptions is one of the important means to reflect the characteristics of TCM, and it is of great significance to improve the curative effect of TCM. Based on working practice, this paper summarized the development current status, significance, technical standards and samples of processing variety and methods etc in order to provide thoughts for the processing technology of temporary prescriptions in medical institutions.

Objective: To explore the effect of different processing methods on the content changes of ferulic acid and total flavonoids in wheat bran and its processed products by HPLC and UV spectrophotometry.Methods: Ferulic acid was determined by HPLC with SHIMADZU C18 (250×4.6 mm,5 μm) as the analytical column, the mobile phase was composed of acetonitrile-0.1% phosphoric acid solution (16∶84), the flow rate was 1.0 ml·min-1, the detection wavelength was 320 nm and the column temperature was 35℃.Total flavonoids was determined by UV spectrophotometry at the wavelength of 258 nm(±2 nm).Results: The content of ferulic acid in the crude bran, the toasted to brown bran, the toasted to dark brown bran, the over toasted bran and the honey toasted bran was 2.07, 2.33, 1.68, 1.11 and 0.83 μg·g-1, and total flavonoids was 5.96, 7.15, 9.73, 13.61 and 9.42 mg·g-1, respectively.Conclusion: The higher the toasted processing degree is, the lower content of ferulic acid and the higher content of total flavonoids are in wheat bran.

Objective: To establish the quality standard for Ehong capsules.Methods: A TLC was adopted to identify safflower, pueraria and prepared Radix Polygoni Multiflori.The contents of hydroxyl-safflor yellow A and puerarin in Ehong capsules were determined by a dual wavelength HPLC.A Wondasil C 18-WR (250 mm× 4.6 mm ,5 μm) was used as the analytical column, the mobile phase was composed of methanol acetonitrile-0.7% phosphoric acid aqueous solution (16∶3∶81), the flow rate was 1.0 ml·min-1 ,the detection wavelength was 403 nm for hydroxy safflower yellow A and 250 nm for puerarin.The column temperature was 30℃.Results: The TLC spots were clear and well-separated without negative interference.The calibration curves of hydroxyl-safflor yellow A and puerarin were linear over the ranges, the average recovery was 101.02% and 100.03% , and the RSD was 1.43% and 2.40% (n =6), respectively.Conclusion: The method is fast and accurate with good specificity, which can be used for the quality control of Ehong capsules.

Objective:To establish the quality standard for Qingchang mixture.Methods:TLC was adopted to identify angelica sinensis,paeoniae radix and phellodendri cortex.HPLC was used to determine the content of ferulic acid on a Wondasil C18 (250 mm×4.6 mm,5 μm) analytical column,the mobile phase was composed of acetonitrile-0.1% phosphoric acid solution(17∶83),the flow rate was 1.0 ml·min-1, and the detection wavelength was 316 nm and the column temperature was 30℃.Results:The TLC spots were clear and well-separated without any negative interference.The calibration curve of ferulic acid was in good linearity over the range of 39.6-316.8 μg·ml-1(r=0.999 9),the average recovery was 98.75% and RSD was 1.21%(n=9).Conclusion:The method is fast and accurate with good specificity,which can be used for the quality control of Qingchang mixture.

Methods:TLC was adopted to identify salvia miltiorrhi-za, rhubarb charcoal and panax notoginseng; the contents of tanshinoneⅡA , chrysophanol and physcion in Huayu pills were deter-mined by RP-HPLC with Wondasil C18 (250 mm × 4. 6 mm,5μm) as the analytical column, the mobile phase was composed of metha-nol-0. 1% phosphoric acid solution (85 ∶15), the flow rate was 1. 0 ml·min-1,the detection wavelength was 254 nm, the column temperature was 35℃, and injection volume was 10 μl. Results:The TLC spots were clear and well-separated without any negative in-terference; the calibration curve of tanshinoneⅡA , chrysophanol and physcion was linear over the range of 0. 212-2. 12 μg ( r =0. 999 9),0. 159-1. 59 μg(r=0. 999 9) and 0. 072 6-0. 726 μg(r=0. 999 9), the average recovery was 99. 55 %, 99. 56% and 100. 60%, and RSD was 1. 57%,2. 26% and 2. 28%(n=6), respectively. Conclusion: The method is fast and accurate with good specificity, and can be used for the quality control of Huayu pills.

Methods:TLC was adopted to identify Paeoniae Ra-dix Rubra, Cyperi Rhizom and Cinnamomi Cortex; the contents of paeoniflorin and cinnamic aldehyde in Juebi capsules were deter-mined by HPLC with Wondasil C18 (250 mm × 4. 6 mm,5 μm) as the analytical column, the mobile phase was composed of methanol-0. 2% phosphoric acid solution with gradient elution, the flow rate was 1. 0 ml·min-1 ,the detection wavelengths were 230 nm and 290 nm, the column temperature was 40℃, and the volume injection was 10 μl. Results: The TLC spots were clear and well-separated without any negative interference;the calibration curves of paeoniflorin and Cinnamic aldehyde were in good linearity over the range of 0. 368-3. 680μg(r=0. 9999) and 0. 191-1. 914 μg(r=0. 9999), the average recovery was 101. 9 % and 99. 66%, and the RSD was 1. 90% and 2. 77% (n=6), respectively. Conclusion:The method is fast and accurate with good specificity, which can be used for the quality control of Juebi capsules.

Tubulin has been shown to be an effective target for the development of cytotoxic agents against prostate cancer. Previously, we reported that Lx2-32c is an anti-tubulin agent with high binding affinity to tubulin. In this study, we investigated the potential of Lx2-32c to act as an effective cytotoxic agent in the treatment of prostate cancer. MTT assays showed that Lx2-32c was cytotoxic to all tested prostate cancer cell lines. The Lx2-32c-treated cells typically exhibited a rounded morphology associated with the onset of apoptosis, as evidenced by immunocytochemical staining. Human prostate cancer cell lines treated with Lx2-32c arrest in the G2/M phase of the cell cycle and the treatment is associated with an increased ratio of cells in the sub-G0/G1 phase as determined by flow cytometry. Furthermore, expression of the cleaved form of poly (ADP-ribose) polymerase in prostate cancer cell lines treated with Lx2-32c was shown by Western blotting assay. Xenograft implants of LNCaP and PC3-derived tumors in nude mice showed that Lx2-32c treatment significant inhibited tumor growth with effects equivalent to those of docetaxel. These findings demonstrate the potential of Lx2-32c as a candidate antitumor agent for the treatment of prostate cancer.

Objective To investigate the effect of ReCell technique on diabetic foot and explore main nursing measures. Methods Forty patients with diabetic foot were divided into control group(n=20)and experiment group(n=20)according to admission time. With wound bed prepared, the control group received only free skin flap grafting and the observation group was managed with ReCell technique plus free skin flap grafting.The skin flap survival and treatment time in two groups were observed and compared. Result The skin flap survival in the experiment group was better than the control group and the treatment time was shorter. Conclusion ReCell technique is effective in raising the survival rate of skin flap in diabetic foot and shortening treatment time.The improvement of nursing measures is critical for the success of ReCell technique.

ObjectiveTo explore the causes and countermeasures for extravasation of intravenous fluids in children. Methods726 cases of intravenous infusion in pediatric surgery department from May to December 2011 in our hospital were observed. ResultsThere were 30 cases with extravasation of intravenous infusion in the 726 cases,the leakage rate was 4.13%.The specific causes of fluid extravasation included:younger age of children,not cooperating with the infusion in 18 cases,accounting for 60.00%,followed by non standardized fixation methods in 5 cases,accounting for 16.67%,and accompanying family members refused to be disturbed accounted for 13.33%,Nursing visits to owe in a timely manner accounted for 6.67%.The rate of younger age of children and lack of cooperation leading to fluid extravasation were significantly higher than other causes,the difference was statistically significant. ConclusionsChildren age,fixation method,cooperation of family members,inspections and other factors can be controlled.Along with the continuous improvement of medical standards and a variety of active and effective implementation of the measures,these factors can be controlled effectively.Strengthening the training of nurses and vocational skills to enhance their own sense of responsibility,to give standardized infusion operation and fixation,and give predictable observation and treatment,can effectively prevent the occurrence of extravasation of intravenous fluids.

Objective To study the mechanism of Chinese Medicine in treating adenomyosis. Methods Thirty-seven patients in the Chinese Medicine treated group (17 cases of adenomyosis) and the control group (20 cases of adenomyosis, non-drug treated) underwent hysterectomy. Endometrium was looked as eutopic endometrium and adenomyosis as ectopic endometrium. Expression of MMP-9 and TIMP-1 in eutopic and ectopic endometrium were measured by immunohistochemical techniques. Meanwhile the levels of CA125 in serum were measured. Results The ectopic endometrium of the Chinese Medicine treated group expressed lower level of MMP-9, higher level of TIMP-1 and lower ratio of MMP-9/TIMP-1 than the ectopic endometrium of the control group (P<0.05). The levels of CA125 in serum were significantly decreased in Chinese Medicine group after treated as contrast with those in control group. Conclusion Chinese Medicine could reduce the level of CA125 and the ratio of MMP-9 and TIMP-1 to prevent the progression of adenomyosis.

@# ObjectiveTo investigate the effective nursing strategy on various psychological problems of patients with head and face chemical burn.MethodsVarious psychic reactions of 52 patients with head and face chemical burn were analyzed and mental nursing was taken to these patients.ResultsThe depression and anxious scores of patients at time of discharge were significantly declined compared with time of hospitalization(P<0.01).ConclusionThe mental nursing can not only benefit to the improvement of patients' mental status but also make them optimum to accept treatment and nursing.
