【Objective】 To establish a dry fluorescent luminescence method for the detection of antibodies to hepatitis C virus (HCV) and evaluate its clinical application. 【Methods】 Anti-HCV antibody was detected by double-antigen sandwich dry fluorescent luminescence method established using multi-epitope chimeric antigen. The established method was used to detect national reference samples(positive 20, negative 20), and a total of 349 clinical samples, including 108 HCV patients, 36 patients with other diseases and 205 healthy individuals, which were tested in parallel with enzyme-linked immunoassay (ELISA) to evaluate the performance of the established method. 【Results】 The concordance rate of positive and negative(each 20) reference samples were both 100% (20/20), and the CV of precision reference sample was 9.16%, which met the requirements of national reference samples. In clinical performance evaluation, the AUC value was 0.984, and the sensitivity and specificity of the dry fluorescent luminescence method were 96.30% (104/108) and 96.27% (233/241). The overall concordance rate between dry fluorescent luminescence method and ELISA was 97.71% (341/349) (Kappa=0.952). 【Conclusion】 The dry fluorescence luminescence method of HCV antibody is simple and rapid, with high sensitivity and high specificity, and can be used in clinical application.

RESUMO

To establish an intelligent cervical vertebra maturity assessment system, and to evaluate the reliability and clinical value of the system. Sixty children aged were recruited in the study. Lateral cephalometric radiograph and cone beam CT (CBCT) were taken at the same period. Based on the CBCT data, the system automatically extracted the patient's facial area through Otsu's method, intercepted the sagittal plane by three-dimensional least squares method, captured the second to fourth cervical vertebrae by superpixel segmentation. And then selected points were marked automatically through morphological algorithm and manual method. Consistency test was performed on the two sets of data to compare the reliability of automated cervical morphology capture. According to the parameters of morphological identification, positioning and staging algorithms were designed to form the intelligent cervical vertebra maturity assessment system. The cervical vertebra maturity was also judged manually on the lateral cephalometric radiograph. The weighted Kappa test and the Gamma correlation coefficient were subsequently applied to evaluate the consistency and correlation. The results showed that the cervical vertebra features automatically captured based on CBCT data had a high accuracy on the overall morphological recognition. In the prediction of 8 inflection points out of 13 points, there was no significant difference between automatic and manual method on both X and Y axes (all >0.05). The assessment results of the cervical vertebra maturity of the intelligent system had strong consistency and correlation with the manual recognition results (weighted Kappa value=0.877, Gamma value=0.991, both <0.05). The intelligent cervical vertebrae maturity assessment system based on CBCT data established in this study presents reliable outcome and high degree of automation, indicating that the system may be used clinically.

Purpose@#There is no optimal prognostic model for T-cell lymphoblastic lymphoma (T-LBL). Here, we discussed the predictive value of total metabolic tumor volume (TMTV) and total lesion glycolysis (TLG) measured on 18F-fluorodeoxyglucose positron emission tomography–computed tomography (PET-CT) in T-LBL. @*Materials and Methods@#Thirty-seven treatment naïve T-LBL patients with PET-CT scans were enrolled. TMTV was obtained using the 41% maximum standardized uptake value (SUVmax) threshold method, and TLG was measured as metabolic tumor volume multiplied by the mean SUV. Progression-free survival (PFS) and overall survival (OS) were analyzed by Kaplan-Meier curves and compared by the log-rank test. @*Results@#The optimal cutoff values for SUVmax, TMTV, and TLG were 12.7, 302 cm3, and 890, respectively. A high SUVmax, TMTV, and TLG indicated a shorten PFS and OS. On multivariable analysis, TMTV ≥ 302 cm3, and central nervous system (CNS) involvement predicted inferior PFS, while high SUVmax, TLG and CNS involvement were associated with worse OS. Subsequently, we generated a risk model comprising high SUVmax, TMTV or TLG and CNS involvement, which stratified the population into three risk groups, which had significantly different median PFS of not reached, 14 months, and 7 months for low-risk group, mediate-risk group, and high-risk group, respectively (p < 0.001). Median OS were not reached, 27 months, and 13 months, respectively (p < 0.001). @*Conclusion@#Baseline SUVmax, TMTV, and TLG measured on PET-CT are strong predictors of worse outcome in T-LBL. A risk model integrating these three parameters with CNS involvement identifies patients at high risk of disease progression.

Objective:To compare the incidence of complications associated with arm and chest ports by Meta-analysis.Methods:To Janurary 2019, studies published in PubMed, Embase, CINAHL, Cochrane, CBM and Wanfang regarding complication comparison between arm ports and chest ports were searched. Newcastle-Ottawa Scale was applied to evaluate the quality of studies. Review Manager 5.3 software was applied to conduct this meta-analysis.Results:A total of 19 articles covering 4 203 patients were included. The results showed that the incidence of pneumothorax ( OR value was 0.22, 95% CI0.05-0.88, P value was 0.03) and artery injury ( OR value was 0.24, 95% CI0.07-0.77, P value was 0.02) between arm ports group and chest ports group were statistically significant. No significant difference was observed in infection ( OR value was 0.81, 95% CI0.58-1.12, P value was 0.20), thrombosis ( OR value was 1.25, 95% CI0.64-2.45, P value was 0.52), extravasation ( OR value was 1.13, 95% CI0.54-2.35, P value was 0.75), catheter misplacement ( OR value was 1.58, 95% CI0.95-2.61, P value was 0.08), skin incision ( OR value was 0.64, 95% CI0.23-1.74, P value was 0.38), sepsis ( OR value was 0.68, 95% CI0.27-1.70, P value was 0.41) and exudate ( OR value was 0.88, 95% CI0.32-2.42, P value was 0.80). Conclusion:The incidence of pneumothorax and artery injury in arm ports is significantly lower than in chest ports, there was no difference in other complications. More studies are needed to further confirm the advantages of arm ports.

Objective@#To explore the relationship between childhood depressive symptoms with behaviors and family factors, and to provide a new perspective for comprehensive treatment of depressive children.@*Methods@#A total of 58 children diagnosed with depressive disorder were recruited as case group in Department of Child Health, Affiliated Hospital of Qingdao University. At the same time, 88 healthy children were selected as age and gender-matched control group. Depressive symptoms, behaviors and family factors in the two groups were investigated. SPSS 22.0 statistical software was used to describe and analyze the data.@*Results@#The total score of CBCL scale in the case group was significantly higher than that in the control group(43.29±30.93, 20.24±12.93, P<0.01), and the number of positive factors was significantly higher than that in the control group(2.57±3.14, 0.97±1.80, P<0.01). The scores of introversion, extroversion, depression, compulsion, hyperactivity, aggression and social withdrawal in the case group were significantly higher than those in the control group(30.29±26.10, 17.10±16.53; 26.29±26.88, 17.45±16.99; 10.14±10.23, 3.48±3.14; 7.29±7.31, 4.83±5.26; 7.00±7.01, 4.86±4.38; 12.86±11.60, 8.38±8.90; 4.29±5.14, 2.72±3.01, P<0.01). There was no significant difference between the two groups in the scores of physical complaints and disciplinary violations (P>0.05). The scores of somatization, hostility and terror of SCL-90 in parents of children in case group were significantly higher than those in control group(17.58±4.05, 15.81±4.00; 9.66±2.67, 8.69±2.45; 8.03±1.49, 7.50±0.88, P<0.05). The score of SDS scale was positively correlated with the total score of CBCL scale, the number of positive factors, introversion, extroversion, depression, compulsion, hyperactivity, aggression and social withdrawal, and negatively correlated with parents’ marital status (P<0.01).@*Conclusion@#Depression is a common emotional disorder in childhood, which has a negative impact on learning and social performance. In the comprehensive treatment of children with depression, the importance of child behavior therapy and parental psychological counseling should be fully considered for mental health improvement.

Objective@#To summarize the nursing experience of a case of pseudomonas aeruginosa septic shock with massive gangrene in infants, and to explore the nursing method of pseudomonas aeruginosa septic shock with massive gangrene in infants.@*Methods@#Shock was treated by close observation, establishment of effective venous channels and effective dilatation; daily infusion of virus inactivated plasma supplemental immunoglobulin; planned detoxification and dressing change, as well as effective management of stool, urine and body position enable gangrene to be effectively controlled;At the same time, necessary nutrition and psychological support were given to the children.@*Results@#After 15 days of careful nursing, the wound surface recovered well and the vital signs were stable. Pediculated skin grafting may be performed at a scheduled time.@*Conclusions@#The practice of this case proved that the above nursing measures were effective for pseudomonas aeruginosa septic shock with massive gangrene.

Objective To establish a rapid method for the detection of SMCY antigen. Methods To Use the technology of colloidal gold immunochromatography with double antibody sandwich assay, the gold labeled pad was coated with colloidal gold labeled SMCY rabbit polyclonal antibody, colloidal gold strip was made for detection of serum, which include 12 serum samples of pig, cattle, dog, chicken and mice and 50 serum samples of human. Results The colloidal gold test strips showed obvious specificity in human and common animal sera and could distinguish between male and female sera. Conclusion This method can be used to identify the serum of women and men, and has certain species specificity, which provides a direction for forensic science to rapidly identify gender of human samples.

High grade B-cell lymphoma was defined as an independent disease in the 2016 new version of the World Health Organization lymphoma classification, including double-hit high grade B-cell lymphoma with myc and bcl-2 or bcl-6 gene rearrangements and high grade B-cell lymphoma, not otherwise specified without myc and bcl-2 or bcl-6 gene rearrangements, both of them are invasive in clinical features. In recent years, there have many studies on it, double-hit lymphoma (DHL) has relatively unique clinical features and poor prognosis. Although high-intensity chemotherapy can prolong the survival of patients, current treatment options have poor efficacy, and specific targeted drug therapies are still required. Single-agent or combination therapy of signal pathway inhibitors can improve the poor prognosis of patients. Immunotherapy is expected to become the direction of future research. This article reviews the definition, diagnosis, prognosis, and latest treatment progress of DHL.

Objective@#To summarize the treatment experience of patients with different degree of acute respiratory distress syndrome (ARDS) caused by inhalation of white smoke from burning smoke bomb.@*Methods@#A batch of 13 patients with different degree of ARDS caused by inhalation of white smoke from burning smoke bomb, including 2 patients complicated by pulmonary fibrosis at the late stage, were admitted to our unit in February 2016. Patients were divided into mild (9 cases), moderate (2 cases), and serious (2 cases) degree according to the ARDS Berlin diagnostic criteria. Patients with mild and moderate ARDS were conventionally treated with glucocorticoid. Patients with severe ARDS were sequentially treated with glucocorticoid and pirfenidone, and ventilator-assisted breathing, etc. were applied. The vital signs, arterial oxygenation index, changes of lung imaging, pulmonary ventilation function, general condition, and the other important organs/systems function were timely monitored according to the condition of patients. The above indexes were also monitored during the follow-up time of 10-15 months post injury. Data were processed with SPSS 18.0 statistical software.@*Results@#(1) The symptoms of respiratory system of patients with mild and moderate ARDS almost disappeared after 3 days′ treatment. Their arterial oxygenation index was decreased from post injury day 1 to 4, which almost recovered on post injury day 7 and completely recovered one month post injury. The symptoms of respiratory system of patients with severe ARDS almost disappeared at tranquillization condition 1-3 month (s) post injury. Their arterial oxygenation index was decreased from post injury day 3 to 21, which gradually recovered 1-3 month (s) post injury and was normal 15 months post injury. (2) Within 24 hours post injury, there was no obvious abnormality or only a little texture enlargement of lung in image of chest CT or X-rays of patients with mild and moderate ARDS. One patient with moderate ARDS had diffuse patchy and ground-glass like increased density shadow (pulmonary exudation for short) at post injury hour 96. Chest iconography of all patients with mild and moderate ARDS showed no abnormalities 10 months post injury. Both lungs of each of the two patients with severe ARDS showed obvious pulmonary exudation at post injury hours 45 and 75, respectively. One patient with severe ARDS showed no abnormality in chest image 10 months post injury, but there was still a small mesh-like increased density shadow in double lobes with slight adhesion of pleura in the other patient with severe ARDS 15 months post injury. (3) All patients showed severe restrictive hypoventilation when admitted to hospital. Pulmonary ventilation function of patients with mild and moderate ARDS recovered to normal one month post injury, and they could do exercises like running, etc. Pulmonary ventilation function of one patient with severe ARDS recovered to normal 6 months post injury, and the patient could do exercises like running, etc. The other patient with severe ARDS showed mild restrictive hypoventilation 15 months post injury and could do exercises like rapid walking, etc. (4) The condition of all mild and one moderate ARDS patients was better on post injury day 3, and they were transferred to the local hospital for subsequent treatment and left hospital on post injury day 21. One patient with moderate ARDS healed and left hospital on post injury day 29. Patients with severe ARDS healed and left hospital on post injury day 81. During the follow-up time of 10-15 months post injury, the other important organs/systems of all patients showed no abnormality, and there was no adverse reaction of glucocorticoid like osteoporosis, femoral head necrosis, or metabolic disorder. Two patients with severe ARDS did not have any adverse reaction of pirfenidone like liver function damage, photosensitivity, anorexia, or lethargy.@*Conclusions@#Early enough and uninterrupted application of glucocorticoid can significantly reduce the ARDS of patients caused by inhalation of white smoke from burning smoke bomb. Sequential application of glucocorticoid and pirfenidone can effectively treat pulmonary fibrosis at the late stage.

Purpose To investigate the expression and significance of MCM5 and p16INK4A in cervical carcinoma and cervical intraepithelial neoplasia (CIN 1,CIN 2 ~ 3) with different degrees of HPV 16 infection.Method RT-PCR and immunohistochemistryof SABC method were used to detect the expression of mRNA and protein in HPV 16 infected normal cervix,CIN 1 tissue,CIN 2 ~3 tissue and cervical squamous cell carcinoma and analyzed the clinical significance.Result The expression of p16INK4A and MCM5 mRNA in cervical cancer tissues were significantly higher than that in normal tissues (x2 =-6.589,P <0.001,x2 =-4.349,P <0.001).The degree of cervical lesions increased gradually (x2 =57.141,P < 0.001,x2 =47.628,P <0.01).Expression of mRNA MCM5 was correlated with pathological grade and clinical stage.Ⅰ a-Ⅰ b period was significantly lower than that of Ⅱ a-Ⅱ b (x2 =-4.93,P <0.01),the expression decreased with the decrease of pathological grade (x2 =-4.017,P <0.01).The expression of p16INK4A mRNA in cervical cancer decreased with the decrease of pathological grade (x2 =8.560,P < 0.01).The most obvious expression of p16INK4A and p16INK4A mRNA in squamous cell carcinoma.Expression of MCM5 protein and p16INK4A protein in cervical squamous cell carcinoma was positively correlated (r =0.497).Conclusion There is high expression of MCM5 and p16INK4A in cervical carcinoma.MCM5 can be a better reaction of cervical malignant hyperplasia,and p16INK4A joint detection for the improvement of CIN classification and prognosis of the significance of the judgment.

Objective with the advantages of rapidity in detection protein, We selected the gender-specific amino acid sequence based on human SMCY and SMCX, cloned and expressed SMCY gender-specific fusion antigens. The rabbits were immunized with purified antigens to obtain the polyclonal antibodies. A new method was established for rapidly sex identification of forensic evidence samples by detecting SMCY antigens with the corresponding polyclonal antibodies. Methods We found three differential fragments by analyzing the sequence of human SMCY and SMCX. Then we cloned this three fragments and ligated as a new recombinant.This SMCY gender-specific fusion antigen gene was sub-cloned into pET-28a and expressed in Escherichia coli. The fusion antigen was purified by Ni-NTA column. The rabbits were immunized with purified antigen to produce the specific polyclonal antibodies.The reactivity of the polyclonal antibody was evaluated by ELISA and Western blotting. We developed a colloidal gold test strip for detecting the gender of human samples. Results We successfully selected gender-specific amino acid sequence, the SMCY gender-specific fusion antigen was expressed by prokaryotic expression and the polyclonal antibody was prepared by immunizing rabbit. The results of colloidal gold strip tests showed that there is a significant difference between male and female serums. Conclusion The results showed that the SMCY gender-specific fusion antigen could be recognized by the polyclonal antibody.The colloidal gold strip tests made by SMCY gender-specific fusion antigens and the corresponding polyclonal antibodies could be used for rapidly determining the gender of forensic evidence samples.

Objective To compare sensitive difference of docetaxel between the triple negative breast cancer(TNBC)cell line CAL-51 and non TNBC line T47D and analyze mechanisms underlying docetaxel resistance in former cells. Methods Cell activi?ty was determined by MTT method and IC50 value was calculated;Wright-Giemsa stain was used to analyze the effect of docetaxel in the morphology of CAL-51 and T47D cell lines. Flow cytometry(FCM)was performed to determine cell cycle distribution and apoptosis. Realtime fluorescence quantitative PCR was used to compare the relative gene expression levels.The anti-apoptosis protein Bcl-2 and caspase family protein expression levels were determined by Western blot. Results Wright-Giemsa stain showed significant morpholo?gy change in T47D cells by docetaxel treatment. Further flow cytometry results confirmed that docetaxel could significantly induce apoptosis in T47D cells compared to CAL-51 cells(P<0.01). The result of realtime fluorescence quantitative PCR revealed that anti-apoptosis protein Bcl-2 was significantly higher expressed in CAL-51 cells(P<0.05). Immunoblot analysis revealed docetaxel treat?ment induced the instrinsic pathways in both CAL-51and T47D cells,but the activated pathway of executioner caspase was different. Conclusion Our present study shows that docetaxel induces different intrinsic apoptosis pathway in CAL-51 and T47D cell lines. An?ti-apoprosis protein Bcl-2 is highly expressed,which might be the underlying mechanism of docetaxel resistance in TNBC cell line-CAL-51.

Objective To construct prokaryotic expression vectors for glutamate dehydrogenase(GDH)of Clostridium difficile(CD),and express recombinant GDH in Escherichia coli,and identify its antigenicityed.Methods The entire gene of GDH was cloned from ATCC43255 genome DNA.The recombinant antigens were expressed in E.coli with IPTG induction and purified by Ni-NATBeads.The antigenicity was detected using CD Qick Chek Complete dual-antigen EIA. Results Prokaryotic expression vectors of CD GDH were constructed successfully.The antigen could be identified by specific anti-GDH antibodies.Conclusion The GDH antigen can be used to prepare corresponding antibodies,which facilitate the development of immunoassay for CD GDH.

Objective To evaluate the clinical application value of reflectance confocal microscopy(RCM) in the diagnosis of several common diseases manifesting as papules in children, including lichen nitidus, verruca planae, lichen striatus, milium, molluscum contagiosum and lichen pilaris. Methods A total of 579 children clinically characterized by papules were recruited into this study. RCM was used to observe lesions and perilesional normal skin. The RCM features of 6 diseases manifesting as papules were analyzed and compared. Results Based on RCM images, 236 patients were diagnosed with lichen nitidus, 70 with verruca planae, 123 with lichen striatus, 40 with milium, 53 with molluscum contagiosum and 57 with lichen pilaris. All the 6 diseases had typical RCM features. Concretely speaking, RCM images of lichen nitidus lesions showed infiltration of dense inflammatory cells and melanophages in enlarged dermal papillae. In RCM images of verruca planae lesions, cells in the granular and spinous layers were arranged in concentric circles, giving a rose cluster?like appearance. RCM images of lichen striatus lesions revealed focal swelling of stratum spinosum, absent or local liquifaction degeneration of basal cells, and clustering of a moderate number of inflammatory cells in the superficial dermis. In RCM images of milium lesions, well?circumscribed round or oval structures containing highly but nonuniformly refractive materials could be seen in the dermis. RCM images of molluscum contagiosum lesions showed intact cystoid structures containing highly refractive molluscum bodies. Lowly to moderately refractive cutin ? like materials were observed along with the dilation of hair follicle infundibula in RCM images of lichen pilaris lesions. In RCM images, the 6 diseases were distinguished mainly based on structural features(patterns and refractivity)of skin lesions shown by continuous vertical scanning. Conclusion RCM is of great value to the diagnosis of diseases manifesting as papules in children.

RESUMO

TcdA and B toxins secreted by Clostridium difficile( CD) are two important causes of diseases in organisms. The expression of tcdA and tcdB genes is regulated by a few factors located in the pathogenicity locus ( PaLoc) .Studies have indicated that the tcdC gene is likely to act as a negative regulator of toxin gene expression.So far, it has been debatable whether tcdC gene is regarded as a negative regulator.The mechanism of tcdC gene in pathogenesis remains unclear.In this paper, the structure and function of the tcdC gene are summarized, which will help study the mechanism of tcdC gene and obtain optimal drug targets.

Objective To clone and express Mycobacterium tuberculosis fusion antigen ESAT-6 and CFP-10 ( EC) and to evaluate the biological activity of the fusion antigen EC in inducing specific cytokines secretion from THP -1 cells. Methods The fusion antigen EC gene was cloned into pET-30 a prokaryotic expression vector and expressed highly in E.coli BL21.Then, the THP-1 cells were stimulated with purified fusion antigen EC of different concentrations (10 and 20 μg/ml).Culture supernatants were collected after 12 h and 24 h, respectively.The secretion levels of IL-2, IL-4, IL-6, IL-8, IL-10, GM-CSF, TNF-αand IFN-γin THP-1 cell culture supernatants were detected using Bio-PlexProTM Assays kit.Results The M.tuberculosis fusion antigen EC was cloned and expressed successfully .The secretion levels of IL-6, IL-8 and TNF-αin EC infected THP-1 cells were significantly higher than those in THP-1 cells (P<0.05).The secretion levels of other cytokines did not change significantly .Conclusion The obtained M.tuberculosis fusion antigen EC has biological activity in inducing the THP-1 cells to secrete a higher level of IL-6,IL-8 and TNF-α.

RESUMO

Objective To generate data on the current medical rehabilitation resources in Hubei province so as to improve the management of rehabilitation quality.Methods Medical institutions on the second level and above were investigated using a questionnaire which covered installations,personnel,equipment and field of operations.Results A total of 176 questionnaires were recovered.Among them,166 (94.3%) of the hospitals had established a department of rehabilitation medicine,and 122 (69.3%) had set up an independent rehabilitation ward,accounting for a total of 3,359 beds.Altogether,there were 1,260 rehabilitation physicians (36.7% of the assigned personnel),904 therapists (26.3%) and 1,272 nurses (37%).Senior physicians,therapists and nurses were 26%,3.1% and 28.3% of the respective totals.Conclusion Rehabilitation medicine in Hubei Province remains relatively poorly developed.It is weak in terms of building the discipline,management and devices,especially in a serious lack of professionals and rehabilitation therapists.

<p><b>OBJECTIVE</b>To study the association between the level of serum DKK1 and disease course of multiple myeloma(MM)as well as myeloma bone disease.</p><p><b>METHODS</b>This study enrolled 145 cases of MM(including 79 newly diagnosed MM, 19 responded MM, 47 relapsed or progressive MM) whose lytic bone disease were evaluated by conventional radiography, ELISA was used to detect the concentration of serum DKK1.</p><p><b>RESULTS</b>Serum DKK-1 elevated in newly diagnosed MM compared with normal donors[2 155(646-35 251)vs 1 487(646-2 577) ng/L, P=0.000], those responded[1 136(431- 3 582) ng/L, P=0.001]and relapsed/progressive MM[1 695(431-3 582) ng/L, P=0.037], and the level of relapsed/progressive MM was marginally higher than the responded ones. Moreover, MM patients without lytic lesions in conventional radiography had significantly lower DKK- 1 levels than those with lytic bone disease[1 910(660-26 925)vs 2 519(646-35 251) ng/L, P=0.005]. Notably serum DKK-1 correlated with the number of bone lesions[0 vs 1-3 vs >3 lesions: 1 910(660-26 925)vs 2 155(1 369-5 974)vs 2 547(646-35 251)ng/L, P=0.018].</p><p><b>CONCLUSION</b>DKK-1 serum concentration correlated with disease course of MM and myeloma bone disease, indicating that DKK-1 was an important factor for the extent of bone disease, which supporting the hypothesis of DKK-1 as a therapeutic target in myeloma bone disease.</p>
