RESUMO

Objective:To investigate the correlation between serum inflammatory markers and carotid plaque and its stability in patients with acute ischemic stroke (AIS).Methods:Patients with AIS admitted to Liaocheng Third People’s Hospital from June 2021 to April 2022 were prospectively enrolled. The demographic and relevant clinical data were collected. Color Doppler ultrasound was used to detect carotid artery plaques. The patients were divided into unstable plaque group, stable plaque group and non-plaque group according to plaque characteristics. The levels of serum hypersensitive C-reactive protein (hs-CRP), serum amyloid A (SAA) and interleukin (IL) -6 were detected and compared. Multivariate logistic regression analysis was used to determine the independent risk factors for carotid plaque and its stability. Results:A total of 201 patients with AIS were enrolled, including 87 patients (43.30%) in the non-plaque group, 57 (28.35%) in the stable plaque group, and 57 (28.35%) in the unstable plaque group. The proportion of patients with hypertension and previous stroke history, and hs-CRP, SAA and IL-6 levels in the unstable plaque group was significantly higher than those in the stable plaque group and the non-plaque group (all P<0.05). Multivariate logistic regression analysis showed that after adjusting for other confounding factors, the increased IL-6 level (odds ratio [ OR] 1.174, 95% confidence interval [ CI] 1.049-1.314; P=0.005) was an independent risk factor for the existence of stable plaques, while the previous stroke history ( OR 3.172, 95% CI 1.123-8.957; P=0.029) and the increased IL-6 level ( OR 1.367, 95% CI 1.107-1.687; P=0.004) were the independent risk factors for the existence of unstable plaques. Conclusion:The serum IL-6 level in patients with AIS increase significantly, which is closely associated with the formation and stability of carotid plaques.

Objective To explore the application effect of key indicators of oncology nursing quality evaluation in the continuous improvement of specialist nursing quality. Methods The Delphi method was used to cons-truct 14 key indicators of oncology nursing quality evaluation, and the key indicators were used to control the quality of specialist nursing in the tumor area. Data were collected and a database was established. Based on the data, the quality of the nursing care and continuous quality improvement were carried out. Results The results of the data from the first year (before the intervention in 2016) and the second year (after the intervention in 2017) were compared with the data of the key indicators of the quality evaluation of the specialist nursing. The results showed that the indicators had different degrees of decline. The data before and after intervention were: chemotherapy drug extravasation rate 0.20% (33/16 861) and 0.10% (20/19 957), and the incidence of severe pain was 1.65% (1 747/106 114) and 1.43% (1 521/106 462). The incidence of anxiety was 2.17% (2 305/106 114) and 1.53% (1 633/106 462), and the incidence of depression was 2.27% (2 408/106 114) and 1.28% (1 359/106 462). The difference was statistically significant (χ2=5.80-300.90, P<0.05); The other 9 indicators of process have improved to varying degrees. Conclusions 14 key indicators of oncology nursing quality evaluation are applied to the oncology ward for specialized nursing quality management and quality improvement, which can monitor the quality of key specialist nursing in real time and dynamically. It can accurately select clinical nursing priority improvement projects and improve specialist nursing. Quality;energy evaluation to improve the quality of specialist nursing quality; help improve the quality management of specialist nursing and ensure patient safety.

Objective To compare the outcomes of patients with non-variceal upper gastrointestinal bleeding (NVUGIB) taking aspirin for primary prophylaxis to those not taking it.Methods Patients without any vascular disease (coronary artery or cerebrovascular disease) who were admitted to the 89th Hospital of PLA between 2006 and 2016 with non-variceal upper gastrointestinal bleeding were included.The frequencies of in-hospital mortality,re-bleeding,severe bleeding,need for surgery or embolization,and of a composite outcome of the 4 bleeding related adverse outcomes were compared between patients receiving aspirin and those on no antithrombotics.We also compared the frequency of in hospital complications and length of hospital stay between the two groups.Results Of the 179 eligible patients,47 were on aspirin and 132 patients were on no antithrombotics (control group).Patients in the aspirin group (58.4 years) were elder than that in controls (66.8 years) (P <0.01).Patients in the aspirin group had significantly more co-morbidities,including diabetes mellitus [25.5 % (12/47) vs 12.1% (16/132)] and hypertension [46.8% (22/47) vs 28.0% (37/132)],respectively,as well as dyslipidemia [21.3% (10/47) vs 6.1% (8/132)] (P < 0.01).Smoking was more frequent in the aspirin group [36.2% (17/47) vs 22.7% (30/132),P < 0.05].The frequencies of endoscopic therapy and surgery were similar in both groups,while the frequencies of in-hospital complications and re-bleeding were similar in the two groups,too.However,the patients who were on aspirin had lower in hospital mortality rates (2.1% vs 13.6%,P <0.01),shorter hospital stay (4.9 d vs 7.0 d,P <0.05),and fewer composite outcomes (10.6% vs 24.2%,P < 0.05).Conclusions Patients who present with NVUGIB while receiving aspirin for primary prophylaxis had fewer adverse outcomes.Thus aspirin may have a protective effect beyond its cardiovascular benefits.

[Methods]The article discusses the essentials of clinical dialectical experience of Professor YU Xinwei in the diagnosis and treatment on Hashimoto's thyroiditis.The article also illustrates and analyses a medical record. [Result]Professor YU considers that treatments of Hashimoto thyroiditis should start from the liver spleen and kidney,combine Chinese traditional and western medicine,medicate stagedly and reasonably,while paying attention to female patients.[Conclusion]Professor YU Xinwei distinguishes characteristics in therapy and the rich experience,simultaneously the clinical efficacy is obvious.

Objective]To research the impact of Danxi’s medical science on Kampo medicine system and summarize its development in Japan,for providing ideas for the research of Chinese medicine. [Method]Learning Zhu Danxi related books and Kampo medicine literature, analyse the impact of Danxi ’s medical science on each genre of Japan Kampo medicine,explain the role of Danxi ’s medical science in the formation and development in Japan Kampo medicine system,and ummarize its development path in Japan.[Result]The development of Danxi ’s medical science in Japan experienced ups and downs, combined with local realities,and derived more Japanese-oriented medical theories. [Conclusion]Danxi ’s medical science not only for the formation of Japanese Kampo medicine system plays an important role, but also has a profound impact on Japan's three Kampo medical genre.Its development track is worth thinking and summarizing.

BACKGROUND: Diffusion tensor imaging is a common technique applied for clinical studies of the brain, but it is rarely used for the diagnosis or prognosis of spinal cord injury. OBJECTIVE: To establish a rat model of spinal cord injury using micrological techniques, and to evaluate spinal cord transection with diffusion tensor imaging technology, thus providing a good animal model for further intervention. METHODS: Twelve healthy Sprague-Dawley rats were applied to establish spinal cord injury models using precise microscopic techniques, and another six rats in the sham operated group served as controls. Spinal cord transection of experimental rats after modeling was observed using diffusion tensor imaging. Motor evoked potentials and somatosensory evoked potentials were used to detect electrical physiological changes of rats.Neurological function changes of rats were evaluated using slope experiments and Basso, Beattie and Bresnahan scores. RESULTS AND CONCLUSION: After experimental rats regained consciousness, their lower extremities exhibited complete paralysis, the tails cannot swing, accompanying urinary retention. Diffusion tensor imaging displayed the T10 segment of spinal cord was completely transected. Motor and sensory evoked potential waveform were not drawn compared than control group. At 1 day, 1 week, 2 weeks, 4 weeks after operation, the test angle of slope experiments was less than 30° and Basso, Beattie and Bresnahan score was less than 10 points; as the time prolonged, lower limb irritating reflections of some rats were visible, but no initiative functional activity was found, local spinal cord structure were severely damaged. Precise microscopic techniques can successfully establish spinal cord injury model in rats, and diffusion tension imaging clearly visualizes the complete transection of the T10 spinal cord.

Objective:To improve the CT bulb tube cooling rate and the testing method of the CT bulb tube temperature, and provide more stable platform for better bulb tube protection and longer life-span. Methods: The designed specified cold air machine could transfer the dry cold wind into machine rack, which can lower the ambient temperature of the machine rack and increase the air flow inside the machine rack and promote the efficiency of the heat exchange of the bulb tube oil cooling circuit. Results:the design of this system’s joint is light and handy, which could collect the instantaneous temperature change of the bulb tube, and carry out simple man-machine interaction monitoring. Conclusion:According to the bulb tube oil-way temperature and ambient temperature and relative humidity, the computer can dynamically adjust the refrigerating capacity and operation/cease of the air cooler. Then it can constitute the closed control loop, and accelerate the efficiency of the bulb tube’s heat dissipation, and promote the heat capacity of the bulb tube equivalently.

Objective To investigate the clinical effect of repairing soft tissue defects of foot and ankle by sural neurovascular adipofascial flap.Methods Application of sural neurovascular adipofascial flap to repaire soft tissue defects of foot and ankle in 19 patients,inclouding 3 csaes with the soft tissue defects of medial malleolus and 16 cases of dorsum pedis from May,2006 to May,2013.The size of soft tissue defects was 3 cm ×3 cm-7 cm ×9 cm.The skin of the donor site was sutured directly.The recipient area was reshaped granulation and underwent free skin graft after the adipofascial flap survived.Results After followed up of 3-18 months,the adipofascial flaps were all survived,the recipient area was almost flat with the surrounding tissue,the donor area only leaving linear scar,the appearance and function of donor area and adopt area were satisfactory.The sensation of lateral dorsal region of foot decreased to S3,and the sensation of recipient site recovered to S2.There were some changes in pigmentation with the skin graft region.Conclusion Sural neurovascular adipofascial flap can be effectively repaired soft tissue defects of foot and ankle,and it can avoid irregularity of donor area and enlargement of adopt area,the appearance and function were satisfactory.

To investigate the correlation between cerebral infarction and carotid atherosclerosis.Methods:Bilateral carotid atherosclerotic plaques,intima-media thickness (IMT),and stiffness coefficient (β) in 60 patients with cerebral infarction were detected by high-frequency color Doppler imaging,and they were compared with the control group (n=60).Results:The detection rate of plaque in the cerebral infarction group was significantly higher than that in the control group (90% vs 28%,P<0.005); the constituent ratio of soft plaques in the cerebral infarction group was significantly higher than that in the control group (P <0.01),and the constituent ratio of flat and hard plaques was significantly lower than that in the control group (P<0.005); IMT and β values in the cerebral infarction group were significantly higher than those in the control group (P<0.01).Furthermore,IMT had significant positive correlation with β values (P<0.01).Conclusions:Carotid atherosclerosis is closely correlated with cerebral infarction.

AIM: Through detecting bone marrow angiogenic mediators and inhibitors in aplastic anemia(AA) patients,the value of angionesis in AA pathogenesis was elucidated.METHODS: The patients were divided into severe AA group(SAA,8 patients),non severe AA group(NSAA,10 patients),and normal control group(7 persons),5 patients were observed before treating(group beginning) and getting improvement(group improving).The angiogenic mediators vascular endothelial growth factor(VEGF) and bFGF were detected by ELISA,angiogenic inhibitors IFN? and TSP were detected by ELISA and flow cytometry,respectively.RESULTS: The levels of VEGF were lower in SAA group and NSAA group than those in control group significantly(P

RESUMO

AIM: To observe the cytotoxicity on myeloma cells mediated by anti-CD20 monoclonal antibody-mabthera, after heightening level of CD20 expression on myeloma cells membrane by ?-interferon. METHODS: 10 untreated(UT) and 10 relapsed or refractory(RR) MM patients'myeloma cells were cultured with human recombinant ?-interferon (hr?-IFN) at concentrations of (0-800)?10~3 U/L to heighten level of CD20 expression, then complement-dependent cytotoxicity (CDC) and antibody-dependent cell-mediated cytotoxicity (ADCC) on myeloma cells mediated by mabthera were studied through MTT methods. RESULTS: When CD20 expression of UT MM and RR MM patients' myeloma cells increased after treated by hr?-IFN, 12 mg/L and 16 mg/L mabthera mediated ADCC and CDC (against) myeloma cells in group UT patients and group RR patients, respectively. CONCLUSION: After heightened level of CD20 expression on myeloma cells membrane by hr?-IFN, mabthera mediated ADCC and CDC against myeloma cells in vitro.
