RESUMO

Objective@#To evaluate the effect of Danqi Yishen decoction on diabetes mellitus(DM) and its influence on proteinuria.@*Methods@#150 DM patients were randomly selected, and they were randomly divided into two groups according to the digital table, 75 cases in each group.The control group was given western medicine, the study group was treated by Danqi Yishen decoction.The treatment efficacy, blood serum creatinine(Cr), 24h urine protein, blood urea nitrogen(BUN) and fasting blood glucose(FBG) before and after treatment were compared between the two groups.@*Results@#The total effective rate of the study group was 84.0%, which was significantly higher than 71.1% of the control group, the difference between the two groups was statistically significant (χ2=7.215, P<0.05). Before treatment, there were no statistically significant differences in the health indicators between the two groups (all P>0.05). After treatment, the improvement of indicators of the study group were significantly higher than those of the control group (all P<0.05). After treatment, the FBG, postprandial 2h plasma glucose and fasting insulin concentration in the study group were significantly lower than those in the control group (all P<0.05).@*Conclusion@#Danqi Yishen decoction has ideal clinical effect in the treatment of DM, and can significantly reduce the proteinuria and FBG, which is worthy of widely popularized.

Objective To explore the value of hydrogen sulfide(H2S) in the evaluation of severity of acute pancreatitis(AP).Methods Fifty-one patients with AP from February 2013 to December 2015 in this hospital were divided into severe acute pancreatitis (SAP,n=21) group and mild acute pancreatitis (MAP,n 30) group.Thirty six non-pancreatitis abdominal patients and 9 healthy persons(NC) were chosen as compare groups.The peripheral venous blood samples from the four groups and the Ranson Score of AP group were obtained after admission,the venous blood samples of 12 h,24 h,48 h of AP group were also obtained.Blood samples were used to detect the concentrations of hydrogen sulfide with ELISA method.We compared the concentrations of H2S between the four groups with one way ANOVA,and post hoc;compared the differences of concentrations of H2S in AP group between after admission,12 h,24 h and 48 h with repeated measures data ANOVA;and explored the relationship between the concentrations of H2S after admission and the Ranson scores in AP group with Spearman correlation analysis.Results The concentra tions of H2S was significant higher in SAP group than MAP,abdominal,and NC group(P=0.000);the concentrations of H2S was also significant higher in MAP group than abdominal,and NC group(P=0.000);there is no significant difference between the abdominal and NC group(P =0.131).There is significant difference of H2S concentrations between the four times in AP group(P =0.000),decreasing gradually over time.The H2S concentrations in AP group after admission was significantly related with their Ranson scores(r=0.578,P=0.000).Conclusion There is certain value of the concentrations of H2S in the diagnosis of AP,the higher H2S level suggests the higher severity of the patients with acute pancreatitis.

RESUMO

Objective To evaluate the diagnosic value of miniprobe sonography (MPS) in eminence lesion of upper gastrointes-itinal tract .Methods MPS were performed in 485 patients with eminence lesion of upper gastrointesitinal tract .334 patients were given histologic examination .The pathological examination results were compared with diagnosis of MPS and gastroenterology .Re-sults 80 stromal tumors ,41 leiomyomas ,22 cases of ectopic pancreases ,67 cancers ,28 cysts ,26 lipomas ,25 hemangioma ,42 in-flammatory hyperplasia ,104 polyps ,25 cases of protuberance from outside organs ,1 lymphoma and 8 cases of duodenal Brunner gland hyperplasia were detected .The accurancy rate of MPS was 76 .65% which is higher than gastroscope′s .Conclusion MPS is helpful to scope the extent and location of the eminence lesion of upper gastrointesitinal tract and display security of endoscopic bi-opsy .M PS do benefit to the diagnosis ,differential diagnosis and treatment of eminence lesion of upper gastrointesitinal tract .

RESUMO

Objectives To investigate the clinical effect of hemoperfusion in severe juvenile dermatomyositis. Meth-ods Two patients with severe juvenile dermatomyositis, who accepted hemoperfusion treatment, were retrospectively analyzed. Results In these 2 patients, the hemoperfusion process were smooth. The muscle pain was reduced and the muscle enzymes were decreased. After hemoperfusion, one patient refused to continue treatment and the disease recurrent. The other patient con-tinued to accept the immunosuppressive therapy and eventually improved. Conclusions For juvenile dermatomyositis, hemo-perfusion was a safe and effective auxiliary treatment. It could be applied when the general treatment was invalid.

Objective To research expression changes of COX-2 gene in Barrett′s esophageal mucosa before and after argon plasma coagulation (APC)operation and to explore it′s clinical curative effect and possible mechanism .Methods 66 barrett′s e-sophageal diagnosed with gastroscope and pathology were randomly divided into normal esophageal group (the negative control group ,group A) ,the APC with acid suppression therapy group(group B) ,Acid antimicrobial drug treatment group(group C) .Ob-serve symptom relief condition ,barrett esophagus epithelium ablation under gastroscope before and after treatment ,esophageal mu-cosa tissue samples was detected by real-time fluorescent quantitative PCR technique .Expression of COX-2 gene in barrett′s esoph-ageal mucosa were compared among groups .Results Compared with group A ,both Group B and group C could effectively relieve symptoms (P<0 .05) .Symptom remission rate had no no statistically significant difference in Group B and group C .Barrett esopha-gus epithelium was not significantly narrowed under gastroscope follow-up Group A and group C ,while Barrett esophageal mucosal occurred ablation with pink mucous covered in Group B .The expression of Cox-2 mRNA in group B decreased ,which was similar to group A .The expression of Cox-2 mRNA in group C also decrease ,but there was no significant differences before and after treat-ment .Conclusion APC with acid suppression therapy could effectively relieve symptoms ,melting Barrett esophagus epithelium ,and is of a safe and effective treatment on Barrett′s esophagus .

Objective To study the effect of multiple IPL treatment on cell senescence markers of skin fibroblasts using UVA as a control and to make clear whether the multiple IPL treatment may result in cell senescence.Methods Cells were divided into three groups: one group without irradiation as a control,one group receiving IPL treatment with 15 J per cm2,and the last group receiving UVA irradiation with 9 J per cm2.IPL and UVA irradiation were performed once a day during five days.On the sixth day,the cells were collected.Senescence-associated β-galactosidase (SA-β-Gal) staining,cell cycle,reactive oxygen species (ROS) and telomere length were determined.Results Our results showed that five consecutive days of IPL irradiation had no effect on the activity of SA-β-Gal and telomere length and decreased the G1 % of cell cycle and the level of ROS in comparison with the control group (P<0.05).On the contrary,five consecutive days of UVA irradiation increased the activity of SA β Gal and the level of ROS,shortened the length of telomere and no obvious change in the G1 % of cell cycle in comparison with the control group.Conclusions Multiple UVA irradiations induce cell senescence.On the contrary,multiple IPL treatments could not induce cell senescence.

Objective To study the protective effects of intense pulse light (IPL) on the injury of normal human skin fibroblasts (FB) induced by ultraviolet A (UVA Ⅰ ) in vitro and to explore its possible mechanism. Methods The human skin fibroblasts were isolated and cultured, and then irradiated by UVA Ⅰ (9 J/cm2) and IPL (15 J/cm2), respectively. The proliferative ability of the cells were detected by CCK-8. Cell cycle was detected by flow cytometry, and cylin D1 and CDK2 protein expression levels were detected by Western blot. Results Different doses of UVA Ⅰ irradiation caused certain damages of cultured fibroblasts. With the increasing of of UVA Ⅰ dose, cell proliferation was decreased. Cells went to death at the exposure to 11 J/cm2 UVA Ⅰ , while the proliferative activity did not change much at 7 J/cm2 UVA Ⅰ . Cells were treated with UVA Ⅰ for other 2 days, then with IPL irradiation for other 2days, showing clear stimulating to the cell proliferation as compared with the cells that received UVA Ⅰ treatment only. Flow cytometry results showed that an increase of cell proliferating index, and cell cycle protein cyclin D1 and CDK2 expression levels were also upregulated after IPL irradiation.Conclusion UVA Ⅰ irradiation may cause cell damage as showed by cell growth index, cyclin D1 and CDK2 expression, and this injury could be protected partly by IPL treatment. The intense pulsed light may regulate the expression of cyclin proteins that may promote normal fibroblast proliferation, which could be one of the mechanisms of IPL skin rejuvenation.

Objective To evaluate the influence of grape seed proanthocyanidin extract (GSPE) on sunburn cell formation and p53 protein expression induced by acute ultraviolet injury. Methods Ten volunteers were enrolled in this study. The buttock region served as the exposed region. Four areas were randomized and delineated on the buttock: one area (control area) received no exposure or product, the other 3 areas were exposed to two minimal erythema doses (MED) of simulated solar radiation (SSR) for 3 days. Of the 3 exposed areas, one area (SSR) received no product before exposure, one area (SSR + Veh) was pretreated with vehicle, the third area (SSR + GSPE) with the samples of GSPE. GSPE or vehicle was applied 30 minutes before each exposure at 2 μL/cm2. Skin biopsy was performed 24 hours after the last exposure, and skin specimens were subjected to hematoxylin eosin (HE) staining and histochemical analysis for p53 protein. Results There was a statistical difference in the number of sunburn cells per high power field (×200) between SSR sites and SSR + GSPE sites (29.8±11.1 cells vs 2.2±0.2 cells, P<0.01). A significant decrease was noticed in the account of p53 protein-positive cells per high power field (×200) in SSR + GSPE sites com-pared with the SSR sites (4.6±0.7 cells vs 19.3±3.4 cells, P<0.05). Conclusion GSPE exerts a poten-tial protective effect against acute ultraviolet injury and can serve as a natural sunscreen.
