BACKGROUND: Bone morphogenetic protein 2 is a most widely studied and osteogenic-inducing bone morphogenetic protein. However, simple bone morphogenetic protein can be diluted by tissue fluid and decomposed by protease after implantation, so it is difficult to maintain sustained drug concentration or play an effective role in bone induction. OBJECTIVE: To observe the effect of repairing osteoporotic fracture through calcium phosphate cement/fibrin glue composite as the carrier of bone morphogenetic protein 2. METHODS: Fifty-four female Sprague-Dawley rats were selected to remove bilateral ovaries for making osteoporosis models. Three months later, the middle femoral fracture models were made, and then randomized into three groups, with 18 rats in each group. Kirschner wire fixation group was injected nothing. The fracture end was injected with 0.5 mL calcium phosphate cement/fibrin glue (composite group) or calcium phosphate cement/fibrin glue loaded with recombinant human bone morphogenetic protein 2 (loaded group) . At 4 and 12 weeks after fracture, X-ray examination, micro-CT examination, biomechanical three-point bending test and pathological observation were performed. RESULTS AND CONCLUSION: (1) The fracture healing score in the loaded group was higher than that in the other two groups (P < 0.05) . (2) The bone volume fraction, trabecular thickness and number of trabeculae at 4 and 12 weeks in the loaded group were higher than those in the other two groups (P < 0.05) , and the trabecular segregation was lower than that in the other two groups (P < 0.05) . (3) The maximum load and stiffness at 4 and 12 weeks in the loaded group were higher than those in the other two groups (P < 0.05) , and the elastic modulus at 4 weeks after fracture was higher than that in the other two groups (P < 0.05) . (4) Fibrocartilage callus was mainly seen at 4 weeks after fracture in the Kirschner wire fixation group, and the callus was reconstructed into lamellar bone at 12 weeks after fracture with less callus content. Obvious fibrocartilage callus appeared at 4 weeks after fracture in the composite and loaded groups, and the callus was reconstituted into a plate-like bone at 12 weeks. (5) These results imply that the calcium phosphate cement/fibrin glue composite loaded with recombinant human bone morphogenetic protein 2 can promote the healing of osteoporotic fracture and improve bone strength.

Objective To investigate the effect of a combination of second generation high pitch scanning technique of dual source CT and Iomeprol 400 on image quality,total iodine dose and injection speed.Methods Eighty-four patients with clinically suspected aortic disease underwent aorta CT angiography (CTA)were randomly divided into two groups (42 cases in each group).The experimental group used high pitch dual source CTA acquisition mode (pitch 3.2)combined with Iomeprol 400 contrast agent.The control group used conventional CTA scan mode combined with Iopamidol 370 contrast agent.The differences in image quality,effective radiation dose, total iodine dose and injection speed between two groups were compared.Results There were no statistical differences in CT value, signal-to-noise ratio,contrast-to-noise ratio and subjective image quality score between two groups (P>0.05).There were significant statistical differences in CTDIvol,DLP and CT value of the superior vena cava between two groups (P<0.01).The total iodine dose and injection speed were reduced in experimental group.Conclusion The combination of a high pitch scanning technique of aorta CTA with Iomeprol 400 contrast agent can reduce radiation dose and potential hazards of contrast agents on the premise of guaranteeing the image quality.

Objective To study whether crystal-8 exerts analgesic effect on the central nervous system,and to observe the relationship between crystal-8 and receptors. Methods A total of 32 SD rats were randomly divided into the 0.9% sodium chloride solution group, positive control group ( rotundine 2 mg·kg-1 ) , high dose of crystal-8 group ( 2 mg·kg-1 ) and low dose of crystal-8 group ( 1 mg · kg-1 ) . The changes of pain threshold were measured by the thermal stimulation test following intracerebroventricular (ICV) injection.The SD rats or mice were randomly divided into the 0.9% sodium chloride solution group, receptor tool medicine group ( including naloxone, reserpine, haloperidol, scopolamine) , crystal-8 group, receptor tool medicine plus crystal-8 group. The pain threshold was detected by the thermal stimulation test at 15, 30, 45, 60, 90, 120 min after dosing, then the influence of analgesic effect of crystal-8 on the neurotransmitter was observed. Results Compared with the 0.9% sodium chloride solution group, the pain threshold of rats was improved after taking the crystal-8 by intracerebroventricular (ICV) injection(P<0.01).Compared with the crystal-8 group, the naloxone plus crystal-8 group, the haloperidol plus crystal-8 group, the scopolamine plus crystal-8 group couldn't increase the pain threshold, there was no significant difference among these groups(P>0.05).However, Reserpine plus crystal-8 group could significantly decrease the pain threshold in rats compared with the crystal-8 group(P<0.01).The analgesic effect of crystal-8 was interfered by reserpine. Conclusion The analgesic effect of crystal-8 may be involved in the central mechanism,which relates to monoamine neurotransmitters, but has nothing to do with the opioid receptor, M receptor, and the inhibition of central DA receptor.

RESUMO

Objective To investigate the application value of low dose high pitch dual source CT angiography technique in diagno-sis of acute aortic dissection.Methods 21 cases of critically ill patients with suspected aortic dissection underwent total aortic angiog-raphy without using ECG by adopting the second generation dual source CT Flash Technology (pitch 3 .2 ,1 0 0 kV and 1 0 0 mAs), using a lower contrast dose (60 mL).Results All subjects accepted as low as possible radiation doses and iodine intake;at the same time,accurate information including aortic intimal tear position,intimal flap,true and false lumen,and lesions involving the range vessel signs and anatomic relationship were received,2 cases of Stanford type A,type B 1 9 cases (2 cases of aortic rupture).Average scan time 2.6 s,average effective radiation dose cm 270 mGy·cm.Conclusion High quality images by using high pitch dual source CT low dose angiography with lower contrast dose provide safe,reliable,real-time screening method for critically ill patients with aor-tic dissection caused by systemic severe compound injury.

Objective To explore the characteristics of MRI images and pathological features of demyelinating pseudotumor (DPT).Methods Five patients with demyelinating pseudotumor were histologically confirmed.Their MR imaging were re-viewed and the enhancement patterns were analyzed.Results Among the five cases with demyelinating pseudotumor,single le-sion was found in 4 cases,and multiple lesions in 1 case.Most lesions had a clear boundary and these space occupying lesions were irregular (3 cases).The signal intensity on T1 WI was lower than edema,and it had a specific “core and halo”feature,du-al-high signal with different intensity on T2 WI,slightly higher signal intensity on DWI,and on enhanced MRI,nodular or patch-y enhancement and open ring enhancement with edema around the lesions were observed.The pathological results showed that the demyelination occurred but the neuraxon survived,lymphocytes were infiltrated,glial cells were proliferated,megakaryo-cytes also could be found,and multiple monocyte-macrophages were all around the lesions.Conclusion The MRI of DPT has some characteristic features.

<p><b>OBJECTIVE</b>A high performance liquid chromatography (HPLC) method was developed to determine the concentration of nitidine chloride in plasma and successfully applied to study pharmacokinetics after i.v. administration in rabbits.</p><p><b>METHOD</b>Twelve rabbits, randomized into 2 groups , were given i.v. at the dose of 4, 6 mg x kg(-1) respectively. Chloramphenicol was used as an internal standard. Nitidine chloride was extracted from plasma with ion pair reagent, and was determined by HPLC.</p><p><b>RESULT</b>The calibration curves of nitidine chloride was linear in the range of 0.03-2.04 mg x L(-1). Its recoveries were more than 95%, intra-day and inter-day precisions were lower than 6%. The concentration-time curve of nitidine chloride in rabbits after i.v. of 4 and 6 mg x kg(-1) were shown to fit a two-compartment model, the main pharmacokinetic parameters showed no significant difference between the low and high dosage, and the AUC values are directly relative to doses. T1/2alpha were (5.46 +/- 0.89), (4.76 +/- 0.33) min respectively, T1/2beta were (263.33 +/- 16.4), (274.71 +/- 16.52) min respectively, AUC were (46.56 +/- 1.80), (69.19 +/- 2.30) microg x min(-1) x mL(-1) respectively.</p><p><b>CONCLUSION</b>It is first time to establish the HPLC method to determine the concentration of nitidine chloride in rabbits plasma. The method is sensitive, accurate and reproducible. It is first time to study the pharmacokinetic characters of nitidine chloride in rabbits after i.v. administration, the elimination of nitidine chloride is linear pharmacokinetics.</p>

RESUMO

Objective To assess the clinical value of ultrasound-guided percutaneous ethanol injection (PEI) in the treatment of hysteromyoma. Methods Fifty patients with hysteromyoma confirmed by pathology were enrolled in this study. Dose of ethanol was identified through tumor size. Ultrasound angiography and biopsy were performed after operation to observe the therapeutic effect of PEI. Results All tumors were treated with PEI 1-4 times. Echo enhancement, disappearance of blood in tumor, tumor atrophy and cavity-like change were observed. Post-operative biopsy indicated necrosis of tumor cells. Conclusion Ultrasound-guided PEI is safe and effective in the treatment of hysteromyoma.

RESUMO

60 years, 13.3% of the total sarcoidosis patients) diagnosed as sarcoidosis based on national criteria from the year 1965 through 1999 were reviewed. Results The clinical degree was as follows: phase I, 8 cases; phase Ⅱ, 6 cases; phase Ⅲ, 2 cases. The elderly sarcoidosis patients manifested variously, with misdiagnosis rate being 50%. The active degree of pulmonary sarcoidosis was correctly evaluated after the diagnosis was confirmed and the patients was strictly observed during the treatment since it was very important for avoiding the occurrence of complications. Conclusions The early diagnosis in elderly patients with sarcoidosis should be emphasized to prevent them from missing and misdiagnosis. The elderly patients with treating indications in phases Ⅱ and Ⅲ could be given steroids and constant observation and regular examinations are important during the treatment.
