RESUMO

Objective To observe the clinical efficacy and safety of Yishen Huoxue Decoction combined with low-frequency pulse magnetic fields (PEMFs) for treatment of hemiplegia with osteoporosis (OP). Methods Totally 144 patients were divided into TCM group and control group with randomized parallel controlled method, with 72 cases in each group. Patients in control group were treated with Western routine therapy, rehabilitation training, PEMFs (putting the magnetic head on the back, five times every week, 20-30 minutes each time). On the basis of control group,TCM group received Yishen Huoxue Decoction, one dosage per day, twice a day, orally. The therapeutic course for both groups was 6 weeks. The Fugl-Meyer assessment (FMA), Barthel index (MBI), NIH stroke scale (NIHSS), and visual analogue scale (VAS) before and after treatment were observed in the two groups of patients. Bone mineral density (BMD) was measured. Blood calcium, serum phosphorus, osteocalcin (BGP) content, type 1 procollagen amino-terminal peptide (P1NP), alkaline phosphatase (BALP) were tested. Safety was tested and adverse reactions were observed. Results Compared with before treatment, the scores of FMA, MBI, NIHSS and VAS improved in both groups (P<0.05); the scores of FMA, NIHSS and VAS in the TCM group were significantly improved than the control group after treatment (P<0.05). Compared with before treatment, the levels of BMD, bone metabolic index were improved in both groups (P<0.05). The levels of BMD, BGP, BALP in the TCM group were better than the control group (P<0.05). There was no abnormality in blood routine, liver and kidney function in the 2 groups after treatment, and no obvious adverse reactions were observed. Conclusion Yishen Huoxue Decoction combined with PEMFs can enhance the motor function, improve the neurological deficit symptoms and decrease the pain in hemiplegia patients with OP, and the bone metabolism.

RESUMO

<p><b>OBJECTIVE</b>To determine the expression of high-mobility group protein box 1 (HMGB1), high-sensitivity C-reactive protein (hs-CRP), and D-dimer (D-D) in the peripheral blood of children with Henoch-Schönlein purpura (HSP) and to investigate the clinical significance of HMGB1 in children with HSP.</p><p><b>METHODS</b>A total of 40 children with HSP (HSP group) and 30 healthy children (control group) were involved in the study. The level of serum HMGB1 was determined using enzyme-linked immunosorbent assay, and the levels of serum hs-CRP and plasma D-D were determined using automatic biochemical analyzer and automatic blood coagulation analyzer, respectively.</p><p><b>RESULTS</b>The levels of HMGB1, hs-CRP, and D-D in the peripheral blood of the HSP group in the acute phase were significantly higher than in the control group (P<0.05). The levels of the three indicators were significantly higher in HSP children with renal damage than in those without renal damage (P<0.05). In children with HSP, the expression of HMGB1 was positively correlated with the expression of hs-CRP and D-D (r=0.878, P<0.001; r=0.625, P<0.001).</p><p><b>CONCLUSIONS</b>The expression of HMGB1 is related to the inflammatory response and hypercoagulability in children with HSP. HMGB1 may be involved in the development of HSP and associated renal damage in children.</p>

RESUMO

Objective To study the effect of intensive trunk muscle training on balance and walking in pa- tients with hemiplegia caused by stroke. Methods A total of 90 stroke patients were recruited and randomly divid- ed into a treatment group(45 cases)and a control group(45 cases). All the patients were given conventional reha- bilitation training.Meanwhile, intensive trunk muscle training was also administered for those in the treatment group as well. The trunk control function, balance ability and walking ability were assessed by using the trunk control test, Berg Balance Scale and the balance subscale of the Fugl-Meyer physical performance test, and Holden's functional ambulation classification, respectively, before and after 6 weeks of training. Results It was found that all the pa- tients scored better with the trunk control test,Berg's balance scale, the balance subscale of the Fugl-Meyer physical performance test and Holden's ambulation classification after treatment, and there were significant differences between the two groups after treatment(P
