The whole process management principles of chimeric antigen receptor T-cell therapy for lymphoma at Peking University Cancer Hospital / 白血病·淋巴瘤

Chimeric antigen receptor T (CAR-T) cell has achieved excellent efficacy in hematological tumors, especially for lymphoma. Many products have been approved to market all over the world, and 2 products targeting CD19 have been approved to treat relapsed and refractory large B-cell lymphoma in China. The current experiences of using CAR-T cells come from previous clinical studies. How to use CAR-T cells in a standardized and rationalized way is still a challenge faced by our clinicians. Based on the CAR-T cell treatment experiences from Peking University Cancer Hospital and the latest research progresses in CAR-T in China and abroad, this article will elaborate on patient screening, peripheral blood mononuclear cell collection, bridging treatment, lymphocyte depletion chemotherapy, CAR-T cell infusion, the monitoring and treatment of adverse events after infusion, and long-term follow-up after infusion, in order to guide clinicians to better use CAR-T cell and to bring maximum benefits to patients.

The research progress of carotid web and cryptogenic stroke / 中华神经科杂志

Carotid web can be defined as an endoluminal shelf-like projection often noted at the origin of the internal carotid artery just beyond the bifurcation.In recent years,it has been found that the carotid web is one of possible risk factors for cryptogenic stroke.Understanding the pathogenesis and pathological characteristics of carotid artery web,and accurately diagnosing the carotid artery web will help to implement targeted intervention for cryptogenic stroke,and reduce the recurrence of stroke events.In this paper,we will review the research progress of relationship between the carotid artery web and cryptogenic stroke,the common imaging features,pathological research,pathogenesis,diagnosis and treatment.

Application of Health Education in Psychological Care of Patients Undergoing Hematopoietic Stem Cell Transplantation and Their Family Member / 中国医学伦理学

Hematopoietic stem cell transplantation has a good clinical effect on the treatment of hematologic malignancies, but during the treatment, it will bring relevant negative psychology to the patient' s psychology and body. This paper analyzed the psychological obstacles often emerging during the treatment of hematopoietic stem cell transplantation, as well as the family members' negative psychological emotion caused by the long-term psychological pressure and economic pressure, and further analyzed the reasons for the psychological problems in the patients and the corresponding nursing methods, and the causes of the psychological disorders of the patients' families and the corresponding nursing methods. In addition, health education was adopted for patients and their family members, and through the methods of doctor-patient communication, disease education, behavior training and so on, this paper found that after the implementation of health education for patients and their family members, the postoperative recovery rate of transplanted patients was obviously improved, and their psychological problems were also obviously alleviated and eliminated. It was proved that health education had a good therapeutic effect on the psychological problems of hematopoietic stem cell transplantation patients and their family member, and was worthy of promotion in clinical practice.

The association of serum levels of MIF and EGF and cognitive function in first-episode schizophrenic patients / 中华行为医学与脑科学杂志

Objective To explore the serum levels of macrophage migration inhibitory factor(MIF)and epidermal growth factor(EGF) in first-episode schizophrenia patients characterized and the correlation of MIF,EGF serum levels with psychotic symptoms and cognitive function.Methods The serum levels of MIF and EGF in patients (53 cases) and controls (58 cases) were measured by enzyme-linked immunosorbent assay (ELISA).The patients' psychotic symptoms were assessed by positive and negative syndrome scale (PANSS).The MATRICS consensus cognitive battery (MCCB) was used to assess the cognitive function.Results MIF serum level in cases group was higher than that of control group and the difference was statistically significant((50.54±23.05)μg/L vs (36.72± 18.52) μg/L) (P＜0.01).EGF serum level in cases group was higher than that of control group and the difference was statistically significant((5 163.40±2 289.76) vs (3 584.83± 1 444.71) ug/L) (P＜0.01).There was a positive correlation between serum MIF level and PANSS score in schizophrenic patients(P＜0.05).Score of TMT in MCCB in cases group was significantly higher than that in control group (P＜0.01),while scores of BACS SC,HVLT-R,BVMT-R and CF in MCCB in cases group was significantly lower than those in control group (P＜0.01).Serum level of MIF in cases group was significantly positively correlated with score of BVMT-R(P＜0.05),and serum level of EGF in cases group was significantly positively correlated with score of BVMT-R (P＜ 0.05).There was a positive correlation between serum MIF levels and serum EGF levels in the cases group (P＜0.05).There was a positive correlation between serum MIF and serum EGF levels in the control group (P＜0.05).Conclusion The clinical symptoms and partial cognitive impairment in patients with first-episode schizophrenia are related to the concentration of serum protein factors,and there are neuroimmunological abnormalities and neurotrophic imbalances and they are related.

Reflections on Humanistic Care in Patients with Malignant Lymphoma during Hospitalization / 中国医学伦理学

This paper introduces the implementation method of humanistic care nursing in the concrete work process,and puts forward some thoughts on the humanistic care nursing process,such as setting up the nursing concept with the patient-oriented;To establish the management norms of humanistic care;To develop the humanistic care model,ward or hospital,etc.In the future work will continue to deepen the connotation of humanistic care,and do a good humanistic care services.

Clinical characteristics and lung CT imaging features in heart failure patients with normal or reduced left ventricular ejection fraction / 中华全科医师杂志

The clinical features and lung CT findings of 174 heart failure patients,including 85 cases with normal left ventricular ejection fraction (LVEF) and 89 cases with reduced LVEF were reviewed.Patients with normal LVEF had a higher proportion of hypertension,diabetes and obesity than patients with normal LVEF; and cough and dyspnea were more common.The lung CT findings in patients with normal LVEF were frequently presented as interstitial lung edema,increased pulmonary texture,subpleural line,Kerley lines and diffuse ground-glass opacity.Patients with reduced LVEF had a higher proportion of coronary heart disease and clinical manifestations of acute left heart failure,and central alveolar edema presented in lung CT images.Results suggest that heart failure patients with normal LVEF usually have longer clinical history and occult symptoms,and have a different lung CT imaging features from those in heart failure patients with reduced LVEF.
