RESUMO

ObjectiveTo understand the health information needs of chronic disease inpatients and the current utilization of community health resources, and to analyze the influencing factors, so as to provide basis for personalized and high-quality health education for chronic disease inpatients and to guide them to make full use of community health resources. MethodsFrom November 2020 to February 2021, we conducted a face-to-face multi-center clinical epidemiological survey with paper questionnaire in three general hospitals of Shanghai. The study included 404 inpatients with diabetes, hypertension, coronary heart disease, chronic respiratory diseases, cancer and other chronic diseases. Results94.01% of the 404 respondents had different needs for disease or health related information, and there was no significant difference between patients of different ages, genders and educational backgrounds in their needs for health knowledge. Among these patients, only 39.11% of them participated in the establishment of a card for chronic disease management in the community or signed for a family doctor. The participation rate of male patients was lower than that of female patients (P<0.05). ConclusionPatients with chronic diseases have a high demand for health related information, and we need to provide health information in multiple ways. We should increase investment in community health resources, improve service quality, and guide residents with chronic diseases to make full use of community health resources.

RESUMO

Objective:To investigate the current status of doctor-patient communication of pediatric residents, and to explore the specific demand of communication skills training in clinical practice. Methods:A self-made questionnaire was conducted to investigate 77 residents taking standardized residency training of pediatrics in Beijing Children's Hospital training base, including basic information, self-satisfaction of communication ability and the needs of doctor-patient communication courses, etc. SPSS 22.0 statistical software was used for data processing and analysis. Results:In terms of the doctor-patient communication ability, the results showed that the satisfaction degree of pediatric resident surgeons was higher than that of pediatric resident physicians (78.9% vs. 58.5%, P=0.108) and the satisfaction degree of high-seniority residents was higher than that of low-seniority residents (78.6% vs. 62.5%, P=0.330; 78.6% vs. 58.9%, P=0.278). Besides, the importance of communication objectives and the degree of difficulty were sorted in order, and the top three were family members, superior and children for importance, and family members, children and nurses for difficulty, respectively. The main reasons of difficult communication included heavy work (41.6%) and lack of communication skills (46.7%), with no statistical difference between different majors and seniorities ( P >0.05). More than 80% residents agreed that effective doctor-patient communication could promote the clinical work. The lower of seniority, the higher demands for the class time allocation and teaching frequency of doctor-patient communication training courses. Conclusion:Pediatric residents have a good cognition of medical doctor-patient communication. There are some obvious communication problems in pediatric physicians and low-seniority pediatric residents. Therefore, it's necessary to adjust courses according to different majors and seniorities, so as to improve the training quality, thereby promoting the clinical work and reform of medical education.

RESUMO

Objective:To investigate the therapeutic effect of human umbilical cord mesenchymal stem cells (MSCs) on psoriasis-like mouse models induced by imiquimod and the underlying mechanisms. Methods:Eighteen C57BL/6 mice were randomly and equally divided into vaseline group, model group and treatment group according to a random number table. The mice in the model group and treatment group received topical treatment with 5% imiquimod cream at a dose of 62.5 mg once a day for 6 consecutive days on the shaved back, and those in the vaseline group received the treatment with the same amount of vaseline ointment; the mice in the treatment group were injected with 1.5×10 6 human umbilical cord MSCs via the caudal vein on days 1 and 4. The severity of skin lesions on the back of the mice was assessed everyday according to the psoriasis area and severity index (PASI) . Twenty-four hours after the last treatment, that is, on day 7, blood samples were taken, and the mice were sacrificed. The dorsal skin tissues were resected and subjected to hematoxylin and eosin (HE) staining. A single cell suspension of the resected spleen was prepared, and flow cytometry was performed to detect the Th1 and Th17 cell subsets in the spleen cells. Enzyme-linked immunosorbent assay was conducted to detect serum levels of cytokines interleukin (IL) -17A and tumor necrosis factor (TNF) -α. One-way analysis of variance was used for comparisons among groups, Tukey test for multiple comparisons, and repeated measures analysis of variance for the analysis of changes in the PASI score over time. Results:On day 7, there was obvious scaly erythema on the back of the mice in the model group, and the skin thickness and number of infiltrating inflammatory cells were significantly higher in the model group (78.73 ± 23.11 μm, 36.16 ± 2.95 cells/mm 2) than in the vaseline group (13.28 ± 4.57 μm, 13.33 ± 1.15 cells/mm 2, q=19.25, 7.21, respectively, both P < 0.001) . The treatment group showed significantly decreased PASI score, epidermal thickness and number of infiltrating inflammatory cells compared with the model group (all P < 0.001) . The percentage of Th17 cell subsets in the spleen cells and serum level of TNF-α were significantly lower in the treatment group than in the model group (both P < 0.05) . There were no significant differences in the spleen weight, spleen index, spleen cell count, Th1 cell percentage or serum IL-17A level between the treatment group and the model group (all P>0.05) . Conclusion:Human umbilical cord MSCs can effectively alleviate skin inflammation induced by imiquimod in the psoriasis-like mouse models, likely by inhibiting Th17 cell formation and TNF-α expression.

RESUMO

OBJECTIVE: To study lung tissue injury induced by long-term aerosol inhalation of 4 kinds of non-aerosol drugs in healthy SD rats, and to evaluate the safety of aerosol inhalation of non-aerosol drugs. METHODS: Totally 40 healthy SD rats (♂) were randomly divided into 8 group, i.e. blank control group, normal saline group (solvent control), budesonide group (non-aerosol drug control, 0.1 g/L) ,silicon dioxide group (lung injury drug control, 40 g/L)and 4 kinds of non-aerosol drugs [Dingchuan decoction group (15 g/mL, calculated by crude drug), cefatriaxone group (200 g/L), Qingkailing group (stoste) and Tangreqing group (stoste)], with 5 rats in each group. Except that blank control group didn’t received any treatment, other groups received aerosol inhalation, 10 mL, twice a day, for consecutive 56 days. After medication, the number of white blood cells in peripheral blood were counted and classified, and the number of white blood cells in bronchus alveolar lavage fluid were counted. The pathological changes of lung tissue were observed by HE staining and the number of dust cells was counted. The expression of leukocyte differentiation antigen 163 (CD163) in lung tissue were determined by immunohistochemical method. RESULTS: The white blood cells in peripheral blood mainly included lymphocyte and neutrophil, of which lymphocyte is the main one. Compared with blank control group, there was no statistical significance in the number of white blood cells, lymphocyte or neutrophil in peripheral blood, the number of white blood cells in alveolar lavage fluid or the number of dust cells in lung tissue of rats in normal saline group (P>0.05); the structures of bronchus and lung tissue were intact, and the expression of CD163 was negative. Compared with normal saline group, there was no statistical significance in the above indexed of rats in budesonide group(P>0.05), the structures of bronchus and lung tissue were intact, and the expression of CD163 was negative, while the number of white blood cells, lymphocyte or neutrophil in peripheral blood, the number of white blood cells in alveolar lavage fluid or the number of dust cells in lung tissue of rats in other 5 groups were all increased significantly (P<0.05); alveolar wall thickening and alveolar interstitial edema occurred in different degrees in lung tissue. The expression of CD163 was positive or strongly positive. CONCLUSIONS: The long-term aerosol inhalation of 4 kinds of non-aerosol drugs can induce pathological changes of lung tissue and increase the number of inflammatory cell and dust cell in alveolin in healthy SD rats.

RESUMO

Objective To approach the brain protective effect and mechanism of Sini decoction on rats with cardiopulmonary resuscitation (CPR) syndrome. Methods Fifty Sprague-Dawley (SD) rats were divided into sham operation group (n = 10), CPR model group (n = 20) and Sini decoction treatment group (n = 20) by random number table. The rat models were established by trachea clipping to induce cardiac arrest, and after heart beat stopped for 5 minutes, CPR was carried out. In the Sini decoction group, Sini decoction 5 g/kg was given through a stomach tube, once per 24 hours, while in the sham and CPR model groups, the same amount of normal saline was given by the same method at the same time. Venous blood was collected before CPR and 6, 12, 24, 48 and 72 hours after CPR, and the levels of serum interleukin-6 (IL-6) and tumour necrosis factor-α (TNF-α) were determined by enzyme linked immunosorbent test (ELISA); after CPR for 72 hours, the rat brain tissue was obtained from all the groups, the content of caspase-3 in brain tissue was detected by immunohistochemistry method, and its protein expression caspase-3 was detected by Western Blot; the apoptosis situation of brain tissue was detected by terminal deoxynucleotidyl transferase-mediated duTP nick end labeling (TUNEL).Results With the prolongation of time, the levels of IL-6 and TNF-α in CPR model and Sini decoction groups showed a tendency firstly increased and then decreased, IL-6 reached its peak value after resuscitation for 24 hours, while TNF-α reached its peak value after resuscitation for 48 hours, and both IL-6 and TNF-α were decreased after resuscitation for 72 hours ; beginning from 6 hours after resuscitation, the levels of serum IL-6 (ng/L: 61.79±1.31, 62.49±1.42 vs. 21.48±0.79) and TNF-α (ng/L: 48.32±1.98, 25.32±1.96 vs. 18.34±2.45) in CPR model and Sini decoction treatment groups were all significantly higher than those in sham group, since 12 hours after resuscitation, the level of IL-6 was significantly lower in Sini decoction than that in CPR model group (ng/L: 70.41±2.21 vs. 88.32±1.59), and since 6 hours after resuscitation, TNF-α was obviously lower in Sini decoction group than that in CPR model group (ng/L: 25.32±1.96 vs. 48.32±1.98, allP < 0.05), both IL-6, TNF-α persisting to 72 hours after resuscitation, and their levels did not return to normal at the end of experiment in the two groups. After the end of resuscitation, the content and protein expression of caspase-3 and rate of cell apoptosis in the brain tissue in CPR model group were significantly higher than those in the sham group [caspase-3 content (A value,×103): 2.59±0.26 vs. 1.57±0.06, caspase-3 protein (gray value): 0.80±0.08 vs. 0.43±0.04, apoptosis rate: (2.01±0.08)% vs. (0.26±0.02)%, allP < 0.05], above indexes in the Sini decoction treatment group were significantly lower than those in the CPR model group [caspase-3 content (A value,×103): 1.89±0.08 vs. 2.59±0.26, caspase-3 protein (gray value): 0.57±0.02 vs. 0.80±0.08, apoptosis rate: (1.03±0.05)% vs. (2.01±0.08)%, allP < 0.05).Conclusion The Sini decoction has a protective effect on rats with post-resuscitation syndrome, and its mechanism is possibly realized by the inhibition of inflammatory factors and reduction of cell apoptosis.

RESUMO

<p><b>OBJECTIVE</b>To investigate the in vitro effects of immune inhibitor tacrolimus on platelet function.</p><p><b>METHODS</b>Fresh venous blood was collected from healthy volunteers at ages of 18-25 years old, who are not taking antiplatelet drugs within two weeks. The platelets were isolated from the blood and incubated with different concentrations of tacrolimus (0.06, 0.6, 6, 60, 120, 240 μmol/L) at 37 °C for 2 hours, and then the changes of mitochondrial membrane potential and P-selection of platelets were detected by flow cytometry, the expression of apoptosis related protein by Western Blot, and the change of the platelet aggregation function by platelet aggregation analyzer.</p><p><b>RESULTS</b>Tacrolimus at concentration of 0.06 μmol/L could promote collagen induced platelet aggregation, inhibit thrombin induced platelet aggregation, have no effect on ristocetin and vWF induced platelet aggregation function. Tacrolimus at concentration of 120 μmol/L and 240 μmol/L could reduce the platelet mitochondrial membrane potential and induce the expression of apoptosis protein caspase-3.</p><p><b>CONCLUSION</b>In vitro experimental results showed that high concentration of tacrolimus could lead to platelet apoptosis. But the current therapeutic dose of tacrolimus at 0.06 μmol/L (which is equivalent to 50 ng/ml blood concentration) could have different effects on platelet aggregation function according to different stimulating agents.</p>

RESUMO

Objective To explore the endogenesis metabolizer character of kidney-yang insufficiency syndrome model rats caused by hydrocortisoni natrii succinas with metabolomics technology. Methods Twenty four Sprague-Dawley(SD)male rats were divided randomly into a normal group, a kidney-yang insufficiency model group, and a oral administration group, eight rats in each group. After producing kidney-yang insufficiency model by injecting hydrocortisoni natrii succinas intramuscularly, oral administration group rats were administered orally with white prepared lateral root of aconite every day for two weeks. After twenty-four hour the last oral administration, the blood plasm were prepared and used for testing endogenesis metabolism with the liquid phase color spectrum-mass spectra (LC/MS) metabolomics technology. Results Apices shape change of lysophosphatidyl choline (LPC) and lysophosphatidyl ethanolamine(LPE) and 468.4m/z unbeknown chemical compound of normal rats were distinct from those of model rats. Above-mentioned chemical compound as chief material symbols of kidney-yang insufficiency syndrome might be farther studied. Conclusion Finding out differential symbol from endogenesis metabolizer with metabolomics technology was redounded to deep exploring the biology essence of traditional Chinese medicine syndrome.

RESUMO

Objective: To investigate the combined preventive effect of folic acid (FA) and vitamin B12 (VB12) on the developmental toxicity of alcohol. Methods: To build animal model with the developmental toxicity by giving 5g/kg bw alcohol (25% ethanol) intragastrically (IG) to ICR mice during gestational day (GD) 6-15. In addition to alcohol, three groups were given FA 60 mg/kg bw, VB12 1 mg/kg bw, FA 60 mg/kg bw + VB12 1 mg/kg bw respectively by IG during GD1-GD16. An alcohol model group and a negative control group were set. All dams were killed at GD18. Results: Compared to the alcohol model group, the pregnant mice of FA + VB12 combined intervention group put on more weight during pregnancy; the live fetal rate; the fetal weight, body length and tail length were all increased; the abnormal ossification rate of sternum, occipital bone, and four limbs dropped (P

RESUMO

Objective:To investigate the association of human leucocyte antigen-DRB1 alleles with chronic hepatitis B and hepatic cirrhosis in Han population of Jilin area.Methods:HLA-DRB1 alleles in 61 patients with chronic hepatitis B,44 patients with hepatic cirrhosis,32 spontaneous rehabilitees after HBV infectin, and 50 healthy controls were analyzed by using polymerase chain reaction/sequence specific primer technique. Results:The frequency of HLA-DRB1*1201-3 allele in chronic hepatitis B group was higher than that in healthy control group(17.21% vs.8.00%,P=0.0427,RR=2.391).The frequency of HLA-DRB1*0701 allele in chronic hepatitis B group and hepatic cirrhosis group was markedly higher than that in healthy controls group(11.48 % vs.2.00%,P=0.006 6,RR=6.35;12.50% vs.2.00%,P=0.004 6,RR=7.00)and in spontaneous rehabilitee group (11.48% vs.1.56%,P=0.0183,RR=8.17;12.50% vs.1.56%,P=0.0136,RR=9.00).No significant differences frequencies of the other HLA-DRB1 alleles were observed among the four groups. Conclusion:Alleles of HLA-DRB1*1201-3 and HLA-DRB1*0701 are closely associated with chronic hepatitis B and maybe susceptible genes or linkage genes of chronic hepatitis B.HLA-DRB1*0701 is also closely associated with hepatic cirrhosis and maybe a susceptible gene or linkage gene-posthepatitis B-cirrhosis in Han population of Jilin area.
