RESUMO

Objective: To compare the effects of thoracotomy and total thoracoscopic surgery on coronary artery fistulae complicated with ectasia


Methods: Forty-six patients with coronary artery fistulae complicated with ectasia were randomly divided into a treatment group and a control group [n=23] which were given totally thoracoscopic surgery and thoracotomy respectively. Both groups were followed up


Results: All patients survived the surgeries and were discharged from the hospital. The treatment group had significantly less intraoperative blood loss, earlier postoperative ambulation and shorter postoperative hospitalization stay more than those of the control group [P<0.05]. The two groups had similar plasm cortisol and adrenocorticotropic hormone levels one day before and after surgery. These levels peaked on the postoperative 3rd day, but those of the treatment group were significantly lower [P<0.05]. The two groups had similar left and right atrial diameters as well as left ventricular ejection fractions [LVEFs] before surgery, but the treatment group had significantly higher postoperative LVEF [P<0.05] as well as significantly smaller left and right atrial diameters in the postoperative 1[st] and 3rd months [P<0.05]. The treatment group was significantly less prone to postoperative complications such as chest pain, atelectasis and pulmonary infection than the control group [P<0.05]


Conclusion: Total thoracoscopic surgery promoted the recovery of coronary artery fistulae complicated with ectasia, improved cardiac remodeling and cardiac function, and alleviated stress reaction, with wellproved safety

Objective To study the cytogenetic features of acute myeloid leukemia (AML) and analyze the association with cytogenetic features and early responses after induction therapy.Methods The karyotypes of 395 patients who had been newly diagnosed with AML were analyzed.These patients were divided into three groups (low-risk,intermediate-risk and high-risk),according to the AML NCCN guidelines.The incidence of different karyotypes in these three groups and the complete remission (CR) rate after the first cycle of induction therapy were analyzed.Results The incidence rates of karyotypes in high-risk,intermediate-risk and low-risk groups were 50.56 % (180/356),39.89 % (142/356),9.55 % (34/356),respectively.All patients with t(15;17) who completed induction therapy reached CR.There was significant difference in the CR rates of t(8;21) groups with or without additional karyotypes [92.00 %(23/25) vs 50.00 %(11/22)] (x2 =10.317,P =0.001).There was no significant difference in the CR rates between normal and-Y karyotype group [61.90 % (39/63) vs 58.82 % (10/17)] (x2 =0.054,P =0.817).Complex cytogenetics ascribed to the low-risk group,of which monosomal karyotype was common,nine of ten patients with monosomal karyotype were associated with an inferior CR rate.Conclusion The cytogenetic features of AML are different from previous reports by other centers.The cytogenetic features of AML patients not only influence the long-term survival,but also the CR rates of induction therapy.

RESUMO

Objective To investigate the clinical effects of treatment of chronic pulmonary heart disease (CPHD) at acute stage with sodium ferulate and Ciwujia injection. Methods 90 cases of CPHD at acute stage were recruited into treatment group 1, treatment group 2 and control group. The control group was treated with consistent inhalation of oxygen,antibiotics, and symptomatic therapies like relieving cough, removing sputum, and stopping wheeze. The treatment group 1was treated with sodium ferulate, Ⅳ 0.2g, once daily for successive 14days, in addition to the conventional treatment. The treatment group 2 was treated with Ciwujia Injection, 100ml Ⅳ, once daily for successive 14 days. Observe the curative effect and the changes of blood gas analysis indexes (PAo2, PaCO2, SAO2) and bemorheological indexes (whole blood high-cutting viscosity, whole blood low-cutting viscosity, whole blood recovery viscosity, erythrocyte aggregation index, and HCT) before and after treatment. Result Compared with the control group, both treatment groups got improvement in blood gas analysis indexes and hemorheological indexes and curative effect, but no significant difference between the two groups (P>0.05).Conclusion both sodium ferulate and Ciwujia injection had good results in treating CPHD at acute stage, and there was no difference between the two medicines in terms of curative effects.

RESUMO

Objective:To detect the influence of the early Class Ⅲ orthopedic treatment on the development of the maxillary.Methods:Four adolescence rhesus monkeys were fitted with casting Twin-block magnetic appliance(TMA),two were put to death after 1.5 months and another two after 3 months,zygomaticofrontal sutures were harvested,and performed histological examination using optical and electron microscope.Another two monkeys without appliance were served as control.Results:The maxillary sutures were rebuilt,after Class Ⅲ functional orthopedic,and the zygomaticomaxillary suture,zygomaticotemporal suture and transverse palatine suture were especially active.Electron microscopy results showed that osteoblasts and fibroblasts were active,and fibroblasts showed more active effects.Conclusion:During the treatment of the Class III functional orthopedics,maxillary sutures take part in the rebuilding actively,and there are lots of new bone formation along the sutures.
