BACKGROUND@#The clinical features of enthesitis in patients with psoriatic arthritis (PsA) have been reported in some Western countries, but data in China are very limited. This study aimed to describe the characteristics of enthesitis in Chinese patients with PsA and compared them with those in other cohorts.@*METHODS@#Patients with PsA enrolled in the Chinese Registry of Psoriatic Arthritis (CREPAR) (December 2018 to June 2021) were included. Data including demographics, clinical characteristics, disease activity measures, and treatment were collected at enrollment. Enthesitis was assessed by the Spondyloarthritis Research Consortium of Canada (SPARCC), Maastricht ankylosing spondylitis enthesitis score (MASES), and Leeds enthesitis index (LEI) indices. A multivariable logistic model was used to identify factors related to enthesitis. We also compared our results with those of other cohorts.@*RESULTS@#In total, 1074 PsA patients were included, 308 (28.7%) of whom had enthesitis. The average number of enthesitis was 3.3 ± 2.8 (range: 1.0-18.0). More than half of the patients (165, 53.6%) had one or two tender entheseal sites. Patients with enthesitis had an earlier age of onset for both psoriasis and arthritis, reported a higher proportion of PsA duration over 5 years, and had a higher percentage of axial involvement and greater disease activity. Multivariable logistic regression showed that axial involvement (odds ratio [OR] 2.21, 95% confidence interval [CI], 1.59-3.08; P <0.001), psoriasis area and severity index (PASI) (OR: 1.03, 95% CI: 1.01-1.04; P = 0.002), and disease activity score 28-C reactive protein (DAS28-CRP) (OR: 1.25, 95% CI: 1.01-1.55; P = 0.037) were associated with enthesitis. Compared with the results of other studies, Chinese patients with enthesitis had a younger age, lower body mass index (BMI), a higher rate of positive human leukocyte antigen (HLA)-B27, more frequent dactylitis, and a higher proportion of conventional synthetic disease-modifying antirheumatic drugs' (csDMARDs) use.@*CONCLUSIONS@#Enthesitis is a common condition among Chinese patients with PsA. It is important to evaluate entheses in both peripheral and axial sites.

Objective@#To construct a scientific, practical and objective quality evaluation system for evaluating clinical probation teaching effects of Psychiatry Nursing in undergraduate students.@*Methods@#Protocol of the system was initially made via literature analysis method and experts interview method; indexes were screened via Delphi method; and the evaluation system was formed by comprehensively considering experts' suggestions. The data was analyzed by using SPSS 19.0.@*Results@#All expert authority coefficients were more than 0.8; expert activeness coefficients in two rounds were 100% and 88.3%, respectively; and expert coordination coefficient was 0.235 (P<0.01). The designed teaching evaluation system contained three first-level indexes, seven second-level indexes and forty-one third-level indexes.@*Conclusion@#The drafted teaching evaluation system for evaluating clinical probation teaching effects of Psychiatric Nursing reflects requirements and characteristics of psychiatric special training during clinical probation and pays attention to cultivate students' humanistic care and clinical practice ability, with high scientificness and reliability, so as to provide objective and quantitative basis for evaluating probation.

Objective@#To Effects of n-hexane on learning and memory and the expressions of nerve growth factor (NGF) mRNA and nerve growth factor receptor (NGFR) mRNA of brain tissue in mice exposed to N-hexane.@*Methods@#40 Kunming mice were randomly divided into low-dose group, meddle-dose group, high-dose dose group and control group, with 10 mice in each group. All the groups were orally exposed to n-hexane in different doses: low-dose group with 43.5 mg/kg, middle-dose group with 86.5 mg/kg and high-dose group with 173.0 mg/kg, 1 time per day for 20 d. After the poisoning, the Y-arm test and the expressions of NGF mRNA and NGFR mRNA and the concentrations of NGF and NGFR in the brain tissues of each group were measured.@*Results@#In the first Y-arm test, there existed a significant difference in correct reaction rate generally in all groups (P<0.05), and correct reaction rate in the middle-dose group and the high-dose group were low significantly compared with that in the control group(P<0.05). In the second Y-arm test, there existed a significant differences in total electric shock time and correct reaction rate generally in all groups (P<0.01), and the total electric shock time prolonged significantly and the correct response rate decreased significantly in 3 dose groups compared with those of the control group(P<0.05). The expression levels of NGF mRNA in brain tissues of low, meddle and high dose-groups were 0.81±0.66, 0.67±0.37 and 0.69±0.26, and the expression levels of NGFR mRNA were 1.22±0.42, 1.98±0.84 and 2.01±2.01, respectively. Compared with the control group, the expressions of NGF mRNA in the 3 dose groups decreased significantly (P<0.05), and the expression of NGFR mRNA in middle-and high-dose groups increased significantly (P<0.05). The concentrations of NGF in brain tissues of low,meddle and high dose-groups were 39.97±7.24 ng/L, 39.26±7.88 ng/L,31.70±8.21 ng/L,and the concentrations of NGFR were 17.37±6.82 ng/L,21.37±7.16 ng/L, 22.46±7.70 ng/L, respectively. Compared with the control group, the concentrations of NGF in high-dose groups decreased significantly(P<0.05), and the concentrations of NGFR in middle-and high-dose groups increased significantly (P<0.05).@*Conclusion@#N-hexane exposure can result in decrease of learning and memory in mice, which may be related to abnormal expression of NGF mRNA and NGFR mRNA in brain tissue.

Objective To explore the relationship and influencing factors between pre‐diabetes mellitus (pre‐DM) and hypertension, providing evidence for formulating strategies for cardiovascular disease prevention and control. Methods We conducted this study from June 2013 to September 2014. Using stratified multistage random sampling, participants were administered a questionnaire survey, their height, weight, waist circumference, hip circumference, other physical attributes, blood pressure and blood lipids were measured. They also underwent the 75‐g glucose tolerance test and other laboratory examinations. A logistic regression model was used to analyze the relationship between pre‐DM and hypertension and its influencing factors. Results A total of 4 002 participants completed the survey. Participants'mean age was 50.3 ± 11.8 years. Of the total participants, 1 962 (49.0%) were males, while 2 039 (51.0%) were females; 1 participant had missing gender data. Further, 2 188 participants had normal glucose metabolism, 1 066 had pre‐DM, and 748 had diabetes. The prevalence of hypertension in participants with normal glucose metabolism, impaired fasting glucose, impaired glucose tolerance, both impaired fasting glucose and impaired glucose tolerance, and DM was 28.3%, 46.5%, 46.3%, 62.0%, and 61.2%, respectively. The prevalence of hypertension varied among people with different glucose metabolism (χ2=306.672, P<0.001). The prevalence of hypertension in the pre‐DM population increased with the aggravation of abnormal glucose metabolism compared to the normal glucose metabolism population, with a linear trend (χ2=299.009, P<0.001). Among those with abnormal glucose metabolism, there were differences in age, cholesterol, triglycerides, low‐density lipoproteins, body mass index, and waist circumference compared to those without hypertension (P<0.05). The risk of hypertension in the pre‐diabetic population was 1.5 times higher than that in the normal glucose metabolism population (OR=2.510, 95% CI: 2.156-2.922, P<0.001). There was no difference in the correlation intensity between pre‐DM and hypertension when gender was taken into account. Age and lipid abnormalities slightly decreased the correlation intensity between abnormal glucose metabolism and hypertension. Considering body mass index and centripetal obesity, the correlation intensity between abnormal glucose metabolism and hypertension could be reduced by controlling these factors. Conclusion The prevalence of hypertension is high in people with pre‐DM. There is a correlation between pre‐DM and hypertension, even when considering factors such as age, dyslipidemia, body mass index, and centripetal obesity. Therefore, it is necessary to strengthen the management of blood pressure in the pre‐diabetic population; improve early intervention for risk factors such as dyslipidemia, body mass index, and centripetal obesity; and reduce the occurrence of hypertension.

Objective To investigate the influence of Joint Commission International (JCI) accreditation on nursing practice.Methods Totally 30 medical institutions which had passed the JCI accreditation were investigated to compare the scores of nursing practice before and after JCI accreditation.Results Before JCI accreditation,the average standardized score of governance,leadership and management dimension was 81.20,but the scores of other dimensions were below 80.After the JCI accreditation,the quality improvement and patient safety(QPS) dimension scored 81.10,and scores of other dimensions were all above 90.The differences of all dimensions were statistically significant (P<0.001) before and after the JCI accreditation,the scores were all much greater than those before the accreditation,and improvement of AOP(77.37%)and IPSG(69.86%)was most obvious with statistical significance (P<0.001).Conclusion JCI accreditation plays a certain role in promoting nursing practice,making nursing process more systematic and comprehensive and the safety of nursing guaranteed.

This study was aimed to evaluate the clinical effect and safety of Qing-Fei Tong-Luo (QFTL) ointment for treating children with pneumonia.Randomized controlled trial (RCT) was conducted among 460 cases of children with pneumonia.The observation group was given QFTL ointment combined with basic treatment.And the control group was only treated by basic treatment.Evaluation was given on the total clinical efficacy,disappeared time of fever,cough,expectoration,shortness of breath,and medication safety.The incidence of respiratory diseases was followed up on the 30th days after drug withdrawal.The results showed that in the aspect of clinical efficacy between two groups,the cure rate of the observation group was 98.26%,and that of the control group was 93.89%,with statistic significance (P < 0.05).The cure rate of the observation group was better than that of the control group.There was statistical difference on expectoration disappeared time (P < 0.05).There was no statistical difference on disappeared time of fever,cough and shortness of breath (P > 0.05).There was statistical difference on the incidence of respiratory diseases on the 30th days followed-up after drug withdrawal (P < 0.05).There was no statistical difference on the incidence of upper respiratory tract infection,pneumonia and asthma (P > 0.05).No adverse reactions occurred in the observation group.It was concluded that QFTL ointment combined with basic therapy on the treatment of pneumonia in children was significantly better than the control group in the aspect of clinical efficacy,expectoration disappeared time and the incidence of bronchitis.It is safe and effective.The prognosis is good and worthy of promotion in the clinical practice.

Mycoplasma pneumoniae (MP) is an important pathogen of community-acquired pneumonia (CAP) in children.Mycoplasma pneumoniae pneumonia (MPP) is a common lower respiratory tract infection (LRTI) in children.It accounts for 10-40% of CAP in hospitalized children.This article discussed the correlation between MPP and windwarmth diseases in traditional Chinese medicine (TCM) from the similar epidemiological features and clinical manifestations such as long disease term and disease process.The treatment method of clearing lung heat and removing pathogenic factors received good curative effect in the clinical practice.

Aim To investigate the effects of isoliquiri-tigenin ( ISL) on anti-angiogenesis both in vitro and in vivo and its mechanisms. Methods We assessed the antiangiogenic activities of ISL on proliferation viabili-ty, migration and tube formation of human microvascu-lar endothelial cell line-1 (HMEC-1) in vitro. The cell proliferation viability was assessed using the Sulforho-damine B ( SRB ) assay. Modified Boyden Transwell chamber assay was done to study the effect of ISL on HMEC-1 cells migration. 2′, 7′-dichlorofluorescein di-acetate ( DCFH-DA) was used to measure the levels of intracellular reactive oxygen species ( ROS ) , which was induced by VEGF. Metalloproteinase-2 ( MMP-2 ) and metalloproteinase-9 ( MMP-9 ) expressions by HMEC-1 cells were assessed through gelatin zymogra-phy assay. HMEC-1 cells cycle was detected by flow cytometry. Moreover, we investigated the in vivo anti-angiogenic activity of ISL on chicken embryos nap al-lantoic membrane model ( CAM ) . Results ISL con-centration-dependently inhibited the growth of HMEC-1 cells as well as SW620 and A549 cells. ISL signifi-cantly and concentration-dependently suppressed the migration activity of HMEC-1 cells. Tube sample struc-ture formation further confirmed the effect of ISL on an-ti-angiogenesis. Moreover, ISL also inhibited intracel-lular ROS level, MMP-2 and MMP-9 expression by HMEC-1 cells. ISL induced endothelial cell apoptosis at a low concentration ( ISL 12 . 5 μmol · L-1 ) and blocked the cells in S phase of mitosis at higher con-centrations ( ISL 25~100 μmol·L-1 ) . Furthermore, ISL distinctly inhibited the angiogenesis of chick em-bryos in vivo. Conclusions ISL has anti-tumor and angiogenesis effects on HMEC-1 cells. The mechanism may be related to intracellular ROS scavenging and ap-optosis induction of HMEC-1 cells.

BACKGROUND:The use of mesenchymal stem cels in the field of tissue engineering for osteoarticular injury repair is a very promising tool since these cels are readily expandable and able to differentiate into chondrocytes. Abundant evidence suggests that microRNAs play critical roles in chondrogenic differentiation of mesenchymal stem cels. OBJECTIVE:To observe the chondrogenic effect of human bone marrow mesenchymal stem cels transfected with lentiviral vectors bearing miR-221-3p/222-3p inhibition, thereby provding new strategies for cartilage injury. METHODS: miRNA microarray technology was applied to detect microRNAs expression profiles at three different stages of chondrogenic differentiation induction after transforming growth factor-β3 treatment and verified by real-time fluorescence quantitative PCR (RT-qPCR). Human bone marrow mesenchymal stem cels were infected with lentivirus bearing miR-221-3p/222-3p inhibition. After co-suppressing the expression of miR-221/222-3p, cel counting kit-8 was used to determine the cel proliferation, the differentiation of bone marrow mesenchymal stem cels towards chondrocytes was verified by type II colagen protein expression through immunohistochemistry and glycosaminoglycan accumulation was also elevated by sarranine O staining. RT-PCR was used to detect type II colagen and aggrecan mRNA expression at 21 days of chondrogenic induction. RESULTS AND CONCLUSION: The expression of miR-221-3p/222-3p was inhibited after Lv-miR221-3p/222-3p inhibition co-transfected into bone marrow mesenchymal stem cels. microRNA microarray and RT-qPCR results showed that the expression of miR-221-3p/222-3p was declined significantly at the anaphase of chondrogenic differentiation. The expression levels of chondrogenic markers, Aggrecan and type II colagen were significantly increased in the miR-221-3p/222-3p inhibition group and cel proliferation was also inhibited significantly compared with non-transduced cels or transduced with the empty lentiviral vector group. miR-221-3p/222-3p knockdown in bone marrow mesenchymal stem cels could inhibit proliferation but promote chondrogenic differentiation of bone marrow mesenchymal stem cels.

OBJECTIVE:To optimize the technology of separation and purification of active constituents from Morus alba L. METHODS:With the adsorption rate and desorption rate as the indexes,static and dynamic macroporous resin adsorption-desorp-tion methods were adopted to optimize the type of macroporous resin,the loading amount of fluid,the volume fraction and amount of eluent in the separation and purification technology of total flavonids. With the yield and content of polysaccharide as the index-es,water extraction and alcohol precipitation method was used to optimize the volume fraction of alcohol for polysaccharide precipi-tation in the separation and purification technology of total polysaccharides. With the absorbance of total alkaloids as the index,ion exchange method was employed to optimize the loading amount of fluid and the amount of eluent in the separation and purification technology of total alkaloids. Then the verification was conducted. RESULTS:The optimal technology of total flavonids from Morus alba L. was with D101 macroporous resin as the adsorbent,loading fluid mass concentration of 0.1 g(material medicinal)/ml,loading fluid amount 9 times as the column volume(BV),70% alcohol of 5 BV for elution. The optimal volume fraction of alcohol for polysaccharide precipitation in the technology of total polysaccharides was 80%. The optimal technology of total alka-loids was with loading fluid amount of 3 BV,0.5 mol/L ammonia water of 8 BV for elution. After separation and purification,the purities of total flavonids,total polysaccharides and total alkaloids from Morus alba L. were 35.68%,47.14% and 55.79%,and the RSDs were 1.28%,1.61% and 1.14%(n=3). CONCLUSIONS:The optimal technology is stable and suitable for the separa-tion and purification of active constituents from M. alba L.

This study is to investigate the effects of ceramide on GSTA1 expression in Caco-2 cells. After being exposed to ceramide for a fixed time, GSTA1 protein expression was detected by Western blotting analysis; GSTA1 mRNA expression was detected by real time PCR; dual luciferase assay was used to analyze GSTA1 transcriptional activity and GSTA1 activity was determined toward androstanedione (AD) as substrate. The data showed that ceramide can significantly induce the expression of protein and GSTA1 mRNA, and increase transcriptional activity and enzyme activity of GSTA1. The results demonstrated that ceramide may increase resistance to chemotherapeutics in Caco-2 cells by up-regulating the expression of GSTA1.

Objective To explore a preliminary health behavior self-efficacy scale for functional dyspepsia patients,and supply a practical evaluation tool for clinical nurses.Methods Through literature review,group discussion and expert interview,the authors established the preliminary index system.The Delphi method was used to organize two rounds of 20 experts consultations.Results In the two rounds of expert consultations,experts positive coefficients were 100%,respectively.The authoritative coefficient of experts was 0.83,respectively.The Kendall's W coefficients were 0.23 and 0.29.The evaluation system included 5 dimensions,11 evaluation indicators,and 42 evaluation items.Conclusions The reliability of the self-efficacy scale developed with Delphi method is relatively high and it can acted as an evaluation tool for self-efficacy and health education of patients with functional dyspepsia.

Objective To study the relationship between social support and quality of life in surgical patients with breast cancer. Methods Quality of life and social support status of 78 patients with breast cancer were evaluated by FACT-B and SSRS,and their correlation analysis was carried out Results The scores of emotional status and functional well - being in the aspact of life quality were lower (9.10±3.99,12.81 ±7.99) for patients with breast cancer, while total score of social support (36.93 ± 6.42) was higher than the average (34.56 ± 3.73). Life quality of breast cancer patients in terms of physiological condition, social/family status and emotional status were significantly positively related with total score of social support. Conclusions Social support was an important factor affecting the quality of life in breast cancer patients. Nurses should pay more attention to and make full use of social support resources of patients, in order to improve and enhance patients' quality of life through a variety of interventions.

Objective 5-azacytidine could induce the diffcrentiation of stem cells into cardiomyocytes (CMs). The aim of this study was to screen the optimal condition for 5-azacytidine inducing differentiation of human mesenchumal stem cells (hMSCs) into CMs, and the effect of 5-azacytidine on adherence, cell vigor and chromosome karyotype ofhMSCs. Methods hMSCs were isolated from human bone marrow and cultured in vitro. The phenotypes ofhMSCs were identified by flow cytometric analyses. MTT test was used to investigate the effect of different concentrations of 5-azacytidine on proliferation ofhMSCs. Four weeks after 5-azacytidine induction, semi-quantitative RT-PCR, transmission electron microscopy (TEM), single-cell action potentials, detection ofcardio-cnzyme AST and LDH, cell adherence, cell viability and chromosome karyotype test were performed. Results The typical morphological features of hMSCs were fibroblast-like in shape, hMSCs expressed CD44 and CD105,and did not express CD34, CD45 and CD31. The optimal concentration of 5-azacytidine was 10μmol/L. The shape ofhMSCs treated with 5-Azacytidine changed from fusiform to polygon or astrocyte gradually, and passaged cells were evenly arranged as polarity structure. Induced-hMSCs connected with neighbouring cells, forming myotube-like structures 4 weeks later. It was confirmed that induced hMSCs shaped myotubule-like structure and had some of micro-histologic structures of CMs by TEM. RT-PCR showed that induced hMSCs expressed cardiac specific product BNNP and early cardio-myogenesis specific transcription factor NKX2.5mRNA. Besides, induced-MSCs led to the weak action potential and secreted cardio-enzyme AST and LDH. There was no significant difference in cell adherence and viability before and after induction. Both hMSCs and induced-hNSCs kept stable normal diploid nucleus. Conclusion The optimal condition for inducing effect of 5-azacytidine is 10 μ mol/L and 24-hour incubation; and under this condition, the adherence, vigor and chromosome karyotype ofhMSCs would not be affected.

Objective To explore the application of experienced teaching method in the teaching of Fundamentals of Nursing.Methods The feasibility and validity of experienced teaching method was evaluated by expounding the utilization and result of role-play method and practice experienced method.Results The experienced teaching method received good result and proved appropriate in the teaching of Fundamentals of Nursing.Conclusion The experienced teaching method overcame the disadvantages of mere traditional teaching method and aimed at the improvement of quality and ability of students.It contributed to developing the critical thinking ability and other abilities of students.

Objective To isolate and cultivate hepatic stem cells(HSCs) from rat fetal liver in vitro and identify their biological features.Methods Collagenase perfusion method and mechanical cutting method were used to isolate HSCs from rat fetal liver which were then cultivated by H-DMEM containing 10% fetal bovine serum.The cell surface antigen expression of HSCs was observed with immunocytochemical method under confocal laser scanning microscope.Results The isolated HSCs from rat fetal liver grew to monolayer 5 d after cultivation in vitro.They presented pykno-round cells and distinct borderline under the light microscope.After 8 d the cells grew like epithelium.These cells expressed AFP antigen,CK 18 and CK19.Conclusion The cultivated cells are proved to be HSCs and can proliferate quickly in vitro.

RESUMO

[Objective] To evaluate the results of surgical treatment of developmental dislocation of the hip (DDH) in children aged from 12 to 18 months. [Methods]A total of 59 patients (72 hips with developmental dislocation) were treated surgically from 2004 to 2006.In them, 53 hips received open reduction and Salter innominate osteotomy, 19 hips received open reduction and Pemberton transiliac osteotomy.[Results]All patients had been followed up for 50 months in average after operation. According to the modified McKay clinical criteria,Severin radiographic classification and Salter AVN diagnostic criteria,the excellent to good rate for Severin classification and modified McKay criteria were 94.4%、93% respectively.Avascular necrosis of the femoral head occured in 2 hips(2.8%).[Conclusion]Surgical treatment without preoperative traction is feasible for the management of DDH in children aged from 12 to 18 months.

RESUMO

Objective To investigate the role of serum specific IgE(sIgE)antibodies,autoantibody and contact allergens in the pathogenesis of urticaria and the possible correlation among them.Methods The serum sIgE antibodies,autoantibody and contact allergens were tested by IVT kit,autologous serum test-ing and patch testing in145cases of urticaria.Results72.4%(84/116)of patients were positive with spe-cific IgE antibodies,among them13.8%(16/116)were strongly positive.37.7%(20/53)patients were pos-itive with autologous serum testing.In twenty-six patients the above two antibodies were tested.Specific IgE antibodies were found to be strongly positive in4(33.3%)of12patients with negative autologous testing,while in none of14patients with positive autologous testing.Patch testing was performed in21patients with chronic urticaria,90.5%(19/21)patients were positive to15of20allergens tested.Conclusion The pathogenesis of urticaria is complicated,which may include allergy to food,aeroallergens,contact allergens,as well as autoimmunity.Different allergic reactions may be present in the same individual.Contact allergens may be the factor responsible for contact urticaria.

Objective To investigate the role of vascular endothelial cell growth factor (VEGF), nm23 and microvessel density (MVD) in the pathogenesis of malignant melanoma(MM).
