RESUMO

Objective To investigate the sanitation status of dental model in medical institutions in Changning District of Shanghai, and to provide evidence for strengthening the disinfection management. Methods 40 medical institutions in Changning District of Shanghai were investigated by field sampling and detection. Results 92.5% of the investigated medical institutions disinfected dental models.In 168 dental model samples, the qualification rate of sanitary quality was 88.1%, while exceeded number of bacteria was the major reason for the unqualified.Among all dental model disinfection methods including ultraviolet irradiation, disinfectant soaking, disinfectant spraying and ozone, ultraviolet irradiation was the most widely used and disinfectant soaking was with the best effect.The storage sites of disinfected dental models including filling room, pollution area of sterilization room and other locations.Dental models stored in the filling room were with the highest qualification rate. Conclusion The current dental model sanitation status in medical institutions in Changning District of Shanghai is relatively unsatisfactory;supervision and inspection administration need to be strengthened by health and family planning administrative departments.

RESUMO

<p><b>OBJECTIVE</b>To investigate the effect of tetrandrine (TET) on zinc finger protein 139 (ZNF139) and multidrug resistance (MDR) of human gastric carcinoma cell lines and possible mechanisms.</p><p><b>METHODS</b>Cultured SGC7901 and SGC7901/ADR were treated with TET (0.5, 1.0, 1.5, 2.0, and 2.5 microg/mL), then inhibition rates were measured by MTT assay in vitro. The expressions of ZNF139, MRP-1, MDR1, and GST-pi were detected by RT-PCR. The correlation between ZNF139 and each multidrug resistance factor was analyzed using Spearman correlation analysis, and the coefficient correlation was calculated.</p><p><b>RESULTS</b>The inhibition rate of TET (< or = 2.0 microg/mL) for SGC7901 and SGC7901/ADR was less than 10% with MTT assay. Expressions of ZNF139, MRP-1, MDR1, and GST-pi mRNA were higher in SGC7901/ADR than in SGC7901 (all P < 0.05). The expressions of ZNF139, MRP-1, MDR1, and GST--pi were down-regulated in SGC7901/ADR cells efficiently (all P < 0.01). Positive correlation existed between ZNF139 and MRP-1, ZNF139 and MDR1 before treated by TET in SGC7901/ADR, and this relationship also existed in SGC7901/ADR cells after treated by TET (all P < 0.05).</p><p><b>CONCLUSION</b>TET could achieve MDR reversion in gastric cancer cells by down-regulating the expression of ZNF139, MRP-1, and MDR1.</p>

<p><b>OBJECTIVE</b>The purpose of this study was to investigate the efficacy and mechanism of oxaliplatin in combination with capecitabine (XELOX) regimen as neoadjuvant chemotherapy in the treatment of patients with advanced gastric cancer.</p><p><b>METHODS</b>Eighty-five patients with advanced gastric cancer (stage IIB and IIIC) were randomly divided into two groups: neoadjuvant chemotherapy group (40 cases) and surgery alone group (45 cases). In the neoadjuvant chemotherapy group, patients received oral administration of Xeloda 1000 mg/m(2) twice a day on days 1-14 and intravenous infusion of oxaliplatin 130 mg/m(2) on day 1 (XELOX regimen). The regimen was repeated every 21 days. In the surgery alone group, patients directly received radical resection of gastric cancer. The R0 resection rate, overall survival and disease free survival (DFS) were observed in all cases. The cycles and apoptosis rate of the gastric cancer cells were detected by flow cytometry. The expression of proliferating cell nuclear antigen (PCNA), p21, p53 and survivin was detected by Western blot.</p><p><b>RESULTS</b>In the neoadjuvant chemotherapy group, the total effective rate was 32.5% (13/40), and the tumor control rate was 90% (36/40), with few side effects. Compared with the surgery alone group, R0 resection rate was significantly higher in the neoadjuvant chemotherapy group (P < 0.05). The survival analysis indicated that both the overall survival and DFS were longer in the neoadjuvant chemotherapy group in comparison with those in the surgery alone group, but no significant differences were found (P > 0.05). In the neoadjuvant chemotherapy group, both the apoptosis rate and the ratio of cells in stage G0 and G1 were significantly higher than those in the surgery alone group (P < 0.05). The expression of PCNA and survivin was lower in the neoadjuvant chemotherapy group, while the expression of p21 and p53 was higher.</p><p><b>CONCLUSIONS</b>XELOX regimen as neoadjuvant chemotherapy in the treatment of patients with advanced gastric cancer can effectively improve the R0 resection rate and prolong the survival time of the patients. Its mechanism is probably that the neoadjuvant chemotherapy can markedly enhance apoptosis in gastric cancer cells and inhibit their proliferation.</p>

PURPOSE: Accurate prediction of lymph node (LN) status is of crucial importance for appropriate treatment planning in early gastric cancer (EGC). Matrix metalloproteinases (MMPs) have been shown to be involved in the pathogenesis of tumor progression and metastasis in gastric carcinoma. In this study, we investigated the association between MMPs expressions and LN metastasis in EGC. METHODS: Thirty-four LN positive and 80 LN negative pT1 tumors were immunohistochemically analyzed for MMP-2, MMP-7, and MMP-9 expression. The relation of MMPs expressions to LN metastasis was analyzed in the univariate and multivariate model. RESULTS: There were 73 men and 41 women with a mean age of 60 years. Among the pathologic characteristics, larger tumor size, submucosal invasion and lymphatic invasion were factors that are significantly associated with LN metastasis in pT1 tumors. Immunohistochemistry showed significantly higher MMP-7 expression (82.4% vs. 54.4%, P=0.005) in LN positive tumors, whereas MMP-9 (85.3% in LN positive vs. 67.5% in LN negative) and MMP-2 (70.6% in LN positive vs. 57.5% in LN negative) expression did not reach statistical significance. Multivariate analysis revealed that MMP-7 expression (OR 4.915, 95% CI 1.375~17.573) is an independent predictor of LN metastasis in EGCs, along with lymphatic invasion by tumor cells (OR 10.337, 95% CI 2.785~38.360). CONCLUSION: Our study shows that MMP-7 expression is significantly associated with LN metastasis in EGC. MMP-7 expression can be used to predict LN status in EGCs in addition to other pathological parameters.

RESUMO

<p><b>OBJECTIVE</b>To observe the relation of calcaneus fractures and compartment syhdromes of the foot taken place.</p><p><b>METHODS</b>From April 2003 to March 2008, there were 78 cases with calcaneus fracures involving 72 males and 6 females, aged from 17 to 56 years (averaged 34.2 years). The intracompartment pressures of 54 feet with the severity swell were measured with the Whiteside's method. Among them, 8 patients were final diagnosed the compartment syndromes of the foot. After incision decompression through the foot bottom wall,the whole foot swell fade away quickly. After 5 to 10 days the decompression incision were sutured.</p><p><b>RESULTS</b>The 8 patients were followed up for from 7 to 12 months,kinesthetic sense of the whole foot was mormal. None of the cases occurrenced claw toes, instep contracture and stiffness of the toes, et al.</p><p><b>CONCLUSION</b>Compartment syndromes of the foot general occurrences during 1 to 3 days after calcaneus fractures. Fractures of Sanders II-IV is easy to complicate compartment syndromes of the foot.</p>

<p><b>OBJECTIVE</b>To establish 2-dimensional gel electrophoresis (2-DE) images and seek differentially expressed serum proteins for understanding the pathogenesis of silicosis.</p><p><b>METHODS</b>2-DE and matrix-assisted laser desorption/ionization time of flight tandem mass spectrometry (MALDI-TOF-MS/MS) were used to screen differentially expressed serum proteins among silica-exposed population, suspect of silicosis (0+), phase one (I) group with silicosis and control group(non silica exposure).</p><p><b>RESULTS</b>Complement C4, leucine-rich alpha-2-glycoprotein and alpha-1-antitrypsin were significantly highly expressed in suspect of silicosis (0+) group(P < 0.01), but lowly in other groups. Inversely, serotransferrin was significantly down-regulated only in suspect of silicosis (0+) group(P < 0.01). Plasma glutathione peroxidase, tetranectin, apolipoprotein A-I and transthyretin were equally expressed in the serum of control group and silica-exposed population group, but decreased in the suspect of silicosis (0+) and phase (I) group.</p><p><b>CONCLUSION</b>Complement C4, leucine-rich alpha-2-glycoprotein, alpha-1-antitrypsin, serotransferrin, plasma glutathione peroxidase, tetranectin, apolipoprotein A-I and transthyretin are differentially expressed in the silica-exposed group and phase (I) group with silicosis, and the result should be validated by other biochemical technologies.</p>
