RESUMO

Background The current oil production determines oil workers’ occupational noise exposure. Without effective protection, noise will affect various aspects of worker’s body functions, including acting on the adrenal cortex system and resulting in renal function damage. Objective To evaluate the associations of noise exposure and its cumulative exposure level with renal function impairment of oil workers. Methods Oil workers from a collective medical examination in a hospital were selected as the study subjects. In accordance with the national standard Measurement of Physical Agents in the Workplace Part 8: Noise (GBZ/T 189.8—2007), noise exposure was measured three times at the oil workers' work site, and their average value was calculated to obtain the cumulative noise exposure (CNE). A questionnaire survey was conducted to collect general information such as socio-demographic characteristics, family history, lifestyles, and occupational history. All blood biochemical indicators were measured in the fasting state. Renal function impairment was judged based on the glomerular filtration rate. The relationship between CNE and renal function was analyzed using receiver operating characteristic curve (ROC) for workers with noise exposure. Results A total of 2 917 subjects were included in the study and their prevalence of renal function impairment was 14.2%. The univariate analysis results suggested statistically significant differences in the prevalence of renal function impairment among the oil workers grouped by having hypertension or not, gender, age, marital status, marital status, smoking, and alcohol consumption (P<0.05); the prevalence of renal impairment was significantly higher in those with abnormal values of uric acid, total cholesterol, triglycerides, high-density lipoprotein, and fasting glucose than in those with normal values (P<0.05); the oil workers with noise exposure [n=1565, 53.7%, equivalent sound level ≥80 dB(A)] showed a higher prevalence of renal function impairment than those without (P<0.05). The results of multiple logistic regression analysis showed that being female (OR=2.811, 95%CI: 1.960-4.030), age at 31 years and above (OR31-40=3.502, 95%CI: 1.402-8.751; OR41-50=4.255, 95%CI: 1.759-10.291; OR≥51=7.179, 95%CI: 2.864-17.996), showing abnormal uric acid (OR=5.932, 95%CI: 4.486-7.843), having hypertension (OR=1.593, 95%CI: 1.230-2.063), alcohol consumption (OR=2.648, 95%CI: 1.346-5.212), and smoking (OR=1.816, 95%CI: 1.133-2.911) had higher risks of developing renal function impairment; besides, those exposed to noise had 1.351 times (95%CI: 1.073-1.702) higher risks of developing renal function impairment than non-exposed individuals. Noise-exposed oil workers in the renal impairment group had higher noise exposure intensity and CNE compared to the noise-exposed oil workers in the normal renal function group (P<0.05), and the workers had an increased risk of renal function impairment when the CNE was >95.85 dB(A)·year versus CNE ≤ 95.85 dB(A)·year (OR=2.583, 95%CI: 1.956-3.411). Conclusion Exposure to noise, higher noise exposure intensity, and higher level of CNE may be associated with developing renal function impairment in oil workers. Oil workers with CNE above 95.85 dB(A)·year are at an increased risk of renal impairment.

Objective To investigate the effects of different doses of oxaliplatin (L-OHP) chemotherapy regimen for treatment of colorectal cancer and its influence on the adverse reactions in the nervous system.Methods A total of 92 cases of colorectal cancer patients in Wuhan Wuchang Hospital from August 2013 to April 2016 were selected as the research objects, which were randomly divided into the observation group (46 cases) and the control group (46 cases) according to the random lottery envelopes. All the patients were given FOLFOX chemotherapy. The observation group was treated with small dose L-OHP chemotherapy(60 mg/m2),the control group was treated with routine dose of L-OHP chemotherapy(120 mg/m2), and the efficacies were evaluated after 3 cycles. Results All the patients completed chemotherapy. The observation group included 26 cases of complete remission(CR),11 cases of partial remission(PR),5 cases of stable disease (SD), 4 cases of progression of disease (PD). The control group included 28 cases of CR, 10 cases of PR, 3 cases of SD, 5 cases of PD. Overall response rate (ORR) in the observation group and the control group were 80.4 % (37/46) and 82.6 % (38/46) respectively, and there was no significant difference between the two groups (χ 2= 0.072, P > 0.05). Nervous system adverse reactions included 6 cases of gradeⅠ, 3 cases of grade Ⅱ, 1 case of grade Ⅲ in the observation group, and 12 cases of grade Ⅰ, 6 cases of gradeⅡ,4 cases of gradeⅢin the control group.The incidence rate in the observation group was lower than that in the control group [21.7 % (10/46) vs. 47.8 % (22/46), χ2= 6.900, P < 0.05]. Up to November 2017, PD and overall survival time in the observation group were longer than those in the control group [(19 ±4)months vs. (16±4) months, t= 4.314, P< 0.05; (24±4) months vs. (20±5) months, t = 4.170, P < 0.05]. Conclusion The low dose L-OHP chemotherapy regimen has a favorable effect in patients with colorectal cancer, and it can reduce the adverse reactions in the nervous system and prolong the survival time of patients.

Objective To evaluate the relationship between endothelin-1 (ET-1) and p38 mitogenactivated protein kinase (p38 MAPK) and phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathways during mechanical stretch-induced enhancement of adhesion of rat pulmonary microvascular endothelial cells (PMVECs).Methods Rat PMVECs were seeded in the culture plate at a density of 0.5×105 cells/ml (2 ml/well) and divided into 5 groups (n=24 each) using a random number table:control group (group C),mechanical stretch group (group MS),mechanical stretch plus specific PI3K inhibitor LY294002 group (LY group),mechanical stretch plus specific p38 MAPK inhibitor SB203580 group (SB group),and mechanical stretch plus selective ETA receptor blocker BQ123 group (BQ group).Cells were exposed to 20% cyclic stretch at 0.3 Hz for 4 h using a sine wave.In LY,SB and BQ groups,LY294002,SB203580 and BQ123 at the final concentration of 10 μmol/L were added,respectively,after mechanical stretch,cells were incubated for 10 min,and then extracted and purified rat polymorphonuclear neutrophil leukocytes (PMNs,5× 105 cells/well) were added and co-incubated with PMVECs for 30 min and then washed out.The concentrations of ET-1 and interleukin-6 (IL-6) in the culture medium were determined using enzyme-linked immunosorbent assay.The expression of phosphorylated p38 MAPK (p-p38 MAPK) and phosphorylated Akt (p-Akt) was detected by Western blot.Adhesion of PMNs was measured by immuno-histochemistry,and the adhesion rate was calculated.The expression of P-selectin mRNA was detected using real-time polymerase chain reaction.Results Compared with group C,the concentrations of IL-6 and ET-1 in the culture medium were significantly increased,the expression of p-p38 MAPK,p-Akt and P-selectin mRNA was up-regulated,and the adhesion rate of PMNs was increased in the other four groups (P<0.05).Compared with group MS,the concentration of IL-6 in the culture medium was significantly decreased,the expression of p-Akt and P-selectin mRNA was down-regulated,and the adhesion rate of PMNs was decreased in LY,SB and BQ groups,the concentration of ET-1 in the culture medium was significantly decreased in group BQ,and the expression of p-p38 MAPK was significantly down-regulated in SB and BQ groups (P<0.05).Conclusion The signaling mechanism underlying ET-1-mediated enhancement of rat PMVEC adhesion may be related to activating p38 MAPK and PI3K/Akt signaling pathways.

Objective To study the effects of Trichinella spiralis (T.spiralis) infection on mice with oxazolone (OXZ)-induced colitis and the possible immunologic mechanism.Methods Female BALB/c mice at age 6-8 weeks were randomly divided into four groups (A to D).Each mouse in groups B and D was infected with T.spiralis strains.Twenty-one days after T.spiralis infection,the mice in groups A and B were treated with 50% ethanol solution,while those in groups C and D were treated with OXZ to induce the murine model of colitis.Several parameters including survival rate,disease activity index (DAI),macroscopic damage and histological score,myeloperoxidase (MPO) activity and the expression of cytokines (IFN-γ,IL-4 and IL-10) in colonic and splenic tissues at mRNA and protein levels were measured 3 and 7 days after modeling.Results No significant differences in the survival rate,DAI score,macroscopic damage score,histological score,MPO activity were observed between mice from groups C and D (P>0.05).Pre-exposing the mice to T.spiralis strains neither alleviated the mucosal damages nor aggravated the condition of colitis.Compared with group C,the expression of IFN-γ on the third day and the expression of IFN-γ,IL-4 and IL-10 on the seventh day at mRNA and protein levels in colon and spleen tissues were significantly increased in mice treated with T.spiralis and OXZ (P<0.05).The expression of IL-10 at transcriptional level in spleen tissues on the third day was higher than that of group C (P<0.05).The expression of IL-4 and IL-10 at protein level in colon tissues on the third day were significantly up-regulated as compared with those of group C (P<0.05).Conclusion The severity of OXZ-induced colitis in the murine model was neither alleviated nor aggravated by pre-exposing the mice to T.spiralis strains.High doses of IL-10 not only weakened the regulatory effects on Th2 responses,but also induced the production of proinflammatory cytokines,resulting in a new drift of Th1/Th2 without aggravating Th2 responses.Further investigation on the mechanism of T.spiralis-induced over-expression of IL-10 should be conducted,which might increase the practicability of using T.spiralis strains against OXZ-induced colitis.

Objective To explore the current status and influencing factors of oral health knowledge,attitudes and practice (KAP) among the teachers of primary and secondary schools in Gongshu District of Hangzhou City.Methods A total of 652 teachers of primary and secondary schools were selected by using multi-stage stratified cluster random sampling method and completed a self-designed questionnaire,which included demographic characteristics and oral health KAP.The status and influencing factors of oral health KAP were analyzed by single and multiple-factor analysis methods.Results The questionnaire was completed by all the 652 teachers,627 of whom were qualified (96.2%).The total ratios of oral health knowledge awareness,attitudes correctness and behaviors formation were 77.1%,68.7% and 46.3%,respectively.There was significant difference of knowledge awareness rate by age,gender,marriage status,monthly income,life status and current cigarette smoking (x2 values were 11.48,8.57,8.12,8.46,23.97 and 4.64,respectively; all P<0.05).There was significant difference in oral health attitudes correctness rate by gender (x2=10.65,P<0.05) and behaviors formation rate by gender,education level and life status (x2 values were 3.95,10.88 and 10.41,respectively; all P<0.05).In logistic regression model,impact factors of oral health knowledge awareness were age,gender,life status and physical exercises; the impact factors of oral health attitudes correctness were gender and monthly income; the impact factors of oral health behaviors formation were gender,level of education and physical exercises.Conclusion The oral health awareness and attitudes correctness are acceptible among the teachers of primary and secondary schools in Gongshu District of Hangzhou City,although the formation rate of oral health behaviors is low.Oral health education should be enforced among the teachers of primary and secondary schools.

RESUMO

Stimulation of the round window (RW) for coupling an implantable hearing system to the cochlea has gained increasing clinical importance. To compare the vibration transfer to the cochlear fluids and partition in response to normal acoustic stimulation and to mechanical stimulation of the RW, we carried out an acoustic-structure coupled finite element analysis using a recently developed finite element (FE) model in our laboratory, which consisted of external ear canal, middle ear and cochlea. Intracochlear pressures were derived during normal forward sound stimulation as well as reverse RW stimulation. A model was utilized to calculate the force required of an actuator at the RW to produce a differential intracochlear pressure that is equivalent to a stimulus produced in normal ear by a given external ear-canal pressure. The current results provided further information to support the optimization of the actuators and adapt existing prostheses for RW stimulation in order to insure sufficient acoustic output.

Objective To study the levels of Th1 and Th2 type cytokines in 3-nitrobenzene sulfonate(TNBS) and oxazolone(OXZ) colitis without or with Trichinella spiralis( T. spiralis) infection. Methods Female BALB/c mice were randomly divided into 4 groups: 50% Ethanol, T. spiralis only, TNBS or OXZ,T. spiralis +TNBS or OXZ(at least 6 in each group when mice were killed). The levels of IFN-γ, IL-12,IL-4, IL-10 in colon in mice with colitis induced by TNBS or OXZ without or with T. spiralis infection 3 d and 7 d post-induction were assessed using ELISA method. Results Colonic protein levels of IFN-γand IL-12 were significantly increased 3 d and 7 d after intra-colonic injection of TNBS( P <0.05 ). Concurrent infection with T. spiralis prevented this rise in IFN-γand IL-12 secretion and tended to induce a rise in colonic IL-4 and IL-10 content(P <0. 05). The levels of IFN-γ, IL-4 and IL-10 protein in colitic mice colon with prior nematode infection on days 3 and 7 post-induction of colitis were significantly higher than that seen in colitic mice without prior nematode infection ( P < 0.05 ). Conclusion T. spiralis infection significantly attenuates TNBS-induced colitis in mice. The local immunologic mechanism is that T. spiralis can down-regulate strongly Th1-type immune response of colitis and up-regulate Th2 response, Tr1-cytokines. According to the change of Th1/Th2 cytokines, prior T. spiralis infection doesn't reduce the severity of OXZ-induced colitis, but without aggravating colitis. The exactly immunologic mechanism deserve to be explored deeply.

Objective To explore the etiopathogenisis of Reinke's edema. Methods To detect 15 samples of Reinke's edema of vocal cord mucosal and 5 samples of common vocal cord mucosal with S-P immunohistochemical method. Then to observe the effect of immunity class under the light microscope. To detect the expression of the VEGF and the production of the blood capillary in the sample of Reinke's edema of vocal cord mucosal with immunohisto-chemical method. Results There are 12 effects of sex in the 15 samples of Reinke's edema of vocal cord mucosal. There are effects of feminine in the 5 samples of common. Conclusion The main etiopathogenisis of Reinke's edema is that blood vessel grew ad libitum, the permeability of blood vessel increase and oxygen lack partly.

In order to screen antigenic genes of Clonorchis sinensis (C.sinensis).Firstly,a cDNA expression library from adult worms of C.sinensis was successfully constructed with lambda ZAP express vector.Total RNA of C.sinensis adult worms were extracted by the Trizol reagent and mRNA were further purified through oligo-dT cellulose.The first strand eDNA was synthesized by using MMLV reverse transcriptase.After the synthesis of the second strand,the cDNA were purified by CHROMA SPIN-400 kit and then ligated with lambda ZAP express vector,then packaged in vitro and amplified.The original library contained 1.5 × 106 pfu cDNA clones,the titer of amplified library reached 1.5 × 1010 pfu/mL,in which about 99% clones were recombinants and most of insert DNA fragments were 0.4-2.0 kb.Secondly,immunoscreened using the naturely infected man serum from C.sinensis.The positive clones were sequenced and analyzed.From 2.0 × 105 recombinant clones of the eDNA library,41 positive clones were obtained.Sequence analysis indicated that the cDNAs encoded proteins including glycine rich antigen 2,proline rich antigen 2 and antigen Cs44 from C.sinensis,anothers were lower similarity to predicted protein from Nematostella vectensis,transcription elongation factor GreA from Bartonella quintana str.Toulouse and NM_132090 CG3446 gene product in Drosophila melanogaster from Schistosoma japonicum.These data may form a foundation for identifying recombinant antigens that can be used in the diagnosis or vaccination against clonorchiasis.

Objective:To investigate the inhibitory effect of Trichinella spiralis polypide protein on H7402 cell line in vitro and to assess its anti-tumor effect against hepatocellular carcinoma(HCC).Methods:The Trichinella spiralis polyp- ide protein was isolated from adult worms and newborn mixture.Trichinella spiralis polypide protein was prepared through homogenization and centrifugation,and its concentrations were determined by UV-visable spectrometers.H7402 cells trea- ted with Trichinella spiralis polypide protein(at 0.035,0.070,0.140 mg/ml)served as treatment group,and normal he- patic cell line HL-7702 treated with Trichinella spiralis polypide protein served as control group.Cell proliferation was de- tected by MTT;the effect of Trichinella spiralis polypide protein on migration and invasion ability of H7402 cell line was assessed by scarification test and invasion test.Cell apoptosis was observed by TUNEL.Apoptosis and cell cycle were ana- lyzed by FCM.Results:Trichinella spiralis polypide protein was successfully prepared.The protein at 0.035,0.070,0. 140 mg/ml inhibited H7402 cells proliferation by(22.40?13.80)%,(29.45?16.80)%,and(39.38?17.80)%, respectively.Apoptosis was observed by TUNEL and FCM,with an apoptosis rate of(39.07?0.90)%;the cell cycle was blocked at S phase.Scarification test and invasion test suggested that Trichinella spiralis polypide protein inhibited the migration and invasion of H7402 cell line,with the inhibition rate being(63.79?13.71)%.Conclusion:Trichinella spiralis polypide protein can inhibit the proliferation,migration,invision of H7402 cell line,and it might be a potential anti-cancer agent.

BACKGROUND: Although the sternohyoid muscular valves are traditionally used as reparative material for functional reconstruction after partial laryngectomy, the late-term results are not so satisfactory.OBJECTIVE: To investigate the efficacy of using external layer mucoperiosteum valve of thyroid cartilage to repair the wound surface and reconstruct vocal function.DESIGN: A self-controlled observational trial using the patients as subjects.SETTING: Department of otolaryngology of a municipal hospital.PARTICIPANTS: A total of 57 patients with laryngocarcinoma were selected from the Department of Otolaryngology, Shantou Second People' s Hospital, from June 1998 to May 2004. Among them, there were 56 men and 1 woman who were aged 37 to 78 years with the average age of 60. 38 years and whose disease duration was two months to one year with the average of five months.METHODS: In the 57 patients with laryngocarcinoma, their external layer mucoperiosteum valves of thyroid cartilages in the affected side were preserved and inverted into laryngeal cavities, which were used to cover the wound surface and rebuild the vocal fold.MAIN OUTCOME MEASURES: The final outcome indices: changes in the symtoms and function and Karnofsky scores before and after treatment. Risk indices: adverse events and side effects.RESULTS: The operations on the 57 patients were successful. The incidence of respiratory tract obstruction and hoarseness in the patients before operation was 38.9% and 98. 1%, but 1.9% and 96. 3% after treatment. Karnofsky score was 40 and 70 before and after treatment, respectively. No complications of pharyngeal stricture occurred in the near and long term.CONCLUSION: Vertical partial laryngectomy and laryngeal reconstruction with mucoperiosteum valve are easy to operate. The recovery of the wound surface is good, phonation efficacy is satisfacory, and respiratory function is not affected. The treatment has low incidence of complications and satisfactory long-term results.

A polymer film of vitamin B1 was obtained at a graphite carbon electrode by cyclic voltammetric method in phosphate buffer solution (PBS). Cyclic voltammograms of the film electrode in PBS (pH 5) exhibited one pair redox waves, Epa=350 mV,Epc=325 mV,different from monomer vitamin B1. The peak currents of poly-VB1 films was proportional to (scan rate)1/2, which showed that the charge transfer was controlled by diffusion in the polymer film. The Poly-VB1 modified electrode showed a good catalytic ability to dopamine and epinephrine. At the modified electrod, the peak potential for oxidation of dopamine occurred at 250 mV and 200 mV for epinephrine, while they were shifted towards the negative direction 100 mV and 300 mV respectively at the bare glassy carbon electrode . And their peak current had a linear relationship to the concentrations in the range of 8.0×10-7~1.0×10-4 mol/L for dopamine and 1.0×10-6~1.0×10-4 mol/L for epinephrine.

Objective The aim of this article is to research the radical mastoidectomy and primary tympanoplasty to attain radical cure of the mastoid focus as well as rebuilding the sound- conducting structure of the tympanic cavity to improve the audition. Method The autobone and temporal fascia were taken as the transplants after the radical mastoidectomy under microscopy, and then to carry out the operation of Portmann` s tempanoplasty of 2nd and 3rd types of 2nd class.Result Non of the cholesteatoma was relapsed after follow- up for 1~ 4 years.The efficiency rate of tympanoplasty is of 89.47% .The average audition increased was 19 dB HL. Conclussion Radical operation for cholesteatoma should be associated with tympanoplasty if having condition. We recommend that the opening tympanoplasty should be used, and use autobone as a artificial auditory ossicles.
