RESUMO

Objective:To explore the relationship between emotional trauma and depressive symptoms. Methods:In November 2021, a total of 1 650 university students were assessed using scales including childhood trauma questionnaire (CTQ), distress tolerance scale (DTS), rumination response scale (RRS) and the depression anxiety stress scale (DASS). SPSS 20.0 and AMOS 25.0 software were used to analyze the data with Spearman correlation analysis, Mann-Whitney U test and mediation analysis. Results:The differences of DTS, RRS and DASS were significant between high and low trauma group(all P<0.01). Emotional trauma (16.70(6.09)) was negatively correlated with distress tolerance(10.95(2.15)) ( r=-0.26, P<0.01), and positively correlated with rumination(39.51(10.74)) and depression(4.93(5.76)) ( r=0.31, 0.38, both P<0.01). The testing of mediating effect indicated that emotional trauma had direct effect ( β=0.20, P<0.001) and indirect effect on depression through distress tolerance and rumination. Conclusion:Emotional trauma, distress tolerance and rumination are important factors influencing depression, and it could provide guidance for undergraduate psychological intervention of depression.

RESUMO

Objective:To explore the risk factors and preventive strategies of pancreatitis after percutaneous transhepatic biliary drainage (PTBD) in patients with pancreatic cancer and obstructive jaundice. Methods:A total of 241 patients were retrospectively analyzed from May 2001 to October 2014 in Tianjin Medical University Cancer Institute and Hospital. The possibly correlated 9 factors were analyzed, including gender, age, hemoglobin level, total bilirubin level, degree of pancreatic duct dilatation, degree of pancreatic atrophy, degree of biliary stenosis, the pancreatic duct visualization, and drainage mode. Results:Univariate analysis suggested that pancreatic duct dilatation, pancreatic atrophy, visualized pancreatic duct and drainage mode were associated with the incidence of pancreatitis after PTBD ( P<0.05). Logistic regression analysis showed that visualization of pancreatic duct ( OR=6.33) was a risk factor for pancreatitis, while pancreatic duct dilatation ( OR=0.14), pancreatic atrophy ( OR=0.12) and external drainage ( OR=0.11) were protective factors for pancreatitis. Conclusion:In pateints with pancreatic cancer and obstructive jaundice, pancreatic duct dilatation and pancreatic atrophy predict low risk of pancreatitis after PTBD,while intraoperative pancreatic duct visualization and internal or external drainage may increase the incidence of postoperative pancreatitis.

RESUMO

Objective:To explore the effects of childhood trauma on depression of college students and the role of depletion sensitivity and self-control in it.Methods:A total of 1 678 freshmen and sophomores from a medical university in Guangzhou were investigated by childhood trauma questionnaire(CTQ), Beck depression inventory-Ⅱ(BDI-Ⅱ), depletion sensitivity scale(DSS) and self-control scale(SCS) through random cluster sampling. The mediating effect of depletion sensitivity and moderating effect of self-control were examined by SPSS 26.0 macro program PROCESS V2.13.Results:(1)The total scores of CTQ and BDI-Ⅱ in college students were (34.56±7.03) and ( M( QR): 5.00(7.00)) respectively, and the scores of DSS and SCS were (41.78±12.21) and (40.02±8.15) respectively. (2)Childhood trauma was positively correlated with depletion sensitivity ( r= 0.20, P<0.01) and depletion sensitivity was positively correlated with depressive symptoms ( r=0.48, P<0.01). There was a negative correlation between self-control and depressive symptoms ( r=-0.49, P<0.01). (3)Depletion sensitivity partly mediates the relationship between childhood trauma and depressive symptoms. The direct effect (effect size=0.28) and mediating effect (effect size=0.07) accounted for 80% and 20% of the total effect (effect size=0.35), respectively. (4)Self-control moderated the path between depletion sensitivity and depressive symptoms. Conclusion:Childhood trauma can indirectly affect college students′ depressive symptoms through depletion sensitivity, and the partial mediatory role of depletion sensitivity is also moderated by self-control.

RESUMO

The healthcare plus aging care is a new type of aging care model that integrates medical services and aging care services. China has successively issued a series of policies, encouraging the localities to proactively explore such a model. The authors refer to the four dimensions of exchange and communication, policy resources, policy executor preferences and organizational structure in the Edwards policy execution model, for analyzing the implementation of such a policy. The study found such constraints for the policy as follows: unclear policy implementation standards, inadequate policy resources of manpower, financial and material resources, differences in preferences among local governments, general hospitals, primary medical institutions, and pension institutions, as well as decentralized and restricted organizational structures. At present, the policy of healthcare plus aging care remains in the stage of exploration and trial. The government should continue to define and revise the policy to ensure the smooth progress and long-term implementation of the policy of healthcare plus aging care.

RESUMO

Family physicians contracted service is an important choice to deepen the reform of China′s healthcare reform and achieve the strategic goal of " health for all" . In order to effectively promote the contracted service, the central government has successively issued a number of policy documents, which are echoed by the localities in their proactive explorations, achieving remarkable results. But there are also multiple roadblocks hindering the contracted service to progress. This article referred to the three-circle theory and studied policy implementation from the three dimensions of policy value, policy resources, and policy impetus. A thorough analysis of the problems in the implementation process revealed such main problems as insufficient family physicians, inadequacy of supporting policies and low level of policy acceptance among the young healthy age groups. The follow-up work will promote the long-term implementation of the family physicians contracted service policy by enriching the family physicians team, enriching supporting policies, and focusing on promotions to key age groups.

RESUMO

Objective To investigate the effect of incomplete cryoablation on the biological behavior of prostatic cancer RM-1 cells and its mechanism. Methods RM-1 cells of prostatic cancer were placed in -20℃ icebox to be frozen for 5 min. After the recovery of the cell state, the RM-1 cells were frozen again for 10 min and 15 min successively. After culture for one day,the cellular morphology was microscopically examined.A total of 20 C57/BL mice were used to establish the tumor-bearing models, which were randomly and equally divided into the control group and the incomplete cryoablation group with 10 mice in each group. At scheduled time points the tumor lesion size was measured for all mice. The mice were sacrificed at 14 days, the lung tissues were collected and were stained with lE;the numbers of metastatic lesions in the lung were calculated.Transwell assay was used to test the cell migration and invasion,immuno-blotting method was adopted to determine the epithelial-mesenchymal transition-related (EMT-related) protein expression level, and the enzyme-linked immunosorbent assay (ELISA) was employed to check the secretion volume of transforming growth factor-beta (TGF-β).Results After incomplete cryoablation,RM-1 cells became disorderly arranged, their morphology was changed, and antenna structure might be formed. At 3 and 7 days after cryoablation, the tumor size in the incomplete cryoablation group was slightly smaller than that in the control group, but only the difference at 7 days after cryoablation was statistically significant between the two groups (P=0.019).At 10 and 14 days after cryoablation, the tumor volume of the two groups was almost equal. The pulmonary metastatic lesions in the incomplete cryoablation group were obviously much more than those in the control group (P<0.001).Transwell assay indicated that the cell migration and invasion ability in the incomplete cryoablation group was stronger than that in the control group (P<0.05).Immuno-blotting test revealed that, when compared with the control group,in the incomplete cryoablation group the expressions of N-cadherin,MMP-9 and Vimentin were up-regulated,while the expression of E-cadherin was downregulated.ELISA test showed that increased secretion of TGF-β was observed in the incomplete cryoablation group. Conclusion Incomplete cryoablation can enhance the migration and invasion ability of RM-1 cells, increase the number of pulmonary metastatic lesions in tumor-bearing mice, and affect the EMT-related protein expression level.

RESUMO

Many studies show that transforming growth factor-β (TGF-β) can suppress tumor by inhibiting proliferation and dedifferentiation of cells and promoting cell apoptosis at early stage. But during the progression of tumor, its tumor-promoting action will be more and more obvious with the variation of TGF-β itself and its signal path.TGF-β signal path inhibitors have anti-tumor activity, but the clinical effects and side effects still need to be observed.

RESUMO

Objective:This study aimed to analyze the feasibility, safety, and efficacy of cryoablation in treating pain caused by malignant paravertebral and mesenchymal tumor. Methods:Cryoablation was performed in 31 patients with unresectable and painful malignant paravertebral and mesenchymal tumors whose pain was poorly controlled by conventional treatment methods. Tumors ranged in size from 3 cm to 20 cm. Pain experienced by the patient was assessed using the Brief Pain Inventory (BPI) at 0, 1 day, 1 week, 1 month, and 3 months post-cryoablation. Results:BPI scores were divided into two categories, i.e., the influence and the se-verity of pain. Both categories showed downward trend after cryoablation. Pain severity score significantly decreased (P=0.001, t=3.862;P=0.031, t=2.261) 1 day and 1 month after cryoablation. Pain influence score also significantly decreased (P=0.016, t=2.566;P=0.036, t=2.195) 1 day and 1 week after cryoablation. Two patients (6.45%) had mild complications, and no serious complication was ob-served. Conclusion:Cryoablation is a low-risk and well-tolerated topical treatment for pain caused by unresectable malignant paraverte-bral and mesenchymal tumors.
